Epidemiology of anaerobic infections.

The clinical importance of the anaerobic organisms, especially the toxicogenic Clostridia and some of the nonsporulating anaerobes, has been recognized for some time. Only within the last 20 years, however, owing to improved methodology, have gram-negative anaerobic bacilli, anaerobic cocci, and streptococci been commonly recognized and encountered in clinical infections. Today, anaerobic organisms are common isolates from infections involving intra-abdominal sites, the female genital tract, soft tissue, and oral areas and from major infections involving the lung, brain, and head and neck. Most of these infections are polymicrobial--involving both anaerobic and facultative or aerobic organisms. In some instances, it is difficult to ascertain what role is played by what organisms. No doubt, synergism is present in many cases. Because anaerobes are prevalent normal flora of the body, almost all anaerobic infections are of endogenous origin. Many of these anaerobes are opportunists; given the appropriate set of conditions, they will penetrate tissue and cause infection. Many have been associated with wound infection subsequent to bowel surgery or trauma, tubo-ovarian abscess, perirectal abscess, subphrenic abscess, postabortal sepsis, appendicitis, and many other infectious conditions. This article reviews the distribution of anaerobes in infected hospitalized patients and their relation to infection over a 5-year period.

Risk factors in first-trimester abortion.

Among 238 women who underwent first-trimester abortion and who were randomized to the placebo group in a clinical controlled trial we studied the possible correlation of the variables age, parity, number of previous spontaneous and induced abortions, previous pelvic inflammatory disease (PID), gestational age, chronic pelvic pain, dyspareunia, dysmenorrhea, social status, and the application of an intrauterine device (IUD) at abortion - to the number of days with pain, bleeding, discharge, fever, absence from work, and day of first coitus after abortion. The ANOVA test of Kruskal-Wallis with the limit of significance p less than 0.05 was employed. Women with one or more previous spontaneous abortions had significantly more days with postabortive bleeding (p = 0.010). Women with previous PID and women with dysmenorrhea had significantly more days with pain after abortion (p = 0.011 and p = 0.001). Women at a gestational age of 11-12 weeks had significantly more days with fever (p = 0.009). Women who had an IUD inserted at abortion suffered more days with pain and bleeding (p = 0.038 and p = 0.043). No one group of women carried a risk of several severe complaints after abortion except those with a history of PID.

Urogenital mycoplasma infections of man: a review with observations on a recently discovered mycoplasma.

Ureaplasma urealyticum organisms (ureaplasmas), Mycoplasma hominis, M. fermentans, M. primatum, M. Salivarium and M. pneumoniae have been isolated from the genitourinary tract. The first two of these microorganisms are found most frequently. M. hominis is a cause of some cases of postpartum and postabortal fever, acute pyelonephritis and pelvic inflammatory disease. Ureaplasmas have been associated with chorioamnionitis, habitual spontaneous abortion, low birthweight, the urethral syndrome in women, and nongonococcal urethritis (NGU) in men; but the difficulty of proving an etiological relationship is emphasized. However, in NGU there is accumulating evidence to indicate that ureaplasmas cause some cases. Some patients suffering from NGU, from whom ureaplasmas, mycoplasmas and chlamydiae cannot be isolated, respond to tetracycline therapy. This has suggested that a tetracycline-sensitive microorganism might be responsible. In this context, the isolation of a glucose-metabolizing mycoplasma from the genitourinary tracts of 2 of 13 men with NGU is of interest. This mycoplasma, serologically different from all other tested, has the structural and biological features of a pathogenic organism.

[Colposcopically "suspicious" and atypical findings from portio vaginalis uteri of women in pregnancy (author's transl)].

Described in this paper is the phenomenon of "suspicious" colposcopic findings and their histomorphological relevance, with reference being made to colpophotographic follow-up checks of pregnant women over several years. Colposcopy has been upgraded, in recent time, and it is against this background that its potential efficacy and limitations are assessed and compared to cytological techniques, on the basis of "suspicious" colposcopic findings. A general assessment also is made of the diagnostic expectations which can be hinged on either method of tracing. Twenty in 450 cases of carcinoma in situ and severe dysplasia were combined with processes of pregnancy, between 1962 and 1979, among them four cases which had been under colposcopic and cytological monitoring. In two of these cases constant behaviour of the epithelium was established up to postpartal or postabortal confirmation by cytograms, while the other two cases were thought to be attributable to gestational developments, with tissue being suspicious or progressive growth.

Emotional distress patterns among women having first or repeat abortions.

Thirty-five percent of a sample of 413 women undergoing first-trimester abortions were repeating abortions. All patients rated their emotional symptoms on an SCL-90 scale and completed a brief demographic questionnaire. Preabortion and postabortion emotional distress factors and associated demographic characteristics were compared for women having first and those undergoing repeat abortions. Elevated distress levels were similar in both groups prior to abortion procedures, particularly depression, anxiety, and somatization. After abortion, repeat aborters continued to have significantly higher emotional distress scores in dimensions relating to interpersonal relationships. The variables that discriminated most between first and repeat abortion groups were number of living children, race, and phobic anxiety.

Tubal ring sterilization: experience with 10,086 cases.

The safety and efficacy of the Silastic ring were examined for interval, postabortion, and postpartum sterilizations. The study includes an in-depth analysis of 10,086 cases generated by 51 collaborating institutions in 23 countries. Failure of the tubal ring is studied via the life-table method, although follow-up is limited to a relatively short period. Difficulties encountered when applying the ring, particularly those resulting in technical failure, are explored in detail. Surgical complications, infection, menstrual irregularity and abnormal bleeding, and pelvic surgery following sterilization are discussed. Comparisons are made among various approaches to the fallopian tubes. Such data are found to be similar to those for other occlusion techniques, demonstrating the comparable safety and efficacy of tubal ring sterilization.

Early medicine and surgery in Calgary.

The medical history of the Calgary area can be divided into three periods: the early period before the coming of the North West Mounted Police in 1874, the middle period from 1875 to 1905, during which Fort Calgary was built and the first surgeons settled in the West, and the third period from 1905 to the present. George Alexander Kennedy was the first surgeon of the middle period. He encouraged proper sanitation, fresh air, and avoidance of cold, damp and dirt. He recommended special treatment for various systemic diseases, used chloroform in October 1880 and was known to have treated skull fractures, extradural hemorrhages, prostatic obstruction, postabortion hemorrhage, ascites and pleural effusion. Henderson, Brett, Lindsay, Rouleau, Lafferty and Mackid were the other six pioneer surgeons of the middle period. Mackid's son Ludwig took over his father's practice after studying in Europe and the United States. When he died in November 1975 at the age of 93, the last link with the pioneer surgeons of Calgary was broken.

A 10-year review of maternal mortality in Chon Buri Hospital, Thailand.

1. The overall maternal mortality rate (MMR) in Chon Buri Hospital in the 10-yr period from 1982-1991 was 51.1/100,000 livebirths. 2. The top causes of death were abortion related complications, pregnancy induced hypertension, puerperal infection and postpartum hemorrhage. 3. What we have done is to improve the quantity and quality of obstetric and medical care, solve the problem of vital statistics reports in our hospital, contact doctor in nearby hospitals in referral and interhospital OB-GYN conferences to meet and discuss both knowledge and management problems. 4. What we still faced in the last 4-yr were deaths from abortion related complications, puerperal sepsis and postpartum hemorrhage. Most of the deaths were preventable. 5. So what we have to target to lessen the MMR is to improve the obstetric and medical care, improve the quality of medical personnel in our area in KAP aspect (knowledge, attitude, practice) especially in the field of family planning to prevent unwanted pregnancies, proper prevention and management of postpartum hemorrhage, and prevention and treatment of puerperal and postabortal infection.

[Anaerobic bacteria and gynecologic infections].

The normal vaginal flora consists of a wide variety of bacteria and the composition of this microflora is influenced by hormonal, metabolic and pharmacological factors. An increased concentration of a mixed anaerobic flora (Mobiluncus, Prevotellae, Peptostreptococci and Gardnerella vaginalis) appears to be an early stage in the development of bacterial vaginosis or in the infection of the upper genital tract. Women with bacterial vaginosis are at increased risk of bartholinitis, skenitis, pelvic inflammatory disease, postpartum and postabortal endometritis, chorioamnionitis and infections following gynecologic surgery or diagnostic procedure.

Follow-up of 50 adolescent girls 2 years after abortion.

Thirty-eight adolescents who underwent an abortion were studied by questionnaire and interview with a psychiatrist and a social worker 2 years after the abortion. Most did not regret their abortion and considered it a positive experience. Most said they would not have another abortion, although adoption was unanimously rejected as a choice for the pregnant teenager. Relations with their parents were generally good and the families were supportive in the decision-making process and in the postabortion period. The girl's father had been absent because of death or separation in 37% of instances. A lengthy relationship with the putative father before the abortion was common, but 37% of the relationships were not able to withstand the pregnancy-abortion crisis. The proportion using contraceptives before the abortion was 2%, and 2 years after the abortion, 84%.

[First trimester sonographic diagnosis of acrania].

Anencephaly, a well-known lethal fetal malformation, was long considered to result from primary nonclosure of the neural tube. In the past few years other pathogenic mechanisms, such as reopening or degeneration of a closed neural tube have been suggested. High-resolution transvaginal ultrasonography, which provides fine visualization of the different stages in embryogenesis, allowed us to detect fetal acrania as early as the 12th week of gestation. Very high levels of alpha-fetoprotein, almost undetectable levels of unconjugated estriol (E3), and postabortion histology were consistent with anencephaly, suggesting that anencephaly is the end result of fetal acrania.

Differential diagnosis of the nonvisualized fetal urinary bladder by transvaginal sonography in the early second trimester.

OBJECTIVE: To describe the pathologic situations associated with the persistently nonvisualized fetal urinary bladder in the early second trimester. METHODS: We analyzed retrospectively 13,458 ultrasound examinations performed between 12-16 weeks' gestation in search of fetal malformations. Seventy percent of the patients were at low risk and 30% comprised a high-risk group for the detection of fetal anomalies. RESULTS: Seven cases of persistent nonvisualization of the fetal urinary bladder were detected by transvaginal sonography among 13,458 fetuses. In one case bladder exstrophy, and in another bilateral multicystic dysplastic kidneys were diagnosed by postabortal examination. Five cases were associated with bilateral renal agenesis. CONCLUSION: When one fails persistently to demonstrate the fetal urinary bladder during an examination lasting 30 minutes or longer, urinary tract pathology should be highly suspected. Failure to visualize the bladder may be associated with bladder exstrophy, bilateral renal agenesis, or late onset of bilateral multicystic dysplastic kidneys.

Elective abortion. Complications seen in a free-standing clinic.

Experience is reported with the elective first trimester abortion of 16,410 pregnancies during a 31-month period by Reproductive Health Services of St. Louis, a free-standing clinic. Incidence of complications was 1.54%. The most common of these were incomplete evacuation, excessive postabortal bleeding, and uterine perforation. In patients with unquestioned perforation, the use of laparoscopy has been very valuable in ascertaining the exact nature of the perforation, in avoiding unnecessary laparotomy, and in giving intraabdominal visual guidance to concomitant suction evacuation in cases of an incomplete procedure. Aspects of the other complications are also discussed. In general, the findings support the view that even in the first trimester, the earlier in pregnancy that suction abortion is performed, the less likely it is to result in major complications. It is possible to perform first trimester abortions in a free-standing clinic with a satisfactorily low complication rate.

The safety of local anesthesia and outpatient treatment: a controlled study of induced abortion by vacuum aspiration.

The safety of early vacuum aspiration performed as an outpatient procedure using local anesthesia was examined in a controlled study sponsored by the World Health Organization and undertaken in two centers in Yugoslavia and Singapore. The sample consisted of healthy women whose pregnancies were between 7 and 12 weeks in duration. Follow-up four weeks after abortion was nearly 100 percent in both centers. No important differences in complication rates were found between the different types of services and anesthesia, even though overall complication rates differed markedly between centers. Overnight postabortion observation was not associated with any decrease in complications.

The quality of abortion services in Turkey.

The quality of abortion in Turkey's public sector hospitals is investigated using multiple research methods including observations of actual procedures, interviews with medical staff and clients, and an inventory of materials and equipment. The results suggest that women who obtain abortions in public sector hospitals have a higher abortion rate than the general population. They are likely to receive a vacuum aspiration with minimal pain control medication. Clinical infection control procedures are insufficient, as are interpersonal communication practices including counseling on reproductive health issues and providing factual information. Links with family planning services are strong and the majority of abortion patients who desire family planning receive a contraceptive method, either through direct provision of postabortion contraceptives or referral.

[The application of ultrasonography: uterovaginal and pelvic masses in black African women].

Echography was prospectively performed in 345 black African female patients with pelvic masses which had been clinically detected in some cases. The women were between 11 and 65 years old. The aims of this study were to list the pelvic masses observed in black. African women, to record any potentially distinguishing features and to evaluate the sensitivity and the specificity of echography as applied here. A total of 477 masses were listed. Ovarian masses were the most frequent (56.66%) followed by uterine masses (31.45%). Most masses were benign. We observed only two cases of ovarian cancer. The sensitivity was 100% and the specificity was 97.69%. Eleven cases (2.3%) of the ultrasound diagnoses did not correspond with the final diagnoses. As for distinguishing aspects, these patients were younger than European patients-this was particularly true for patients with fibroid masses (a range of 31 to 40 years compared to 40 to 50 years). Genetic factors in these patients could explain the high rate and early appearance of fibroids. Because the diagnosis of vaginal masses is well established by clinical examination rather than by echography, this lesion was rarely observed. The most frequent clinical complaint was pelvic pain (36.94%), followed by pelvic mass impression (28.65%). We found high rates of both past histories of miscarriage and abortion (60.7%) and postabortion pelvic hematoma. The hematomas comprised 45.4% of the non-genital pelvic masses. Misdiagnosis of pyosalpinx was often a source of diagnostic error. Thus, to avoid false diagnoses of pyosalpinx in evaluating pelvic masses with a void in ultrasound, the possibility of an infectious episode should be evaluated by definitive questioning. Ultrasound examination is of value in evaluating the pelvic masses of women; however, this should be considered within the appropriate disease context.

Managing residual trophoblastic tissue. Hysteroscopy for directing curettage.

OBJECTIVE: To describe our experience with selective removal of residual intrauterine trophoblastic tissue via hysteroscopy. METHODS: This is a descriptive report. Eighteen patients, 16 postabortion and 2 postpartum, underwent a hysteroscopic procedure for removal of residual trophoblastic tissue causing continuous bleeding. At hysteroscopy, a cutting loop was used as a curette for selective removal of the adherent residual tissue, while interference with the rest of the endometrial surface was avoided. RESULTS: Complete removal of the suspected residual tissue was achieved in all patients. Histology confirmed the curettings as trophoblastic remnants. No complications were reported during or immediately after the procedure. The median operative time was 10 minutes (range, 8-20). In all cases the bleeding stopped shortly after the procedure. In each patient, postoperative ultrasonography revealed a uterine cavity free of residual tissue. Five of the patients underwent second-look hysteroscopy several weeks later, and no signs of further residual tissue were observed. CONCLUSION: Selective curettage of residual trophoblastic tissue directed by hysteroscopy is an easy and short procedure and might be preferable to conventional, nonselective, blind curettage.

[HELLP syndrome in the 21st week of pregnancy in mosaic trisomy 9].

We report for the first time the case of postabortional HELLP-syndrome in the 21st week of gestation. In this case mosaic trisomy 9 was confirmed by amniocentesis prior to induction. Pertinent history, clinical course and pathoanatomical morphology are described. We emphasize the early onset of the HELLP-syndrome in association with trisomy 9 after abortion. The possibility of interconnections between trisomy 9 and the occurrence of HELLP-syndrome (sparse blood, vessels in the villi, circulatory deficit on the fetal side of the placenta, increased production of e.g. vasopressive substances) is discussed.
