Simultaneous direct determination of amiloride and triamterene in urine using isopotential fluorometry.

A method for the simultaneous fluorometric determination of two diuretics in urine is proposed. The combination of matrix isopotential synchronous fluorescence (MISF) and first derivative techniques provides good analytical results. MISF spectra are obtained by calculating the isopotential trajectory in the three-dimensional fluorescence spectrum for a urine solution. In the spectral contour, the trajectory is taken to be the portion of the line that passes by the fluorescence maxima of both diuretics (lambda(ex) = 365 and lambda(em) = 413 nm for amiloride and lambda(ex) = 365 and lambda(em) = 437 nm for triamterene). Because contour lines connect points of identical intensity and the trajectory is part of a contour line, it is called "isopotential." Analyses was carried out in a 1/1 (v/v) ethanol/water mixture, using an apparent pH of 6.3 provided by 0.01 M sodium/citrate citric acid buffer. Urine samples are diluted 50 times and provide linear calibration plots at amiloride and triamterene concentrations up to 320 and 100 ng mL(-1), respectively. The goodness of the analytical signal was checked by using variance analysis. Signals recorded throughout the calibration range were subjected to three calibrations per each analyte, both in the absence and in the presence of variable amounts of the other analyte. Differences between individual calibrations and slopes were compared with those within individual calibrations. Based on the results, triamterene and amiloride can be accurately quantified in the presence of each other. The limit of detection calculated according to Clayton who uses error propagation throughout the calibration curve and a noncentralized security factor was 16.8 and 2.4 ng mL(-1) for amiloride and triamterene, respectively.

Histological and cytological criteria in the diagnosis of malignant melanomas by cryostat sections.

Although cryostat sections in general allow a distinction to be made between malignant melanomas and other pigmented lesions in clinically doubtful cases, the differential diagnosis may be difficult. The histological and cytological criteria taken into account can be classified as major, minor, and insufficient. Knowing the diagnostic value of each makes a conventionally established diagnosis safer. Variance analysis does not contribute to the problem but it can nevertheless be shown that the evaluation of six major criteria makes a quick and reliable cryostat section diagnosis possible. If these results are confirmed in a prospective study it would be a decisive step on the way to a quicker and safer cryostat section diagnosis of malignant melanoma, even for the less experienced histopathologist.

Reappraisal of the clinical and etiologic significance of immunoglobulin deviations in autoimmune hemolytic anemia ('warm type').

56 patients with autoimmune hemolytic anemia ('warm type') (AIHA) were investigated for immunoglobulin deviations. Of these, 43 were repeatedly analyzed (mean 4 times). The mean observation time was 20 months. The immunoglobulin values were correlated with clinical (degree of hemolysis) and serological (immunoglobulin class of autoantibodies; strength of antiglobulin reaction) parameters and statistically evaluated by variance analysis. Although no significant deviations of immunoglobulins in AIHA were found as compared to a normal control group, the immunoglobulin disturbance most frequently seen was an elevation of IgM. This is interpreted as a possible lack or functional impairment of immunoregulatory T cells in AIHA.

Short-wavelength automated perimetry in patients with diabetes mellitus without macular edema.

BACKGROUND: The short-wavelength-sensitive (SWS) cone-mediated sensitivity is a sensitive indicator of functional changes of the macula in diabetic maculopathy. This study was performed to investigate whether functional losses of the macula are detectable in patients without a significant macular edema. METHODS: In 45 patients with diabetes mellitus with clear optical media and no macular edema, conventional white-on-white perimetry (WWP) and short-wavelength automated perimetry (SWAP) were performed in the central 10-deg field. Fifty-eight healthy subjects ranging in age from 16 to 62 years served as controls. The two groups did not differ in age. RESULTS: Variance analysis (ANOVA) revealed significantly lower sensitivity in patients with diabetes than in controls. SWAP thresholds were significantly more greatly reduced by diabetes than those of WWP (ANOVA interaction: P=0.003). Post-hoc testing revealed a sensitivity reduction of 2.8 dB ( P=0.0003) in patients with diabetes for SWAP versus 0.46 for WWP ( P=0.15). Subgroup analysis revealed that mean thresholds of SWAP and WWP predominantly were reduced in patients with advanced disease. In patients with no retinopathy, sensitivity was not affected at all. CONCLUSION: SWS sensitivity may be affected in patients with diabetic retinopathy without clinically significant macular edema. Sensitivity loss was pronounced with increasing severity of retinopathy, reflecting the global status of the eye.

An evaluation of histomorphometric properties of coronary arteries, saphenous vein, and various arterial conduits for coronary artery bypass grafting.

PURPOSE: We evaluated the histomorphological properties of the coronary arteries, saphenous vein, and five arterial conduits for coronary artery bypass grafting, and compared them with each other. METHODS: All vessels were harvested from seven cadavers, eight autopsied individuals, and 14 patients. The length, lumen diameter, wall thickness, and histological structures were examined based on samples obtained from the coronary arteries and the conduits. The dimensions and wall thicknesses of the coronary arteries and the conduits were compared using a one-sided variance analysis. The similarities between the coronary arteries and the conduits were investigated by Duncan's multiple range tests. RESULTS: The internal mammary and intercostal arteries were elastic, while the others were muscular. The lengths of all conduits were adequate for use as coronary artery grafts. The dimensions and wall thicknesses of the coronary arteries and the conduits showed no statistically significant differences, except for the proximal portion of the saphenous vein. CONCLUSIONS: These findings emphasize the justification of continued use of the ideally matching internal mammary artery, either as an in situ or free graft, in coronary artery bypass grafting, although other arterial grafts can be used in coronary artery bypass grafting. In addition, the intercostal artery was found to have relatively favorable properties regarding its potential suitability as a coronary bypass conduit.

Heterogeneity of reperfusion after reversible regional myocardial ischemia in a canine model.

Variability of regional myocardial blood flow (RMBF) during reflow after 20 minutes of left anterior descending (LAD) coronary occlusion was measured by the radioactive microsphere technique in nine open-chest dogs. Preocclusion RMBF in the LAD territory was 0.89 +/- 0.27 ml/min/gm. Twenty minutes of LAD occlusion resulted in uniform and severe ischemia (RMBF < or = 0.25 ml/min/gm). After 1 minute of reperfusion, RMBF in the LAD territory rose to 3.48 +/- 1.88 ml/min/gm, and declined to 1.06 +/- 0.29 ml/min/gm after 20 minutes of reperfusion. RMBF variance increased significantly from 0.046 preocclusion to 0.2857 after 1 minute of reperfusion (p < 0.01) and declining to 0.086 after 20 minutes of reperfusion. By contrast, RMBF variance analysis of myocardial segments from the nonischemic left circumflex territory exhibited no significant change throughout the experiment. In any given dog this heterogeneous reperfusion of previously ischemic tissue resulted in a disorganized topography of blood flow rates. Myocardium with relatively high regional flow was intermingled with islands of tissue with relatively low blood flow. In conclusion, despite a relatively uniform and severe myocardial ischemic insult, the subsequent initial hyperemic response during reperfusion exhibits marked spatial heterogeneity. The juxtaposition of myocardial regions exposed to vastly differing rates of oxygen delivery and washout of toxic metabolites may set the stage for nonuniform recovery of myocardial function.

An association between serum testosterone level and HLA phenotype.

The concentration of testosterone was determined in sera of 122 HLA-typed women. Subsequently the women were classified in the category below or above the mean serum testosterone concentration. When the frequencies of HLA antigens were compared in these two categories of women, it was found that HLA-B5 and HLA-B12 antigens were significantly increased in the category of women with serum testosterone level above the mean concentration (P less than 0.01 and P less than 0.05, respectively). The frequency of HLA-B8 antigen was significantly decreased in the same category of women (P less than 0.05). The comparisons of the mean testosterone values of each HLA antigen and the variance analysis have also shown significant differences between the mean of HLA-B8 antigen and the means of other HLA antigens--HLA-A2, A3, A9, B5, B12 and Bw35. These results gave further conclusive evidence that gene(s) inside HLA region influence either the androgen hormone metabolism itself or cellular sensitivity to hormonal action as it has been presented for congenital adrenal hyperplasia.

Developmental changes of antioxidant enzymes in kidney and liver from rats.

The five principal antioxidant enzymes superoxide dismutase, catalase, glutathione peroxidase, glutathione-S-transferase, glutathione reductase in the kidney and liver, and the total hepatic glutathione were determined in rats of different ages (1, 2, 3, 6, and 12 months). Variance analysis proved the effect of age on the measured enzymes in the respective organ with the exception of glutathione S-transferase. The behavior of the enzymes was not uniform, and there were both increased and decreased changes in the two organs. A clear correlation between cellular antioxidative capacity and the age-specific processes of growing or aging could not be seen. A far more complicated network of interactions has to be assumed.

The relationship between synonymous codon usage and protein structure in Escherichia coli and Homo sapiens.

The role of silent position in the codon on the protein structure is an interesting and yet unclear problem. In this paper, 563 Homo sapiens genes and 417 Escherichia coli genes coding for proteins with four different folding types have been analyzed using variance analysis, a multivariate analysis method newly used in codon usage analysis, to find the correlation between amino acid composition, synonymous codon, and protein structure in different organisms. It has been found that in E. coli, both amino acid compositions in differently folded proteins and synonymous codon usage in different gene classes coding for differently folded proteins are significantly different. It was also found that only amino acid composition is different in different protein classes in H. sapiens. There is no universal correlation between synonymous codon usage and protein structure in these two different organisms. Further analysis has shown that GC content on the second codon position can distinguish coding genes for different folded proteins in both organisms.

VIVA (from virus variance), a library to reconstruct icosahedral viruses based on the variance of structural models.

VIVA is a software library that obtains low-resolution models of icosahedral viruses from projections observed at the electron microscope. VIVA works in a fully automatic way without any initial model. This feature eliminates the possibility of bias that could originate from the alignment of the projections to an external preliminary model. VIVA determines the viewing direction of the virus images by computation of sets of single particle reconstruction (SPR) followed by a variance analysis and classification of the 3D models. All structures are reduced in size to speed up computation. This limits the resolution of a VIVA reconstruction. The models obtained can be subsequently refined at best with use of standard libraries. Up today, VIVA has successfully solved the structure of all viruses tested, some of which being considered refractory particles. The VIVA library is written in 'C' language and is devised to run on widespread Linux computers.

Intracranial epidermoid cysts: diffusion-weighted, FLAIR and conventional MR findings.

PURPOSE: To compare diffusion-weighted echo-planar imaging (DW) with spin-echo (SE), and fluid-attenuated inversion recovery (FLAIR) sequences in the evaluation of epidermoid cysts (ECs), and to evaluate T2 shine-through effect. MATERIALS AND METHODS: Fifteen patients were imaged prospectively in two different 1.5 T magnetic resonance (MR) units with standard head coils with SE, FLAIR and DW echo planar imaging sequences. The qualitative and quantitative assessments were performed by two radiologists in consensus. Apparent diffusion coefficient (ADC) values were obtained from all ECs. Exponential DW images are obtained in 11 cases to eliminate T2 shine-through effects. The results are analyzed with variance analysis (ANOVA) and Bonferroni t method. RESULTS: FLAIR sequence was superior to T1- and T2-weighted sequences in showing ECs. In 13 cases, the borders of the lesions could be delineated from the surrounding structures with only DW imaging where ECs were markedly hyperintense. The ADC values of ECs are significantly lower than CSF (P < 0.001), and significantly higher than deep white matter (P < 0.01). On exponential DW images, ECs had similar intensity with brain parenchyma showing that the real cause of the hyperintensity of the lesions on trace images is the enhanced T2 effect of the tissue. CONCLUSION: FLAIR sequence is superior to the conventional MR sequences in demonstrating the ECs and DW imaging is superior to other MR sequences in delineating the borders of the ECs. Exponential DW images had shown that the hyperintensity in the trace images are caused by increased T2 effect of the lesion rather than the decrease in ADC values.

A comparison of conflict-resolving tendencies of nursing college students and medical school students in Turkey.

PURPOSE: The purpose of this descriptive and comparative study was to define and compare the tendencies of nursing college and medical students in resolving conflicts. METHOD: The scope of the research was all the students of the nursing college and faculty of medical. The sample included 359 students of the nursing college and 367 medical students. The data gathering tools used were a questionnaire covering socio-demographical attributes and the conflict-resolving tendency scale. Percentage computations, t-test (significance test between two independent groups) and one-way variance analysis (ANOVA) were utilized during the evaluation of the data. FINDINGS AND CONCLUSION: In the light of the findings of the study, it was defined that there were significant differences between the students of the two schools in their conflict-resolving tendencies, with nursing college students having higher levels of conflict-resolving tendencies, as well as in their empathy, listening skills and requirement-focused approach.

Color stability of heat-polymerized and autopolymerized soft denture liners.

STATEMENT OF PROBLEM: The physical and mechanical properties of soft denture liners are influenced by a number of factors. Aging is one factor that has an effect on the characteristics of polymers. The resistance of denture liners to aging can potentially influence the color stability of these liners. PURPOSE: This study attempted to determine the color stability of soft liners by subjecting them to an in vitro accelerated aging test. MATERIAL AND METHODS: Two autopolymerizing and 3 heat polymerizing commercially available soft denture lining materials were tested. Seven sample disks were made of each material according to manufacturers' instructions. Color measurements were recorded before and after the accelerated aging process. Color changes (DeltaE) were calculated by measuring tristimulus values at several wavelengths in the visual spectrum with the use of Commission International de l'Eclairge Lab (CIE-LAB) uniform color scale. RESULTS: Kruskall-Wallis variance analysis and Mann-Whitney U-Wilcoxon rank sum W tests were performed for statistical comparison of the DeltaE values. Ufigel P and Simpa liner materials showed significantly greater color changes than the other tested materials. There were no significant differences in color change among Ufigel L-Molloplast B, Ufigel L-Flexor, and Molloplast B-Flexor liner materials. However, a significant difference in color change was found between Ufigel P and Simpa liners. CONCLUSION: Heat-polymerized soft liners were more color stable than autopolymerized soft liners. No significant differences were found among heat polymerized materials; however, significant differences in color change were found between the 2 autopolymerized liners.

Hyperdynamic circulation in patients with cirrhosis: direct measurement of nitric oxide levels in hepatic and portal veins.

BACKGROUND/AIMS: Peripheral vasodilation represents the main vascular dysfunction associated with the hyperdynamic circulation of liver cirrhosis. This study was intended to measure directly regional and systemic levels of nitric oxide, a potent vasorelaxing mediator, in order to assess its role in the development of hemodynamic changes of cirrhosis. METHODS: We compared nitric oxide levels in the splanchinic and systemic circulation of 25 patients with cirrhosis undergoing transjugular intrahepatic portosystemic stent shunt and in the hepatic vein and peripheral blood of 10 patients without cirrhosis submitted to venous catheterization. Nitric oxide levels were measured through electron paramagnetic resonance spectroscopy as nitrosylhemoglobin complexes. RESULTS: Significantly higher nitric oxide levels were calculated in patients with cirrhosis with respect to controls, both in the peripheral and hepatic veins. In patients with cirrhosis, nitric oxide levels in the portal vein (3.44 +/- 2.17, expressed in arbitrary units) were higher than in the systemic circulation (1.89 +/- 1.15), but lower than in the hepatic vein (4.75 +/- 2.53; p < 0.001 by variance analysis). CONCLUSIONS: These data suggest that nitric oxide synthetic pathway activity as well as nitric oxide release are enhanced at the level of splanchnic vasculature and, more important, in the hepatic tissue, confirming evidence of the predominant role of nitric oxide in the pathogenesis of hemodynamic changes in patients with cirrhosis with portal hypertension.

Health education for pregnant women: the role of background characteristics.

This was a pilot study with the purpose of determining the effect of health education in enhancing the self-care agency of pregnant women and to define the role of their background characteristics in the success of this education. Subjects of this study were 30 pregnant women who visited a university hospital for check. The success of the given education was measured by pre- and post-tests that were applied before and after health education using "self-care agency scale". Data were evaluated according to t-test and variance analysis. After health education, the self-agency scores of the pregnant women increased significantly (P < 0.05). It was defined that pregnant women with the least self-care agency scores before health education, displayed the best progress after the education.

A study on the pharyngeal size in different skeletal patterns.

In the present study, pharyngeal size was investigated on the lateral cephalometric head films of 90 subjects, 45 males and 45 females, having different ANB angles. All of the subjects were aged 13 to 15 years. The films were taken at natural head position, and all were divided into three groups according to the ANB angle: ANB angles smaller than 1 degree, between 1 degree and 5 degrees, and larger than 5 degrees. In addition, each group was also divided into two subgroups according to sex. The effects of the ANB angle and sex on the pharyngeal size were investigated by means of variance analysis. It has been observed that two measurements, hy-apw4 and oropharynx area measurements, were affected by the change of ANB angle, and two other measurements, t-ppw and hy-apw2 measurements, by the sex; and that hy-apw4 measurement and oropharynx area became smaller with the increase of ANB angle.

Isokinetic assessment of spasticity in subjects with traumatic spinal cord injury (ASIA A).

OBJECTIVE: To assess resistance to passive isokinetic movements at specified speeds and range of motion, as a measure of muscular tonus. METHODS: A group of 12 subjects with complete traumatic spinal cord injury and a group of 12 able-bodied subjects were analysed, distinguishing the level of spasticity in the SCI group. Maximum eccentric peak torque was observed in passive isokinetic flexion (F) and extension (E) knee movement in displacements of 30, 60 and 120 per second. RESULTS: The Student t-test showed that the average torque in the injured group was significantly higher than in the control group (F= 120 degrees per second, P<0.001). Variance analysis showed that the lower level presented lower torque levels (E=60 and 120 degrees per second, P<0.032), while those in the higher level presented higher torque levels (F and E=60 and 120 per second, P<0.032) when compared to the control group. In the control group the Student t-test (P<0.0006) showed a significantly different muscle behavior (Flexion>Extension Torque). CONCLUSION: By using isokinetic assessment it was possible to quantify hypertonic spasticity in a group of subjects with spinal cord injury, distinguishing groups with higher and lower levels of spasticity as compared to a control group.
