[Dosage of conservative therapy of the mammary carcinoma by means of opposing Co-60 wedge filter fields (author's transl)].

A method of measurement is described which allows to determine the maximum dose (tumor dose) for routine radiotherapy of the breast with opposing Co-60 wedge filter stationary fields. Furthermore it could be demonstrated that--independently of the form of the mamma--a sufficiently precise dosage can also be achieved by means of a simple calculation. For this purpose one must only know the base (= distance of the dorsal field marks on the skin) and the irradiation time for the dose to be applied into the depth dose maximum of a rectangular stationary field without filter.

[Measurement of skin temperature in breast cancer--a new possibility of early diagnosis].

The local skin temperature was measured by infrared thermometry in 100 women suffering from malign neoplasms of the breast. After thermal stimulation the reaction of the skin temperature was determined in a repeated measurement. The results were compared with standard temperature values obtained in 100 healthy women. Statistical evaluation of the data using discriminant analysis resulted in a score, which allowed to classify 90% of the women with mamma carcinomas and 92% of the controls correctly. The possible application of this passive diagnostic method to the early detection of malign tumors in the breast is discussed.

[Bone scintigraphy in the diagnosis and follow up care of malignant head and neck neoplasms].

In diagnosis and localisation of malignant tumors with destruction of the bone, scintigraphy has its special importance in diagnosis as an addition to X-ray tomography. When carcinoma are localized in the middle or dorsal part of the base of the skull scintigraphy may help to calculate the chance for an operation. The exact knowledge of the dimension of the tumor is a valuable help in radiotherapy. In primary diagnosis of destructions of the base of the skull scintigraphy might give an earlier information than X-ray. Malignant tumors of the ear however are sufficiently detected by means of X-rays alone. The main field of scintigraphy is the observation of irradiated and operated malignoma. We recommend 6 or 12 months controls in combination with a clinical observation and with tomographic controls. Scintigraphy became a most valuable tool in the search and diagnosis of metastases in primary malignoma of the mammae, the lungs or the kidneys. In case of metastases in the skull scintigraphy is most helpful to detect them often earlier than by means of X-rays.

Possible factors influencing the immunoglobulin M concentration in swine colostrum.

The immunoglobulin (Ig) M concentration in swine colostrum was determined by the single radial immunodiffusion method, using 157 samples collected from the same number of farm-raised sows in Yamaguchi Prefecture of Japan during 1976 and 1977. The mean IgM value was 4.16 +/- 1.11 mg/ml, and the maximum and minimum values were 7.74 mg/ml and 1.50 mg/ml, respectively. To elucidate the possible factors influencing the IgM concentration in swine colostrum, the following 12 items were surveyed in the present study: season, district, breed, age, udder section (mamma) from which colostral samples were collected, kind of feed, 4 vaccinations (swine erysipelas live-organism vaccine, hog cholera live-virus vaccine, Japanese encephalitis live-virus vaccine, and transmissible gastroenteritis live-virus vaccine), type of farming, and number of sows raised on a farm. Relationships between the IgM concentration in swine colostrum and each item were analyzed. Of the 12 items, 3 items--season, breed, and vaccination of swine erysipelas live-organism vaccine--were the most influential on the IgM concentration in colostra of farm-raised sows. Five items--district, age, kind of feed, vaccination with transmissible gastroenteritis live-virus vaccine, and number of sows raised on a farm--were moderately influential. The remaining items--udder section, vaccinations with hog cholera live-virus vaccine and Japanese encephalitis live-virus vaccine, and type of farming--were poorly influential. The multiple correlation coefficient obtained here was 0.5605 (P greater than 0.05).

[Danazol for treatment of fibrocystic mastopathy].

59 patients with extensive mastopathia, aged between 23 and 58 years, had been examined over a period of approximately 36 months. They were treated with danazol (Winobanin, Sanofi-Winthrop, Munich) at doses between 100 and 800 mg/day because of an extensive and partly persisting mastopathia. The documented period of treatment ranged between 40 and 774 days. According to the chosen medium daily dose the patients were divided into three groups (< 400 mg/d, 400-600 mg/d, > 600 mg/d). These groups were then subdivided according to criteria of symptoms, palpation findings, mammography and sonography. Between the onset and the end of treatment there was a significant improvement of mastopathia up to a complete recovery in more than 60% of the women. There were no significant systematic differences between the various dosages of danazol. In this respect it was impossible to verify assumed relation between dosage and success of treatment according to anamnestic or demographic factors. The objective evaluation by mammography and ultrasound showed an improvement in 81.4% of the patients, demonstrated by a decreased density of parenchyma and an increased subcutaneous fatty tissue of the mammae. The subjective success of therapy of the patients concerned was judged to be good in 75% of the cases by the doctor and in more than 63% by the patient herself. In conclusion, this study confirms that danazol offers very good therapeutic results for the treatment of the frequently persisting benign breast disease.

[Gynecological dispensary and stationary services in the treatment of mammary disorders (author's transl)].

The authors examined 29,230 women. In addition to clinical examination, they were also examined mammographically. The methods used were mass mammography, standard mammography, contact thermovision, xerography, pneumocystoraphy, galactography, exfoliative and aspiration cytology, and pathohistological diagnosis. The diagnosis revealed 964 (2.4%) solid or cystic tumours, 453 mastopathies, 216 secreting mammae, and 215 malignomas. By the use of a large number of gynecological consulting units and dispensaries it is possible to select atypical and suspected changes in the breast, while in large stationary gynecological institutions, by an interdisciplinary collaboration of radiologists, surgeons, and cytologists, it is possible to make a definite diagnosis and provide treatment for patients with atypical changes. The ultimate goal is to refer to therapeutists latent and early forms of malignant mammary disorders.

[Breast blood supply in various diseases as evidenced by color Doppler ultrasonography].

Color doppler sonography, traditional sonography, and mammography without contrast staining, as well as physical and pathomorphologic methods were used in examinations of the mammary glands of 76 women aged 18 to 65. Characteristic features of the blood supply to the gland were detected, and differential diagnostic criteria defined on the basis of these findings, to simplify differentiation between malignant and benign nodular formations and the diffuse changes in the mamma.

[Human dirofilariasis in southern Italy. I. The Puglia Region].

Nine new cases of human Dirofilariasis by Dirofilaria repens, in subjects living in Apulia, aged 30 to 60 years, are reported. Six were subcutaneous and three subconjunctival. Many of them occurred a number of years ago and were identified thanks to a specific joint investigation carried out by a parasitologist, some histopathologists and ophthalmologists. Of special relevance is one subcutaneous case with contemporary presence of 2 nodules in different regions of the body (mamma and forearm) and another subconjuctival case with strong inflammatory reaction (episcleritis, chemosis, orbital cellulitis) associated with the passage of the nematode under the Tenon's capsule.

[An unusual constellation of findings in polymyositis--differential diagnosis and clinical course].

Clinical and biochemical findings are described in a 34-year-old female with atypical polymyositis. Uncommon clinical features in this patient included distally accented decreased muscle strength and myalgias, atypical electromyographic findings, a remarkable discrepancy between clinical findings and laboratory parameters of myolysis, unexplained episodes of somnolism, presence of increased serum lactate levels, and a unilateral mamma aplasia. For this combination of signs a polymyositis or an inclusion body myositis, but also a metabolic or heredodegenerative myopathy was considered. Finally, the idiopathic polymyositis was confirmed histologically and a marked improvement in the clinical and biochemical signs occurred after commencement of high-dose methylprednisolone.

Effects of oestrogenic silage on some clinical and endocrinological parameters in ovariectomized heifers.

The influence of phytoestrogens was studied in 3 ovariectomized Swedish Friesian heifers fed 20 kg of 100% red clover silage per heifer/day for 14 days. Behaviour, reproductive organs and pituitary response to exogenous gonadotropin-releasing hormone (GnRH) injections were monitored. Clinical effects like oedema and mucous discharge in the vulva, presence of milky fluid in the mammae and increases in teat size and the cross-sectional distance of the uterus were observed in heifers fed red clover silage. Fluid accumulation in the uterus, visualized by means of ultrasonography, had still not disappeared 30 days after the red clover silage had been completely withdrawn. Red clover silage appeared to reduce the magnitude and duration of the pituitary response to GnRH injections.

[Malignant cystosarcoma phyllodes: clinico-pathological conference in connection with two cases].

Clinical and morphological characteristics of two malignant cystosarcoma phyllodes mammae were reported with the review of recent literature. One of the patients went to see her doctor 2 years after she had noticed the tumour because of "problems in her family". In the removed giant fibroadenoma osteo-chondrosarcomatous transformation was found. The other patient tried "to mobilise her inner energies against the growth" for 3 years. The removed tumour weighed 2100 grams and there were metastases in the axillary lymph nodes. Histological examination showed malignant cystosarcoma phyllodes with vasoformative tendency. Electron microscopical studies confirmed in both cases that the tumors with varying structures originated from immature mesenchymal cells. Authors agree on wide extirpation of the tumour. According to the literature adjuvant chemo- or radiotherapy do not improve patients survival. In case of early radical operation the malignant transformation is less probable.

[Use of iodamide 300 and iodamide 380 in galactography].

The authors analyze the results of iodamide use for galactography. 434 females with discharge from the nipples were examined making use of galactography, and 136 women were selected for surgical treatment; the local processes in the mamma were previously unpalpated, nor were they confirmed by the laboratory findings. Clinical and morphologic analysis of the preparations resected in surgery has lead the authors to a conclusion that galactograms made with use of iodamide are sufficiently contrast and informative. The authors recommend this agent for the diagnosis of unpalpated mammary tumors.

[Clinical observations on Stewart-Treves syndrome].

Adding to the existing literature three cases of Stewart-Treves-syndrome in a secondary lymphedema of the leg, a postmastectomy lymphedema with progressing mamma-ca and an uncomplicated postmastectomy lymphedema are described and compared with former observation. Thanks to the physical therapy of edema the frequency of Stewart-Treves-syndrome seems to be lowered compared to former statistics.

[Nutrition and cancer (author's transl)].

There is evidence of the epidemiological role of nutrition in the pathogenesis of various forms of human diseases; several components of nutrition are known to have a carcinogenic effect in man. In terms of epidemiology, however, alcoholism is most important which through the development of malnutrition enhances the risk of carcinosis of the upper gastrointestinal tract particularly in smokers. Nutritional deficiences have an influence on the incidence of cancer of the stomach, cervix and thyroid. Overnutrition favours the development of certain forms of cancer such as malignant diseases of the colon, pancreas, kidney, mamma, ovary, endometrium and prostate.

[Internal breast support grids in the surgery for breast hypertrophy and ptosis. Technique and indications].

In breast surgery, the use of a Grade for Internal Support of the Mamma (GISM) is an elegant way to reduce the length of the scars as it becomes possible to conserve only the peri-areolar incision. The technique has been imagine by Bustos in 1985. Its principles are: the reshaping of the gland with a posterior pedicle; the use of a silicone sheet that supports the gland; the spontaneous adaptation of the skin over the reshaped breast. A new sheet (GISM) is presented allowing a modification of the Bustos's technique that becomes easier and shorter. Technique and indications are described, and results are analysed.

Biodistribution of 111In-pentetreotide and dosimetric considerations with respect to somatostatin receptor expressing tumor burden.

The general features of the biodistribution of the labeled somatostatin analog 111In-pentetreotide are already known. We describe some details of 111In-pentetreotide accumulation in the thyroid gland, mammae, spleen and gastrointestinal tract with respect to endocrine parameters, kinetics and time of imaging. In addition, dose estimations were performed for liver, spleen and kidney in patients without neuroendocrine tumor load and in patients with large tumors positive for somatostatin receptors. The overall absorbed dose turned out to be within the range reported previously. However, in patients with extensive tumor burden the radiation dose in liver, spleen and kidney tended to be lower than in patients without such malignancy. The dependence of estimated organ doses on the presence of somatostatin receptor-expressing tumors will have to be considered if radiotherapy with suitable labeled somatostatin analogs becomes available.

[Suction-radiography in mammography (author's transl)].

The value of the suction apparatus used in over 500 examinations as a simple addition to mammography has been convincing in our hands. Suction of 30 mm Hg, produced with a test-rubber bulb, achieves distension or stretching of the mamma. This leads to changes of position, shape and density of normal and pathologic structures and gives further information, compared with routine mammography. It is often possible to distinguish between spaces filled with fluid (cysts, hematomata) and solid tumours, pseudotumours and genuine growths, and between benign and malignant tumours. One essential indication is the "small" breast of various types (constitutional, post-operative and in men), because often suction will enable us to do a mammography. Suction radiographs are added in every 10th patient to previously taken compression radiographs with, of course, large and coned radiographs, which will yield a safe diagnosis in one half of the cases or confirm suspected findings.

[Wobenzyme and diuretic therapy in lymphedema after breast operation].

The authors of this clinical study report the results of a controlled clinical trial in randomized parallel groups (Wobenzym vs. diuretics) of 55 female patients suffering from brachial arm lymph edema subsequent to ablatio mammae. All patients received manual and machine lymph drainage as well as gymnastics as concomitant therapy. After 7 weeks of therapy the results of the volometric assessments of the arm, the circumference of the arm and the skinfold thickness showed significant improvements compared to diuretics. In addition, the patients receiving Wobenzym reported a significantly higher proportion of patients free of pain compared to the diuretics patients. Overall safety assessment results are satisfactory thus resulting in a superior benefit/risk relation of the Wobenzym group.
