[Toxicological characteristics of teturam in an alcoholic intoxication model in rats].

Study of toxicological characteristic of teturam (200 mg/kg, administered into the stomach through a tube daily for 2 weeks) on the model of alcoholic intoxication in rats (ethanol, 8 g/kg, administered into the stomach through a tube daily for 1 month) confirmed the literature data on the drug's ability to enhance or maintain ethanol-induced pathological changes in the blood and internal organs.

[Features of the course of retrobulbar neuritis in alcoholic intoxication].

The authors analyzed the course of bilateral retrobulbar neuritis (RN) in 39 patients with alcoholic intoxication. In almost one-third of those studied RN was first the only sign of the disease (24 patients suffered from chronic alcoholism of different stages). They were followed up for 1 to 3 years. Characteristics of disorders of the central and peripheral vision and of the status of the ocular fundus were found to correlate with general signs of intoxication. The data obtained my be of a definite significance for the timely specification of the diagnosis and aetiology of the process in patients with syndromes of bilateral RN.

Hypocalcaemia and chronic alcohol intoxication: transient hypoparathyroidism secondary to magnesium deficiency.

The authors report the observation of an alcoholic patient admitted for tetanic manifestations, in whom severe hypocalcaemia associated with hyperphosphatemia were suggestive of hypoparathyroidism. Administration of magnesium supplementation alone improved the clinical features and led to the correction of the calcium abnormalities. The mechanisms of hypomagnesemia in alcohol intoxication are reviewed as well as the links with hypocalcaemia.

[Myocardium in acute and chronic experimental alcoholic intoxication].

The myocardium of both heart ventricles in acute (AAP) and chronic (CAP) alcoholic poisoning was studied in 90 randombred rats. Functional-morphological changes typical of alcoholic cardiomyodystrophy were shown to develop in the heart in AAP and CAP. In AAP, alcoholic cardiomyodystrophy may lead to acute cardiac insufficiency and in CAP to progressive reduction of the contractile function of the heart and disturbances of conductivity in it. In AAP, however, the leading factor is the disturbance of bioenergetics as a result of toxic effect of ethanol and its metabolites on mitochondrial membranes under conditions of markedly disordered microcirculation. In CAP, along disordered microcirculation. In CAP, along with compensatory-adaptative processes in cardiomyocytes there increase the defects of the contractile apparatus, and diffuse fine-focal cardiosclerosis. This is the result of a long-term effect of ethanol and progressive hypoxia due to the affected vessels and disorders in lipid metabolism. Disorders in the function of sarcoplasmic reticulum may contribute to reduced contractile capacity of the heart.

Alcohol intoxication in teenagers using inhalant stupefacients.

The study was carried out 11 boys, 12-17 years old, treated in the Therapeutic-Educational Guidance Center for the Young because of using inhalant stupefacients and incliniation to alcohol abuse. The inhalant stupefacients were taken for a period of 6 months to three years, of alcohol--from 6 months to 2 years. The most common inhalant stupefacients were "Butapren" glue, trichlorethylene and "Roxy" fluid; wine and vodka were the alcohols used. No one patient displayed the full dependence syndrome, most of them (83%) had organic CNS damage, in 63% pathological EEG changes were recorded. The alcohol inebriation was accompanied by marked psychomotor excitation with cognitive disturbances, aggressiveness, anxiety, self-mutilation. One patient developed visual illusions and hallucinations. The findings suggest that the consequences of abuse of inhalant stupefacients significantly influence the syndrome of alcohol intoxication, potentiating the psychopathological signs.

Efficacy of flumazenil in acute alcohol intoxication: double blind placebo-controlled evaluation.

Flumazenil acts as an antidote for pharmacological and toxic effects due to benzodiazepines. Several isolated observations and short uncontrolled series have also suggested a possible effect against the impairment of consciousness induced by pure alcohol intoxication. Patients admitted in the emergency department with coma related to acute alcohol (ALC) or pure benzodiazepine (BZD) intoxication were randomized and treated blindly with either placebo or 1 mg flumazenil. A modified Glasgow score was used to observe the evolution of consciousness. In the 18 ALC patients, 1 mg flumazenil was not more effective than placebo, whereas it appeared to be very active in the BZD group. However, an open administration of higher doses of flumazenil (2-5 mg) in 11 ALC patients, whose condition had not initially improved, was followed by clear improvement of consciousness in five of them. Flumazenil, administered at a dose usually active against BZD sedation, does not improve CNS depression induced by ALC intoxication. Higher doses could be more effective in some patients, but it should also be verified in a placebo-controlled trial.

[Study of pharmacological activity of complex magnesium-containing preparation based on mineral bishofit and pyridoxine hydrochloride in a rat model of chronic alcoholic intoxication].

Nervous disturbances accompanying alcoholic illness were studied in relation to the depletion of magnesium ion content in the organism. The possibility of correcting the development of psychic (behavioral) pathologies by treatment with a complex magnesium-containing preparation based on mineral bishofit and pyridoxine hydrochloride (below, Mg-containing preparation) was studied in rats upon three-month voluntary alcoholization. A decrease in the locomotor (number of crossed squares) and vertical (number of standings) activity as evaluated in the open-field test and an increase in the immobilization time in the forced swim test showed evidence of depressive state in animals after long-term ethanol administration. After treatment with the Mg-containing preparation (50 mg Mg/kg in 2.5 ml volume, p.o.), the immobilization time of alcohol-preferring rats decreased in comparison to that before treatment and showed no statistically significant differences from the value in the intact control group. A decrease in the immobilization time (the main sign of antidepressant action) allows the Mg-containing preparation to be considered as antidepressant. The level of magnesium in rat blood erythrocytes decreased upon three-month voluntary alcoholization by 40.95 +/- 2.41% relative to control (p < 0.05). After a 5-week treatment with Mg-containing preparation under conditions of free access to alcohol, the content of magnesium in the erythrocytes of alcohol-preferring rats restored on a normal level. Chronic alcoholism reduces the content of microelements and vitamins (in particular, B6), these changes being mutually related.

[Influence of complex plant polyphenol preparation Eklikit on process of liver functions recovery after alcohol intoxication].

The comparative study was carried out to estimate hepatoprotective activity of the preparation "Eklikit", that represents the extract from the Shizandra chinensis crests and reference hepatoprpotector "Legalon" under acute alcohol intoxication. It was found that "Eklikit" posseses membrane protective and expressed hepatoprotective activity. "Eklikit" exceeds the reference hepatorpotector, "Legalon", in its ability to increase the oxidized form of NAD+ and reduced glutatione, cholesterol ethers and reduce the time of thiopental sleep. In other cases "Eklikit" was equally effective to "Legalon".

Effect of acute alcohol intoxication on the metabolism and plasma kinetics of chlordiazepoxide.

1. The metabolism and plasma kinetics of chlordiazepoxide have been determined in a group of volunteers and in a group of patients with acute alcohol intoxication. 2. Using the SAAM 26 non-linear least squares fitting programme, all chlordiazepoxide plasma concentration v time data following oral administration could be analysed in terms of a one-compartment open model with metabolic conversion of chlordiazepoxide to desmethylchlordiazepoxide. 3. Acutely intoxicated patients showed a prolonged elimination of chlordiazepoxide and a reduced clearance when compared with alcohol-free volunteers. The elimination of desmethylchlordiazepoxide, on the other hand, appeared to be faster in the alcoholics. 4. Alcohol exerts significant effects on the metabolism of chlordiazepoxide in acutely intoxicated patients.

[Clinico-hormonal correlations in patients with chronic alcoholic intoxication].

Radioimmunochemical assay was used to study the hypophyseal and peripheral hormones activity in 60 patients with chronic alcoholism, stage II. A correlation has been established between the patient's age and prolactin and FSH concentrations, as well as between the duration of the recent hard drinking and the concentrations of prolactin, testosterone, FSH and interstitial cell stimulating hormone. It has been shown that the manifestation of the alcoholic abstinent syndrome depended on the prolactin concentration. The test sensitivity estimated by the prolactin level rise and the testosterone level reduction reached 92.3%. The specificity of the changes detected comprised 25%. A conclusion has been made that the disorders noted in the patients with chronic alcoholic intoxication can be used as an objective test in the alcoholism diagnosis.
