The pathophysiology of the juvenile bunion.

While the surgeon may tend to use one procedure in the repair of a hallux valgus deformity, versatility is most important when treating the juvenile bunion. Using a distal soft-tissue repair when subluxation is solely at the metatarsophalangeal joint is an acceptable approach. A metatarsal or cuneiform osteotomy is necessary if the intermetatarsal angle is abnormally large. It is important not to stretch the indications for a bunion technique in order to correct the hallux valgus deformity. If a more severe deformity is present, a more aggressive technique must be used to correct the abnormality. That varying success rates are reported with different techniques testifies to the fact that the juvenile bunion is not suited for a standard hallux valgus repair. The surgical technique used to repair a specific juvenile bunion depends upon the anatomic and physiologic abnormalities present in each patient.

Surgical treatment of arthritis in the elderly.

Arthritis in the elderly can be very effectively treated by a combination of careful orthopaedic and rheumatological assessment. The overall functional assessment, and definition of the aims and likely results of surgical treatment by the team approach, involving occupational therapists, physiotherapists and social workers, are important for success. There are many worthwhile surgical procedures available to relieve pain, and restore mobility and independence to those affected by osteoarthritis and rheumatoid arthritis.

A statistical analysis on the reliability of the proximal articular set angle.

The authors investigated the statistical relationship and significance between the transverse and sagittal plane proximal articular set angles both radiographically and intraoperatively. The analysis revealed a highly significant difference between the means of the respective measurements. Although the radiographic and intraoperative findings were found to be related for the transverse plane proximal articular set angle, the transverse plane PASA was approximately 7 degrees greater when measured intraoperatively. The concept of a sagittal plane PASA was also introduced. The significance of accurate measurements in the surgical correction of hallux valgus was also examined.

Digital fractures and dislocations. Diagnosis and treatment.

A detailed discussion of the often underestimated dilemma of digital fractures and dislocations is presented. Illustrations accentuate the important concepts. An aggressive attitude should be adopted to avoid mismanagement and prolonged convalescence. Should the need arise, surgical intervention is emphasized.

Metatarsophalangeal fusion for hallux valgus: indications and effect on the first metatarsal ray.

Arthrodesis of metatarsophalangeal joint was performed in 31 cases of severe hallux valgus. With the surgical technique used, which is described in detail, internal fixation and plaster cast immobilization were not necessary. Analysis of the results suggested that the operation is indicated for severe deformities in elderly patients, preferably women. The basic metatarsal deformity was corrected by the procedure.
