The 1999 international emergency humanitarian evacuation of the Kosovars to Canada: A qualitative study of service providers' perspectives at the international, national and local levels.

BACKGROUND: In response to the Kosovo crisis, Canada received 5,500 Albanian Kosovar refugees in 1999 as part of the emergency humanitarian evacuation and settlement effort. This study attempts to describe the experiences of service providers at the international, national, and local levels, involved in the organization and delivery of health and settlement services in Canada for the Kosovar refugees. METHODS: A qualitative case study design using key informant interviews was used. Nominated sampling was used to identify 17 individuals involved in the organization and delivery of health and settlement. Key themes were identified and recommendations made to provide a framework for the development of policy to guide response to future humanitarian emergencies. RESULTS: Six themes emerged: (1) A sense of being overwhelmed, (2) A multitude of health issues, (3) critical challenges in providing health care, (4) access to health and settlement services, (5) overall successes and (6) need for a coordinated approach to migration health. CONCLUSIONS: For those involved, the experience was overwhelming but rewarding. Interviewees' major concerns were the need for a more comprehensive and coordinated approach to the flow of medical information and handling of specific health problems.

Report of an international workshop to standardize response criteria for non-Hodgkin's lymphomas. NCI Sponsored International Working Group.

Standardized guidelines for response assessment are needed to ensure comparability among clinical trials in non-Hodgkin's lymphomas (NHL). To achieve this, two meetings were convened among United States and international lymphoma experts representing medical hematology/oncology, radiology, radiation oncology, and pathology to review currently used response definitions and to develop a uniform set of criteria for assessing response in clinical trials. The criteria that were developed include anatomic definitions of response, with normal lymph node size after treatment of 1.5 cm in the longest transverse diameter by computer-assisted tomography scan. A designation of complete response/unconfirmed was adopted to include patients with a greater than 75% reduction in tumor size after therapy but with a residual mass, to include patients-especially those with large-cell NHL-who may not have residual disease. Single-photon emission computed tomography gallium scans are encouraged as a valuable adjunct to assessment of patients with large-cell NHL, but such scans require appropriate expertise. Flow cytometric, cytogenetic, and molecular studies are not currently included in response definitions. Response rates may be the most important objective in phase II trials where the activity of a new agent is important and may provide support for approval by regulatory agencies. However, the goals of most phase III trials are to identify therapies that will prolong the progression-free survival, if not the overall survival, of the treated patients. We hope that these guidelines will serve to improve communication among investigators and comparability among clinical trials until clinically relevant laboratory and imaging studies are identified and become more widely available.

Internalization of non-permeant materials into cultured mammalian cells by vortex-stirring in the presence of a high molecular weight polyacrylic acid: direct evidence of internalization and its dependence on the cell lines.

Laser scanning confocal fluorescence microscopic analysis provided a direct evidence of the internalization of non-permeant Lucifer Yellow and a dextran of MW 4400 into leukemia L1210 cells when the cells were vortex-stirred in the presence of a high molecular weight polyacrylic acid, A-119 (MW ca. 9 x 10(6)). The morphology of A-119-treated cells was probed by scanning electron microscopic analysis using 2% glutaraldehyde for fixation. The cells immediately after vortex-stirring with A-119 showed many blebs on the cell surface, indicative of local weakening of the plasma membrane. The blebby surface returned to normal within 5 min at 37 degrees C, but not completely at 0 degree C even after 20 min. Several cultured cell lines, murine splenocytes, and Ehrlich ascites carcinoma cells were particularly susceptible to permeabilization by this method, although the efficiency varied from one type of cell to another.

Results of internal mammary artery-coronary artery bypass surgery and the characteristics of internal mammary artery grafts.

Coronary artery bypass grafting utilizing the internal mammary artery (IMA) was performed in 108 patients with an operative mortality (less than 1 month) of 0% and a hospital mortality of 1.9%. The IMA was used most often in the left anterior descending artery system in combination with saphenous vein grafts (SVG) to the right and left circumflex artery systems. Although the IMA flow was smaller than the SVG flow when measured intraoperatively by an electromagnetic flow meter, postoperative clinical, electrocardiographic, isotopic, angiographic and coronary sinus flow-metric studies all demonstrated that the IMA can respond well to myocardial blood flow demand both at rest and during exercise, resulting in excellent clinical improvements with no detectable signs of flow deficiency. In addition, no signs of ischemia were detected in any of the 15 patients with stenosis in the left main trunk treated with an IMA graft. The IMA graft appears to have a great adaptive capacity to meet increased myocardial demand. Postoperative angiography performed at an average of 3 months after surgery in 60 unselected patients demonstrated an IMA patency rate of 98% in comparison with 88% patency in SVGs to the left anterior descending artery (p less than 0.05). Not only the patency rate, but also the graft wall characteristics were much better in IMA grafts than in SVGs. International Guidelines for Neonatal Resuscitation: An excerpt from the Guidelines 2000 for Cardiopulmonary Resuscitation and Emergency Cardiovascular Care: International Consensus on Science. Contributors and Reviewers for the Neonatal Resuscitation Guidelines.

The International Guidelines 2000 Conference on Cardiopulmonary Resuscitation (CPR) and Emergency Cardiac Care (ECC) formulated new evidenced-based recommendations for neonatal resuscitation. These guidelines comprehensively update the last recommendations, published in 1992 after the Fifth National Conference on CPR and ECC. As a result of the evidence evaluation process, significant changes occurred in the recommended management routines for: * Meconium-stained amniotic fluid: If the newly born infant has absent or depressed respirations, heart rate <100 beats per minute (bpm), or poor muscle tone, direct tracheal suctioning should be performed to remove meconium from the airway. * Preventing heat loss: Hyperthermia should be avoided. * Oxygenation and ventilation: 100% oxygen is recommended for assisted ventilation; however, if supplemental oxygen is unavailable, positive-pressure ventilation should be initiated with room air. The laryngeal mask airway may serve as an effective alternative for establishing an airway if bag-mask ventilation is ineffective or attempts at intubation have failed. Exhaled CO(2) detection can be useful in the secondary confirmation of endotracheal intubation. * Chest compressions: Compressions should be administered if the heart rate is absent or remains <60 bpm despite adequate assisted ventilation for 30 seconds. The 2-thumb, encircling-hands method of chest compression is preferred, with a depth of compression one third the anterior-posterior diameter of the chest and sufficient to generate a palpable pulse. * Medications, volume expansion, and vascular access: Epinephrine in a dose of 0.01-0.03 mg/kg (0.1-0.3 mL/kg of 1:10,000 solution) should be administered if the heart rate remains <60 bpm after a minimum of 30 seconds of adequate ventilation and chest compressions. Emergency volume expansion may be accomplished with an isotonic crystalloid solution or O-negative red blood cells; albumin-containing solutions are no longer the fluid of choice for initial volume expansion. Intraosseous access can serve as an alternative route for medications/volume expansion if umbilical or other direct venous access is not readily available. * Noninitiation and discontinuation of resuscitation: There are circumstances (relating to gestational age, birth weight, known underlying condition, lack of response to interventions) in which noninitiation or discontinuation of resuscitation in the delivery room may be appropriate.

Ankle arthrodesis: internal non-compression arthrodesis versus internal compression arthrodesis.

Ankle arthrodesis is still considered to be the standard treatment for most disabling types of ankle arthritis, but fusion methods are varied. We report our experience of ankle arthrodesis and compare a group of 34 cases treated by Blair's non-compression arthrodesis to another group of 32 cases treated by internal compression arthrodesis using two crossed screws. The same surgeon performed all the operations. The Blair's non-compression arthrodesis group included 21 males and 13 females with an average age of 42 y/o (range 18-70 y/o) and an average follow up period of 38.6 months (range 26-62 months). The union rate was 91.2% and the average union time was 5.6 months (range 2-10 months). There were three cases of non-union. The cross-screw compression arthrodesis group included 20 males and 12 females with an average age of 45 y/o (range 20-86 y/o) and an average follow up period of 38.3 months (range 15-81 months). The union rate was 96.9% and the average union time was 2.7 months (range 1.5-4.4 months). There was one case of non-union. We conclude that our cross-screws compression arthrodesis with its shorter fusion time was found to be superior to the Blair's non-compression arthrodesis.

International multicentre comparison of cerivastatin with placebo and simvastatin for the treatment of patients with primary hypercholesterolaemia. International Cerivastatin Study Group.

An international multicentre double-blind randomised trial compared the efficacy and safety of cerivastatin (0.025, 0.05, 0.1 and 0.2 mg once daily) with placebo and simvastatin (20 mg) over a period of 12 weeks, with study extensions to 52 and 100 weeks. The primary efficacy parameter was the percentage change in low density lipoprotein cholesterol (LDL-C). This was reduced from the baseline by 12.5% (0.025 mg) to 30.6% (0.2 mg) compared with falls of 2.0% on placebo and 40.3% on simvastatin. All four cerivastatin doses and simvastatin (20 mg) produced significantly greater falls than placebo (p < 0.0001) and the decrease in LDL-C was dose-dependent for cerivastatin. Simvastatin produced significantly greater falls than any cerivastatin dose or placebo (p < 0.0001). The effect was maintained at 1 year but somewhat attenuated at 100 weeks. Significant falls were also seen in serum total cholesterol and triglycerides. High density lipoprotein cholesterol (HDL-C) levels were significantly increased by cerivastatin (0.1 and 0.2 mg) and simvastatin (20 mg) at 12 weeks and increased further by 100 weeks. Mean fasting apolipoprotein A1 and lipoprotein A1 were increased and apolipoprotein B decreased by cerivastatin and simvastatin therapy. All doses of cerivastatin produced significant falls in the total cholesterol/HDL-C ratio at 12 weeks (0.5-1.6) compared with a fall of 2.1 for simvastatin (20 mg). Cerivastatin was well tolerated. Elevations in creatine phosphokinase, aspartate aminotransferase and alanine aminotransferase were mostly minor and transitory. Vital signs, electrocardiogram determinations, urinalysis and ophthalmic assessment showed similar results for both drugs. Cerivastatin, at doses of 0.1 mg and 0.2 mg daily, is considered to be of therapeutic value in the treatment of patients with primary hypercholesterolaemia, with 0.2 mg cerivastatin achieving reductions of LDL-C and total cholesterol similar to those achieved in the WOSCOP and CARE studies.

The Systemic Lupus International Collaborating Clinics/American College of Rheumatology (SLICC/ACR) Damage Index for Systemic Lupus Erythematosus International Comparison.

OBJECTIVE: To compare patients with systemic lupus erythematosus (SLE) from different centers with respect to demographics and Systemic Lupus International Collaborating Clinics/American College of Rheumatology Damage Index (SLICC/ACR DI) scores, and to assess whether the SLICC/ACR DI changed over time, and whether initial DI scores were related to outcome. METHODS: Members of SLICC completed DI scores and patient demographics on patients followed in their centers. Information was provided at 2, 5-10, and > 10 years of followup. Data were entered on computer and analyzed on SPSS/PC+ and SAS using descriptive statistics and analysis of variance. RESULTS: Information for 1297 patients within 2 years of first clinic visit was submitted from 8 centers. There were 1187 women and 110 men with a mean age at diagnosis of SLE of 32 years. Seven hundred sixty-two were Caucasian, 423 were black, and the remainder were of other races. There were more blacks in the American centers than in Canadian or European centers. Five centers provided information for the 3 time periods. The DI increased over time. Ninety-nine patients had died. Higher SLICC/ACR DI scores were documented in patients who went on to die. CONCLUSION: The SLICC/ACR DI is a valid measure for damage in SLE.

INTERNATIONAL REFERENCE PREPARATIONS OF TYPHOID VACCINE. A REPORT ON INTERNATIONAL COLLABORATIVE LABORATORY STUDIES.

Before their establishment as International Reference Preparations of Typhoid Vaccine, an acetone-inactivated vaccine (vaccine K) and a heat-phenol-inactivated vaccine (vaccine L) were tested on man in large-scale controlled field trials and simultaneously subjected to extensive collaborative laboratory studies on their relative potencies. Some 20 laboratories in 12 countries took part in these collaborative studies. This paper describes in detail the results of the investigation of 14 different assay procedures.One of the main objects of these studies was to determine whether one or more of the laboratory methods showed a correlation with the protective effectiveness of the vaccines against typhoid fever in man. It is clear from the results reported that none of the potency tests in animals reliably reflected the evidence obtained in human beings in the controlled field trials. Some of the assays, however, seem to offer promise of ability to distinguish between the potencies of typhoid vaccines in a manner reflecting their efficacy in man. Further research is needed before the value of these procedures can be definitely assessed.

Delayed chronic type A dissection with a functional midline crossing right internal thoracic artery after CABG: surgical approach to an ascending aorta without dissecting the midline crossing internal thoracic artery.

We successfully treated chronic type A dissection and coronary artery disease with a functional midline crossing right internal thoracic artery (RITA) after coronary artery bypass grafting (CABG). A 68-year-old man was incidentally diagnosed as chronic type A dissection by follow-up cardiac catheterization after CABG, with 90% stenosis in the right coronary artery (RCA) No. 2. The dissecting aneurysm (max. 6 cm in diameter) was localized at the right portion of the ascending aorta with a functional RITA to the left anterior descending coronary artery. The dissecting aneurysm was treated with patch closure and the RCA was revascularized with a right gastroepiploic artery and saphenous vein composite graft through combined right antero-axillar thoracotomy and lower mini-sternotomy without RITA dissection. Treatment of chronic type A dissection following CABG becomes more challenging with a functional midline crossing RITA. It is important that a safe and less invasive surgical strategy be implemented for such complicated case.

International trends in salmonella serotypes 1998-2003--a surveillance report from the Enter-net international surveillance network.

One of the objectives of any surveillance activity is to monitor trends in infections. The international surveillance network for human enteric infections, Enter-net, has been collecting and reporting data on laboratory-confirmed human salmonella infections since 1993. The number of cases identified rose in the mid-1990s, with the peak being in 1997. This paper describes the subsequent decline in salmonella serotypes being reported by the national reference laboratories participating in the Enter-net surveillance network between 1998-2003. The total number of human cases of salmonellosis reported by the Enter-net participating countries has fallen from 22,698 to 142,891 during this period. Even at these reported levels salmonellosis remains a major cause of morbidity in humans.

Angioleiomyoma of the internal auditory meatus: a rare occurrence in the internal auditory canal.

We describe a case of angioleiomyoma of the internal auditory meatus. A finding of this tumor at this site is very unusual. In fact, our review of the literature revealed that only 1 case has been previously reported. In our patient, the tumor was clinically and radiologically difficult to distinguish from an acoustic neuroma. It would be important to recognize this rare small tumor preoperatively because it may be appropriate to manage it conservatively.

[Right internal mammary artery reimplantation into the left internal mammary artery. Y anastomosis. 25 cases].

Distal latero-circumflex arteries (DLCA) and posterior descending artery (PDA) are anatomically too far and cannot be revascularized with internal mammary artery (IMA) by a conventional procedure. The reimplantation of the right IMA into the left IMA in situ increases (2 times) the length of right IMA graft available. Fifty-five patients underwent this technique. Their preoperative status was: 22 males, 3 females; mean age: 57 years, 38% myocardial infarction (MI). Coronary angiography showed: stenosis of the left main coronary artery: 3; stenosis of 3 vessels: 15; 2 vessels: 10; 2.3 anastomoses by patient were performed with Y right and left IMA procedure: 24 LDA, 8 diagonals, 25 DLCA and 1 PDA anastomosis. No deaths were observed in this short series. Morbidity was: 1 MI, 2 sternal sepsis, 1 bilateral phrenic paralysis (all were cured without sequelae). To date (March 90) 15 patients have been followed for 3 to 12 months, 12 are angina-free, 3 are significantly improved, 11 have a negative exercise test. Thallium test is normal in the revascularized area in 14 patients. Seven angiographies have been performed (6 months to 1 year) and all Y right IMA are patent.

[Porosity, microhardness, roughness and internal stresses of PMMA base materials depending on the isolation method. 2. Roughness and internal stresses].

In the present paper hot and cold polymerized denture basic materials were investigated after isolation by means of alginate, tin foil and silicone. Measurements of roughness and internal stresses after polymerization show that the advantages of a silicone isolation correspond to those of a tin foil isolation, where the clinically important smoothing effect of a silicone isolation is convincing using simple laboratory handling.

Report from the International Bone Marrow Transplant Registry. Advisory Committee of the International Bone Marrow Transplant Registry.

The International Bone Marrow Transplant Registry (IBMTR) receives and analyses detailed information contributed by transplant teams at more than 175 institutions worldwide. This collaborative research effort has grown rapidly; there are now more than 8000 cases in the database. This is a summary of the current status of bone marrow transplantation in leukemia and severe aplastic anemia, a brief summary of key findings reported by the IBMTR during the past year, as well as studies planned for the coming year.

Reactivity of human isolated internal mammary artery to constrictor and dilator agents. Implications for treatment of internal mammary artery spasm.

Perioperative spasm of the internal mammary artery (IMA) may occur after coronary artery bypass surgery. To establish the most appropriate dilator agent, we tested the reactivity of ring segments of human IMA in organ baths to various constrictor and dilator agents. We found that the thromboxane mimetic U46619 was the most potent IMA constrictor agent, followed by norepinephrine, serotonin, phenylephrine, and potassium chloride (K+). In K+- or U46619-precontracted IMA, glyceryl trinitrate and papaverine caused full relaxation. In K+-precontracted arteries, nifedipine, verapamil, and diltiazem caused full relaxation, but nifedipine was 15-fold more potent than the other calcium antagonists. In contrast, pretreatment of vessels with glyceryl trinitrate failed to alter subsequent contraction to U46619 or K+ while nifedipine pretreatment abolished subsequent contraction to K+ and reduced sensitivity of the IMA to U46619. We conclude that perioperative IMA spasm could be treated with the rapid-onset, nonspecific, vasodilator glyceryl trinitrate, but for prophylaxis of IMA spasm, calcium antagonists or specific receptor antagonists should be tested in the clinical setting.
