Dynamic magnetic resonance sialography as a new diagnostic technique for patients with Sjögren's syndrome.

OBJECTIVE: To evaluate the clinical utility of dynamic magnetic resonance (MR) sialographic images as a diagnostic tool for patients with Sjögren's syndrome. METHODS: The morphological findings and various kinds of functional parameters in volunteers on dynamic MR sialographic images were compared with those in five patients with definite Sjögren's syndrome. RESULTS: On the MR sialographs of all five patients with Sjögren's syndrome, the so-called 'apple-tree appearance' was seen. The difference in two functional parameters using the dynamic MR sialographic data was elucidated between the two groups. The maximum area of the detectable ducts in the group of patients was significantly smaller (P < 0.001) than that in the group of volunteers. The ratio of change in the detectable ducts in the group of patients was significantly lower (P = 0.011) than that in the group of volunteers. CONCLUSIONS: Our study suggests that dynamic MR sialographic data in addition to MR sialographic images might be useful for the diagnosis of Sjögren's syndrome.

[Role of diagnostic imaging in chronic recurrent parotitis in childhood].

BACKGROUND: Chronic recurrent parotitis represents a disease of still uncertain etiology that can be divided into a childhood and an adult form. Sialography is still the imaging procedure of choice. METHOD: We report about a nine-year-old girl who had had six episodes of parotitis in the past two years. MR sialography is presented as an alternative to conventional sialography. MR was performed with a 1.5 T wholebody tomograph using a high resolution 3D-CISS sequence. RESULTS AND CONCLUSION: MR sialography does not require any contrast medium. It primarily images liquid structures, and the flow of saliva can be adequately demonstrated after stimulation with ascorbic acid. These special features of MR sialography allow its use also during acute episodes of sialadenitis, thus providing a significant improvement over conventional sialography. Furthermore, no overlapping occurs as in conventional summation images, due to the multiplanar reconstruction features.

[A diagnostic and etiologic studies of Sjögren's syndrome--I. Sialographical and histopathological findings of the major and minor salivary glands].

This work was an attempt to elucidate the diagnostic evaluation of sialography+ and histopathological examination of the salivary gland in Sjögren's syndrome. The author investigated the correlation of the sialography and histopathology features of the salivary gland in 89 confirmed cases of Sjögren's syndrome and 26 suspected cases, a total of 115 subjects and the following results were obtained. 1) In determining the histopathological severity of the salivary gland lesions from sialography, the parotid sialography was more useful than the submaxillary sialography. 2) The severity of parotid sialography correlated well that of labial gland histopathology. 3) Biopsy of the labial gland was useful for histopathological diagnosis on Sjögren's syndrome but the histopathological change occurred to a greater degree in the submaxillary gland than in the labial gland. 4) From above results, in histopathological examination of severe case of Sjögren's syndrome, biopsy of the labial gland will be sufficient, but biopsy of the submaxillary gland must be employed for studying mild cases.
