[How to manage biliary injuries caused by Dormia catherers?].

The authors report the complications of intraoperative biliary exploration, particulary for the utilisation of basket Dormia. If the biliary injury, in general, are an problem for the surgeon, the especific injury for the basket Dormia are underestimate. The authors makes, for theirs observations, an review of the literature and an clasification anatomical-clinic are propose for help therapy repair.

Current management of hilar cholangiocarcinoma.

The goal in the treatment of hilar cholangiocarcinoma is the relief of biliary obstruction with the quality survival. The choice of therapy for an individual patient requires complete preoperative staging to determine the resectability. This is best accomplished in a multidisciplinary setting with radiologists, gastroenterologists and surgeons participating in the diagnostic and therapeutic strategies. Operative removal of the tumor can be performed with low mortality and, when possible, provides the longest palliation and offers potential for cure. For tumors found at operation to be unresectable, biliary enteric bypass provides a durable method of palliation. The relative merits of the newer methods of radiologic stenting are encouraging, but further study will require prospective trials in comparison to operative biliary enteric bypass.

Adenoma, Bile Duct↗

[A new approach to the intraoperative diagnosis of cholangitis and to the choice of surgical procedure in operations on the bile ducts].

The authors have developed a prompt and informative method for assessment of the depth and reversibility of the morphofunctional changes in the common bile duct (CBD) wall with the use of staining by hematoxylin basic, fuchsin-picric acid. Four types of the morphologic changes in CBD wall, which characterize the different degree of reversibility, were established. Depending of presence of these types, the corresponding surgical tactics is recommended.

[The optimal drainage of the bile ducts and subhepatic recess in the surgical treatment of complicated cholecystitis].

On the basis of the analysis of the results of surgical treatment of 478 patients with complicated cholecystitis, it was established that the Robson-Vishnevsky method (95 cases) was the optimal one for drainage of the bile ducts after diagnostic (19) or therapeutic (115) choledochotomy, and that of Halsted-Pikovsky (9)--in manipulations through the cystic duct. Drainage of the subhepatic space by means of a rubber tube drain is effective. The methods for active decompression and transdrain therapeutic measures used in presence of purulent-inflammatory process in the bile ducts, or subhepatic space are presented.

[Meta-analysis on curative effects of surgical procedures for intrahepatic bile duct lithiasis].

OBJECTIVE: To compare curative effects of various surgical procedures of bile duct stones. METHODS: Two thousand nine hundred and fifty-five patients with intrahepatic bile duct lithiasis who had undergone various surgical procedures were analysed with Meta-analysis. Some of these cases were reported in Chinese Medical Journals from January 1990 to March 2001 and others were from Tongji Hospital. RESULTS: There was a significant difference between curative effects of non-hepatectomy and that of hepatectomy (chi(2) = 62.945, P < 0.01), and the outcomes of hepatectomy were much better than those of non-hepatectomy with OR(S) equalled to 0.303 (0.222 - 0.413). There was not a significant difference between curative effect of interposed jejunum and that of hepatectomy (95% CI of RR from 0.98 to 1.04). All the other operation, effects were worse than hepatectomy (upper limit of 95% CI of RR < 1). CONCLUSIONS: Hepatectomy is the most ideal surgery for intrahepatic bile duct stones and operation methods should be diversified since good effect could also be obtained when other operations are performed on suitable cases.

Bile Duct Diseases↗

[Repeated surgeries of the bile ducts].

The results of examination and operative treatment, of 74 patients with the postcholecystectomic syndrome, who underwent 81 restorative and reconstructive operation 1 month to 21 year after cholecystectomy, have been summarized. The postoperative lethality was 8%. Peritonitis and hepatic failure were the main causes of death. Necessity of early performance of a reoperation has been substantiated.
