[Antibacterial effects of antiseptics in vitro and in an experiment in vivo].

The effect of topical antiseptics routinely used in the Department of plastic and aesthetic surgery was tested against bacteria commonly infecting burns and major skin defects. In vivo, the effect on Staphylococcus aureus and Pseudomonas aeruginosa was examined, in vitro also on Proteus mirabilis. A very good effect on tested microorganisms was observed with 0.1% Persteril, Peruvian balsam and Dermazin crm. (Lek, Ljubljana). Gentian violet in a 0.5% concentration and 3% boric acid were not effective on Pseudomonas aeruginosa, 0.05% permanganate and 2% Jodonal were moreover not effective against S. aureus.

[Antibacterial action of the polymeric antiseptic catapol].

Activity of a polymer derivative of catamine AB, the antiseptic catapol, against gram positive and gram negative bacteria was studied. The effect of catapol on structural components of bacterial cells and permeability of bacterial cell membranes was investigated. The possibility of varying the susceptibility of bacteria to catapol and the effect of catapol on bacteria susceptible to antibiotics were evaluated. It was shown that by its in vitro antibacterial activity, catapol does not significantly differ from catamine AB.

Contact photosensitivity to nonoxynol used in antiseptic preparations.

Nonoxynols are nonionic surface-active agents used as surfactant in numerous cosmetic products and antiseptic preparations. Recently nonoxynol contact allergy has been described. We report two patients who presented a contact photosensitivity to nonoxynol 10 contained in Hexomédine transcutanée. Among 32 control subjects, we found 13 positive photopatch tests to Hexomédine transcutanée and 4 positive photopatch tests to nonoxynol 10. Surprisingly, the authors observed that only undiluted nonoxynol was phototoxic.

[Septo-clean* new disinfecting agent and antiseptic].

The study was aimed at evaluation of usefulness of Septo-Clean, which contain in 100 cm3 of distilled water 1 g of didecyl-dimethylammonium chloride and 70 cm3 of isopropyl alcohol. This preparation is prepared for quick disinfection of surfaces and antiseptic action on hand skin. Investigations were carried on test strains of S. aureus, E. coli, P. aeruginosa and C. albicans. It was demonstrated that MIC of Septo-Clean amount to 0.04% for S. aureus to 0.88% for P. aeruginosa. MBC 5 min values were respectively 0.19 and 1.05%. It was found that this preparation effectively lowers natural contamination with bacteria of hospital usable surfaces by 98%, exhibiting effectiveness similar to preparations containing 2.5% of chlorhexidine in 70% ethanol or isopropanol.

[Effect of antiseptics and redox-cycling agents on Corynebacterium ammoniagenes and related microorganisms in relation to synthesis of a new macroergic compound].

Sublethal concentration of the antiseptic composition Desoxon-1 was shown to provoke in cells of Corinebacterium ammoniagenes in a liquid medium the biosynthesis and accumulation of a novel macroergic 2-methylbutane-1,2,3,4-tetraol-2,4-cyclopyrophosphate. This substance is also synthesized when C. ammoniagenes is cultivated in a solid agar medium supplemented with benzylviologen. Cells preloaded with the new cyclopyrophosphate maintain its content when treated with 4% phenol, DP-2, Desoxon-1 or boiled and heated in an autoclave. Experiments with Mycobacterium tuberculosis and BCG revealed the ability of these bacteria to grow in a medium supplemented with BV++ possibly due to ability of synthesis of a new cyclopyrophosphate which was shown to correlate with resistance toward redox-cycling drugs. Accumulation of polyphosphates in the control cells of M. tuberculosis was illustrated by 31P-NMR spectroscopy and disappearance of the polyphosphates during cultivation in a BV(++)-supplemented medium. No signal of the new cyclopyrophosphate was yet registered in cells of M. tuberculosis by 31P-NMR.

[Evolution of resistance to antibiotics and antiseptics of hospital Staphylococcus aureus strains isolated from 1980 to 1991].

From 1980 to 1991, 925 non epidemic hospital isolates of S. aureus were selected and phage typed. MIC of 20 antibiotics and 4 antiseptics were determined by agar dilution method. The proportion of isolates susceptible to all antibiotics remains constant; however the trend to the resistance is strong during the study period (oxacillin 10-->20%, erythromycin 17-->28%, pefloxacin 4-->19%...). Strains resistant to oxacillin become more and more multiple resistant; some of recent isolates are resistant to 7 antibiotic families. There are very few products active against these strains i.e, glycopeptides (100%), pristinamycin (96%), fusidic acid (94%). This finding implies the need for continuous surveillance at the local and national level.

Study of the antiseptic efficacy of a quick drying rubbing type povidone-iodine alcoholic disinfectant solution by the glove juice method.

The antiseptic efficacy of HAD Hand Wash (83% ethanol containing 0.5% povidone-iodine) was investigated by the glove-juice method in 29 cases amongst staff members of our university. The number of viable bacteria on the hands and fingers was counted three times, and the mean of these counts was used as the baseline value for each subject. The number of viable bacteria on both hands was the same prior to disinfection. After the hands were disinfected by rubbing with 3 ml of HAD Hand Wash until it dried, the subjects put on sterile gloves. The number of viable bacteria was counted on the right hand of all the subjects immediately after disinfection. The number of viable bacteria on the left hand was counted in 8 subjects 30 minutes after disinfection, and in 7 subjects each 1, 2 and 4 hours after disinfection. The bacteria were identified by subculture. The ratio of bacteria sterilized (bacterial reduction rate) was 75.4% immediately after disinfection, and 69.0%, 71.9%, 35.8% and 27.6% at 30 minutes, 1 hour, 2 hours and 4 hours, respectively. All of the bacteria detected were Gram-positive bacteria that were native to the sebaceous glands, sweat glands or skin surface; i.e. Staphylococcus, Micrococcus, Bacillus, and Streptococcus. The frequency of detection of Staphylococcus warneri was the highest before disinfection, followed by Staphylococcus epidermidis and Staphylococcus hominis, in that order. After disinfection, all these species showed a marked decrease in the number of bacteria. Micrococcus remained in half the subjects, while other species remained in less than one third of the subjects.

[Experiences with closed irrigation-suction drainage and simultaneous administration of an antiseptic].

In 47 patients a closed instillation drainage system was used with concomitant systemic antibiotic therapy. The schedule was strictly followed and is described in detail in the text. Prerequisites for a successful therapy are thorough surgical débridement, good coverage with soft tissue and a very careful skin closure. We propose a shortened duration of instillation of 3 to a maximum of 5 days. The Draintec suction pumps now used evidently create less problems than the classic pumps. During a mean follow-up of 3 years a definite recurrence was observed in two cases and a possible recurrence in one. In view of these results we consider this mode of treatment is still valid. Lavasept was added as an antiseptic drug in three quarters of the cases and was tolerated well. In view of the low rate of recurrence in all cases its potential cannot yet be fully estimated. We recommend the instillation-drainage system for use in thigh, upper arm and pelvic areas in the hands of a fully cooperating management team with some experience in the application of instillation-drainage systems.

Failure of antiseptic bonding to prevent central venous catheter-related infection and sepsis.

Infection associated with the use of triple lumen catheters in hospitals is a frequent and serious complication. The prevailing hypothesis for the origin of catheter-related infection (CRI) is bacterial colonization and subsequent infection of the skin insertion site and catheter interface. The recently released ARROWgard catheter contains a bonded synergistic combination of silver sulfadiazine and chlorhexidine, which is thought to render the catheter resistant to bacterial colonization and subsequent sepsis. The purpose of this study is to compare the incidence of CRI and catheter-related sepsis (CRS) between a standard triple lumen catheter and ARROWgard antiseptic coated catheter in patients receiving total parenteral nutrition (TPN). A randomized, prospective clinical trial was conducted at a community referral center from January 1993 through April 1994. One-hundred-ninety-one patients with need for TPN were randomized to receive either the ARROWgard or a standard triple lumen catheter placed under a strict sterile protocol. CRI was defined as >/= 15 colony forming units by semiquantitative culture technique of the catheter tip or intracutaneous segment. CRS was defined as growth of the same organism on the catheter and at least one peripheral blood culture. All catheters were cultured. Ninety-two patients received the ARROWgard catheter, and 99 patients received the standard catheter. There were no differences between the average age, sex, length of hospital stay, days on TPN, number of catheters/patient, indications for TPN, primary diagnoses, or duration of the central line between the two groups. The overall rate of CRI was 11.5 per cent, and CRS was 8.4 per cent in this study. The rate of CRI for the ARROWgard was 10.9 per cent, compared with 12.9 per cent for the standard catheter (P = NS). The rate of CRS for the ARROWgard was 8.7 per cent, compared with 8.1 per cent for the standard catheter (P = NS). The coating of central venous catheters with silver sulfadiazine and chlorhexidine does not reduce the rate CRI or CRS when compared with standard central venous catheters in patients receiving TPN.

[The dynamics of the antiseptic resistance of the causative agents of soft-tissue suppurative-inflammatory diseases in children].

Opening of purulent foci in children with pyo-inflammatory diseases (PID) of soft tissues often leads to detection of staphylococcus aureus in the monoculture and different associations of this microbe with gram-negative bacteria in amounts exceeding the "critical level" responsible for the development of PID. Repeated examinations revealed a 2 times decreased frequency of detection of staphylococcus monocultures and 1.5 time less diversity of the association of microorganisms. Most efficient antiseptics among the investigated ones in relation to bacteria isolated in the opened purulent foci were found to be iodopyron, pervomur, boric acid, resorcinum, dioxidine.

[The comparative characteristics of the antibacterial activity of new antiseptics and the prospects for their use in dental practice].

Antibacterial activities of modern antiseptics listerine, myramistine, cidipol, octenisept, tantum verde, matecide, and rautine drugs chlorhexidine and dioxidine are compared. The prospects of their use in dentistry for the treatment of inflammations associated with anaerobes are discussed. Metacide, listerine, chlorhexidine, and dioxidine are characterized by the widest spectrum of activity towards obligate anaerobic bacteria; moreover, these drugs inhibit bacterial growth when used in lower concentrations than other drugs.

[Comparison of the efficacy of 2 antiseptic solutions in the prevention of infection from peridural catheters].

Two antiseptic solutions (iodine polyvidone and chlorhexidine) were compared-in a prospective non-randomized study including 294 parturient women. This study aimed to assess their efficacy against infections through epidural catheters. All catheters were subsequently cultivated. Cultures were significantly positive in 3% of cases after iodine polyvidone and 1% after chlorhexidine decontamination (not significant). No clinical or biological infections were detected. Notwithstanding some apparently unavoidable but moderate contaminations, prevention of infections post epidural analgesia depends principally on a complete adherence to asepsia rules.

[Antiseptics in the combined treatment of patients with perimaxillary abscesses and phlegmons].

The efficacy of local use of antiseptic solutions (chlorhexidine bigluconate, chlorfillipt and furaciline) on clinical presentation and microflora of purulent wounds was studied in 236 patients with maxillofacial abscesses and phlegmons. The wound microflora was represented by a wide spectrum of opportunistic agents. A pronounced therapeutic effect was achieved in patients with staphylococcal infection. Marked resistance to the drugs was noted in alpha-hemolytic streptococcus.

[Use of antiseptic myramistin in the multimodality treatment of nonspecific suppurative pleuropulmonary diseases].

To treat patients with suppurative diseases of the bronchi, lung, and pleura, the new aseptic myramistine was topically applied as 0.005% myramistine solution in isotonic sodium chloride solution in 28 patients with acute and chronic pleural empyema, acute abscess of the lung, and suppurative endobronchitis-complicated bronchoectatic disease. The therapy made it possible to arrest the intoxication syndrome, to achieve abacillation of contents in the cavities of empyema, abscesses, and bronchoectases, to cure endobronchitis, to obliterate destructive cavities in the lung more rapidly and to prepare patients for a planned surgical intervention more valuably and to improve the results of surgical treatment. The studies allow the antiseptic myramistine to be recommended for topical treatment of pyodestructive diseases of the lung.
