Importance of a balanced-gap technique in rotating-platform knees.

Rotating-platform knees are constructed of highly conforming polyethylene to reduce contact pressures that potentially cause wear. The mobile nature of the rotating platform can lead to bearing spin-out. The purpose of this study was to determine the prevalence of spin-out in a single series. A total of 426 knees in 393 patients were included in the study. All knees were implanted using a balanced-gap technique. The average follow up was 3.2 years. There were no cases of bearing spin-out. The author concludes that the balanced-gap technique reliably prevents spin-out in mobile-bearing total knee implantation. This implantation technique provides the potential wear benefits of the rotating platform design with a nominal bearing spin-out rate.

Metallomesogenic stationary phase for open-tubular capillary electrochromatography.

A synthetic coppermesogenic polymer is prepared and then covalently bonded to the siloxane-based deactivated column as the stationary phases of open-tubular CEC with essentially high phase ratio. The EOF generated from the modified phase is surveyed through conventional aqueous buffers and hydroorganic mobile phases. Zeta potentials, which are computed from the EOF data and the ratio of dielectric constant to viscosity, are plotted as a function of pH, ionic molarity, and compositional range. These plots responsible for the electroosmotic characteristic of the bonded phases are found to be like those of bare fused-silica or deactivated columns through decreasing or increasing the ACN content in the mobile phase, respectively. This two-phase characteristic is basically derived from the polymeric configuration with carboxylato ligands attached onto the polysiloxane backbone. Phthalates and amino acids are suitable probes to examine the two phenomena, more-polar and less-polar mediums, respectively, and to judge whether the chromatographic retention is the major source of separation mechanism. With the mixing modes of Lewis acid-base interaction, dispersive force, and shape discrimination, the chromatographic partition adequately accomplishes the uneasily resolved separations by only CZE mode, although the electrophoretic migration is truly somewhat involved.

A trans-lentiviral packaging cell line for high-titer conditional self-inactivating HIV-1 vectors.

Lentiviral vector safety has been the impetus underlying the progress in packaging cell line development. The prospects of generating replication-competent lentiviruses (RCLs) and the potential for vector mobilization continue to be the driving force for the advancement of packaging cell lines. We have exploited the trans-lentiviral packaging system to develop the SODk3 packaging cell line for the generation of conditional self-inactivating (cSIN) vectors. Separating the gag-pol genome into two distinct expression cassettes (gag-pro and vpr-RT-IN) may reduce the potential for RCL formation, while concurrently employing cSIN vectors supports retention of the SIN phenotype in target cells and alleviates technical constraints associated with generating producer cell lines. Through development of the SODk3 packaging cell line we determined that the ratio of Gag/Pol in vector particles may be used as an indicator for packaging cell clones that yield high vector titers. Conditional SIN vector titers (1 x 10(7) TU/ml) were augmented through clonal selection. Distinct producer cell clones revealed a parallel between vector titer and transgene expression levels. We exploited this observation to demonstrate that incorporation of an internal ribosome entry site between the GFP marker and a relevant transgene affords efficient selection of high-titer producer cell lines. Furthermore, cSIN vectors generated from SODk3 packaging cells imparted efficient transduction of primary human fibroblasts, an indication of the future applicability of the SODk3 packaging cell line.

An experimental investigation of the stability of ethylcellulose latex: correlation between zeta potential and sedimentation.

This paper aims at explaining the experimental observations of the stability and redispersibility of an aqueous ethylcellulose latex through the electrokinetic characterization of the particles. The surface charge and the electrical double layer thickness play an essential role in the stability of the system, hence the need for a full characterization of the polymeric particles. The effect of both pH and ionic strength of the dispersion medium were investigated. It was found that at acid pH values the latex displays "delayed" or "hindered" sedimentation: in such conditions, the electrophoretic mobility and zeta potential are rather low, indicating a small electrokinetic charge on the particles. At alkaline pH, when the dissociation of ionizable surface groups must be complete, the zeta potential is high and negative. The electrostatic repulsion between polymer particles is responsible for the low sedimentation volume and poor redispersibility of the latex. The effect of NaCl and CaCl(2) concentration on both the zeta potential and stability of the latexes was also investigated: it was found that CaCl(2) has the greatest influence, yielding flocculated, easily re-dispersible systems when its concentration in the dispersion medium is high enough. There qualitative observations were ascertained by means of calculations of the potential energy of interaction between particles. In the case of NaCl solutions, a high and relatively wide potential energy barrier was predicted, that may prevent the particle aggregation. Above 5mM NaCl a shallow minimum in the potential energy curves must lead to the formation of aggregates. Similar results were found with CaCl(2) solutions, although in this case the secondary minima are deeper and appear at lower concentrations.

A mobile clinical e-portfolio for nursing and medical students, using wireless personal digital assistants (PDAs).

This paper outlines the development and evaluation of a wireless personal digital assistant (PDA) based clinical learning tool designed to promote professional reflection for health professionals. The "Clinical e-portfolio" was developed at the University of British Columbia School of Nursing to enable students immediately to access clinical expertise and resources remotely, and record their clinical experiences in a variety of media (text, audio and images). The PDA e-portfolio tool was developed to demonstrate the potential use of mobile networked technologies to support and improve clinical learning; promote reflective learning in practice; engage students in the process of knowledge translation; help contextualize and embed clinical knowledge whilst in the workplace; and to help prevent the isolation of students whilst engaged in supervised clinical practice. The mobile e-portfolio was developed to synchronise wirelessly with a user's personal Web based portfolio from any remote location where a cellular telephone signal or wireless (Wi-Fi) connection could be obtained. An evaluation of the tool was undertaken with nurse practitioner and medical students, revealing positive attitudes to the use of PDA based tools and portfolios, but limits to the use of the PDA portfolio due to the inherent interface restrictions of the PDA.

Adsorption of the cationic antitumoral drug celiptium to phosphatidylglycerol in membrane model systems. Effect on membrane electrical properties.

The binding of the cationic antitumoral drug Celiptium to the anionic phospholipid phosphatidylglycerol was studied by measuring surface potentials and surface pressures in monolayers, and by determination of electrophoretic mobility on liposomes. Surface potential and zeta potential data were interpreted in terms of the Gouy-Chapman-Stern theory of the diffuse electrical double layer. A unique drug-to-lipid adsorption constant KaD, could not be calculated. KaD was observed to increase rapidly from 10(4) M-1 to 10(6) M-1 with an increase in drug concentration from 5 x 10(-7) M to 7 x 10(-6) M. This was accompanied by a marked decrease (in absolute value) in the corresponding electrophoretic mobilities which, from negative at low drug concentrations, became positive at drug concentrations of 10(-5) M and above. This indicates that the drug-to-lipid binding cannot be accounted for by a simple Langmuir adsorption isotherm, but corresponds to a more complex process, probably of a cooperative nature. Comparison of delta V and zeta potential data shows that adsorption of Celiptium to phosphatidylglycerol not only lowers the electrical surface potential, psi 0 (in absolute value) but also markedly reduces the polarization potential, delta Vp. These observations suggest that Celiptium destabilizes the electrical properties of cell plasma membranes.

Decrease in circulating hematopoietic progenitor cells by trapping in the pulmonary circulation.

BACKGROUND: When stem-cell grafts are infused into the venous circulation and stem/progenitor cells egress from BM, pulmonary capillary beds are the first microcirculation site that they encounter. This provides the potential for circulating progenitor cells to be trapped in the pulmonary circulation. METHODS: We compared the number of progenitor cells [CD34(+) cells, colony-forming unit-granulocyte-macrophage (CFU-GM), CD34(+) CD41(+) cells and CFU-megakaryocyte (CFU-meg)] and their expression of cell-adhesion molecules (CAM) in samples taken simultaneously from radial arteries and central veins of 21 patients following PBSC mobilization. RESULTS: The mean (+/- SD) frequency of progenitor cells in the radial arteries was reduced to 79% +/- 25% for CD34(+) cells, 73% +/- 27% for CFU-GM, 77% +/- 25% for CD34(+) CD41(+) cells and 70% +/- 29% for CFU-meg of the number in the central veins. This suggests that some progenitor cells might be trapped in the lung. No association between progenitor-cell expression of CAM and pulmonary trapping was observed. DISCUSSION: Our data demonstrate pulmonary trapping of PBSC during mobilization, suggesting a potential inhibitory effect on PBSC harvest and medullary trafficking following graft infusion. However, the impact associated with pulmonary PBSC trapping may be negligible in the clinical setting if sufficient cells are infused.

Dynamic Electrophoretic Mobility of a Cylindrical Colloidal Particle

Accurate approximate formulas are obtained for the dynamic electrophoretic mobility of a cylindrical hard colloidal particle in an oscillating electric field for two cases where the cylinder is in a transverse field or in a tangential field. These formulas, expressed in terms of Hankel functions and modified Bessel functions, are suitable for numerical calculation for all kappaa (kappa is the Debye-Huckel parameter and a is the particle radius) at zero particle permittivity and low zeta potentials. The dynamic mobility in a tangential field is shown to depend on kappaa in contrast to the static case, where it is independent of kappaa. The dynamic mobility of a cylinder averaged over a random distribution of orientation of the cylinder axis is roughly equal to the mobility of a sphere with a radius of 1.5 times the cylinder radius.

Drug-induced zeta potential changes in liposomes studied by laser Doppler spectroscopy.

The technique of laser Doppler spectroscopy is used to measure the electrophoretic mobility of liposomes under the influence of one beta-blocking agent and three local anesthetics. All four drugs decrease the mobility (i.e., the zeta potential) of negatively charged phospholipids (soybean lipids, phosphatidylserine and cardiolipin). The mobility of electrostatically neutral pure phosphatidylcholine (zero mobility under control conditions at pH 7 and 4) is increased linearly with the logarithm of drug concentration, indicating binding and incorporation of positively charged drug molecules. The sequence of strength of activity, measured by zeta-potential changes, corresponds to that found in biological tissues: propranolol greater than tetracaine greater than lidocaine greater than procaine. For purely negatively charged lipids (phosphatidylserine, cardiolipin) the activity of the drug is higher at acidic pH, (pH 4), while for electrostatically neutral (phosphatidylcholine) or partly neutral (soybean) lipid liposomes drug activity is about the same at pH 9, 7 and 4. A Hill plot of the data reveals noncooperative drug binding. From the line width of the scattering power spectrum the mean particle radius and the average interparticle distance in the samples are determined.

Functional outcome measures to assess interventions for spasticity.

PURPOSE: Clinicians use functional loss as a criterion to treat spasticity, but the connection between function and severity of spasticity is not well established for monitoring spasticity treatment effect. Studies were reviewed which have implemented outcome measures to assess functional changes relative to changes in spasticity. Criteria for review included the reliability and internal validity of the functional measures used and the strengths/weaknesses of the study designs that likely affected the external validity of the measures for this application. Guidelines are provided for the use and development of functional outcome measures in futures studies of spasticity treatment based on this review. DATA IDENTIFICATION: An English-language literature search using MEDLINE and bibliographies of published articles and textbooks was conducted. RESULTS: Very few functional measures demonstrated changes concurrent to a reduction in spasticity. There were multiple potential confounding factors in study protocols, reporting of results, and data analysis that might account for the limited number of measures shown to be valid for this application. Selected standardized ordinal functional outcome scales (the PECS and PEDI) and specific functional tasks were identified as measures that show promise for assessing changes concurrent with altered spasticity level. CONCLUSION: Based on a review of previous studies, functional measures involving posture, positioning, balance, and certain mobility skills have potential, with further test development, to provide needed information regarding the impact of spasticity on functional outcome.

Accumulation and chemical fractionation of heavy metals in andisols after a different, 6-year fertilization management.

GOAL, SCOPE AND BACKGROUND: Andisols are widespread in Japan and have some special properties such as high anion exchange capacity, low bulk density, and high organic matter content, which might influence the accumulation or chemical fractionation of heavy metals. However, few such data exist in Japanese andisols. The primary objective of this study was to investigate the distribution and chemical fractions of Cu, Zn, Ni, and Cr in the soil profiles and subsequently to assess their potential environmental hazard. MATERIALS AND METHODS: Soil samples were taken from a field experiment conducted on Japanese andisols, which had received either swine compost or chemical fertilizers for 6 years. Concentrations of Cu, Zn, Ni, and Cr were determined for all of the obtained extract solutions by ICP-AES. RESULTS AND DISCUSSION: Considerably higher total concentrations of Cu and Zn were observed in the top 20 cm layer of the compost-amended soil, relative to the unfertilized soil, while chemical fertilizers had little effect. Application of the swine compost increased the concentrations of Cu and Zn, but not Ni and Cr, in all fractions in the top 20 cm layer. The greatest increase in the organically bound fraction (OM) Cu and dilute acid-exchangeable fraction (DAEXCH) Zn was observed. This suggests that Cu and Zn are potentially bioavailable and mobile in the andisol profiles after 6-year consecutive applications of the swine compost. On the other hand, distribution of Cu, Zn, Ni and Cr among various soil fractions was generally unaffected by chemical fertilizers. CONCLUSIONS: We observed that 6-year consecutive applications of the swine compost led to an increase in total metals of Cu and Zn, as well as their all-chemical fractions, in the top 20 cm soil layers. Potential hazard of heavy metals, especially of Cu and Zn, as a result of the use of swine compost on andisols, must be taken into account. RECOMMENDATIONS AND OUTLOOK: The long-term effect of the accumulation of heavy metals, particularly Cu and Zn, in various plant tissues and soils, as well as their potential risk to surface water via runoff and groundwater via leaching, needs to be carefully considered. Further investigations in the long-term experiments are therefore necessary.

[Methods of autologous peripheral blood stem cell (PBSC) mobilization in oncological and hematological patients].

The potential of peripheral blood stem cell (PBSC) mobilization with granulocyte (G-CSF) and granulocyte-macrophage (GM-CSF) colony-stimulating factors of chemotherapy has been assessed in patients with tumor or hematological disease. The study was intended to aid PBSC transplantation. It was shown that G-CSF administration increased the number of leukocytes in leukemia patients dramatically while mononuclear cell and colony-forming unit levels in G-CSF-mobilised PBSC were significantly higher than those collected with GM-CSF or chemotherapy.

Influence of surfactants and polymeric adjuvants on physicochemical properties, droplet size spectra and deposition of fenitrothion and Bacillus thuringiensis formulations under laboratory conditions.

The effect of two surfactants and two polymeric adjuvants on droplet size spectra and deposition patterns of nine spray formulations was investigated following atomization in a laboratory chamber using a spinning disc atomizer that can produce a narrow droplet size spectrum. Spray droplets were sampled using Kromekote cards and deposit recoveries were examined on glass plates. Physicochemical properties studied were: viscosity-shear rate relationship, surface tension, volatility, pH, conductance, electrophoretic mobility and zeta potential. Formulations containing low surfactant concentrations provided Newtonian liquids with low viscosities. These liquids atomized into small droplets and provided low recoveries of spray deposits on sampling units. However, formulations containing polymeric adjuvants, and a high concentration of a non-ionic surfactant provided pseudoplastic liquids with high viscosities. These formulations resulted in large droplets with high recoveries of spray deposits on sampling units. Among the physicochemical properties studied, viscosity, surface tension, volatility and electrophoretic mobility, played important roles on liquid atomization and droplet deposition.

Oxidative damage and platelet activation as new predictors of mobility disability and mortality in elders.

Mobility disability is an early phase of the disablement process in older adults, and represents a major risk factor for physical disability and mortality. Pathophysiological mechanisms responsible for the onset of mobility limitation are still largely unknown. Oxidative damage, responsible for the disruption of the equilibrium of biological systems by damaging major constituent molecules, might play an important role in the pathway leading to major health-related events. It has been suggested the existence of a vicious cycle involving oxidative damage, platelet activation, and inflammation as promoter of pathophysiological changes occurring with aging. This hypothesis is based on the following observations: (a) oxidative damage is associated with diseases and clinical conditions potentially leading to disability and mortality; (b) oxidative damage is associated with platelet activation, and a vicious cycle involving oxidative damage, platelet activation, and inflammation has been demonstrated in several metabolic disorders potentially leading to mobility disability; (c) the age-related physical decline may be associated to the oxidative damage due to the excess of free radicals; (d) antioxidant defense and behavioral factors (e.g., physical activity, dietary restriction, smoking cessation) play an important role in the reduction of oxidative damage levels and are associated with improved physical performance and muscle strength.

Electrophoretic measurements about the relation between transition voltage and zeta-potential of biological membranes.

The effects of inorganic cations, n-hexanol, saccharose and 2H2O on the electrophoretic mobility and zeta-potential of membrane vesicles from nerve myelin were measured and the results compared with the corresponding effects of the same reagents on the transition voltage, VTr, of the nerve axon membrane. Different cation concentrations and 2H2O affect both potentials, the zeta-potential and VTr, in a parallel way. Saccharose and n-hexanol, however, shift VTr but leave the electrophoretic mobility of the myelin vesicles unchanged. These results suggest that VTr shifts are not necessarily linked to changes in the membrane surface charge density but may also be caused by an interaction between the reagent and non-polar groups of the membrane interior.

The Phoenix Indian Medical Center experience with laparoscopic cholecystectomy during pregnancy.

Since the advent of laparoscopic cholecystectomy, there has been concern about its efficacy in the pregnant patient. This article reports the largest series to date and adds to the growing evidence that the procedure can be safely performed and has the potential to decrease morbidity and mortality. At the Phoenix Indian Medical Center, we performed laparoscopic cholecystectomy in eight women in the second trimester and one in the first trimester of pregnancy, and we attempted it in one patient in her late second trimester. Intra-abdominal pressure was maintained at or below 15 mm Hg. Subsequently, all 10 patients had term vaginal deliveries. In the pregnant patient, laparoscopic cholecystectomy appears to be safe with no morbidity or mortality seen in this series. Early mobilization has the potential to decrease postoperative complications and shorten hospital stay.

The effect of mobile phone to audiologic system.

Mobile phones have come into widespread use. There are a lot of possible adverse effect to health. Use of mobile phone generate potentially harmful radiofrequency electromagnetic field (EMF) particularly for the hearing aspect. 98 subjects underwent hearing evaluations at Department of Otolaryngology, Faculty of Medicine, King Chulalongkorn Memorial Hospital, Chulalongkorn University. 31 males and 67females, mean age was 30.48 +/- 9.51 years old, all subjects were investigated the hearing level by audiometry, tympanometry, otoacoustic emission (OAE) and auditory brain stem evoked response (ABR). The average of using time were 32.54 +/- 27.64 months, 57 subjects usually used the right side and 41 the left side. Average time of use per day was 26.31 +/- 30.91 minutes (range from 3 to 180 mins). When the authors compared the audiogram, both pure tone and speech audiometry, between the dominant and nondominant side, it indicated that there is no significant different. When the authors focused on the 8 subjects that used the mobile phone more than 60 mins per day. It indicated that the hearing threshold of the dominant ears was worse than the nondominant ears.

Flow cytometric assessment of lymphocyte subsets, lymphoid progenitors, and hematopoietic stem cells in allogeneic stem cell grafts.

Currently, bone marrow (BM), cord blood (CB), and G-CSF-mobilized peripheral blood progenitor cells (PBPCs) are the most commonly used sources for allogeneic stem cell transplantation (SCT). The aim of this study was to assess the yields and distribution of lymphocyte subsets, lymphocyte progenitors and hematopoietic stem cells (HSC) in each type of allograft by three-color flow cytometry. The yields of CD34(+)CD38(-) HSCs did not differ significantly between BM grafts (2.80 +/- 0.74 x 10(6)) and leukapheresis products (LPs) (1.82 +/- 0.64 x 10(6)), and were lowest in CB grafts (0.21 +/- 0.05 x 10(6)). For most lymphocyte subsets yields were lowest in CB grafts and significantly higher in LPs than in BM grafts. BM grafts, however, contained the highest yields of CD34(+)CD19(+)CD20(-) B cell progenitors and CD19(+)CD20(-) B cells. The relative frequencies of the naive CD45RA(+)CD45RO(-) phenotype among CD4(+) and CD8(high) T cells were highest in CB grafts (P < or = 0.001), and higher in LPs than in BM grafts (P < or = 0.02). The latter finding was in accordance with a preferential G-CSF mobilization of naive T cells relative to the total lymphocyte population (P < or = 0.014). CD3(+)CD8(low) and CD3(+)CD8(low)CD4(-) subsets, which facilitate engraftment in murine transplantation models, demonstrated a tendency towards lower frequencies among T cells in CB grafts and LPs compared to BM grafts. This observation coincided with a significantly reduced mobilization of subsets potentially enriched for facilitating cells as compared to the total lymphocyte population (P < or = 0.036). The CD34(+) compartment of CB grafts contained a significantly higher percentage (12.1%) of CD34(+)CD7(+)CD3(-) T cell progenitors than those of BM grafts (5.1%) and LPs (3.6%). In addition, CB lymphocytes contained the highest fraction of CD3(-)CD16/56(+) NK cells (P < or = 0.013) and almost no CD3(+)CD16/56(+) NKT cells (P < 0.001) compared to adult cell sources. In summary, LPs, CB allografts and BM allografts differ widely with respect to the cellular composition of their lymphocyte compartments, which is partially affected by a varying mobilization efficiency of G-CSF for distinct lymphocyte subsets.
