The influence of tooth wear on root canal dimensions.

The objective was to investigate the relations between tooth wear and root canal dimensions. The experimental group comprised 100 cuspids (canines), 50 of which had advanced abrasion and 50 were without it The dimensions of root canal were investigated at the light microscope level in order to define their vestibulo-oral and mesio-distal diameters in all three thirds. In the groups of teeth without abrasion the MD diameters of root canaliculi in their cervical thirds were greater than in the teeth with advanced abrasion. In the apical third the MD diameters of canaliculi were highly variable in teeth with no signs of abrasion. In the group of teeth with abrasion the MD and VO diameter values of the cervical thirds were significantly smaller than in the group of teeth without abrasion. In the mid-thirds the values of MD diameters were highly variable in the group of teeth with abrasion, whereas the VO diameter values were much greater than in the teeth without abrasion. As regards to the apical thirds, the VO diameter values were greater than in the control teeth. The data presented herewith contribute to better knowledge about tooth wear and are highly useful in restorative and reconstructive dental procedures.

Maxillary canine impactions: orthodontic and surgical management.

With the exception of the maxillary and mandibular third molars, the maxillary canine is the most commonly encountered impaction. In North America, palatally located impactions appear to predominate over labially impacted canines and occur more frequently in women. Because of the common nature of this phenomenon, the general dentist should know how to properly diagnose and manage potential disturbances in the eruption of the maxillary canine. If diagnosed early, and when clinically and radiographically indicated, extraction of the deciduous canine may help correct the eruption path of the permanent tooth and prevent impaction. This article reviews the etiology, prevention, and surgical and orthodontic management of impacted cuspids. Because the general dentist is often the first dental care provider to come in contact with patients with impacted canines, knowledge about this common dental anomaly is essential to provide proper comprehensive therapy.

[The predication equation of the permanent canine and premolar crown].

OBJECTIVE: It is important to predict mesiodistal crown diameter of the permanent canines and premolars in the mixed dentition, which helps to determine malocclusion and its further development. But there is still no article about this equation of Chinese used to predict mesiodistal crown diameter of cuspids and premolars. This research is to measure mesiodistal crown diameters, to study relationship between lower incisors and canines and premolars, and to provide the most accurate and easy basis in orthodontic diagnosis and treatment for Chinese people. METHODS: 140 Chinese students (60 male, 80 female) who were from eighteen to twenty-five years old without orthodontic history Chengdu were randomly collected in. All teeth must be free of visible malform, aberration, and fully erupted. Impressions were taken, and measurements were made, parallel to the occlusal plane at the greatest mesiodistal width of each tooth on dental cast with electronic caliper (0.01 mm). Each tooth was measured twice and the average of the two readings was taken as the dimension for that individual tooth. Mesiodistal crown diameters were statistically analyzed with t-test and linear regression, using the SAS (Statistic Analyses System) computer package. RESULTS: It was found that mesiodistal crown diameters had significantly statistical difference in gender. Hence the mesiodistal crown diameters were assessed and investigated separately for the male and the female. It was also found that the total mesiodistal crown diameter of lower incisors was closely correlated with the total mesiodistal crown diameter of canines and premolars. The correlation coefficients between the independent variable and dependent variables were 0.599 for upper canines and premolars and 0.654 for lower canines and premolars in the males and 0.586 and 0.635, respectively, in the females. The P were found to be less than 0.001 for both arches. The correlation coefficients had significantly statistical difference. CONCLUSION: The equations predicting overall mesiodistal crown diameters of upper and lower unerupted canines and premolars for male and female Chinese people were established respectively: Maxillary (y = 10.87 + 0.51 x (male)), y = 11.88 + 0.46 x (female), Mandibular (y = 10.36 + 0.50 x (male), y = 10.03 + 0.49 x (female)), y (total mesiodistal crown diameter of canines and premolars), and x (total mesiodistal crown diameter of lower incisors).

[Prevalence of impacted permanent upper canine and its treatment in 11-18-year-old orthodontic patients].

Data of literature show very high prevalence of impacted upper permanent canine. The aim of the present study was to evaluate prevalence of retentioned or impacted upper permanent canines and to present their therapy. Data on 11-18-year-old children presented for treatment at the Department of Dentistry for Children and Orthodontics in Budapest over a period of 10 years (between 1990 and 2000) were analysed. OP X-ray films of 1858 children and adolescents--1102 female and 756 male--were examined and the data of anamnesis and the administered therapy were considered. Results revealed 101 cases of impacted cuspids, the male:female ratio was 46:55. Considering the totality of administered treatment methods, both surgical exposure and orthodontic treatment of the affected canines were needed in a high rate, i.e. 96.92% of the analysed cases. In 40.91% of the cases extraction of the correspondent upper first premolar was needed. Following the premolar extraction spontaneous occlusions of the impacted canine was established in 3.03%. Authors present the complex therapy of one case of palatally impacted upper permanent canine.

Delayed retraction utilizing ceramic brackets.

The purpose of this study was to evaluate clinically the delay in retraction utilizing ceramic orthodontic brackets versus stainless steel brackets. Twelve patients with Class II division 1 malocclusions requiring the extraction of maxillary first premolars were employed in the study. The following observations were obtained: 1. Retraction of all cuspids bonded with ceramic brackets displayed delayed retraction when compared to their stainless steel counterparts. 2. Over a 36 week period, teeth bonded with stainless steel brackets travelled 2mm farther or 1.8 times faster than those bonded with ceramic brackets.

Fiber-reinforced composite bridge and splint. Replacing congenitally missing teeth.

The congenital absence of permanent teeth, also known as partial anodontia, is a relatively common problem. The patient missing one or both lateral incisors is a frequent occurrence. The treatment of missing laterals involves both functional and esthetic considerations. Options include: cuspid lateralization; implant placement; conventional three-unit bridge; Maryland bridge; and a fiber-reinforced composite bridge (direct or indirect). Direct fiber-reinforced composite bridges provide a minimally invasive restorative option that allows for future implant placement. Selection of appropriate fiber reinforcement and placement of the fibers allows long-term clinical success. Adherence to appropriate adhesive protocol is mandatory.

Effectiveness of preparation guidelines for an all-ceramic restorative system.

PURPOSE: To evaluate, in a clinical field-test, the implementation of manufacturer's preparation guidelines for the all-ceramic CICERO system. METHODS: General dental practitioners from the northwest region of The Netherlands were asked to make complete crown preparations in accordance with the specific guidelines of the CICERO system. 3,446 tooth preparations were evaluated with regard to shoulder angle, shoulder width and top angle. They were quantified using a special software program. The results were compared with criteria defined in the manufacturer's preparation guidelines. RESULTS: On a multivariate level all (main and interaction) effects were significant (P < 0.05) excluding the interaction effect of the location of measurement on the tooth by the upper or lower jaw. The value of the shoulder angle showed a strong relation with the tooth position in the mouth as well as with the location of measurement on the tooth. The shoulder width in the lower jaw was significantly smaller when compared to the width in the upper jaw. The shoulder width of the lower incisors was the smallest and also showed the largest variance per tooth. On a group level (incisor, cuspid, premolar, molar), except for the shoulder width of the lower incisors, the average values of all preparation parameters were within the borders as defined in the preparation guidelines of the manufacturer. However, on an individual tooth level nearly all preparations showed to have one or more locations with imperfections.

[Craniofacial characteristics of the Class II division 1 malocclusion patients with mouth-breating: a posteroanterior].

OBJECTIVE: To evaluate the craniofacial characteristics of the Class II malocclusion patients with mouth-breating by posteroanterior cephalometry. METHODS: To measure craniofacial width of the 12 Class II malocclusion patients with mouth-breathing, and to compared these measures with corresponding measures in a group of normal children. RESULTS: The width of the maxillary base bone (J-J) was less than that in normal children significantly (P < 0.01). The mouth-breathing children's upper and lower arch width (at first molar and cuspid) were comparatively narrower, and lateronasal width (Lap-Lap) was narrower too. CONCLUSION: Mouth breathing may lead to craniofacial morphological abnormal development in craniofacial transverse structures.

The prevalence of the retrocuspid papilla among three indigenous groups of patients. Study of the subjects in Ecuador, Honduras and Nicaragua.

The retrocuspid papilla (RCP) is a soft well circumscribed nodule located lingually to the mandibular cuspid on the gingival tissue surface. The RCP is observed more frequently in young children and regresses or disappears with age. Little information is available in the literature on this structure. The prevalence of the RCP is discussed as it relates to its distribution among three groups of Latin American patients: those of southern Ecuador, northeastern Honduras and eastern Nicaragua. The distribution by age, sex, symmetry and gingival position is evaluated. Based on the analysis of the results, it was determined that: the RCP is most prevalent in under-five age group at a relative frequency of 25 per cent; its prevalence decreases progressively with age; it is present significantly more often among females in the Honduras group; a unilateral distribution is more frequent than a bilateral distribution, except for the Nicaragua group, where a bilateral distribution prevails; and it is most frequently located on the attached gingiva as compared to free gingiva and mucosa. The clinical significance of the RCP resides in the fact that it may simulate pathological gingival conditions from which it must be differentiated.

[Traction resistance of cast metal posts with and without retention, cemented with zinc phosphate cement in smooth and rough canals].

The purpose of this research was to test the traction resistance of cast posts and cores with and without roughness cemented with zinc phosphate cement in cuspids roots, with smooth and roughness canals walls. From the results of the tests and statistical analysis there was significant difference between roughness posts cemented in root canals with roughness walls and smooth posts cemented in smooth root canals. It was also analyzed, the retention of cast crows cemented on roughness posts and canals and it was concluded that posts are responsible for crows retention, when the cores were done at the correct way.

[Pulp and dentin responses to topical dexamethasone application].

The Authors have proposed to develop this research employing a preparation containing exclusively a cortisonic, the desametazone, in order to evaluate the alterations of the dentinogenesis to be attributed to such component. 32 cavities have been carefully drilled in dogs' cuspids. The 16 experimental cavities were dressed with desametazone, asbestos and amalgam in successive strata while in the 16 control cavities only asbestos and amalgam were used. Stainless steel bands were cemented on the processed teeth. The animals received 9 I.V. injections of tetracycline (10 mg per kg) each at 7 days interval. In the observation of the tetracycline markings under the experimental cavities (with desametazone) alterations of the dentinogenesis were not observed. Consequently, it seems that the action of desametazone is not to be considered harmful for the pulp tissue under the freshly cut dentine, as far as duration of the experiment is concerned.

Interceptive approach to tooth eruption abnormalities: 10 year follow-up of a case.

A child affected by lack of eruption of the upper right primary cuspid from the presence of a compound odontoma was followed from 3 to 13 years of age. The early removal of the obstacle to eruption and the correct control of the case evolution allowed for a satisfactory alignment of the permanent teeth by means of minimal therapeutical intervention.

Effect of canine guidance of maxillary occlusal splint on level of activation of masticatory muscles.

The effect of canine guidance of a full-arch maxillary flat occlusal splint on the level of activation of the anterior and posterior, temporal, masseter and suprahyoid muscles during maximal clenching, were studied in 14 subjects without craniomandibular disorders. The results revealed that, the level of electromyographic activity of anterior and posterior temporal and suprahyoid muscles during maximal clenching on the occlusal splint in habitual closure was unchanged, as compared to biting in the intercuspal position (ICP), while the activity in the masseter muscle, on average, was increased slightly (13 percent). In contrast, the level of activation of the jaw elevator muscles decreased significantly during maximal clenching on the cuspid ramp of the splint, as compared to the biting in ICP or clenching on the splint. However, the degree of reduction of activity was not symmetrical, and was most pronounced in the masseter muscle of the biting side and in the anterior and posterior temporal muscles of the non-biting side. No significant difference was observed in the activity of the suprahyoid muscles.
