[On the encapsulated intraneural granular cell tumor (author's transl)].

Two cutaneously and subcutaneously localized nodules of the left mamma from a 66-year-old patient were excised. The lesions were flesh-colored, firm and well circumscribed. Sections of the tumors show the typical aspects of granular cell tumor. The lesions were both surrounded by a condensation of fibrous tissue which formed a capsule. This condensaton of fibrous tissue resembled and on some sections was in continuity with the perineurium of a nerve. In addition to that is the finding of granular cells situated within a nerve fascicle and apparently replacing several, or all, of the nerve fibres.

[Benign tumours of the female breast (author's transl)].

In a period of 25 years 289 women suffering from benign tumours of the breast were seen at our Dept. This number comes up to a percentage of 20.5 of all female patients treated in this time for diseases of the mamma. Pathology, symptoms, treatment and prognosis of benign tumours of the female breast are dealt with.

[Relationship between nutrition and tumor formation].

The tumours of man mainly develop under the influence of the long-term intake of carcinogens and cocarcinogens. There is a correlation between the level of the intake of meat and the development of tumours of the colon and the mamma. In vegetarians these forms of tumours very rarely appear. For their prevention it is necessary to decrease the consumption of sorts of meat and sausage rich in fat, so that less than 50 g of animal fat are taken in a day. It is to be recommended an increase of the consumption of food-stuffs rich in raw fibers. The caretenes, the vitamin A and the vitamin-A-acid proved as anticarcinogens. In smokers an intake of carotene and vitamin A which overwhelms the need is useful. With smoked food-stuffs in small quantities benzpyrene and nitrosamines are taken in, a restriction of the consumption of smoked meat and fish is not to be recommended. a carcinogenic effect have also aflatoxines, for which purpose no food-stuffs infested by fungi must be taken in.

[Importance of cytological study in the diagnosis of phyllode tumor, sarcoma and carcinosarcoma of the breast].

The paper deals with the results of application of clinical, cytologic and complex clinico-cytologic procedures of diagnosis of fibroadenoma phylloides mammae (160), sarcoma developing from libroadenoma phylloides (17), carcinosarcoma (5) and stromal or primary sarcoma (7). All the cases were treated at the Institute in 1960-1980. The difficulties involved in clinical diagnosis of these tumors are discussed. Correct clinical diagnosis was made only in 17% whereas benign and malignant tumors were identified in 94.7%. Cytological examination serves to improve the differential diagnosis between such neoplasms. Due to its application, the histological pattern of tumor was established in 30.6% and the benign or malignant nature of tumor--in 81.5%. When a complex clinico-cytologic examination was conducted diagnosis was improved in nearly half the cases (44.9%).

Effectiveness of radiation therapy and combined radiation and surgical treatment in advanced breast cancer.

The effectiveness of two modes of combined radiation-surgery treatment and the correlation between the therapeutic results and the clinical type of primary breast cancer were followed in a group of 221 patients with Ca mammae in the III clinical stage. As criteria of evaluating, use was made of analysis of the temporal sequence of relapses and metastases formation following primary treatment. The higher effectiveness of the mode--surgery combined with pre- and postoperative irradiation as against that of surgery with only post-operative radiotherapy became manifest in the frequency of relapses but not in that of distant metastase formation. Simultaneously, a relationship has been established between the course of the disease after treatment and the various categories of T, or its dependence on the relation between the primary tumor and the skin and this within the IIIrd clinical stage of breast cancer.

Induction of malic enzyme in explants of human adipose tissue by L-3,5,3'-triiodothyronine.

The influence of L-3,5,3'-triiodothyronine (triiodothyronine), dexamethasone and insulin on malic enzyme was investigated in cultures of explants of adipose tissue from mammae of healthy female subjects. The enzyme was present mainly in adipose tissue; connective tissue possessed very low activity. After preincubation of explants of adipose tissue from five different subjects for 96 h to deplete endogenous thyroid hormones addition of triiodothyronine (final concentration 1.0 microM) stimulated enzyme activity by 16 to 72%. Dexamethasone (170 nM) increased enzyme activity by 32%; a combination of triiodothyronine and dexamethasone did not lead to an increase beyond that observed for dexamethasone alone. Insulin (final concentration 0.7 I. U./ml) alone stimulated malic enzyme activity minimally, but together with triiodothyronine stimulation was additive. Simultaneous treatment of explants with actinomycin D or cycloheximide and triiodothyronine abolished the stimulatory influence of the latter on malic enzyme. These results suggest that human white adipose tissue is a suitable model to study the effects of triiodothyronine on lipid metabolism. The triiodothyronine stimulated malic enzyme may play an important role in providing reducing equivalents for lipogenesis in human adipose tissue.

[A new method of ovarian transfer in patients with cervical cancer].

As a general treatment for women with invasive carcinoma of the cervix with pelvic lymph node metastasis and/or invasion into the cardinal ligament, a radical operation has been necessary followed by pelvic radiation. However, the irradiation leads to complete cessation of function of ovaries which were retained intact at the radical operation. In order to avoid the influence of irradiation on the ovary, a new operation has been considered; this consists of the transfer of the ovary (1) to the breast or (2) to the lateral upper abdomen. Both sites for transplantation are entirely removed from the pelvic radiation field. The procedure in the former is that the ovary with its own vessels, A. and V. ovarica separated from the retroperitoneum by at least 20 cm, is transferred into the breast through subcutaneous tissue and is followed by fixation into the breast adipose tissue through semicircular incision on areola mammae. In the latter technique, however, a sufficient length of ovarian vessels separated from retroperitoneum is mostly 10 cm or less in order to transfer the ovary to the upper lateral abdomen and fix it on the fascia of this site. A total of 51 patients, in which the age ranged from 24 to 45 years old, have received the ovarian transfer at the radical operation; 31 patients by the former technique and 20 by the latter. After the operation, angiography showed that the circulation of the transplanted ovary was sufficient. Monthly cyclic change in size of the transplanted ovary (CCOS) was evident after operation. This change was related to BBT and levels of serum hormones (FSH, LH, E2 and P4). The frequency of occurrence of the evident CCOS was 20 out of 27 cases (74%) in the former technique and 18 out of 18 (100%) in the latter technique. Radiation doses measured on both the transplanted locations at pelvic irradiation were low enough to avoid disfunction in the transplanted ovary.

[Criteria of radical sector resection in mastopathy].

Statistical investigations of morphological structure in 2 groups of patients with localized forms of fibroadenomatosis (93 observations) have been carried out. Histological findings of the morphological picture have been received both within the localized fibroadenomatosis sites and beyond them. The character of the distribution of morphological forms has been shown to remain the same in all the sites of a single and in both mammary glands. Close relationship between the morphological picture in the localized sites and in so called "normal" sites of the mammae has been established. This proves the expedience of radical sectoral resection and the necessity of longlasting conservative treatment.

Field outbreaks of hyperoestrogenism (vulvo-vaginitis) in pigs consuming maize infected by Fusarium graminearum and contaminated with zearalenone.

During the spring and summer of 1979 field outbreaks of porcine hyperoestrogenism characterized by swelling and reddening of the vulva and teats in prepubertal gilts and enlargement of the mammae in young males occurred in the mistbelt of the Natal Midlands. On a farm in the Ixopo district, pigs were fed a mixed ratio containing home-grown yellow maize stored on the cob in a crib and all the young pigs with a mass of 25-85 kg (=a total of 350) were affected. Maize ears from the crib had a high percentage (91%) of kernels infected by Fusarium graminearum and contained 10 mg/kg of zearalenone. The mixed ration contained 0,95 mg/kg zearalenone. After dilution of the contaminated maize with good quality white maize, a dramatic decrease in both the incidence and severity of clinical signs occurred within 3-4 days. On a farm in the Winterton district, approximately 80% of the growing pigs with a mass of more than 40 kg fed a mixed ration containing shelled yellow maize from the silo of the local Agricultural Co-operative were affected. The incidence as well as the severity of the swelling of the vulvas and/or treats were lower than on the farm at Ixopo, thus indicating a lower level of oestrogen intake. A sample of maize from the silo delivered to this farm contained only 0,06 mg/kg zearalenone while the mixed ration did not contain chemically detectable levels of zearalenone. This is the first published record of field outbreaks of porcine hyperoestrogenism associated with the ingestion of F. graminearum infected maize contaminated with zearalenone in the Republic of South Africa.

Serologic analysis of isolates of Pasteurella haemolytica and Staphylococcus aureus from mastitic ewes.

Milk samples (10 ml) were collected aseptically from infected and healthy mammary glands of 20 range ewes in the early stages of unilateral acute mastitis. The ewes were on 5 different ranches in the Northern Rocky Mountain region of the United States. The samples were plated on tryptose blood agar and examined for bacteria of possible etiologic significance. Twelve of the 20 ewes were infected with Pasteurella haemolytica, 4 ewes with Staphylococcus aureus, and 1 ewe with both bacteria. Twelve of the P haemolytica infections were in pure culture as were 4 S aureus infections. The 13 isolates of P haemolytica represented 6 different serotypes. Isolates of P haemolytica from ewes on the same ranch were as serologically diverse as were isolates from ewes in different herds. The 5 isolates of S aureus were similar antigenically. Bacterial isolates were not obtained from the milk of clinically healthy mammae.

[Gigantomastia in pregnancy developing in a case of status thymicolymphaticus].

An 18-year-old female patient is described, suffering from the syndrome of the immunoendocrine insufficiency, status thymicolymphaticus with hyperplasia of the reticulo-epithelial cells and Hassall's corpuscles of the thymus, generalized hyperplasia of the lymphoid tissue and adrenal hypoplasia. When pregnant, she developed a giantism of the mammary glands with a high blood prolactin level. Death occurred several hours after surgery for artificial abortion from acute adrenal insufficiency resulting from adrenal hypoplasia possibly aggravated by the use of parlodel. Eosinophilic (prolactin-somatotrophic) adenohypophyseal hyperplasia which was much higher than that expected in pregnancy evidenced the role of both prolactin and somatotrophic hormone in the pathogenesis of the mamma giantism. The morphological manifestations of the latter were proliferation of the intralobular connective tissue and end ducts and marked interstitial edema of the mammary glands.

[Telethermography in the diagnosis of breast neoplasms. Report on the experience with 5198 studies].

Four-year experiences made in the course of 5198 examinations of the female breast with the help of telethermography are reported. Altogether 40 mamma carcinomas were found, 29 of them being cases in women over 50 years. Consequently, the participation of women older than 50 years in cancer screening must be considerably increased. Taking all diagnostic possibilities and their rates of failure into consideration, any sound method of early diagnosis of breast cancer, including telethermography, should be integrated into the available screening methods.

[Clinical effect and transfer into the wound exudate of cefroxadine used in the treatment of soft tissue infection].

Clinical effect and excretion into wound exudate of a new semisynthetic cephalosporin cefroxadine (CXD), were studied. CXD was given in 25 cases of surgical infections; 6 cases of wound infection, 9 cases of abscess, 9 cases of infected atheroma and 1 case of furuncle. CXD was orally administered in daily dose of 750 to 1,500 mg. Clinical results were excellent in 1 case, good in 18 cases, fair in 3 cases and poor in 3 cases. The overall clinical efficacy rate was 76.0%. Clinical efficacy classified by diagnosis was 66.7% in wound infection, 66.7% in abscess, 88.9% in infected atheroma, and 100% in furuncle. Side effects were not observed in all cases among 25 patients in CXD trials. Studies of excretion into wound exudate of CXD were performed in 1 postoperative case of mamma carcinoma after oral administration of 500 mg of CXD. The concentration of CXD in exudate was 1.12 micrograms/ml in 2 hours, 3.48 micrograms/ml in 3 hours, 4.13 micrograms/ml in 4 hours, 5.56 micrograms/ml in 5 hours and 4.41 micrograms/ml in 6 hours after administration, which was observed that CXD was excreted in wound exudate in high concentration.

[Significance of the venous system of the spine in the formation of metastases].

The vertebral veins and their anastomoses to the other venous vascular regions are represented in their anatomy, physiological and pathophysiological function, with a special reference to the spreading of tumor cells through the veins of the vertebral column. The causes of the accumulation of metastases in the trunk skeleton and brain is pointed out by malignant tumors of the prostata, of the ren, of the lung, of the mamma, of the genital organs and the thyroid gland. The signification of the vertebral venous system as a spread of metastases is emphasized under consideration of the tumor cell kinetics in the blood-flow and the different forms of the metastatic formations, and is verifying by experienced clinical cases and reviews of the literature.

[Pressure-induced pseudoangioma of obese women].

Due to the pressure by the straps of brassieres, lichen sclerosus-like lesions have been observed on the shoulders of obese women with hyperplastic mammae. Histologically the lesions are characterized by hyperkeratosis, acanthosis and pseudoangionatous proliferation of the capillaries in the dermis. Elastic fibers are sparse, but there is no marked edema and only a moderate perivascular infiltrate of mononuclear cells.

[Influence of surgery method and post-operative irradiation dose in case of mastocarcinomas (author's transl)].

The authors present the therapy results and side effects of 422 patients who had been submitted to a radical or partially radical operation or to ablatio mammae. The post-operative irradiation was effected under high voltage conditions with doses between 40 and 50 Gy corresponding to the stage. The survival times were not influenced either by the surgery method or by the post-operative irradiation dose. As compared to radical surgery, however, edemas of the arm were significantly less frequent and serious after a less radical operation. No significant increase of recurrences was observed after partially radical operations.

[Metastatic deposits of carcinoma in the choroid (author's transl)].

The authors report on choroid metastatic deposits from primary tumors of other organs with regard to a survey of the literature and personal observations. 85% of the choroid secondary growths occur after breast cancer in women with an average latency of 3 1/2 years. The ophthalmological appearances and the fluorescence-angiographical phenomena are discussed with regard to the differential diagnosis of melanoma and haemangioma of the choroid. The therapeutic procedure for secondary deposits of mamma carcinoma is considered.

In vitro phagocytosis and killing of coliforms by bovine mammary leukocytes.

Four coliform isolates derived from cows with bovine mastitis-1 serum-sensitive (SS) Klebsiella pneumoniae, 1 intermediate serum-sensitive (IS) K pneumoniae, 1 IS Escherichia coli, and 1 serum-resistant (SR) E coli-were studied in vitro to determine factors associated with their phagocytosis and killing by leukocytes of mammae. The SS coliform was most easily phagocytosed, whereas the SR coliform was the least phagocytosed, and IS coliforms were ingested at an intermediate rate. The SS coliform was more readily killed in freshly collected serum alone, than by leukocytes and freshly collected serum or heat-inactivated serum. The IS coliforms were killed more readily by leukocytes with freshly collected serum, than by freshly collected serum alone or leukocytes and heat-inactivated serum. The extent of the killing of SR strain was equal for leukocytes with either freshly collected serum or heat-inactivated serum. The differences in killing of various coliforms in serum alone were not as great in a system including serum and leukocytes.
