Periodontal repair in dogs: effect of a composite graft protocol on healing in supraalveolar periodontal defects.

This study evaluated the effect of a composite graft as an adjunct to gingival flap surgery in induced chronic supraalveolar periodontal defects in the mandibular premolar region in beagle dogs. The vertical dimension of the defects, measured from the cemento-enamel junction to the alveolar bone, approximated 5 mm. Root surface treatment in quadrants receiving the graft protocol included conditioning with both critic acid and tetracycline. The composite graft (including: hydroxyapatite, freeze-dried decalcified bone, tetracycline, and fibronectin) was then fitted to the defects. Flaps were placed and sutured to cover most of the crowns of the teeth but the tips of the cusps. The root surfaces in contralateral jaw quadrants were conditioned with critic acid and the flaps similarly placed and sutured. Dogs were sacrificed 6 weeks after surgery and tissue blocks including teeth and surrounding structures processed for histometric analysis. Connective tissue repair to the root surface in teeth treated with the graft protocol approximated 60% of the defect height. Connective tissue repair in teeth treated with citric acid only was significantly greater and averaged 98% of the defect height (P less than or equal to 0.01). Cementum formation was limited following both treatments (approximately 6% of the defect height). Bone regeneration was significantly smaller in grafted sites than in sites treated with citric acid only (approximately 2% and 10% of the defect height, respectively; P less than or equal to 0.05). Root resorption was observed in almost all teeth. Ankylosis was present in two citric acid-treated specimens, both from the same dog.(ABSTRACT TRUNCATED AT 250 WORDS)

Destructive membranous periodontal disease (Ligneous periodontitis).

Generalized membranous gingival enlargement due to accumulation of amyloid or fibrin-like material is a rare, destructive and poorly defined disease entity. Some patients also show extraoral manifestations. The lesion is an involvement of periodontal tissues caused by the same process as ligneous conjunctivitis. In this report, 3 new cases, two of whom are siblings, are presented. Defective fibrinolysis and abnormal wound healing seem to be the main pathogenetic mechanism of this unusual disease, which should be evaluated systemically considering other mucosal involvement.

[A case report of esthetic and functional rehabilitation by a combination of periodontal and orthodontic treatments for advanced periodontal disease].

PATIENT: The patient was a 44-year-old woman who presented with masticatory and esthetic problems. She had discoloration of CR filling of a maxillary anterior tooth, marginal discoloration, anterior protrusion, median diastema, and closed bite where the mandibular anterior tooth region occluded with the maxillary alveolar region, tooth 8 elongation and tooth 4 defect. Periodontal and orthodontic treatments were therefore performed for esthetic and functional recovery. DISCUSSION: After prosthodontic therapy, postoperative management techniques are crucial, including thorough plaque control, use of a retainer to prevent teeth from moving back towards the original positions (Begg technique), muscle function therapy (improving tongue movement patterns and strengthening the lingual and oral sphincter muscles) and occlusal adjustment. CONCLUSION: In the present patient, the effectiveness of orthodontic therapy before prosthodontic therapy was reaffirmed: dentine can be preserved, prosthodontic therapy is easier to perform, and therapeutic outcomes are highly predictable.

[The prevalence of periodontal disease. Periodontal disease in black Africa. The impact of socioeconomic factors and cultural customs].

The lack of reliable and recent information about the oral and dental health conditions of many African countries is a serious object of concern. There is an urgent need of up-to-date information about the oral and dental diseases existing among the various African populations and about their deciding factors. This study takes stock of the periodontal diseases in black Africa and mention the influence of the socio-economic advancing and the cultural habits.

[Distribution of 3 kinds of periodontal pathogens in subgingival plaques of patients with chronic periodontitis].

OBJECTIVE: To observe the prevalence of Porphyromonas gingivalis(Pg), Treponema denticola(Td) and Bacterides forsythus(Bf) subgingivally in diseased sites of chronic periodontitis (CP) patients. METHODS: Samples of subgingival plaque were detected by 3 kinds of oligonucleotide DNA probe from 60 sites of CP patients and 10 healthy sites of 10 healthy people. RESULTS: The positive rates of Pg, Bf and Td in patients were 91.67%, 90.00% and 95.67% respectively; Pg, Bf and Td were detected simultaneously in 83.33% patients and Pg, Bf and Td were found to be related with each other(P < 0.01). CONCLUSION: Pg, Bf and Td were prominent periodontopathic bacteria and related each other, might exist in complexes in subgingival plaque and coaggretate together.

[Microbiology of periodontitis. 1. The infective nature of periodontitis].

In the last years, guided tissue regeneration (GTR) and implantology have opened new possibilities for practical periodontologists in the treatment of advanced cases. For both techniques the successful therapy of the local infection of the pocket is a prerequisite. In the near future rapid tests for microbiological diagnoses and slow release devices for antibiotics will be available to achieve this aim easier. The following article will present the microbiological aspects of periodontitis and plaque control and show their relevance for the practitioner.

Periodontal and prosthodontic treatment for patients with advanced periodontal disease.

This article describes prosthetic management of advanced periodontally involved dentitions. Overall prognosis and means of assessing scores to prospective abutment teeth are presented. Esthetic treatment and prognosis of surgically elongated dentitions is discussed, and a method to determine what the patient will look like after completion of treatment is presented. A specific approach using ceramometal restorations with modified long bevel and minimum gold collar and the use of electrosurgery for subgingival access is covered.

[The periodontal-prophylactic effect of the toothpaste "Silka". 2. Effects of circulation-stimulations additives on the periodontium in relation to age and sex and in smokers based on Kötzschke's periodontal index].

On subdiving the patients into age-groups, it was found that the relative improvement in the 46--55 year-old patients amounted only to 18% as compared to nearly 27% in patients of age-groups in which hormonal influences are likely to occur. The sex-linked differences in the periodontal condition became evident by higher initial values for the inflammatory component and a low relative improvement (25.53%) in women than in men (28.25%). The relative improvement in smokers (43.49%) as compared to that in non-smokers (23.01%) demonstrated the beneficial effect of benzylnicotinate on the nicotine-induced vasoconstriction.

The effect of ascorbic acid supplementation on periodontal tissue ultrastructure in subjects with progressive periodontitis.

In an attempt to determine the optimal daily ascorbic acid requirement, the effect of different levels of ascorbic acid intake on collagen synthesis was studied in a double-blind experimental design. By use of electron microscopy, interproximal gingival tissue sections in subjects consuming between 20 to 35 mg of ascorbic acid daily were examined before and after the administration of tablets containing 70 mg of ascorbic acid. The results showed that practically all subjects were affected by progressive periodontitis with marked changes in the connective tissue of the gingival lamina propria. After six weeks of supplementation with ascorbic acid, differences in the shape and activity of fibroblasts in the regenerative tissue of lamina propria were observed. This has resulted in the increased number of collagen bundles in fibroblasts' periphery, increased tonofibril content and an enlarged number of desmosomes between adjacent cells. It is concluded that the obtained results, though suggesting that the optimal daily ascorbic acid intake should be set above the presently recommended 30-50 mg, have to be quantified by a more objective analytical method.
