Long-term results of surgery for the treatment of sustentacular fractures of the calcaneus.

The authors reviewed 55 sustentacular fractures of the calcaneus treated between 1979 and 1988. Our therapeutic objective was that of reconstructing the subtalar joint. This purpose was always achieved in a short amount of time; nonetheless, there were some cases of secondary unreduction in time (13% poor results in 4.5 years). Essex-Lopresti closed surgery was used to treat fractures with limited damage of the joint surface; open osteosynthesis with associated grafting was used to treat fractures with more than one fragment.

Osteolysis of the calcaneus in a child with localized scleroderma.

In a young girl with localized scleroderma a circumscribed area of lysis developed in the calcaneus beneath the involved skin and soft tissues of the foot. A biopsy revealed vascular changes characteristic of scleroderma associated with infarction and severe resorption of the bone. A geographic pattern of bone destruction in a child with localized scleroderma has not previously been reported.

Results of operative treatment for intra-articular fractures of the calcaneus.

Since 1980 our treatment of displaced intra-articular calcaneal fractures consists of open reduction with distractor, bone grafting, and internal fixation. Thereby no splints are applied and early postoperative movement is possible. For precise preoperative planning a CT scan is required. Proper timing and careful preoperative planning are essential to prevent soft-tissue complications. In a total of 16 fractures good clinical and radiologic results were achieved in 50% of the cases; results were satisfactory in 25% and poor in 25%. The patients returned to work after an average of 5 months. Only three patients (19%) received disability compensations, which compares favorably with nonoperative treatment. We believe that a majority of comminuted intra-articular fractures of the calcaneus profit from open reduction and internal fixation and that more reliable functional results are achieved than with conservative therapy.

[Anti-calcaneus carbon fiber orthosis in children with myelomeningocele].

An anti-calcaneus carbon fibre orthosis (ACCFO) is an appliance combining a plantar orthosis with an anterior counter-support at the tibia and two side supports, medial and lateral, leaving the posterior aspect of the leg and heel free. It is made of polyethylene and polycarbolactone resin reinforced by a covering of tubular carbon fibres and polymerized epoxy-resin. Fifty-five pairs have been used in 18 children aged from one years and five months to 12 years suffering from a paraplegia at L4-L5 or S1 level due to myelomeningocele. Two children were unable to walk before the use of the ACCFO and 16 walked with various aids. All the children were followed-up with regular muscle and joint assessments and video recordings at their respective day-care hospitals. The orthoses were made by the same supplier. Correction of the calcaneo-valgus was improved in all the cases. Balance in standing was improved in 15 cases, including three children who became able to stand without other aids. This orthosis is well adapted to children suffering from spina bifida. It is well accepted because of its lightness and good skin tolerance and was preferred to earlier appliances in all cases.

[Dermatoplasty of a cavity in the calcaneus].

A total of 12 patients with chronic traumatologic and hematogenic osteomyelitis of calcaneus were subjected to radical removal of the pathological focus, followed by cutaneo-dermofascial plasty of the residual cavity. The analysis of this complex treatment showed that such a method allowed to yield hopeful results. Till the age of 7 rather safe supporting surface with heel function preservation was being formed in the heel area. Under mechanical load skin grafts didn't turn into the scar tissue and were not ulcerated. Favourable outcomes of the offered method of plasty indicate the possibility of its wide use both in surgical and traumatological practice.

The ilizarov apparatus in the treatment of thalamic fractures of the calcaneus.

The authors present a new method for the treatment of severe thalamic fractures of the calcaneus by the Ilizarov apparatus. The versatility of this apparatus makes it possible to restore the anatomy of the area, in particular, the total height of the talo-calcaneal complex and correction of varus or valgus deviation while leaving the ankle joint free. This method was used to treat 10 cases of severe thalamic fracture. If the apparatus is maintained for at least 3 months there is no subsequent loss of correction and the results are very good.

Intra-articular fractures of the calcaneus: a review.

Intra-articular fractures of the calcaneus can result in prolonged or permanent disability. The systematic approach to analysis based on anatomy, mechanism of injury, and x-ray films allows determination of the various elements of the lesion. Management is difficult because treatment is not standardized, and, even when good reduction has been achieved, long-term complications are commonplace. The authors review different types of intra-articular calcaneal fractures, various methods of treatment, and examples of possible complications.

[Computed tomography in the evaluation of recent articular fractures of the calcaneus].

After mentioning the results of a survey carried out on 28 patients suffering from recent calcaneus articular fractures, the authors point out 9 main points to take into account when a TDM analysis of these lesions is to be done: they recommend to practice a double obliquity incidence that seems to be an efficient guide for a therapeutic decision. They insist on the usefulness of the TDM examination provided it is undertaken after the operation and the removal of the osteosynthesis equipment.

An unusual case of cystic chondroblastoma of the calcaneus: a case report.

A rare case of chondroblastoma of the calcaneus with a significant cystic component is presented. The difference in cure rate and clinical behavior between this lesion and others that would be considered in the differential diagnosis makes awareness of its existence and an accurate histologic diagnosis essential.

[X-ray computed tomography in the study of fractures of the calcaneus].

In a series of 28 calcaneal fractures, computed tomography scanning has shown axial deformities in the coronal and horizontal (plantar) planes not previously recognized. Two-thirds of the fractures investigated showed a deformity in at least two planes. A combination of dorsiflexion, valgus and adduction was present in one-third of the cases. This examination allowed a precise measure to be made of the enlargement of the calcaneus which very frequently occurs and was greater than 25 per cent in two-thirds of the cases. It makes it possible to visualize subtalar and calcaneocuboid incongruence with great precision and, in particular, to determine the size of the incongruous surface and its severity. Computed tomography scanning showed the existence of subtalar incongruity without depression of the thalamus, which is a concept which is not well recognized. It shows incongruity which is not evident on normal radiographs or, conversely, which may be falsely interpreted from deceptive pictures. The results of this study indicate that the usual classifications based on radiological examination are inadequate and need to be revised. The accuracy of this examination leads to the hope of more precise indications for treatment in the future.

The results of tenodesis of the tendo achillis to the fibula for paralytic pes calcaneus.

Sixty-six consecutive tenodeses of the tendo achillis to the fibula were done in sixty patients who had paralytic pes calcaneus. The patients were followed for an average of 5.7 years (range, two to 10.8 years). The preoperative disturbance of gait was eliminated in all of the patients, and radiographic improvement was noted in the feet that had been operated on. Sixteen feet (23 per cent) required revision of the tenodesis because equinus deformity had developed. The development of equinus deformity was found to occur more often in patients who had the procedure at a younger age and in patients in whom the calcaneotibial angle measured more than 70 degrees at the time of the tenodesis. Residual cavovarus deformity was successfully treated by a plantar release.

[Clinicopathological studies of fractures of the calcaneus with special reference to findings of CT and prescale].

A comparative study was undertaken in 116 cases of fracture of the calcaneus using prescale footprints, clinical and radiological findings. According to the CT findings, fractures can be classified into four types: two part, three part, four part and crush fracture. Based on the CT images taken at the time of injury, prognosis for two part fractures was good, whereas the prognosis for crush fractures was poor. There was a significant correlation between the lateral protrusion rate (PR) as seen on the CT images and the clinical results. In fact, in those cases where the lateral protrusion rate was over 30%, clinical results were poor. With regards to footprints, in cases with poor clinical results, weight applied on the forefoot was decreased. Significant correlation was also noted between clinical results and sole pressure distribution. Finally, tendovaginitis of the peroneal tendons caused by a widening of the calcaneal body no less than incongruity of the subtalar joint was a factor of pain induction.

The talar facets of the calcaneus--an anatomical note.

The number and extent of articular facets for the talus on the superior surface of the calcaneus were studied in 272 Indian specimens. These included 66 pairs of male, 39 pairs of female, 29 unpaired male, 4 unpaired female and 29 unsexed bones. The results indicated that there were only 2 major types of calcanei, as far as the number of these facets were concerned: type A (3 facets, 35%) and type B (2 facets, 65%). There was no single case of type C (1 facet) of BUNNING and BARNETT (1965). The frequency of incidence has no association with sex and side.

Treatment of displaced intra-articular fractures of the calcaneus using medial and lateral approaches, internal fixation, and early motion.

Twenty-two displaced intra-articular fractures of the calcaneus (in twenty-two patients) were treated by open reduction and internal fixation with a screw and one or two staples using a lateral and, when needed, a medial approach, followed by early motion. Despite the early motion, a nearly normal anatomical reduction was achieved and maintained in nineteen feet. After follow-up that ranged in duration from sixteen to seventy-two months, the clinical and radiographic result was rated as good in seventeen patients and fair in one, and there were four failures. Twenty-one of the twenty-two patients returned to work within six months. The average subtalar motion at follow-up was 75 per cent of normal.

Fractures of the calcaneus.

Fractures of the calcaneus account for approximately 60 per cent of all tarsal injuries and 1 to 2 per cent of all diagnosed fractures. The length of treatment and rehabilitation and disability has a marked effect on the working population. This helps to explain the great disparity in the classification and treatment of calcaneal fractures.
