Dynamic responses of O2 uptake at the onset and end of exercise in trained subjects.

Inconsistencies about dynamic asymmetry between the on- and off-transient responses in .VO2 are found in the literature. Therefore the purpose of this study was to examine .VO2on- and off-transients during moderate- and heavy-intensity cycling exercise in trained subjects. Ten men underwent an initial incremental test for the estimation of ventilatory threshold (VT) and, on different days, two bouts of square-wave exercise at moderate ( VT) intensities. .VO2 kinetics in exercise and recovery were better described by a single exponential model ( VT). For moderate exercise, we found a symmetry of .VO2 kinetics between the on- and off-transients (i.e., fundamental component), consistent with a system manifesting linear control dynamics. For heavy exercise, a slow component superimposed on the fundamental phase was expressed in both the exercise and recovery, with similar parameter estimates. But the on-transient values of the time constant were appreciably faster than the associated off-transient, and independent of the work rate imposed ( VT). Our results do not support a dynamically linear system model of .VO2 during cycling exercise in the heavy-intensity domain.

Factors affecting the components of the alveolar CO2 output-O2 uptake relationship during incremental exercise in man.

The VCO2-VO2 (alveolar CO2 output-alveolar O2 uptake) relationship (V-slope) during increasing work rate (ramp) cycle ergometer exercise has two approximately linear components: a lower component slope (S1) with a value of about 0.95 and a steeper, upper component (S2). We examined the effect of muscle glycogen depletion (protocol 1) and the rate of increase in work rate (ramp rate) without muscle glycogen depletion (protocol 2) on S1 and S2. In protocol 1, ten healthy men with a mean age of 31.4 years (S.D. 6.2) were studied on each of 3 days (days 1 and 3 were control days). They performed a ramp exercise test to maximum tolerance and steady-state tests at rest, during unloaded pedalling and at two constant work rates below their anaerobic threshold (AT). To deplete muscle glycogen before the test on day 2, the subjects performed 2 h of very heavy cycle exercise on the preceding day and fasted overnight. S1 was reduced on day 2 (0.79 compared with 0.95, P less than 0.001), as was the VCO2-VO2 slope derived from steady-state measurements (0.81 compared with 0.99, P less than 0.001), but AT and the slope difference (S2 - S1) were unchanged. In protocol 2, seven healthy men with a mean age of 20.6 years (S.D. 2.4) performed ramp tests at three different rates of increasing work rate (15, 30 and 60 W min-1), each ramp rate being performed twice in random sequence. The ramp rate did not affect S1 but S2 was steeper with the faster rates of work rate increase (1.27, 1.43 and 1.63, respectively, P less than 0.01). Our findings support the concept that the lower component of the V-slope plot (below AT) represents muscle substrate respiratory quotient (RQ) while the difference between S1 and S2 reflects 'excess CO2' derived from bicarbonate buffering of lactic acid.

Prediction of normal values for lactate threshold estimated by gas exchange in men and women.

Lactate threshold (LT) is an index of exercise capacity and can be estimated from the gas exchange consequences of a metabolic acidosis (LT(GE)). In recent years, it has emerged as a diagnostic tool in the evaluation of subjects with exercise limitation. The purpose of this study was to develop LT(GE) prediction equations on a relatively large sample of adults and to cross-validate each equation. A total of 204 healthy, sedentary, nonsmoking subjects (103 men and 101 women), aged 20-70 years, underwent graded exercise testing on a cycle ergometer. The V-slope technique was used to detect LTGE as the oxygen uptake (VO2) at the breakpoint of the carbon dioxide output versus VO2 relationship. Multiple linear regression was used to develop 12 equations with combinations of the following predictor variables: age, height, body mass, and fat-free mass. Eight of the equations are gender-specific and four are generalized with gender as a dummy variable. The equations were cross-validated using the predicted residual sum of squares (PRESS) method. The results demonstrate that the equations had relatively high multiple correlations (0.577-0.863) and low standard errors of the estimate (0.123-0.228 1 x min(-1)). The PRESS method demonstrated that the equations are generalizable, i.e., can be used in future studies without a significant loss of accuracy. Since we tested only healthy, sedentary subjects, our equations can be used to predict the lower limit of normal for a given subject. Using individual data for healthy and diseased subjects from the literature, we found that our gender-specific equations rarely miscategorized subjects unless they were obese and mass was a predictor variable. We conclude that our equations provide accurate predictions of normal values for LT(GE) and that they are generalizable to other subject populations.

A new densitometric approach to the assessment of mean coronary flow.

RATIONALE AND OBJECTIVES: The authors present an angiographic method to measure absolute coronary blood flow in patients. METHODS: The left or right coronary tree is three-dimensional (3D)-reconstructed from biplane coronary angiograms. This allows the determination of the intravascular volumes needed for flow measurement. The 3D distance traveled by the contrast medium during one cardiac cycle is determined by appropriately thresholding the "concentration-distance", curves computed on two pairs of images taken one cardiac cycle apart. RESULTS: The angiographic flow measurements were compared with nearly simultaneous flow determinations obtained with an intracoronary ultrasonic Doppler flow velocity measuring device. The mean relative difference between the Doppler and the 3D measurements was 11% and the two sets of flow values correlated well (r = 0.81). CONCLUSIONS: A method for the determination of mean coronary flow is presented. The procedure is simple and can be incorporated easily into clinical routine.

Respiratory and skeletal muscles in hypogonadal men with chronic obstructive pulmonary disease.

Hypogonadism, found in about one-third of patients with chronic obstructive pulmonary disease (COPD), has potential for decreasing muscle mass and muscle performance. Compared with eugonadal patients, we hypothesized that hypogonadal patients with COPD have decreased respiratory and skeletal muscle performance. Nineteen hypogonadal and 20 eugonadal men with COPD (FEV(1) 1.14 +/- 0.08 and 1.17 +/- 0.11 L [standard error], respectively) were studied. Diaphragmatic contractility, assessed as transdiaphragmatic twitch pressure generated by phrenic nerve stimulation, was similar in hypogonadal and eugonadal patients: 20.6 +/- 2.2 and 19.8 +/- 2.5 cm H(2)O, respectively. During progressive inspiratory threshold loading, hypogonadal and eugonadal patients had similar respiratory muscle endurance times (302 +/- 29 and 313 +/- 48 seconds, respectively) and airway pressure sustained during the last minute of loading (38.2 +/- 3.0 and 40.5 +/- 4.7 cm H(2)O, respectively) (similar to predicted values in healthy subjects). Hypogonadal and eugonadal patients had equivalent limb muscle strength and endurance. During cycle exercise to exhaustion, exercise performance, gas exchange, and respiratory muscle recruitment (estimated by esophageal and gastric pressure swings during tidal breathing) were similar in both groups. In conclusion, hypogonadism does not decrease respiratory or limb muscle performance and exercise capacity in men with moderate-to-severe COPD who, for the most part, are not underweight.

The effects of intermittent (trapezoidal) noise on man.

Eight healthy students were exposed to intermittent noise to evaluate the effects of traffic noise on man. The experimental noise exposure conditions were as follows: Exp. 1: Control. Exp. 2: Peak level, 80 dB(A); rise-decay time, 1 sec; duration, 3 sec; and duty cycle 8 sec (on fraction 25%). Exp. 3: Peak level, 75 dB (A); rise-decay time, 500 msec; duration, 4.5 sec; and duty cycle, 8 sec (on fraction 50%). In all experiments, the exposure time was 14 hr. Temporary threshold shift (TTS) growth was followed for 14 hr while saliva cortisol levels were measured for a 24-hr period. Although the coefficient of the regression line of TTS growth recorded in Exp. 1 or Exp. 3 was not statistically significant, the value in Exp. 2 was statistically significant. The circadian rhythm pattern of the saliva cortisol level (during a 24-hr period, mean of 8 examinees) was observed in the three experiments. Essentially, the early morning level of saliva cortisol was high, while the daytime level was relatively low.

Three-dimensional display of the bronchi using helical CT.

The purpose of this study was to evaluate the clinical usefulness of three-dimensional (3D) images of the bronchi obtained using helical CT. Thirteen patients with lung cancer, one with tracheal diverticulum, and one with bronchial amyloidosis were examined. The CT scanner employed was the Toshiba Xforce. The helical CT scan cycle consisted of 20 continuous rotations, each requiring 1.5 sec, for a total scanning time of 30 sec. Scans were obtained using a 5-mm X-ray beam width, a 5-mm/1.5 sec couchtop sliding speed, and a 2-mm reconstruction interval. 3D images were reconstructed using a CEMAX VIPstation. The optimal lower and upper threshold CT values for 3D images of the bronchi were -650 and -100 HU, respectively, and 3D images clearly depicted endobronchial lesions. Cartilage crescents were also demonstrated, but longitudinal and circular mucosal folds could not be visualized. In conclusion, 3D images of the bronchi acquired using helical CT were useful in evaluating endobronchial lesions.

Does anaerobic threshold correlate with maximal lactate steady-state?

The aim of this study was to compare the 'anaerobic threshold' (AnT) of subjects determined during a continuous 2-min incremental exercise test until exhaustion and the 'maximal lactate steady-state' (BLaSsmax) determined during prolonged exercise at constant loads corresponding to the subjects' AnT and/or 5-25% above and below it. Seventeen subjects performed an incremental exercise test and 1-5 prolonged exercise tests on a cycle ergometer until exhaustion at intervals of 1 week, and work rates, oxygen uptake (VO2) values and brachial venous blood lactate (BLa) levels were measured. It was proposed that when exercising at a constant workload below AnT, BLa would fall after having reached its peak; at the level of AnT, BLa reaches maximal steady-state (BLaSsmax); and above AnT, BLa increases continuously. Altogether, in 34 of 45 tests with a constant workload between 80 and 125% AnT, BLa values were as expected. In those cases in which BLaSsmax was reached, BLa increased on average by 3.8 mM from resting levels. This increase was 2.0 mM greater than that seen between resting levels and AnT during incremental exercise. There was no correlation between BLa values at BLaSsmax and at AnT, both when expressed as an increase in BLa (delta BLa) and absolute BLa concentration. Altogether, 81% of the variation in BLa concentration at BLaSsmax could be explained by the subjects' age, the percentage of slow-twitch fibres and BLa levels at rest. The AnT and BLaSsmax did not differ significantly, and these values were correlated (r = 0.83). Together, AnT and age accounted for 85% of the variation seen in BLaSsmax. The BLaSsmax did not correlate with AnT when fixed at a BLa concentration of 4 mM (AnT4mM). The three hypotheses tested in this study were confirmed, and the present results demonstrate that AnT correlates with BLaSsmax. The few exceptions to anticipated BLa kinetics were small in magnitude and could be explained by physiological variations.

Plant growth, development and embryogenesis during Salyut-7 flight.

The growth and geotropic movements of roots and hypocotyls of lettuce have been studied on board the Salyut 7 station in a stationary position and on the centrifuge at 0.01, 0.1 and 1 g. On the centrifuge at 0.1 and 0.01 g as well as under weightlessness, the final length of hypocotyls was by 8-16% greater than in control plants on the centrifuge at 1 g. The length of roots, however, was reduced by 17% at 0.01 g and under weightlessness; at 0.1 g their growth is much the same as at 1 g. On the Earth, while growing in a vertical position, and in space at 0 < or = g, the roots and hypocotyls deviate from the longitudinal axis of the seed. Average values of deviation eagles on the Earth are always equal to zero, while this is not always the case in space, which indicates the biological effect of microgravity conditions on board a spacecraft. The threshold of geotropic sensitiveness of lettuce hypocotyls, calculated from the linear regression parameters of the dependence of the response geotropic reaction upon the value of the centrifugal force, comprised 2.9 x 10(-3) g. In the Fiton 3 micro-greenhouse under spaceflight conditions, the plants of Arabidopsis thaliana (L) Heynh have, for the first time, undergone a full cycle of individual development. The seeds sown during the flight germinated, performed growth processes, formed vegetative and generative organs and, judging by the final result, they succeeded in fecundation, embryogenesis and ripening. Despite the noted modification of growth and development of plants in space, 42% of formed seeds appeared to be valuable biologically.

Effect of the total motile sperm count on the efficacy and cost-effectiveness of intrauterine insemination and in vitro fertilization.

OBJECTIVE: To determine prognostic factors for achieving a pregnancy with intrauterine insemination (IUI) and IVF. To compare the effectiveness and cost-effectiveness of IUI and IVF based on semen analysis results. DESIGN: Retrospective cohort study. SETTING: Academic university hospital-based infertility center. PATIENT(S): One thousand thirty-nine infertile couples undergoing 3,479 IUI cycles. Four hundred twenty-four infertile couples undergoing 551 IVF cycles. INTERVENTION(S): IUI and IVF treatment. MAIN OUTCOME MEASURE(S): Multiple logistic regression analysis was used to assess the significance of prognostic factors including a woman's age, gravidity, duration of infertility, diagnoses, use of ovulation induction, and sperm parameters for predicting the outcomes of clinical pregnancy and live birth rate after the first cycle of IUI and IVF. The relative effectiveness and cost-effectiveness of these treatments were then determined based on sperm count results. RESULT(S): Female age, gravidity, and use of ovulation induction were all independent factors in predicting pregnancy after IUI. The average total motile sperm count in the ejaculate was also an important factor, with a threshold value of 10 million. For IVF, only female age was an important predictor for both clinical and ongoing pregnancy. When the average total motile sperm count was under 10 million, IVF with ICSI was more cost-effective than IUI in our clinic. CONCLUSION(S): An average total motile sperm count of 10 million may be a useful threshold value for decisions about treating a couple with IUI or IVF.

Approaches for establishing predicted-no-effect concentrations for population-level ecological risk assessment in the context of chemical substances management.

The establishment of rational frameworks for population-level ecological risk assessment (PLERA) in the context of chemical substances management is an important issue. We illustrate two feasible approaches for establishing predicted-no-effect concentrations (PNECs)for PLERA through a case study of 4-nonylphenol (4-NP) using life-cycle toxicity data for medaka (Oryzias latipes). We first quantified the potential impacts of 4-NP on medaka in terms of reduction of population growth rate (i). An age-classified population matrix model (daily time-step) was developed and used to combine life-cycle survivorship and fecundity data obtained from individual-level responses of medaka expDsed to 4-NP into population-level responses defined by the parameter lambda. Thereafter, from the resulting lambdas, two approaches for establishing population-level PNEC values were proposed and examined. We then derived the PNEC values for population-level impacts, based on (a) the threshold concentration, defined as the chemical concentration at which lambda = 1 as a value with a 95% confidence interval, and (b) the no-observed-effect concentration (NOEC) and the maximum-acceptable-toxic concentration (MATC). The results suggest that PNEC values of 4-NP ranging between 0.82 and 2.10 microg/L affect medaka population growth. Although these approaches have their limitations, current knowledge indicates that they are reasonable and practical for evaluating population-level impacts of chemicals, thereby serving as a case study for establishing PNEC values for PLERA in the context of chemical substances management and decision-making.

Ghrelin response to acute aerobic exercise in boys at different stages of puberty.

The aim of this study was to investigate the changes in serum ghrelin and leptin concentrations during acute aerobic cycle ergometer test in 60 boys at different pubertal stages. Boys were divided according to their pubertal status as group I (Tanner stage 1, n=20), group II (Tanner stages 2 and 3, n=20) and group 3 (Tanner stages 4 and 5, n=20). Maximal oxygen consumption and individual ventilatory threshold of the subjects were measured directly using stepwise increasing loads on cycle ergometer. Second exercise test consisted of a 30 minute constant load exercise on the same ergometer at the level of approximately 95% of the individual ventilatory threshold. Venous blood samples were obtained before, immediately after and after 30 minutes of recovery for the measurement of serum ghrelin, leptin, testosterone and insulin. At baseline, prepubertal children had significantly higher values for serum ghrelin compared to the groups II and III. Acute exercise altered significantly only insulin concentration. In all the groups, the maximal oxygen consumption/kg correlated positively with basal levels of testosterone (r=0.60, p<0.001) and insulin (r=0.34), and negatively to ghrelin (r=-0.35) and leptin (r=-0.32) (p<0.05). We conclude that moderate acute aerobic exercise does not change serum ghrelin or leptin level in boys at different pubertal stages.

A prospective study evidencing rhinomanometric and olfactometric outcomes in women taking oral contraceptives.

BACKGROUND: The aim of this prospective study was to evaluate the changes in olfactory sensitivity of oral contraceptive (pill) users. METHODS: Sixty women underwent rhinomanometric and olfactometric determinations during the follicular, periovular and luteal phases of the menstrual cycle, and at day 7, 14 and 21 of contraceptive intake. Thirty-one women used 30 microg ethinyl oestradiol plus 75 microg gestodene and 29 women used 20 microg ethinyl oestradiol plus 150 microg desogestrel. RESULTS: Rhinomanometry showed higher but not statistically significant values during the periovular phase than in the follicular and luteal phases. Olfactometry showed a higher sensitivity during the follicular and periovular phases than during the luteal phase of the menstrual cycle. The rhinomanometric surveys in pill users were statistically different from those of the luteal phase (P < 0.02) and the follicular and periovular phases (P < 0.001). The olfactometric thresholds during the period of contraceptive use were statistically different from those of the follicular phase for a few odorous substances, and from those of the periovular phase for each odorous substance, but similar to those of the luteal phase (P = NS). CONCLUSIONS: Unlike the rhinomanometric airflow and trans-nasal pressure, the olfactory threshold to odours seems to depend on the variations of the ovarian steroids during the menstrual cycle and on the iatrogenic effects of oral contraceptives.

A discrete fully recurrent network of max product units for associative memory and classification.

This paper defines the truncated normalized max product operation for the transformation of states of a network and provides a method for solving a set of equations based on this operation. The operation serves as the transformation for the set of fully connected units in a recurrent network that otherwise might consist of linear threshold units. Component values of the state vector and outputs of the units take on the values in the set [0, 0.1,..., 0.9, 1]. The result is a much larger state space given a particular number of units and size of connection matrix than for a network based on threshold units. Since the operation defined here can form the basis of transformations in a recurrent network with a finite number of states, fixed points or cycles are possible and the network based on this operation for transformations can be used as an associative memory or pattern classifier with fixed points taking on the role of prototypes. Discrete fully recurrent networks have proven themselves to be very useful as associative memories and as classifiers. However they are often based on units that have binary states. The effect of this is that the data to be processed consisting of vectors in R(n) have to be converted to vectors in [0, 1]m with m much larger than n since binary encoding based on positional notation is not feasible. This implies a large increase in the number of components. The effect can be lessened by allowing more states for each unit in our network. The network proposed demonstrates those properties that are desirable in an associative memory very well as the simulations show.

Effects of acute hypoxia on the estimation of lactate threshold from ventilatory gas exchange indices during an incremental exercise test.

The purpose of this study was to investigate the validity of non-invasive lactate threshold estimation using ventilatory and pulmonary gas exchange indices under condition of acute hypoxia. Seven untrained males (21.4+/-1.2 years) performed two incremental exercise tests using an electromagnetically braked cycle ergometer: one breathing room air and other breathing 12 % O2. The lactate threshold was estimated using the following parameters: increase of ventilatory equivalent for O2 (VE/VO2) without increase of ventilatory equivalent for CO2 (VE/VCO2). It was also determined from the increase in blood lactate and decrease in standard bicarbonate. The VE/VO2 and lactate increase methods yielded the respective values for lactate threshold: 1.91+/-0.10 l/min (for the VE/VO2) vs. 1.89+/-0.1 l/min (for the lactate). However, in hypoxic condition, VE/VO2 started to increase prior to the actual threshold as determined from blood lactate response: 1.67+/-0.1 l/min (for the lactate) vs. 1.37+/-0.09 l/min (for the VE/VO2) (P=0.0001), i.e. resulted in pseudo-threshold behavior. In conclusion, the ventilatory and gas exchange indices provide an accurate lactate threshold. Although the potential for pseudo-threshold behavior of the standard ventilatory and gas exchange indices of the lactate threshold must be concerned if an incremental test is performed under hypoxic conditions in which carotid body chemosensitivity is increased.

CD34+ cell dose and CD33- subsets: collection and engraftment kinetics in autologous peripheral blood stem cells transplantation.

BACKGROUND AND OBJECTIVE: We analyzed the factors that affected the number and quality of peripheral blood stem cells (PBSC) collected for transplant in order to establish a minimum threshold for rapid hematopoietic recovery. DESIGN AND METHODS: From January 1995 to November 1996, a consecutive series of 67 patients, with hematologic and solid tumors underwent autologous PBSC transplantation. Collection of PBSC was performed after mobilization with granulocyte-colony stimulating factor (G-CSF) or with chemotherapy (CT) plus G-CSF. We calculated the factors that influenced PBSC collection, the kinetics of granulocyte and platelet recovery and the threshold value of CD34+ cells for a rapid recovery. The data were analyzed by means of multivariate Cox regression model and the receiver operating characteristic (ROC) methodology. RESULTS: Our results showed that mobilization with chemotherapy plus G-CSF was associated with a higher yield of PBSC in comparison with mobilization with G-CSF alone. Disease status, fewer cycles of conventional prior chemotherapy and absence of prior radiation therapy also influenced the yield of PBSC. The number of CD34+ cells, CD34+CD33- cell subsets, the mobilization schedule, and the conditioning regimen correlated significantly with time to hematopoietic recovery. In the multivariate analysis only the CD34+CD33- cell content and the total number of CD34+ were related with rapid neutrophil and platelet recovery, respectively. Use of G-CSF after transplant significantly shortened the neutrophil recovery time only in patients transplanted with suboptimal dose of PBSC. INTERPRETATION AND CONCLUSIONS: These data suggest the utility of quantitation of CD34+ cells subsets to predict quick engraftment.

[Cardiopulmonary exercise evaluation: value and influence of the use of a ramp protocol versus triangular protocol].

The aim of the study was to evaluate a ramp bicycle ergometer exercise protocol consisting of a very small but rapid increase in load (2 watts/12 seconds) for direct measurement of the VO2 max. Eighteen sedentary healthy subjects were in Group I (16 men, mean age 29 +/- 6 years) and 10 men with chronic stable cardiac failure in Group II (mean age: 51 +/- 16 years). All subjects performed two exercise tests within a period of 8 days, one with a ramp protocol (protocol R) and the other with a standard triangular exercise protocol (protocol T) of 30 watts/3 minutes for Group I and 20 watts/2 minutes for Group II. All procedures were performed until the subjects were exhausted. Direct measurement of VO2 max was performed by rapid response analysers enabling a cycle to cycle study. The parameters were measured and compared by a paired Student t test at each stage of the test: at rest, at submaximal exercise (third level), at anaerobic threshold and at maximal exercise. There was no significant difference between VO2 max and VO2 at the threshold of ventilation between the two populations. An increase in carbon dioxide production at different stages was observed with the ramp protocol. Furthermore, the highest maximal respiratory quotient and anaerobic threshold appeared earlier in Group II with the ramp protocol. Therefore, with respect to the standard triangular protocol, the ramp protocol provides comparable VO2 values. Its potential value, reported in the literature, is a better correlation between the observed and theoretical VO2 values. However, an increase in CO2 production should be underlined, probably related to greater recruitment of anaerobic metabolism in the absence of a steady state. This may be the reason for attaining the anaerobic threshold earlier.
