Electrophysiological correlates of pathology and surgical results in temporal lobe epilepsy.

Routine pre-operative EEG studies as well as direct brain recording and stimulation carried out during operations were analysed for 59 patients subjected to a standard unilateral anterior temporal lobectomy for the treatment of epilepsy. All patients in the present series were 16 years old or older at the time of operation, which was invariably carried out under local scalp analgesia only. Electrophysiological findings was correlated with pathological changes noted in the resected temporal lobes, and with the effects of surgery upon seizure activity. Pre-operative EEG data correlated with each of four pathological categories when sphenoidal electrodes and intravenous barbiturate narcosis were emplyed. Thirty of 31 patients with mesial temporal sclerosis demonstrated medial temporal primary spike foci, frequently with independent contralateral and extratemporal secondary foci. In addition, one-third of these patients demonstrated unilateral focal decreased barbiturate-induced fast activity in the corresponding sphenoidal to ear channels. Twelve patients with other specific medial focal lesions (mostly hamartomas) also had medial temporal primary foci, often with independent contralateral secondaries but never with extratemporal foci. Two patients in this group also demonstrated focal decreased fast activity in the appropriate sphenoidal-ear channel. Both of these groups did very well post-operatively with respect to their epilepsy. Five patients with large temporal convexity cicatrices antedating seizures all demonstrated lateral temporal primary spike foci without independent secondary foci or focal decreased fast activity and did not do as well post-operatively as the first two groups. Eleven patients had only non-specific changes in the resected temporal lobe and in general did not benefit from surgery. Various combinations of primary and independent secondary spike foci were seen. Only this group demonstrated diffuse or bifrontal spikes during initial EEG recording, and basal mid-line spikes with intravenous thiopentone. Pecilar sharp notched spike were also very common in this group, but not unique to it. Focal decreases in barbiturate-induced fast activity were not noted.

Insect eggs exert rapid control over an oxygen-water tradeoff.

In terrestrial environments, the exchange of respiratory gases exacts a water cost: obtaining oxygen or carbon dioxide requires losing water. Insect eggs should be especially sensitive to this tradeoff-because they are unable to forage for water, have high surface area-to-volume ratios, and experience large temperature-driven changes in oxygen demand. Previous work from our laboratory, on eggs of a common hawk-moth, Manduca sexta, has shown that, during development, metabolic rate and water loss rates rise in parallel. These correlative data suggest that eggshell conductance increases to accommodate increasing metabolic demand. Here, we test this idea experimentally by subjecting eggs of M. sexta to 15, 21 (normoxia) and 35% oxygen for 24h, while measuring rates of metabolism (as carbon dioxide emission) and water loss. Hypoxia depressed egg metabolic rates, but led to pronounced, rapid increases in water loss. By contrast, hyperoxia had no significant effect on metabolism or water loss. These data demonstrate that insect eggs actively participate in balancing oxygen gain and water loss, and that they use tissue oxygen status, or some correlate of it, as a cue for increasing eggshell conductance. Rapid control over conductance may allow eggs to conserve water during an initial period of low metabolic demand, thereby deferring water costs of respiratory gas exchange until late in development.

Narcissism and object relations.

Questionnaire measures of the narcissistic personality disorder can predict healthy and unhealthy self-functioning. That this outcome might support Heinz Kohut's psychoanalytic psychology of the self was tested in a sample of 354 undergraduates. In canonical correlations, factors from the Narcissistic Personality Inventory were associated more strongly with grandiose than with idealizing immaturities in Kohut's bipolar self, while difficulties in interpersonal relationships (i.e., poor object relations) were associated more strongly with idealizing deficits. Zero-order and partial correlational data were congruent with Kohut's hypothesis that self-grandiosity can include elements of both "pathology" and relative mental health, but canonical correlations did not support Kohut's claim that narcissism can be described in a bidimensional self-structure.

The validity of cardiac output measurement by thoracic impedance: a meta-analysis.

A structured review of existing literature was undertaken to provide an objective evaluation of the validity of cardiac output measurement by thoracic impedance. Relevant studies were obtained from journals, advertising literature, and N.A.S.A. contract publications, using MEDLINE retrieval and references from other studies. Studies describing concurrent measurement of impedance cardiac output with other techniques were selected if they met certain technical and methodological criteria known to improve the validity of impedance cardiac output measurement. All studies provided Pearson's correlation coefficient as a measure of association. A weighted mean coefficient was calculated for selected studies using Fisher's Z transformation of the data. Correlation of impedance with thermodilution gave r = 0.82; with dye dilution, r = 0.83; with Fick technique, r = 0.80; and with radionuclear angiography, r = 0.65. Studies done on ICU patients showed a lower correlation than studies done on non-ICU patients (p = 0.003). A moderately good correlation exists between impedance cardiac output measurement and other techniques, although correlation is not so good when ICU patients are studied. Better techniques and methodology may improve this correlation.

Right ventricular involvement in transmural inferior wall infarction: two-dimensional echocardiographic and clinical correlations.

We used two-dimensional echocardiography to assess asynergy in the right (RV) and left (LV) ventricles in 34 selected patients with electrocardiographic evidence of acute transmural inferior myocardial infarction (IMI) 40 +/- 3 (SE) h from the onset of pain. We measured the extent of asynergy, defined as akinesis and/or dyskinesis, for RV and LV in 4 short axis sections and computed asynergy for both ventricles. The incidence of asynergy was 100% in the inferior LV, 76% in the inferior ventricular septum and 76% in the inferior RV. We found a direct correlation between peak creatine phosphokinase levels and LV asynergy (R = 0.71) or (RV + LV) asynergy (R = 0.72). The asynergy and right heart catheterization data correlated with clinical findings in 4 subgroups identified on the basis of hypotension (systolic blood pressure less than 100 mmHg) and pulmonary congestion, (confirmed radiographically): 1A, extensive biventricular asynergy (32% RV, 32% LV) with hypotension and congestion (N = 10); 1B, predominant RV asynergy (49% RV, 26% LV) with hypotension but no congestion (N = 5); 2A, predominant LV asynergy (7% RV, 26% LV) with congestion (N = 10); 2B less extensive biventricular asynergy (15% RV; 16% LV) and uncomplicated IMI (N = 9). Hypotension identified a high risk group (N = 15) prone to complications and death. The extent of RV asynergy was the third strongest discriminator for hypotension, next to systolic blood pressure and jugular venous pressure. The extent of LV asynergy was the strongest discriminator for pulmonary congestion. Among hypotensive patients, the RV/LV asynergy ratio was the strongest discriminator for pulmonary congestion, being significantly greater in those without congestion than in those with congestion (2.2 vs 1.0, P less than 0.001). The increased RV/LV asynergy ratio may be a useful index for predicting predominant RV infarction in IMI.

[Enamel resistance to acid dissolution and its correlation with dental caries].

Enamel resistance to acid dissolution is a factor which has an influence upon dental caries susceptibility. The objectives of this study were to determine enamel resistance to acid dissolution by applying the RM technique, and to correlate data obtained to the prevalence of dental caries. Two hundred and seventy one children between seven and nine years of age were chosen by non probabilistic sampling in two city districts, (six public schools in Mexico City). These children's central permanent incisives had already erupted. The DMF-T and dmf-t indexes were recorded, and the RM enamel resistance test was performed on them. A total of 56.4% of the subjects in the sample had very resistant enamel and 27.3%, less resistant enamel. A proportion of 57.9% was free of dental caries on the permanent dentition and 10% in the temporary dentition. The average obtained for the DMF-T index was 0.93 +/- 1.34 and that for dmf-t was 4.7 +/- 3.1. Data suggest that enamel resistance distribution is not homogeneous and this increases proportionally in relation to the eruption third (p < 0.05.) Spearman's correlation coefficient was found to be negative and statistically significant at p < 0.05. The RM technique showed the presence of individuals with different enamel resistance to acid dissolution.

Hippocampal perfusion and pituitary-adrenal axis in Alzheimer's disease.

The hippocampus is involved in Alzheimer's disease (AD) and regulates the hypothalamus-pituitary-adrenal axis (HPAA). Enhanced cortisol secretion has been reported in AD. Increased cortisol levels affect hippocampal neuron survival and potentiate beta-amyloid toxicity. Conversely, dehydroepiandrosterone (DHEA) and its sulfate (DHEAS) are believed to antagonize noxious glucocorticoid effects and exert a neuroprotective activity. The present study was aimed at investigating possible correlations between hippocampus perfusion - evaluated by SPECT - and HPAA function in AD. Fourteen patients with AD and 12 healthy age-matched controls were studied by (99m)Tc-HMPAO high-resolution brain SPECT. Plasma adrenocorticotropin, cortisol, and DHEAS levels were determined at 2.00, 8.00, 14.00, 20.00 h in all subjects and their mean values were computed. Cortisol/DHEAS ratios (C/Dr) were also calculated. Bilateral impairment of SPECT hippocampal perfusion was observed in AD patients as compared to controls. Mean cortisol levels were significantly increased and DHEAS titers were lowered in patients with AD, as compared with controls. C/Dr was also significantly higher in patients. Using a stepwise procedure for dependent SPECT variables, the variance of hippocampal perfusional data was accounted for by mean basal DHEAS levels. Moreover, hippocampal SPECT data correlated directly with mean DHEAS levels, and inversely with C/Dr. These data show a relationship between hippocampal perfusion and HPAA function in AD. Decreased DHEAS, rather than enhanced cortisol levels, appears to be correlated with changes of hippocampal perfusion in dementia.

Assessing student performance on a pediatric clerkship.

OBJECTIVE: To determine whether any of 4 parameters used as evaluation methods in the pediatric clerkship at Children's National Medical Center, Washington, DC, could predict a student's performance, as measured by the final grade. DESIGN: A retrospective study in which the following data were collected: (1) a record of the diagnoses and total number of patients seen during the rotation, (2) clinical performance grade, (3) the National Board of Medical Examiners (NBME) pediatric shelf test score, (4) case presentation grade, and (5) the final clerkship grade for overall performance. SETTING: Third-year pediatric clerkship in the 1987-1988 academic year at Children's National Medical Center, the George Washington University School of Medicine. RESULTS: A total of 128 students had complete data. Correlations among the clinical parameters, ie, number of patients seen, clinical grade, case grade, and the NBME shelf test score were all statistically nonsignificant. In addition, multiple regression modeling of NBME test scores, using measures such as clinical grade, average case grade, and number of patients seen, was unsuccessful, with only clinical performance entering the regression model. In contrast, modeling of the final clerkship grade resulted in explaining almost 80% of the variation on a student's final grade (R2 = 0.79). Variables submitted to the model were number of patients seen, clinical grade, average case presentation grade, and ordered examination score. Last, a highly significant relationship (chi 2 = 15.98, P < .001) was noted between students receiving a final grade of honors and an "A" on their case grade. CONCLUSIONS: Results confirmed that there is no single best predictor of performance on a pediatric clerkship. While there was only a weak correlation between each single measure and the final grade, together these accounted for 80% of the variation in students' scores. Based on the study data, the use of varied evaluative methods to determine a student's final grade is recommended.

Ratio scales of sensory and affective verbal pain descriptors.

The results of two experiments show that ratio scales of sensory and affective verbal pain descriptors are valid, reliable and objective. In the first experiment, 16 subjects rated 15 sensory and 15 affective verbal pain descriptors by numerical magnitude estimation and by cross-modality matching to handgrip force. Ratio scales of sensory and affective verbal pain descriptors computed for two separate groups were highly correlated between the groups (sensory, r = 0.97; affective, r = 0.98), as well as over session (r = 0.99, 0.98). Scales based on an individual's data correlated equally with either another set of scales from the same individual (r = 0.96, 0.98) or a mean scale from a similar group (r = 0.96, 0.89). Sensory and affective verbal descriptor scales from the first experiment correlated highly (r = 0.99, 0.99) with those from the second experiment in which 40 subjects rated verbal pain descriptors by cross-modality matching to time duration and to handgrip force. The ratio responses to the verbal descriptors in both experiments demonstrated specific functional relationships found for measurable psychophysical stimuli. This result supports the validity of cross-modality matched ratio scales of verbal stimuli. The reliability of these scales is shown by the high between-session, between-group and between-experiment correlations. The objectivity is shown by the similarity of within-subject and between-subject correlations for both group and individual descriptor scales.

Measurement of myocardial infarct size by technetium pyrophosphate single-photon tomography.

The primary determinant of prognosis after acute myocardial infarction (AMI) is the size of the acute infarct. The present study evaluates 46 patients with different infarct distributions and sizes to test the hypothesis that single photon emission computed tomography with technetium-99m pyrophosphate (Tc-99m-PPi) and blood pool overlay allows measurements of AMI size that provide insight into prognosis irrespective of infarct location. Identical Tc-99m-PPi and ungated blood pool projections were acquired over 180 degrees with a rotating gamma camera. Reconstructed sections were color-coded and superimposed for purposes of infarct localization. Areas of increased pyrophosphate uptake within myocardial infarcts were thresholded at 65% of peak activity. The blood pool was thresholded at 50% and subtracted so as to determine an endocardial border for the left ventricle. Using this method, myocardial infarcts weighed 2.5 to 81.2 g. The correlation of infarct mass with prognosis showed that patients without previous AMI and with acute infarcts that weighed more than 40 g had an increased frequency of death and congestive heart failure (p less than 0.001). The correlation of measured infarct mass with peak serum creatine kinase level was significant (r = 0.83, p less than 0.001; y = 0.015x + 13.20). The correlation coefficients for anterior, inferior and nontransmural AMI were not significantly different from those for the entire group. In conclusion, tomographically determined infarct mass data correlate with subsequent clinical prognosis, and Tc-99m-PPi tomography with blood pool overlay is a safe and effective means of sizing infarcts in patients with AMI.

What importance do women attribute to the size of the penis?

OBJECTIVE: A great deal of men consider that the size of the penis is directly proportional to its sexual power. Some men, who are worried that their penis is too small, wish to be considered for surgical lengthening or thickening procedures. The argumentation for this chiefly points in the direction of women. However, have women actually been asked about the extent to which they consider the size of the penis to be of importance from a sexual point of view? Or asked what they think about ideas surrounding the size of the penis in relation with actual sexual functioning? PATIENTS AND METHODS: To address these questions, 375 sexually active women who had recently given birth at the University Hospital Groningen were asked a number of questions about sexual functioning and the importance they attach to the size of their partner's penis. RESULTS: A total of 170 questionnaires were returned (response rate 45%); 20% of the women found the length of the penis important and 1% very important; 55% and 22% of the women found the length of the penis unimportant and totally unimportant, respectively. Opinions about the girth of the penis followed the same trend. Length was less important than girth: 21% and 32%, respectively. The women who found the girth of the penis important had the same opinion about the length of the penis (correlation 0.71, p=-0.001). Median division into two subgroups (girth important/unimportant; t-test) did not reveal any significant differences in relation with demographic data. Correlation analysis did not reveal any significant correlation between sexual functioning (measured with the NSF) and opinions about the girth of the penis. CONCLUSION: Although clearly in the minority, a nevertheless considerable percentage of the women respondents attached substantial importance to the size of the male sexual organ.

Immunotoxicogenomics: the potential of genomics technology in the immunotoxicity risk assessment process.

Evaluation of xenobiotic-induced changes in gene expression as a method to identify and classify potential toxicants is being pursued by industry and regulatory agencies worldwide. A workshop was held at the Research Triangle Park campus of the Environmental Protection Agency to discuss the current state-of-the-science of "immunotoxicogenomics" and to explore the potential role of genomics techniques for immunotoxicity testing. The genesis of the workshop was the current lack of widely accepted triggering criteria for Tier 1 immunotoxicity testing in the context of routine toxicity testing data, the realization that traditional screening methods would require an inordinate number of animals and are inadequate to handle the number of chemicals that may need to be screened (e.g., high production volume compounds) and the absence of an organized effort to address the state-of-the-science of toxicogenomics in the identification of immunotoxic compounds. The major focus of the meeting was on the theoretical and practical utility of genomics techniques to (1) replace or supplement current immunotoxicity screening procedures, (2) provide insight into potential modes or mechanisms of action, and (3) provide data suitable for immunotoxicity hazard identification or risk assessment. The latter goal is of considerable interest to a variety of stakeholders as a means to reduce animal use and to decrease the cost of conducting and interpreting standard toxicity tests. A number of data gaps were identified that included a lack of dose response and kinetic data for known immunotoxic compounds and a general lack of data correlating genomic alterations to functional changes observed in vivo. Participants concluded that a genomics approach to screen chemicals for immunotoxic potential or to generate data useful to risk assessors holds promise but that routine use of these methods is years in the future. However, recent progress in molecular immunology has made mode and mechanism of action studies much more practical. Furthermore, a variety of published immunotoxicity studies suggest that microarray analysis is already a practical means to explore pathway-level changes that lead to altered immune function. To help move the science of immunotoxicogenomics forward, a partnership of industry, academia, and government was suggested to address data gaps, validation, quality assurance, and protocol development.

Formulation and evaluation of sustained release paracetamol tablets.

Slow-releasing paracetamol tablet formulations were produced and their in vitro dissolution profiles and in vivo absorption characteristics were determined and compared with a conventional rapidly-disintegrating commercial formulation. A correlation between saliva and plasma levels of the drug was confirmed and this allowed estimation of some simple pharmacokinetic parameters from the saliva paracetamol data. The in vitro data correlated with the in vivo parameters and this enabled the dissolution test results to be used in predictive manner in the development of a compression-coated slow release formulation which produced therapeutic levels of the drug for up to 5 h.

Serum cytokine levels in heart allograft recipients: correlation with findings on endomyocardial biopsy.

The ability to detect rejection of human cardiac allografts depends on endomyocardial biopsy diagnosis. Because cytokines are known to mediate allograft rejection events, we chose to examine serum levels of specific cytokines and receptors (interleukin-2 [IL-2], IL-2 receptor [IL-2R], and tumor necrosis factor alpha [TNF-alpha]) and to correlate those levels with findings on endomyocardial biopsy. Sequential sera samples from 23 cardiac allograft recipients were examined for the cytokine levels mentioned, and data correlated with findings on endomyocardial biopsy. Briefly, no statistically correlation of serum cytokine or receptor levels with the stage of allograft rejection was found. When sequential serum cytokine levels were determined in patients experiencing humoral and cellular allograft rejection events, the levels of TNF-alpha appeared to correlate well with endomyocardial biopsy findings. IL-2 and IL-2R levels in two patients who never experienced rejection were elevated on occasion, but TNF-alpha levels were always negative. In summary, measurement of serum cytokine (IL-2, IL-2R) levels in cardiac allograft recipients does not appear to correlate with findings on endomyocardial biopsy; however, elevated levels of TNF-alpha appear to predict more severe humoral allograft rejection episodes and may be helpful in this regard.

Application of semilinear canonical correlation to the measurement of opioid drug effect.

To examine the relationship between the electroencephalograph (EEG) and plasma opioid concentration, one would like to collapse the high-dimensional EEG signal into a univariate quantity. Such a simplification of the EEG is desirable because a univariate quantity can be modeled using standard nonlinear regression techniques, and because most of the information in the EEG is redundant or unrelated to drug concentration. In previous studies of the EEG response to opioids, the manner in which a univariate component was extracted from the EEG was ad hoc. In this paper, this extraction was performed optimally using a new statistical technique, semilinear canonical correlation. Data from 15 patients who received an intravenous infusion of the semisynthetic opioid alfentanil were analyzed. The components of the EEG that were nearly maximally correlated with plasma drug concentration were found, based on a standard pharmacokinetic-pharmacodynamic model. Two new EEG components were produced from the powers in the frequency spectrum of the EEG: a weighted sum of the logarithms of the powers, and a weighted sum of the powers expressed as percentages of the total power. These components both had a median R2 of 0.84, compared to median R2s ranging from 0.37 to 0.83 for five commonly used ad hoc EEG components. The new components also had less variability in R2 between subjects.

Generalized atrophic benign epidermolysis bullosa: a case of severe hemidesmosomal deficiency.

A case of generalized atrophic benign epidermolysis bullosa was followed from birth to 14 years of age. Electron microscopy of the blistered skin showed either the absence or greatly reduced hemidesmosomes of a very poorly developed type. In perilesional skin, the frequency of hemidesmosomes was 1/3 to 1/2 of the normal controls. These hemidesmosomes were small and lacked normal ultrastructures. Anchoring filaments did not cluster underneath them, and the anchoring fibrils were also diminished. In view of the published data correlating the genotype to the phenotype, this patient seems to have a homozygous nonsense mutation of the COL17A1 gene encoding collagen XVII. An accumulation of data may eventually enable electron microscopists to estimate the types of gene mutation.

A critical role for CCR2/MCP-1 interactions in the development of idiopathic pneumonia syndrome after allogeneic bone marrow transplantation.

Idiopathic pneumonia syndrome (IPS) is a major complication after allogeneic bone marrow transplantation (allo-BMT) and involves the infiltration of donor leukocytes and the secretion of inflammatory cytokines. We hypothesized that leukocyte recruitment during IPS is dependent in part upon interactions between chemokine receptor 2 (CCR2) and its primary ligand monocyte chemoattractant protein-1 (MCP-1). To test this hypothesis, IPS was induced in a lethally irradiated parent --> F1 mouse BMT model. Compared with syngeneic controls, pulmonary expression of MCP-1 and CCR2 mRNA was significantly increased after allo-BMT. Transplantation of CCR2-deficient (CCR2-/-) donor cells resulted in a significant reduction in IPS severity compared with transplantation of wild-type (CCR2+/+) cells and in reduced bronchoalveolar lavage (BAL) fluid cellularity and BAL fluid levels of tumor necrosis factor-alpha (TNF-alpha) and soluble p55 TNF receptor (sTNFRI). In addition, neutralization of MCP-1 resulted in significantly decreased lung injury compared with control-treated allogeneic recipients. Experimental data correlated with preliminary clinical findings; patients with IPS have elevated levels of MCP-1 in the BAL fluid at the time of diagnosis. Collectively, these data demonstrate that CCR2/MCP-1 interactions significantly contribute to the development of experimental IPS and suggest that interventions blocking these receptor-ligand interactions may represent novel strategies to prevent or treat this lethal complication after allo-BMT.

Functional connectivity during working memory maintenance.

Neurophysiological experiments with monkeys have demonstrated that working memory (WM) is associated with persistent neural activity in multiple brain regions, such as the prefrontal cortex (PFC), the parietal cortex, and posterior unimodal association areas. WM maintenance is believed to require the coordination of these brain regions, which do not function in isolation but, rather, interact to maintain visual percepts that are no longer present in the environment. However, single-unit physiology studies and traditional univariate analyses of functional brain imaging data cannot evaluate interactions between distant brain regions, and so evidence of regional integration during WM maintenance is largely indirect. In this study, we utilized a recently developed multivariate analysis method that allows us to explore functional connectivity between brain regions during the distinct stages of a delayed face recognition task. To characterize the neural network mediating the on-line maintenance of faces, the fusiform face area (FFA) was defined as a seed and was then used to generate whole-brain correlation maps. A random effects analysis of the correlation data revealed a network of brain regions exhibiting significant correlations with the FFA seed during the WM delay period. This maintenance network included the dorsolateral and ventrolateral PFC, the premotor cortex, the intraparietal sulcus, the caudate nucleus, the thalamus, the hippocampus, and occipitotemporal regions. These findings support the notion that the coordinated functional interaction between nodes of a widely distributed network underlies the active maintenance of a perceptual representation.
