Synchronous solitary fibrous tumor of the pleura and lung cancer.

We report herein on a 57-year-old woman with comorbid malignant solitary fibrous tumor (SFT) of the pleura and adenocarcinoma of the lung. To the best of our knowledge, this is the first report of a patient presenting with these two pathological entities simultaneously. The patient was treated successfully for both diseases via a one-stage operation through median sternotomy with good results. Although the incidence of multiple primary malignancy is rare, clinicians should be cautious not to discount the possibility of two coexisting primary malignancies.

[Comparative studies of immunologic characteristics in plasmapheresis and hemosorption in patients with purulent diseases of the lungs and pleura].

The article deals with the comparative characteristics of the changes in immunological indices in 40 patients with non-specific purulent diseases of the lungs and pleura when plasmapheresis and hemosorption were included in the complex of therapeutic measures. The changes were found to be hetero-directional in character: a non-specific effect of immunostimulation in hemosorption, and immunodepression, particularly marked in the first 24 hours, in plasmapheresis. The post-aggressive reaction after plasmapheresis is either absent or delayed significantly, which makes it possible to conduct detoxification in extremely grave conditions of the patients without making the mechanisms of the post-aggressive reaction operative, as observed in performance of hemosorption.

Solitary fibrous tumor of the pleura with hypoglycemia: the Doege-Potter syndrome.

The occurrence of hypoglycemia with an intrathoracic tumor was reported in 1930 independently by Doege and Potter. Since then, the presence of a nonpancreatic tumor associated with low blood sugars has often been referred to as the Doege-Potter syndrome. We report a patient with a recurrent solitary fibrous tumor of the pleura who experienced symptomatic hypoglycemia attributable to his tumor.

Recurrence of a solitary fibrous tumor of the pleura: a case report.

We report a case of an 80-year-old female with two prior thoracotomies for benign solitary fibrous tumor of the pleura (SFTP) presenting with a two-month history of shortness of breath. Computed tomography revealed a pleural-based recurrence and operative excision revealed multiple adherent tumors throughout the thoracic cavity. Pathologic examination demonstrated malignant degeneration of this previously benign tumor. We consider the importance of recurrence of benign SFTP and the significance of surgical care and follow up of patients with this rare tumor.

Primary pulmonary botryomycosis with parietal pleura involvement.

Botryomycosis is a rare chronic suppurative disease characterized by the presence of eosinophilic granules containing the causative bacteria on histological study of an abscess specimen. The majority of cases involve the skin and visceral involvement is rare. A 54-year-old man with primary pulmonary botryomycosis which involved parietal pleura presented with exertional dyspnea for 2 weeks. Echo-guided aspiration of lung mass yielded viridans streptococci and positive Gram stain of the surgical specimen indicated viridians streptococci was the causative pathogen. There was no recurrence during 2 years of follow-up after surgery and antibiotic treatment. Early differentiation of botryomycosis from lung cancer, pulmonary tuberculosis, and actinomycosis is essential.

Primary malignant lymphoma arising in the pleura without preceding long-standing pyothorax.

We report a very rare case of primary malignant lymphoma arising in the pleura with no history of persistent pyothorax. A 72-year-old male was hospitalized with dyspnea on effort and chest CT demonstrated a mass along the right chest wall. Right thoracotomy with complete en bloc resection of the pleural tumor was performed. Immunohistochemical examination of the pleural tumor showed that the histology was marginal zone B-cell malignant lymphoma. We considered that this tumor had originated from the soft tissue in the chest wall based on radiographical and surgical findings. As diagnosis and treatment of pleural malignant lymphoma seems to have been difficult in most cases reported in the literature, it is thought that early active and accurate biopsy with large-bore needles, or, if possible, surgical incision for early diagnosis and aggressive surgery to achieve complete resection combined with radiation therapy and/or chemotherapy would be very important for a good prognosis.

[Treatment of acute pyoinflammatory diseases of the lungs and pleura under control of microorganism persistence factors].

Results of treatment of patients with acute pulmonary abscess, acute empyema of the pleura and pyopneumothorax were analyzed. The local treatment of the main group of patients consisting of 50 persons included oxitocin in combination with antiseptics and antibiotics which facilitated maximal reduction of the persistent potential of the pathogenic agent, its rapid elimination from the purulent focus and allowed to get better results using conservative therapy. The obtained clinical effect confirmed the experimental data on pronounced inhibition of persistent properties of the bacterial pathogens with the complexes "antibiotic-oxitocin".

[Solitary fibrous tumor of the pleura in 75-year old woman].

A 75-year-old asymptomatic woman with stable coronary heart disease presented tumor in lower left lobe on routine chest radiograph. A CT scan showed a large sharply delineated mass at this site (84 x 52 x 90 mm). There were no signs of infiltration, no abnormalities were seen in mediastinal structures and on the right side. The pedunculated tumor was resected during left thoracotomy (posterolateral incision). Histological examination revealed spindle-like cells and rich collagen net. Mitoses and necrosis were absent. Final diagnosis was: solitary fibrous tumor of the visceral pleura. During 7-year follow-up recurrence was not observed.

[Cytokines in complex treatment of posttraumatic empyema of the pleura].

Results of treatment of 114 patients with posttraumatic empyema of the pleura (EP) after closed injury of the thorax (62), stab-incised (43) and missile (9) wounds are presented. In complex treatment of 25 patients one of cytokines - recombinant interleukin-1v (betaleukin) -- was administered intravenously (10 patients) and intrapleurally (15 patients). Control group consisted of 35 similar patients treated with traditional methods including drainage and sanitation of empyema's cavity with protheolytic enzymes, antibacterial and detoxication therapy. Comparative analysis has demonstrated that patients of the study group showed fast normalization of immunogram's parameters, decrease of duration of purulent process (51+/-4 days, on the average, in the control group and 28+/-3 days in the study group) due to fast obliteration of purulent cavity, and decrease of rate of chronic forms of EP (from 20 to 4%) which required surgical treatment.

The spectrum of histologic growth patterns in benign and malignant fibrous tumors of the pleura.

A review of the histologic growth patterns in 50 cases of benign and malignant fibrous tumors of the pleura (localized or solitary fibrous tumor, fibrous mesothelioma) is presented. Two major histologic growth patterns were observed admixed in various proportions: solid spindle and diffuse sclerosing. The solid spindle growth pattern assumed various configurations, including fascicular areas, storiform and herringbone formations, angiofibroma and hemangiopericytoma-like areas, synovial sarcoma-like areas, and neural-type palisading, thus simulating a variety of soft-tissue neoplasms. The diffuse sclerosing pattern, although rarely assuming a dominant role, was present in varying proportions in virtually all cases. In areas with extensive sclerosis, focal degeneration of collagen simulating tumor necrosis was often present. Other less frequently observed features were the formation of "amianthoid" fibers, multinucleated giant cells, and foci of metaplastic ossification. On ultrastructural and immunohistochemical examination, the tumor cells showed nondistinct features. Due to the extreme variability in morphologic appearances and the lack of distinctive ultrastructural or immunohistochemical characteristics, these tumors can pose a significant diagnostic problem. Familiarity with their histologic appearances and correlation with the gross findings and clinical setting are essential for arriving at the correct diagnosis.

[Use of laser irradiation for the prevention and treatment of complications following surgery of the lungs and pleura].

Low-energy laser irradiation was used in treatment of 77 patients with surgical diseases of the lung and pleura. Laser therapy was performed by means of transcutaneous irradiation of the patients with the use of a semiconductor arsenide-gallium laser and influencing with helium-neon laser on the biologically active points. The use of laser therapy contributed to decrease in incidence of postoperative complications, shortening the duration of treatment of the patients at a hospital.

[Disorders of erythrocyte deformability in patients with acute suppurative-septic diseases of the lung and pleura].

In 163 patients with acute suppurations of lungs and pleura a substantial reduction of deformability of erythrocytes was established. The maximum increased rigidity of blood red cells was noted at the peak of the pyo-destructive process and postoperative period as well as in the complicated course of the postoperative period. The curative plasmapheresis, general hyperbaric oxygenation exert positive influence upon plastic properties of erythrocytes.

[Solitary fibrous tumor of the pleura (SFTP) hidden in the pleural effusion].

UNLABELLED: The patient with a non-malignant type of solitary fibrous tumor of the pleura (SFTP) has been presented as an example; this tumor manifested itself in long-lasting pleural effusion. The tumor, submerged in the liquid, with the diameter of 16 cm, was invisible in standard and computer X-rays of the chest. The tumor was found and removed during the thoracotomy. The patient has been alive for 9 years, without symptoms of the disease since the operation was performed. CONCLUSIONS: 1. The source of one-sided exudation can be a non-malignant SFTP. 2. Tumor submerged in the liquid can be invisible on standard and computer pictures of the chest.

[Malignant mesothelioma of the pleura. Analysis of its immunohistochemical aspects].

To evaluate the usefulness of immunohistochemistry in the diagnostic distinction between pleural mesothelioma and metastatic adenocarcinoma to the pleura, the authors studied formalin-fixed paraffin-embedded tissue sections from 14 pleural mesotheliomas and 20 primary adenocarcinomas of the lung, stomach, ovary and breast by using 16 commercially available antibodies to cytokeratin (KL1), vimentin, EMA, CEA, CA19.9, CA125, Egp 34 (detected by HEA 125), secretory component, S100 protein, SP1-béta 1, Leu M1, alpha-1-AT, alpha-1-ACT, lysozyme, desmin and factor VIII. Keratin positivity was found in all mesotheliomas and adenocarcinomas. A coexpression of keratin and vimentin was present in 8/14 (57%) mesotheliomas but only in 2/20 (10%) adenocarcinomas. CEA and CA 19.9 were detected in 80% and 65% of the adenocarcinomas respectively, but not in any of the mesotheliomas. Interestingly, two adenocarcinomas (of the ovary and the stomach) that failed to stain for CEA, were immunoreactive to anti-CA 19.9 antibody. Thus, the combined use of anti-CEA and anti-CA 19.9 antibodies results in staining 90% of the adenocarcinomas. S100 protein, SP1-beta and Leu M1 were also absent in mesotheliomas but present only in less than half of the adenocarcinomas. Adenocarcinomas and mesotheliomas did not significantly vary in reaction to the remaining above mentioned antibodies. The authors conclude that the coexpression of keratin and vimentin and the absence of CEA and CA 19.9 might be the best criteria in the distinction of mesothelioma from metastatic non mucosecreting adenocarcinoma.

[Clinico-immunological indices, lipid peroxidation and contents of middle mass molecules in young children with different course of suppurative diseases of the lungs and pleura].

The results of examination of 83 children with purulent diseases of the lungs and pleura at the age under 6 yrs are presented. The indices of activity of the immunity, intensity of the processes of peroxide oxidation of the lipids, and level of the middle mass molecules were studied. The correlative relationship between them depending on the severity of the course of the disease was revealed.

Cytopathology of alpha chain disease involving the central nervous system and pleura.

Alpha chain disease, a lymphomatous disorder characterized by the synthesis and secretion of an abnormal IgA immunoglobulin devoid of light chains, involves mainly the gastrointestinal tract. This paper presents the cytologic and histologic findings in two cases of alpha chain disease involving the central nervous system and pleura. Most of the cell populations in the cerebrospinal fluid (CSF) and pleural fluid resembled immature plasma cells (immunoblasts); many of these cells were degenerated, with well-preserved plasma cells seen more rarely. While the definitive diagnosis of alpha chain disease depends on immunochemical analysis of serum proteins, cytology can play a role by the identification of malignant cells in CSF and pleural fluid specimens. A positive staining of such cells by the methyl green pyronin reaction may permit the correct diagnosis to be suggested.
