[Clinical aspect of herpes zoster and the pathogenesis of symptomatic cranial nerve zosters].

The prognosis of shingles is generally good, however if cranial nerves are affected, the course of disease may be very serious. It is still doubtful if latent virus-in the sense of a trigger mechanism-may be activated by dental surgical interventions and cause a symptomatic herpes zoster of cranial nerves. Serious loss of teeth as an oral manifestation of shingles, with panostitis and retarded healing show the seriousness of the tissue changes. The virus can invade the plexus between the facial and trigeminal nerves and can thus travel within the same epineural sheath from one nerve to the other. It follows that in each case of a herpes zoster of cranial nerves, irrespective of location, no dental surgery should be undertaken, even in the initial stage, because - it may increase the extension of the disease along the nerves - it may cause a viraemia - it may initiate uncalled for intraoperative and postoperative complications. Therapy with high doses of antibiotics is indicated.

Premature eruption of the premolars.

This paper presents a variety of cases in which very early loss of abscessed primary molars caused early eruption of the permanent successors. Clinical sequelae including ectopic eruption, alteration of eruption sequence, arch-length inadequacy and tooth impaction are illustrated by five case reports.

Teeth for life: oral disease prevention in research and practice.

All branches of dentistry have benefited in recent years from the rapid advances made in scientific research. Caries has been reduced in industrialized countries thanks to the combined efforts of epidemiologists, chemists, experimental and clinical scientists, identifying fluoride and refining fissure sealants. Genetic engineering may be pointing the way towards a possible caries vaccine as well as enhancing regimens against periodontal diseases. In addition research has provided advances in preventing and treating soft-tissue pathologies. Over the next 20 years a research and action programme aims to eliminate toothlessness in the USA.

[Dental study of hypophosphatasic child].

Hypophosphatasia is a hereditary disease characterized by congenital deficiency of tissue alkaline phosphatase. One of its dental features is the shedding of teeth. We have experienced a case which was referred to our clinic with the chief complaints of loss and shedding of lower deciduous central incisors and was diagnosed to be highly suspected of hypophosphatasia as a result of hematological examination at the pediatric clinic. As a result of close systemic and dental examination and further study of changes in the course for two and a half years, the following findings were obtained: 1. Low serum alkaline phosphatase level and hypercalcemia were the systemic findings of the disease. 2. There was no delayed ossification in terms of carpal bone age. 3. X-ray cephalography revealed no particular cephalic abnormality, except the lack of development of frontalis and mandibular. 4. Dental findings comprised marked resorption of upper alveolar bone in the front teeth area and pronounced instability of front deciduous teeth. 5. As for changes in oral symptoms, extraction of A because of pronounced instability was followed by spontaneous shedding of A. About one year later, A was extracted because of marked instability. At the time of this writing, A was slightly instable without any abnormality in the other teeth. 6. Pathohistological study of the extracted teeth disclosed only a little resorption of cement of root apex and dysgonic periodontium without any marked tissue changes.

["Tooth mortality" in a Marseille S.C.T.D. The effect of periodontal disease: comparison with some results of a national epidemiological study].

1,247 avulsions have been noted for a year in the S.C.T.D. (tooth, age and sex of the patient). 44.6% were caused by tooth decay. However periodontal disease was responsible for 39.5% of tooth avulsions. The tooth lost because of periodontal disease were in most part extracted after 40 years. A weakness of some type of teeth regarding tooth mortality may be quoted. A sexual dimorphism of tooth mortality was present in the studied population.

[Prevalence of first permanent molar loss in 6 to 12-year-old students of both sexes in Ribeirão Preto].

The authors study the prevalence of lost of the first permanent molars in 6 to 12 years old schoolchildren of medium and low social-economic levels, in both sexes, from Ribeirão Preto, SP., on right and left hemiarcs, and both maxilars. The results indicate that teeth los was significantly higher in females, and in both sexes the teeth lost was higher in the lower maxilar. No differences were also observed between hemiarchs.

[Dental caries in preschoolers and students in Monte Patria, region 4].

An epidemiological study on caries prevalence was undertaken on school children and pre-school children aged from 2 to 14 years, of Monte Patria, a small city located in the North of Chile, following the criteria established by the World Health Organization. It was found that the 90.54% of the studied sample had caries, the 21% of the patients lost permanent teeth due to caries, and the presence of filled teeth was detected only in 1.75% of the children. DMFT index of the sample was 5.16 and dmft index was 4.60.

[Dentigerous cyst. Review of the literature and report of a case].

A case of circumferential dentigerous cyst is presented, with a review of the literature. This lesion occurs when the tooth is exfoliated through the cystic epithelium surrounding a crown, so that the cyst remains within the cervical third. When these teeth are extracted without previous diagnosis, the cyst could become residual. In all dentigerous cyst, the microscopic features must be determined, since some reports mention their eventual transformation in an ameloblastoma or, even worse, in a squamous-cell carcinoma.

Patterns of alveolar bone loss in Nigerian Yorubas.

Unusual patterns of alveolar bone loss in 33 Nigerian Yorubas with severe periodontal disease are described. The subjects, aged 15-50 yr represented 1.4% of a population of dental patients in Western Nigeria. Periodontal pocketing and alveolar bone loss in the incisor and first molar regions, far in advance of other mouth areas, were the criteria for inclusion in this report. Intra-oral radiographs showed advanced horizontal or crescentic bone resorption in incisor regions, and vertical or arc-like bone defects around first molars. Incisors and first molars were frequently missing as a result of periodontal disease. Surveys of tooth mortality and periodontal pocketing in Western Nigeria, and immunological findings elsewhere, suggest these cases represent a severe form of plaque-induced inflammatory periodontal disease. The role of protein-calorie malnutrition as an aetiological factor is discussed.
