A 46-year-old male sought periodontal care for a swelling on his right mandibular gingiva. An excisional biopsy revealed a well-differentiated squamous cell carcinoma. Surgical treatment consisted of a right segmental mandibulectomy with ipsilateral right neck dissection and fibular free flap reconstruction. Two days after the surgical procedure, a weakened Doppler signal suggested vascular compromise of the graft. The patient was returned to the operating room where complete thrombosis of the internal jugular vein (recipient vessel) was observed. This event prompted a complete hematological evaluation that disclosed low serum levels of protein S. The patient was started on systemic heparin and local medicinal leeches. A week later, systemic warfarin sodium was added and successfully resolved the vascular compromise of the graft. Two years later, the patient is active and lives a full life with occasional adjustments of warfarin sodium. This case represents the first report on the treatment of gingival carcinoma that led to the serendipitous discovery of an unrelated and unusual systemic condition, protein S deficiency.
A 64-year-old Japanese male had a squamous cell carcinoma (T4N1M0) in the left gingival and buccal mucosa, so a radical wide resection involving left radical neck dissection was performed. An anterolateral thigh flap measuring 15 x 8 cm was raised from the left thigh and transferred to the defect. The postoperative course was eventful. There was no postoperative flap necrosis, infection, not even a cervical fistula.
The treatment of oral cancer usually involves extensive resection of the mandible, excision of oro-facial soft tissue and often radiotherapy. This causes difficulty in the subsequent oral rehabilitation. The present paper reports our experience with implant treatment of this patient group. The material comprises 38 implants in 12 patients. Four patients died during follow-up, two before loading the implants. The observation time after implantation was 7 to 47 months with a median of 14 months. Six of the patients had been subjected to preoperative radiation therapy. All 38 implants osseointegrated without complications. It is concluded that healing after implantation in irradiated mandibles can be achieved without hyperbaric oxygen. Provided certain guidelines are complied with i.e. careful surgical technique, use of antibiotics and prolonged period of healing prior to loading of the implants, a radiation dosage of up to 50 Gy appears to be of little significance.

The results of chin augmentation by porous hydroxyapatite blocks were evaluated clinically and radiographically in nine patients. Recovery of a satisfactory facial profile was achieved by the procedure, and all patients but one, with removal of the implant because of infection, were satisfied with the results of the surgery. Radiographically, the implants were incorporated into the bony structure of the mandible by deposition of bone at and around the bone-implant interface and ingrowth of bone into pores. Cephalometrically, the procedure was found to provide very stable results, with little change in the position of the implants and no appreciable resorption of the implants and bone. Despite heavy chemotherapy, histologic examination of the removed hydroxyapatite showed the presence of areas with numerous inflammatory cell infiltration and no bone formation. Thus, the procedure is quite useful for chin augmentation, but utmost care should be taken to avoid infection.
Polydimethylsiloxane (PDMS) is used for augmentation of the chin, malar and nasal bones and is available through three different manufacturers. PDMS has been widely accepted by surgeons for bone augmentation because of its variability in hardness, its ability to be easily molded and shaped, and its biological inertness. Wound repair around PDMS implants is achieved through capsule formation. Fibrous capsule formation can be interrupted by infection, immune reaction, implant migration, or extrusion. Recent studies have shown no correlation between silicone implants and connective tissue disease.
A part of the people who need prosthetic treatment in the maxillofacial area are in the stage of after-surgical operation of tumors. One of the most commonly discovered defects in the mandible is the lateral resection with exarticulation. Our goal is to examine the possibilities of achieving optimal prosthetic restoration of the patients with lateral resection and exarticulation of the mandible through clinical application and combination of the modern technology and supportive-retentioned elements.

Four cases of interpositional TMJ anthroplasty are presented. All patients had been operated on previously. The initial success was compromised by reunion of the proximal and distal surgical sites. These failures were due to inadequate gap arthroplasty; lack of complete coverage of the superior distal stump; or lack of retention of the alloplast at the site of placement. The case of manipulation and good tissue tolerance of methyl methracrylate make it an excellent material for interpositional arthroplasty.

By inserting auxiliary prosthetical means (anchored dental replacement with splinting elements) functional failures can be reduced. By using molding surfaces in the one-piece model cast denture the displaced mandibular stump is repositioned and the occlusal relationship between upper and lower teeth reconstructed largely without periodontal damage to the residual dentition.
