In search of meaning: identifying competencies relevant to evaluation of the community health nurse generalist.

The purpose of this study was to identify the competencies that faculty look for in evaluating the performance of baccalaureate community health nursing students. The approach to the research was qualitative and utilized an inductive analysis methodology. Eight faculty members provided data on how they judged the clinical performance of 92 students in a semester-long community health nursing course. Thirty-five competencies emerged from the data which were grouped into two domains of performance evaluation: socialization to the community health nurse role and community health nurse clinical practice.

Infectious disease and transplantation: messages for the generalist.

Today, more patients than ever are receiving organ or bone marrow transplants and are surviving longer afterward. Because these patients often live far from the transplant center, their local primary care physicians may be called on to evaluate problems as they arise, although all significant issues should be handled in conjunction with the transplant team. This paper reviews the primary care physician's role in the pretransplantation evaluation, and in coordination of long-term care, as well as illustrative cases.

Strategies for training generalists and subspecialists.

A strategy is proposed for attracting trainees to careers in general internal medicine coupled with a program that attracts a smaller number of trainees to research careers without attracting trainees into careers in the practice of the subspecialties. A novel training track analogous to the Medical Scientist Training Program, the Specialty Training and Academic Research (STAR) Program has been established. It is anticipated that within the next 5 years, 80% of the trainees in the department of medicine at the University of California, Los Angeles, Medical Center will be training for practice in general internal medicine; the remaining 20% will be enrolled in the STAR Program.

The essential role of generalists in health care systems.

Viewing primary care as merely one of the many niches in individual patient care fosters a narrow perspective that neither accounts for the full value of primary care nor offers proper guidance for reforming it. Instead, a systems view better articulates primary care's essential role in increasing the effectiveness and efficiency of other health care services. Primary care improves health care system functioning through such services as managing and triaging undifferentiated symptoms, matching patient needs to health care resources, and enhancing the system's ability to adapt to new circumstances. By acting as an appropriate filter for high-technology care, primary care helps ensure that it is appropriately applied, a major determinant of outcomes. Furthermore, because primary care is the most financially and geographically accessible arm of the health care system, it reduces socioeconomic and geographic disparities across the population. Even in the de facto health system that exists in the United States, these functions are necessary for the efficient, equitable, and sustainable delivery of health care and the adaptability and resilience of the system as a whole. As primary care reforms proceed, it will be important to explicitly consider how to support and strengthen its system-level benefits. Needed reforms include enabling universal access to primary care and providing the leadership, training, and infrastructure support to improve its performance as a central hub in the network of patients, clinicians, and communities. It is important that health systems realize the essential integrative function of primary care.
