Spirits and spiritist therapy in southern Brazil: a case study of an innovative, syncretic healing group.

This paper examines the treatment of patients by a group of Spiritist healers in southern Brazil. After describing and analyzing a healing session, the practices are shown to be deviant from conventional Spiritism in two directions: (1) they employ a technique, called apometry, that they claim makes possible the transportation of a part of the patient's body to the astral world where it is treated by disincarnate doctors who do past life regressions; and (2) although a conventional Spiritist disobsession is performed, the healers invoke rival Afro-Brazilian spirits who often are shown to have caused the patient's symptoms. Building on the work of Csordas (1983), I hypothesize that the discourse employed by the healers moves the patient to a new reality or phenomenological world in which s/he is healed "not in the sense of being restored to the state in which s/he existed prior to the onset of illness, but in the sense of being rhetorically 'moved' into a state dissimilar from both pre-illness and illness reality...(Csordas 1983:346)." The new state, in this case, is the world of Spiritism. Unlike the Catholic Pentecostals Csordas studied who already were members of a primary group of believers, however, the patients treated by the Brazilian healers are mostly unaffiliated individuals who face the increasing uncertainty and insecurity of life in disorganized, anomic, urban Brazil. By encompassing modern science on the one hand, and aspects of the Afro-Brazilian traditions on the other, this healing group appeals to the often distraught white middle and lower-middle classes, providing them with therapeutic meaning that in many cases leads to healing, conversion, and the sense of security and safety that often accompanies identifying with and belonging to a religious group.

Characteristics of hospital-treated schizophrenia in São Paulo, Brazil.

This paper reports a study on the prevalence of hospital-treated schizophrenia in a large urban centre in Brazil. A sample of 124 individuals from a defined catchment area of São Paulo consecutively admitted to psychiatric hospitals due to acute episodes of non-affective psychoses were assessed by standardized instruments for mental state and social adjustment. The sample was predominantly white (72.6%), single (65.2%), Catholic (61.3%) and not born in that city (58.9%). Eighty-six subjects fulfilled DSM-III-R criteria for schizophrenia (69.3%), 15 were classified as schizophreniform psychosis (12.1%), and 7 as schizoaffective (5.6%). Almost two-thirds showed Schneider's 'first rank' symptoms. Social adjustment before admission was poor or very poor in nearly half the sample. Mean age at onset was 3.2 years earlier in men than in women (P = 0.007). No gender differences in mental state or social adjustment were observed. Individuals born outside São Paulo State had significantly more florid symptoms than those born in that state. These findings suggest that the features of schizophrenia in a large urban centre of Brazil are consistent with the patterns of schizophrenia described in Western developed countries.

Analysis of pathogenicity by restriction-endonuclease digestion of amplified genomic DNA of Entamoeba histolytica isolated in Pernambuco, Brazil.

The pathogenicity of 47 strains of Entamoeba histolytica isolated in Pernambuco, Brazil, was examined using the polymerase chain reaction (PCR) followed by restriction-endonuclease digestion. Electrophoretic patterns of PCR products digested with HinfI revealed that all strains were nonpathogenic. The results were entirely in accord with phenotypic properties such as isoenzyme patterns and the failure to bind a pathogenic-isolate-specific monoclonal antibody. When the sensitivity of PCR was examined, amplified products could be detected from template DNA equivalent to five trophozoites. These observations indicate that PCR amplification of genomic DNA and subsequent restriction-enzyme digestion is a useful strategy for obtaining a sensitive and accurate diagnosis. The present study also demonstrates that nonpathogenic strains of E. histolytica predominate in northeastern Brazil.

Mycoflora of the toxic feeds associated with equine leukoencephalomalacia (ELEM) outbreaks in Brazil.

The mycoflora of 39 feed samples associated with 29 Equine Leukoencephalomalacia (ELEM) outbreaks was studied from 1988 to 1990, in Brazil. Microbiological examination indicated Fusarium spp. as the most frequent mold which occurred in 97.4% of samples followed by Penicillium spp. in 61.5% and Aspergillus spp. in 35.9%. The moisture content of feed implicated in death of horses was above 15% which can favor the development of Fusarium spp. From the genus, F. moniliforme was the predominant species with an occurrence of 82.0%. Two additional species, not commonly associated with animal toxicosis, were isolated in low frequency, F. proliferatum (12.8%) and F. subglutinans (2.6%). It is important to emphasize that the isolation of F. proliferatum and F. subglutinans from feed obtained from the epizootic areas has not been documented previously in Brazil.

Characterization of human rotavirus genotype P[8]G5 from Brazil by probe-hybridization and sequence.

We report the molecular characterization of rotavirus genotype P[8]G5 strains found in fecal specimens collected in four different regions of Brazil, using digoxigenin(dig)-labeled oligonucleotide probes, sequence analysis, and RNA-RNA hybridization. The closest sequence relationships of the neutralization antigens of these strains were to the VP4 protein of P1A[8]G1 strain KU (93.3% identity in amino acids 11 to 282) and to the VP7 protein of G serotype 5 strain OSU (87.6% identity in amino acids 8 to 232). Based on VP7 sequence differences, we designed dig-probes that allowed us to discriminate porcine OSU-like strains from G5 strains isolated from Brazilian infants. The genetic relationships of two P[8]G5 isolates to other rotavirus genogroups were analyzed by RNA-RNA hybridization with [32P]-GTP probes representative of serotypes P1A[8]G1 (Wa), P[8]G3 (AU17), and P9[7]G5 (OSU). The Brazilian P[8]G5 strains showed sequence homology with genes of Wa-like and OSU-like strains, suggesting that these two strains were naturally occurring reassortants between members of the Wa and porcine rotavirus genogroups. The identification of these strains in diverse geographic areas of Brazil underscores their stability and demonstrates the emergence of clinically important rotavirus diarrhea strains by reassortment.

Current status of myocarditis and endomyocardial biopsy in Brazil.

The results presented here show the inadequate attention that has been paid to myocarditis as a clinical entity and to employing endomyocardial biopsy examination as a valuable tool for diagnostic and research purposes. The results must be interpreted with caution, as otherwise false conclusions may be drawn as far as the epidemiological profile of cardiovascular diseases in Brazil is concerned. The data are likely to be influenced by the striking regional differences in development which are reflected in the scientific output. The high incidence of positive serology in patients in endemic areas of Trypanosoma cruzi infection is a further complication. It is possible that Chagas' disease may be overestimated clinically, decreasing therefore the interest in clarifying diseases with comparable clinical features. Myocarditis (excluding a protozoal etiology) and dilated (congestive) cardiomyopathy may serve as examples. Considering these various points, together with the complex and multi-disciplinary requirements of obtaining and interpreting catheter biopsies of the heart explains the limited clinical application of this technique in Brazil.

Abscesses and caseous lymphadenitis in goats in tropical semi-arid north-east Brazil.

This study reports the incidence of abscesses in relation to Corynebacterium pseudotuberculosis infection in goats in north-east Brazil. Among 656 goats examined periodically over almost two years 41.6% were found with superficially palpable abscesses. Most of the infected areas were in the anterior half of the body in front of the pre-scapular region; the inguinal region formed the other prominent area. Among 486 dead or slaughtered goats, 56 (11.5%) revealed abscesses in the internal organs. Lungs and epididymis were affected most, liver, spleen, udder and lateropharyngeal nodes were next in order and kidneys and mediastinal nodes were least affected. Thus the overall incidence of external and internal abscesses was 28.8%; of all the abscesses 27.7% were due to C. pseudotuberculosis and the rest due to infections of Corynebacterium pyogenes and several species of streptococci or staphylococci. The growth of C. pseudotuberculosis-infected and non-infected goats was of the same order and it seems that caseous lymphadenitis is not the cause of much financial loss in north-east Brazil but in view of its high incidence remedial measures should be considered necessary in regions where the goat meat industry is likely to become more intensive and important.

Volvulus of the sigmoid colon in Brazil: a report of 230 cases.

Etiologic and physiopathologic aspects of volvulus of the sigmoid colon in Brazil are presented. It is believed that sigmoidal volvulus in Brazil is a frequent complication of megacolon caused by Chagas' disease, differing in some characteristics from volvulus found in other countries. A review of 230 cases treated between 1938 and 1974 in the Surgical Department of Hospital das Clinicas, University of Sao Paulo School of Medicine, is presented. The successive variations used to treat this disease occurred parallel to those introduced in the surgical treatment of uncomplicated megacolon. From the results, the following treatment is recommended: endoscopic emptying in cases without clinical, roentgenographic or endoscopic signs of intestinal ischemia. Laparotomy should be performed when a complicated volvulus is suspected or when it is not possible to empty the loop. When a simple volvulus is found, the loop should be untwisted and the gaseous contents siphoned off by menas of a rectal catheter. When there is necrosis of the colon, the Hartmann operation is recommended. It is important to submit patients to a definitive treatment of the megacolon soon after endoscopic emptying or surgical detorsion of the volvulus, since recurrences following these measures are frequent.

Environmental control in the uranium mine Lagoa Real, Brazil.

Uranium production in Brazil will be restarted in the year 2000, through exploration of the Jazida da Cachoeira, located in the uraniferous region of Lagoa Real, Bahia State, northeast Brazil. Because of the mining operations, an open cast mine of approximately 27 x 10(4) m2 will be produced and the material removed will constitute approximately 14 x 10(6) tons, occupying an estimated area of 50 x 10(4) m2. Currently, there is a new concept about project developing, where the impact assessment is addressed during the initial phases. Beside this, legislation requires an environmental impact assessment before starting mining activities. In this context purpose, it is the purpose of this work to evaluate the chemical composition of the future waste; thus, samples were collected from the rocks that surround the mineral deposits. Instrumental neutron activation analysis was used for the elemental quantification, the mean elemental concentrations were calculated, and the ratios were normalized using North American Shale Composite (NASC). In the raw material eight main mineral structures exist whose elementary composition were compared using the statistical techniques of analysis of variance, ranking ANOVA, and multivariate ANOVA.

Post-partum thyroiditis in South Brazil presenting as thyrotoxicosis: prevalence and risk factors.

The prevalence of post-partum thyroiditis (PPT) has been reported in several countries (1.9 to 16.7%) but is not known in Brazil. Several factors have been associated to its development, such as a female sex of the newborn, PPT in a previous pregnancy, a family history of thyroid disease and cigarette smoking. To investigate the prevalence of PPT and its risk factors in a southern Brazilian city, a three-cross-sectional observation study was performed. PPT was diagnosed in 14/284 subjects (5.3%) and all cases had thyrotoxicosis (13 sub-clinical and one clinical). Serum total T4 and free T4 were higher and serum TSH was lower in PPT subjects. Anti-thyroid antibodies were positive in 16.7% of PPT subjects and in 4.5% of those with no thyroid dysfunction. Goiter was identified in 14.3% of PPT subjects and in 15% of no PPT subjects. Thyroid was hardened more frequently in PPT subjects (21.4%) than in others (5.2%). Male sex of the newborn was associated to PPT, increasing 11 times the risk of PPT. Cigarette smoking was associated to PPT in group II subjects. There was no clinical sign or symptom able to contribute to this diagnosis, except the presence of hardened thyroid. Based on these findings, PPT, manifesting itself as mild thyrotoxicosis, is a common problem in southern Brazil and is associated to male sex of the newborn.

Short stature and gestational diabetes in Brazil. Brazilian Gestational Diabetes Study Group.

AIMS/HYPOTHESIS: To examine the association between maternal stature and gestational diabetes mellitus. METHODS: We studied a sample of 5564 consecutive Brazilian women 20 or more years old, who were pregnant for approximately 21-28 weeks, had no history of diabetes outside pregnancy and were attending general prenatal care units in six state capitals in Brazil from 1991 to 1995. We did a 2-h, 75-g oral glucose tolerance test, defining gestational diabetes by World Health Organisation criteria. RESULTS: Those in the shortest quartile of height (< or = 151 cm) had a 60% increase in the odds of having gestational diabetes, independently of prenatal clinic, age, global obesity, family history of diabetes, skin colour, referral pattern, waist circumference, parity, previous gestational diabetes, education, ambient temperature and gestational age compared with the tallest quartile [odds ratio (OR) = 1.60, p = 0.005]. This association was observed for those with above median values of skinfold thickness (OR = 1.74, p = 0.006) but not for those with below median values (OR = 1.22, p = 0.51). Associations of short stature with high 2-h glycaemia (> or = 7.8 mmol/l) (OR = 1.61, p = 0.005) were essentially the same as those for gestational diabetes. There was, however, no association between short stature and gestational hyperglycaemia when the latter was defined exclusively by fasting values (OR = 0.97, p = 0.90). CONCLUSION/INTERPRETATION: In Brazil short stature associates with gestational diabetes, principally in women with greater fat mass. This difference in glycaemic levels is present postprandially but not in the fasting state.

The quality of life of people with schizophrenia living in community in Rio de Janeiro, Brazil.

INTRODUCTION: The influence of sociodemographics and clinical factors on the quality of life (qol) of an individual with schizophrenia is being widely studied, particularly in developed countries. The de-institutionalization process that is taking course in Brazil, to a less symptoms-focused approach, makes necessary the assessment of the qol of these patients living in the community. OBJECTIVES: To evaluate factors related to the qol in patients diagnosed with schizophrenia or schizo-affective disorder, and who are users of one of the two municipal community mental health services of a Rio de Janeiro district, Brazil. MATERIAL AND METHODS: A cross-sectional study was carried out. Selected patients were interviewed with Brazilian versions of Lancashire Quality of Life Profile, Brief Psychiatric Rating Scale and Calgary Depression Scale for Schizophrenia. RESULTS: A total of 136 patients participated in the study. Most of them were single and lived with their relatives, had low educational level, were unemployed and had low income. A great number of the interviewed did not have any social contact outside the family and leisure activity, except watching television and/or listening to the radio. Finances and work situation had the lowest satisfaction level. Educational level, presence of positive and depressive symptoms, hospital admissions were negatively associated with qol. For those receiving state benefit, perceived qol increased with age. These predictors explained more than 42% of the total subjective qol score variance.

Oil biodegradation by Bacillus strains isolated from the rock of an oil reservoir located in a deep-water production basin in Brazil.

Sixteen spore forming Gram-positive bacteria were isolated from the rock of an oil reservoir located in a deep-water production basin in Brazil. These strains were identified as belonging to the genus Bacillus using classical biochemical techniques and API 50CH kits, and their identity was confirmed by sequencing of part of the 16S rRNA gene. All strains were tested for oil degradation ability in microplates using Arabian Light and Marlin oils and only seven strains showed positive results in both kinds of oils. They were also able to grow in the presence of carbazole, n-hexadecane and polyalphaolefin (PAO), but not in toluene, as the only carbon sources. The production of key enzymes involved with aromatic hydrocarbons biodegradation process by Bacillus strains (catechol 1,2-dioxygenase and catechol 2,3-dioxygenase) was verified spectrophotometrically by detection of cis,cis-muconic acid and 2-hydroxymuconic semialdehyde, and results indicated that the ortho ring cleavage pathway is preferential. Furthermore, polymerase chain reaction (PCR) products were obtained when the DNA of seven Bacillus strains were screened for the presence of catabolic genes encoding alkane monooxygenase, catechol 1,2-dioxygenase, and/or catechol 2,3-dioxygenase. This is the first study on Bacillus strains isolated from an oil reservoir in Brazil.

Brazil's Samuel Dam: lessons for hydroelectric development policy and the environment in Amazonia.

Brazil's Samuel Dam, which formed a 540-km2 reservoir in the state of Rondônia in 1988, provides lessons for development decisions throughout Amazonia and in other tropical areas. The decision to build the dam was heavily influenced by its role in the political strategies of key decision makers. Samuel illustrates both impacts and benefits of electricity supply and the dilemmas facing decision makers regarding the various options for planned electricity generation. Environmental costs included flooding forest and stimulating illegal logging activity throughout western Amazonia because of an exception opened for Samuel in Brazil's prohibition of export of raw logs. Samuel emitted substantially more greenhouse gases than would have been emitted by generating the same amount of electricity from oil. Contamination of fish in the reservoir resulted from methylation of mercury present in the soil. Social costs of the dam included resettlement of 238 families of farmers; impacts on indigenous people were indirect. Mitigating measures included faunal rescue and creation of a forest reserve. The lessons of Samuel include the need to consider a full range of alternatives prior to making decisions in practice and the importance of adhering to the logical sequence of decision making, where information is gathered and compared prior to the decision. It also shows the need to maintain flexibility when the costs and benefits of different alternatives change significantly over the course of the project's planning and execution, as occurred at Samuel.

BR-319: Brazil's Manaus-Porto Velho highway and the potential impact of linking the arc of deforestation to central amazonia.

Brazil's BR-319 Highway linked Manaus, in the state of Amazonas, to Porto Velho, Rondônia, until it became impassable in 1988. Now it is proposed for reconstruction and paving, which would facilitate migration from the "Arc of Deforestation" in the southern part of the Amazon region to new frontiers farther north. The purpose of the highway, which is to facilitate transport to São Paulo of products from factories in the Manaus Free Trade Zone, would be better served by sending the containers by ship to the port of Santos. The lack of a land connection to Manaus currently represents a significant barrier to migration to central and northern Amazonia. Discourse regarding the highway systematically overestimates the highway's benefits and underestimates its impacts. A variety of changes would be needed prior to paving the highway if these potential impacts are to be attenuated. These include zoning, reserve creation, and increased governance in various forms, including deforestation licensing and control programs. More fundamental changes are also needed, especially the abandonment of the long-standing tradition in Brazil of granting squatters' rights to those who invade public land. Organizing Amazonian occupation in such a way that road construction and improvement cease to lead to explosive and uncontrolled deforestation should be a prerequisite for approval of the BR-319 and other road projects for which major impacts are expected. These projects could provide the impetus that is needed to achieve the transition away from appropriation of public land by both small squatters and by grileiros (large-scale illegal claimants). A delay in reconstructing the highway is advisable until appropriate changes can be effected.

Lifetime prevalence of mental disorders among homeless people in a southeast city in Brazil.

Several studies have shown a high prevalence of mental disorders among homeless people. Data from developing countries, like Brazil, are conspicuously lacking, making it difficult to plan interventions targeted to this group's mental health. The sample was selected from street dwellers in Juiz de Fora, a city with 424,479 inhabitants in the Southeast of Brazil. People of 18 years of age or more who had been living outdoors for at least 12 months were eligible for the study. The psychophathology was assessed by using SCAN (Schedule for Clinical Assessment in Neuropsychiatry). After 3 months, it was possible to assess 83 persons. All but one had at least one psychiatric diagnosis according to ICD-10. The most frequent diagnoses were alcohol abuse/dependence (82%), mood disorders (32.5%), drug abuse/dependence (31.3%), and schizophreniform psychosis (9.6%), with a high rate of comorbidity (78.3%). The occurrence of mental disorders was present in almost the entire sample. The authors link this to sampling framing and weakness of the homeless care network system, due to economic constrain and social inequality within the country.

Investigations on the biology, epidemiology, pathology and control of Tunga penetrans in Brazil. II. Prevalence, parasite load and topographic distribution of lesions in the population of a traditional fishing village.

Tungiasis is hyperendemic in many poor communities in Brazil and is associated with considerable morbidity. In order to understand the factors that determine the distribution of this ectoparasitosis in a rural community, an epidemiological study was carried out in a traditional fishing village in Ceará State, north-eastern Brazil. Based on active case detection and voluntary participation, 91% of the population (belonging to 95% of all families) was examined. Embedded fleas were looked for over all parts of the body, counted, and the lesions were staged. The overall prevalence of infestation was 51.3% (95% CI: 47.0-55.5). More males than females were infested (54.8% vs 48.3%); however, this difference was not statistically significant. Age-specific prevalence rates followed an S-shaped curve with peaks in children aged 5-9 years and people elder than 60 years. The parasite burden was high (range 1-145 lesions; arithmetic mean: 8.9) and particularly elevated in males, children <15 years and the elderly. The distribution of the parasite burden was uneven within the population with the majority of the lesions in a few individuals: the 23 subjects (8% of all infested) with severe infestation (>30 lesions) accounted for 1,366 of the 2,493 lesions (54.8%) documented. The study shows that tungiasis is a highly prevalent ectoparasitosis in this deprived community with a peculiar distribution of prevalence and parasite burden.

The first report of Hepatozoon spp. (Apicomplexa, Hepatozoidae) in domestic cats from São Paulo state, Brazil.

Hepatozoon sp. was diagnosed in three naturally infected cats from São Paulo state, Brazil. The first animal was admitted to the veterinary clinic with renal failure. During the hematological examination, gamonts of Hepatozoon sp. were observed within polymorphonuclear cells. Another two cats, which lived in the same house as the first cat, were also positive for this hemoparasite. This is the first report of a Hepatozoon sp. infection in domestic cats from Brazil.
