[Systematic preoperative phlebography in patients with leg injuries, surgically treated after a delay].

Post-traumatic preoperative deep vein thrombosis (DVT) was studied by routine bilateral venography in 98 orthopaedic patients admitted more than 48 h (mean: 7 days) after the limb trauma, who had not been given prophylactic anticoagulation. All these patients had undergone long distance travel under medical supervision, having been injured a long way from the medical centre. Ages ranged from 15 to 77 years (mean age: 38 years). 85 patients had been previously free of any of the usual risk factors for DVT. Routine venography showed up DVT in 39 patients (40%). Four DVT were bilateral; only seven were symptomatic. When DVT was proved, heparin treatment by infusion pump was given for 15 days before surgery was performed.

[Retrograde pancreatic phlebography with simultaneous balloon occlusion of portal and arterial flow. An experimental study (author's transl)].

The possibility of demonstrating the venous system of the entire pancreas by retrograde portal contrast injection with simultaneous balloon occlusion of the portal vein, coeliac axis and superior mesenteric artery was investigated in 12 dogs. In seven cases good filling of the larger pancreatic veins was obtained. Portal and arterial pressure relationships altered considerably during various experimental conditions. A rise in serum transaminases was observed only once when there was thrombosis of the portal vein and branches of the hepatic veins. Apart from this, there was no rise in transaminases or alphaamylase. Histologically there were only minor changes in the liver after total occlusion of 40 minutes, the occlusion having been performed over periods of six minutes with two to five minutes of flow alternating. The application of this method to man is discussed.

[Diagnosis of floating venous thrombi by spiral CT phlebography].

PURPOSE: Evaluation by means of spiral CT as to whether or not a thrombus in a proximal deep vein of the leg is floating or adherent to the vessel wall. METHODS: Local application of contrast agent into an ipsilateral dorsal foot vein and spiral CT were used to examine 16 consecutive cases with deep venous thrombosis proven at conventional venography; in addition, colour Doppler flow imaging was performed. RESULTS: At conventional venography, 8/16 thrombi appeared to be floating and the remaining 8/16 were adherent to the vessel wall. Spiral-CT showed 15/16 thrombi to be adherent to the vessel wall; the floating thrombus correlated with findings in conventional venography. At colour Doppler flow imaging 3/16 thrombi were considered floating, one of them was discordant to conventional venography. The comparison of conventional venography to spiral-CT demonstrates complete agreement for adherence to vessel wall seen in conventional venography (p = 1,0) and significant discordance in cases with free-floating appearance in conventional venography (p = 0,0002). CONCLUSION: Adherence of thrombi to the wall of the vessel at conventional venography is in agreement with computed tomography. Conventional venography probably overestimates the prevalence of free floating thrombi.

[Phlebography of the upper extremity. I. The technic and findings in 230 studies].

We report on 230 phlebographic examinations in the shoulder arm region which were performed by conventional X-ray technique and/or digital subtraction angiography (DSA). The detailed method of examination as well as anatomical variants of the vessels are explained. The group of patients was analysed concerning its composition and was subdivided with regard to clinically relevant diagnoses. Contrary to statements in the literature, the number of pathological findings in men or women were equal. The main age was between 40 and 50 years. The brachial vein was found to be doubled so often that this might be accepted as a normal situation. Phlebographic examinations were performed in the right arm twice as often as in the left one. 172 primary examinations, more than 60% showed a pathological result.

[MR venography in deep venous thromboses of the leg and pelvis. A comparison of 2D single layer images and 3D MIP reconstructions with phlebography].

A total of 22 MR venograms were performed in 7 volunteers and 15 patients suspected of deep vein thrombosis of the lower limb and pelvis. MR findings were compared to conventional venography in all patients. MR venography is a reliable method for the exclusion of thrombosis proximal to the popliteal vein. In the calf veins, diagnosis of thrombosis is not yet reliable. For MR venography 2D-time-of-flight-inflow-technique and secondary 3D-MIP reconstructions were used and compared to each other. With both methods there were no false negative results in comparison to conventional venography. 2D single slice MR venography showed two false positive results in iliac and one in popliteal vein. MIP 3D reconstructions led to seven false positive results (three iliac, two femoral, two popliteal). The exclusive interpretation of MIP-3D reconstruction is not reliable for decision-making in deep venous thrombosis.
