[Anabolic therapy in patients with HIV infections].

AIM: To assess effectiveness of Deka-Duraboline--preparation of anabolic steroid hormone (nandrolone decanoate)--in the treatment of weight loss in AIDS patients. MATERIALS AND METHODS: 13 AIDS patients with weight loss < 10% admitted to Moscow Center of AIDS Prevention and Treatment were given Deka-Durabolin 1 time a week 50 mg intramuscularly for 12 weeks after failure of attempts to arrest weight loss. Body mass, diarrhea, appetite and general condition (visual analogue scale) were studied. RESULTS: Deka-Duraboline treatment arrested weight loss in 12 of 13 cases, improved well-being. All the patients displayed better appetite, reduced diarrhea. CONCLUSION: Deka-Duraboline is effective in improving body weight, appetite, diarrhea and well-being in AIDS patients. It is advisable to further study Deka-Duraboline in a controlled randomized trial.

[Modification of hematotoxic effects of cyclophosphamide by anabolid steroids].

Human malignant tumors have been shown to be highly resistant to the chemotherapeutic agents so far available. Since these have to be administered in high doses, hematotoxic effects occur frequently during such treatment. Androgens and anabolic steroids being potent stimulators of normal and of some forms of impaired erythropoiesis, studies were initiated in rats to assess the possibility of a protective role of these steroids against the toxicity of cyclophosphamide. The following results were obtained: 1. A single dose of cyclophosphamide (100 mg/kg) significantly lowered the erythrocyte, reticulocyte, hemoglobin, leucocyte and platelet counts. 2. An initial treatment with nandrolone phenylpropionate signifiantly reduced the decrease of both the erythrocyte and the hemoglobin counts. 3. An initial treatment with metenolone enanthate (Primobolan-Depot) completely prevented the reduction of erythrocytes in appropriate experimental conditions and significantly reduced the decrease of hemoglobins. 4. There was no effect of these steroids on the drop of granulocytes, lymphocytes and platelets. These observations suggest that combination of anabolic steroid therapy with cyclophosphamide therapy may be worthy of clinical trial in patients whose tumors are not subject to growth stimulation by androgenic steroids.

Hormonal therapy of breast cancer with special reference to Masteril therapy.

From 1968 to 1972, a trial of a new agent, drostanolone propionate (Masteril), was conducted. This agent was tested against oophorectomy in a group of premenopausal patients, against nandrolone phenylpropionate (Durabolin) in a perimenopausal group, and against ethinyl oestradiol in a postmenopausal group. In the premenopausal group, 44% responded to Masteril and 22% to oophorectomy. In the perimenopausal group, 34,5% responded to Masteril and 39% to Durabolin. In the postmenopausal group, 57% responded to oestrogens and 38,5% to Masteril. The side-effects of Masteril are generally not serious and rarely contra-indicate its use. Masteril has been shown to be a useful and safe agent for all age groups, even though it may appear to be less effective than oestrogens in the postmenopausal patients.

[Anabolic steroids: damages, effect on performance, and on metabolism (author's transl)].

10 normal persons (age: x=22 years) and 15 weight lifters (age: x=27 years) were studied before and three months after taking Nandrolone danoate (ND). Strength exercises, hear rate during ergometer work, physical working capacity and certain biochemical variables were measured in the experimental group, as well as in a controll group of 7 weight lifters. Six weight lifters in the experimental group who had been taking ND for at least 3 years were also studied to determine whether there were any deleterious effects on their health. In addition, 75 athletes who had been taking anabolic steroids were investigated to determine the possible effects. It was found that: 1. In spite of an 7% increase in performance, the maximal heart rate was not higher when the subjects took ND. During submaximal work loads the heart rate (p less than 0.025) and the blood lactate level (p less than 0.05) were lower. During physical work, there was essentially no effect of ND on glucose or total lipids in serum. 2. The results from the three-months study, from the weight lifters taking ND for 3 years, as well as from 26 of the 57 athletes who had been taking ND showed no evidence of a deleterious effect of ND (based on 26 biochemical measurements). It would appear, therefore, that the general suggestion of a detrimental effect of anabolic hormones is not justified. 3. Damages or functional disturbances were found in 31 athletes and 3 weight lifters of the experimental group after oral application of anabolic alkyl-steroids. After a period of time without alkyl-steroid administration, all investigated biochemical parameters returned to normal levels; thus it appears that the aforementioned pathological findings are reversible changes in liver function.

Nebulized heparin and anabolic steroid in the prevention of postoperative deep vein thrombosis following elective abdominal surgery.

One hundred eighty three patients, all over 40 years old, who underwent major abdominal surgery, were randomized into 3 groups: Group I received a single dose of nebulized heparin (800 IU per kg b.w.) administered by inhalation one day prior to surgery. Group II besides the above, also received a single injection of 50 mg of long acting anabolic steroid (nandrolone phenylpropionate) intramuscularly. Group III received 5000 IU heparin subcutaneously on hr prior to surgery as well as every 12 h for the next 5 postoperative days. Postoperatively the patients were evaluated for deep vein thrombosis (DVT) using the 125-I-fibrinogen test. The occurrence of DVT was determined as: in Group I--16%, in Group II--7.9%, in Group III--7.8%. Haemorrhagic complications (clinically important) were observed in 7.8% of patients from Group III, but only in 1.7% of patients in Group I and 1.6% in Group II. For DVT prophylaxis following abdominal surgery a single application of nebulized heparin and long acting anabolic steroid is as effective as conventional low-dose subcutaneous heparin administration, but gives less haemorrhagic complications. This method is also more advantagenous in term of acceptance by the patients and represents considerable saving of nursing time.

Sudden cardiac death during exercise in a weight lifter using anabolic androgenic steroids: pathological and toxicological findings.

A 21-year-old, previously healthy weight lifter collapsed during a bench press workout. He had taken anabolic androgenic steroids parenterally for the previous several months. Pertinent autopsy findings included marked cardiac and renal hypertrophy and hepatosplenomegaly, with regional myocardial fibrosis and focal myocardial necrosis. Nandrolone (19-nor-testosterone) metabolites were identified in postmortem urine. The possible etiologies of the cardiac findings are discussed.

Anabolic steroid-associated hypogonadism in male hemodialysis patients.

Hypogonadism in male hemodialysis patients has been previously reported. However, its precise pathogenesis has not yet been clarified. Mepitiostane and nandrolone decanoate are anabolic steroids prescribed for uremic anemia, and those may possibly exacerbate uremic gonadal damage. We studied the influences of these steroids on male gonadal function. Seventy-six hemodialysis patients were selected and examined for levels of luteinizing hormone (LH), follicular stimulating hormone (FSH), total testosterone, and prolactin. Twenty-three patients who received anabolic steroids showed lower testosterone values (205.2 +/- 35.6 ng/dl) than did patients without these steroids (449.7 +/- 21.3 ng/dl). Gonadotropins and prolactin showed no significant differences between the patients with and without the steroids. The testosterone values of three patients with mepitiostane increased after they stopped taking steroids. One patient suffering from complete aspermia recovered (sperm count: 0/ml to 1300 x 10(4)/ml) after discontinuation of mepitiostane and administration of human chorionic gonadotropin (HCG). This clinical study suggests that some anabolic steroids play a role in uremic hypogonadism; thus mepitiostane or its analogues should be carefully prescribed for young male patients.

Androgen trial in renal anaemia.

A double blind cross-over trial of Nandrolone decanoate (Decadurabolin) was carried out in 27 patients with anaemia due to end stage renal disease, stabilised on regular haemodialysis. Sixteen patients completed the study, the other patients being excluded from the final analysis for a variety of reasons including side effects related to the androgen. There was no sustained significant rise in haemoglobin concentration or in red cell mass. Erythropoietin levels did not alter, they were within or below the normal range, but were lower than would be expected for the degree of anaemia. A majority of patients reported increased well-being including exercise tolerance, appetite and libido. Voice changes and hirsutism were noted, mainly in the females. Instability of anticoagulant therapy and abnormalities in liver function were found in some patients. The benefits, though real, were restricted essentially to the improvement in subjective findings and were unrelated to laboratory measurements. These effects might be obtained with a lower dosage of the drug.

Effect of topically applied steroidal and nonsteroidal anti-inflammatory agents on skin repair and regeneration.

We studied the effects of topically applied steroidal and nonsteroidal anti-inflammatory agents on dermal and epidermal wound healing. Superficial wounds (0.3 mm deep) on the skin of domestic pigs were treated daily with either 0.1% triamcinolone acetonide (TA), 1% hydrocortisone (HC), 1% nandrolone decanoate (ND), 1% ND + 0.1% TA, 10 mg ibuprofen, 10 mg meclofenamate sodium, 3 mg indomethacin, vehicle (USP petrolatum or 70% ethanol), or control (untreated). Wounds were excised on days 2-7 after wounding and the epidermis was separated from the dermis. The dermis was assayed for collagen biosynthesis and the epidermis was evaluated for reepithelialization. A significant decrease (P less than 0.01) in relative collagen synthesis was observed in the wounded dermis in both HC- and TA-treated groups on day 3 after wounding, but there were no significant differences on days 4-7. Depressed collagen and noncollagenous protein production was also noted in vehicle-treated wounds on day 3. Topical application of ND did not affect collagen synthesis, but when combined with TA it eliminated the inhibitory effect observed as a result of TA alone. Topical ND accelerated wound reepithelialization by 12.5% compared with vehicle and by 26% compared with untreated controls. TA delayed epidermal resurfacing by 22%, but when combined with ND (ND + TA) the rate of reepithelialization was similar to vehicle-treated wounds. HC enhanced resurfacing when compared with untreated wounds but did not differ markedly from its vehicle. The nonsteroidal anti-inflammatory drugs when topically applied markedly reduced inflammation (erythema, heat, and edema) but did not influence the healing process.

Testosterone and synthetic and androgens improve the in vitro survival of human marrow progenitor cells in serum-free suspension cultures.

The direct effect of testosterone and its synthetic analogs on the maintenance of human erythroid progenitor cells (BFU-E) and GM-CFC was studied in suspension cultures. To avoid interference by serum components, the experiments were conducted in serum-free media containing insulin, transferrin, selenium, BSA, and alpha-thioglycerol. When added at concentrations between 10(-7)M and 10(-9)M, testosterone improved survival of BFU-E and GM-CFC. Of the nine analogs tested, eight improved BFU-E maintenance in vitro: nandrolone norethandrolone, and oxymetholone were most active, whereas etiocholanolone had only marginal effects. The influence of these compounds on GM-CFC was less pronounced, with testosterone showing the highest activity. It is concluded that testosterone and some of the synthetic analogs tested exert their hemopoietic effect, at least partly, by affecting the maintenance of erythroid and granulocytic stem cells, directly by increasing their survival or proliferation or indirectly by increasing the input from multipotent stem cell pool, or by both mechanisms.

A stereological analysis of the effect of 19-nortestosterone (Durabolin) on rat skeletal muscle.

It is known that some anabolic steroids are capable of increasing the sizes of skeletal muscles or increasing their mechanical performance. The present study was undertaken to establish quantitatively the alterations which occur in skeletal muscle fibres as a result of the injection of 19-nortestosterone (nandrolone phenyl proprionate). Specimens of diaphragm were obtained from the experimental and control animals and prepared for electron microscopy and stereological analyses. Following low doses of the steroid stereological analyses have shown that in Type I and in intermediate fibres their mitochondrial volume proportions increase though their myofilament contents decrease. With higher doses, stereological analyses have shown that the mitochondrial volume proportions were increased particularly in intermediate fibres whilst the myofilament content of Type I and II fibres was significantly reduced.

Double-blind investigation of two corticoid preparations for the treatment of eczematous skin disorders. Short communication.

In a double blind fully randomised trial, three Belgian dermatologists independently treated 81 patients suffering from various eczematous skin disorders. After four weeks both the dermatologists and their patients assessed the results of a twice-daily topical application of an ointment containing 17 beta-hydroxy-4-estren-3-one-17-decanoate (nandrolone decanoate, Dexatopic) ointment or a reference ointment. Both the dermatologists' and the patients' assessments of the two preparations showed that there was no significant difference between the two treatments although Dexatopic did seem to have a more favourable profile in the "much improved" area. In the total score decrease, there was an indication that Dexatopic had a more favourable effect than the reference preparation.

Anabolic steroid abuse and tooth size-arch dimensions in the rat.

Anabolic steroids are misused by adolescents and adults to increase muscle mass and improve appearance and athletic performance. Since anabolics strongly enhance protein synthesis, it was speculated that alterations in tooth size and arch length could occur. This study quantified the effects of the anabolic steroid nandrolone phenpropionate on these parameters in a rat model. The steroid significantly increased mandibular arch length. No difference in mesiodistal dimensions of the molars occurred. In consequence, the increased arch dimensions combined with unaltered tooth size may result in dental spacing and/or other malocclusions.

Anabolic-androgenic steroid increases running endurance in rats.

The study was designed to determine whether treatment with an anabolic-androgenic steroid enhances running performance in rats by increasing their freely chosen training distance. Forty male Long-Evans rats were randomly divided into either a sedentary control group or an exercising group caged in specially designed running wheels in which the rats were able to run spontaneously. After 4 wk, both groups were further subdivided into two groups receiving either 0.5-mg Durabolin (nandrolone phenylpropionate) (im) or 0.5-mg saline, every second day. After 8 wk, running distance was similar in both exercising groups. Rats receiving the anabolic-androgenic steroid ran 41% longer during the test of submaximal running endurance compared to the trained rats receiving saline (P < 0.05). Submaximal running endurance was not increased in sedentary rats receiving the anabolic-androgenic steroid. After 4 wk of training, the maximal sprinting speed increased by 29% in trained rats. There was no further increase in maximal sprinting speed after an additional 4 wk of training and treatment with either anabolic-androgenic steroid or saline treatment. Therefore, rats that train spontaneously while being treated with an anabolic-androgenic steroid had increased submaximal running endurance compared with trained rats treated with saline, despite the similar voluntary training distance and skeletal muscle oxidative capacity between the two groups. The mechanism by which treatment with an anabolic-androgenic steroid, combined with training, enhances submaximal running performance could not be identified and needs to be addressed in future studies.

Factors affecting erythroid colony growth (CFU-E) from human marrow.

Human marrow grown in a plasma clot gives rise to discrete colonies of nucleated red cells in response to erythropoietin (EPO). In 10 studies of human marrow with normal erythropoiesis, human EPO in doses up to 1 unit/ml produced a significant increase in colonies when compared to sheep EPO. To achieve 50% of maximum colony growth, only 0.1 units/ml of human EPO was needed in contrast to 0.5 units/ml of sheep EPO. In seven other experiments, nandrolone decanoate, 10(-7) to 10(-9) M, yielded a mean increase in colonies of 38% when compared to cultures without added androgen. This effect was entirely dependent on the presence of EPO. We conclude that the method is useful for the invitro study of factors affecting early events in erythropoiesis.

Determination of anabolic steroids by gas chromatography/negative-ion chemical ionization mass spectrometry and gas chromatography/negative-ion chemical ionization tandem mass spectrometry with heptafluorobutyric anhydride derivatization.

A gas chromatography/mass spectrometry (GC/MS) method is described which uses negative ion chemical ionization (NCI) and tandem mass spectrometry (MS/MS) for the determination of eight anabolic steroids in human urine. Eight anabolic steroids were derivatized by heptafluorobutyric anhydride (HFBA), and were determined using GC/NCI-MS and GC/NCI-MS/MS. The linear correlation coefficients for calibration in NCI-MS/MS were in the range 0.9880-0.9988. This method of derivatization with HFBA for use with GC/NCI was useful in determinations of 19-norandrosterone, boldenone, 19-noretiocholanolone, 2-methylandrosterone, nandrolone, 1-methyleneandrosterone, 1-methylandrosterone, 4-dihydroboldenone and mesterolone. The detection limits of this procedure were 5-20 ppb at a signal-to-noise (S/N) ratio of 3.

Critical evaluation of the role of nutritional support with chemotherapy.

Although weight loss has an adverse impact on cancer patient survival, the ability of caloric provision via total parenteral nutrition (TPN) to favorably influence outcome in chemotherapy-treated populations is not established. In randomized trials, no significant improvement in either response or survival was associated with TPN addition to chemotherapeutic treatment of adult patients with lymphoma, sarcoma, colon cancer, adenocarcinoma and small cell carcinoma of the lung, or testicular carcinoma. In two instances, TPN addition was associated with decreased survival, again raising the concern that caloric support in the absence of effective antitumor therapy might stimulate cancer growth. In any event, the hypothesis that nutritional repletion of a malnourished cancer patient receiving chemotherapy will improve clinical outcome remains to be critically tested, as studies demonstrating sequential improvement in lean body mass have not been reported. Most recently, consideration of potential mechanisms underlying the development of cancer cachexia has led to new strategies for nutritional intervention. For example, hypogonadism or low testosterone levels have been described in male patient populations with advanced cancer and correlated with weight loss and adverse outcome, leading to trial of replacement therapy with nandrolone decanoate. Similarly, the frequent identification of abnormal glucose metabolism in the patients with cancer cachexia has stimulated clinical trials with agents such as hydrazine sulfate and insulin designed to reverse the metabolic abnormality. Whether such efforts designed to alter metabolic abnormalities associated with cancer cachexia will improve clinical outcome will be determined in ongoing clinical trials.
