[Retinal hemodynamics in cerebrovascular accidents].

In 32 patients the times of retinal passage were determined in 112 examinations immediately after cerebrovascular insult and following in daily intervals. In this case after the insult a significant change of the retinal haemodynamics could be observed: the bolus-tension time and the time V2 showed a continuous deterioration of the blood supply of the retina possibly parallel to the cerebral haemodynamics. Furthermore it was shown that the initial prolongation of the arm-retina-times over 17 seconds refers to serve cerebral lesions with poor prognosis. It seems that the method described is not suitable for the preliminary diagnosis of extracranial vascular processes: In acute extracranial vascular occlusion we found equality of the sides of the inflow of dye as well as differences of the sides of more than 30 seconds in free extracranial vascular lumina.

[Adverse events following immunization: AEFI in 17 children between 0 and 2 years of age].

Adverse Events Following Immunization (AEFI) are disadvantageous side effects of preventive vaccination. In 1993 we found 17 cases of AEFI out of 1440 children between 0 and 2 years of age who had received BCG, diphtheria-tetanus-pertussis, measles or poliomyelitis vaccine. They were classified as reactions in 14 children (0.9%) or complications in 3 children (0.2%). Twelve adverse reactions followed DTP vaccination (0.8%), two followed BCG vaccination (0.14%), another two measles vaccination (0.14%) and one followed poliomyelitis vaccination (0.07%). Both generalized and local symptoms were present and they regressed with no further complications. Two children who had received BCG were noted to have a deeply placed abscess at the injection site remaining scar as well as axillary, submandibular and cervical lymph nodes enlargement within 6 months. In a 3 months old child, after the first injection of DTP vaccine, convulsions and consciousness disorder occurred. Transfontanel ultrasonography revealed intraventricular haemorrhage. After one year of intensive neurological care child's health state was improved. In spite of using still more and more safe vaccines none of them is the ideal one--the one with no adverse events following vaccination. Vaccination technics, distribution and storage of vaccines are to be improved which may result in decrease number of AEFI.

[Migraine and epilepsy: clinical comparisons and the characteristics of the cerebral blood circulation].

Clinical, electroencephalographic, rheoencephalographic and linear cerebral circulation findings were compared for 71 epileptics (group 1), 62 migraine patients (group 2) and 6 patients with epilepsy which had developed in the presence of severe migraine with aura (group 3). One-third of migraine patients, mostly of young age, suffering from migraine with aura and basilar migraine had such consciousness disorders as stupefaction and syncopes. EEG changes in migraine patients were considered nonspecific, paroxysmal spike activity arose only in addition of epileptic seizures. Cerebral blood flow measured by radionuclide tracing in migraine-free interval tended to speeding up, while in groups 1 and 3 hemocirculation was slow. Epileptogenic provocation in group 1 patients accelerated cerebral blood flow on the side of the focus, but decelerated it in group 3. The authors hold that the nature of migraine and epilepsy is different. By disintegration of cerebrovascular regulation and vascular supply of the neuron, migraine may promote secondary epileptogenesis.

[State of the nervous system following acute chlorophos poisoning].

The authors describe the clinical traits of myeloradiculopolyneuritis (2 cases) and encephalopolyneuritis (1 case) developing 2--3 weeks following acute poisoning by chlorofos. Along with a distinct toxic polyneuritic syndrome there were also signs of pathology in the spinal cord and brain, as well as in the internal organs in the forms of toxic hepatitis and nephritis. The report contains data indicating a protracted development of the pathology, its direct dependence upon the depth of intoxication and disordered consciousness in the acute period. The authors express the opinion that in the described pathology there may be autoallergic mechanisms in the formation of myeloradiculopolyneuritis and encephalopolyneuritis following poisoning by chlorofos.

[Clinical effects of urokinase and sodium ozagrel in patients with acute symptomatic lacunar infarction].

The clinical effects were compared between a thrombolytic agent (urokinase) and a thromboxane synthetase inhibitor (sodium ozagrel) in patients with acute lacunar infarction. All patients had some degree of neurological deficits, which corresponded to the lesions on computerized tomography or magnetic resonance imaging. Urokinase of 420,000 units was given over two days in 11 patients, 160 mg/day of sodium ozagrel was administered for two weeks in 23 patients. The study was followed up to one month after the onset. Urokinase treatment improved motor paresis in 45.5-62.5% of the patients, sodium ozagrel in 68.4-86.7%. Using the combined score of motor paresis and conscious disorder, urokinase group revealed 44.4-45.5% improvement, but sodium ozagrel group 81.0-89.5% (p < 0.05). The rates of suppressive effect in progressing stroke and complete recovery were higher in sodium ozagrel group. Sodium ozagrel was clinically more efficient than urokinase in patients with lacunar infarction.

[A case of surgically treated left atrial myxoma early after cerebral embolism].

A 48-year-old female had been visiting the outpatient clinic for treatment of polyarthritis before she was admitted to the hospital due to consciousness disorder and right hemiplegia. Brain computed tomography revealed a small infarction in the left cerebrum. Echocardiography revealed a mass in the left atrium. She was diagnosed cerebral embolism caused by left atrial myxoma. Extirpation of left atrial myxoma was performed 16 days after the onset of cerebral infarction. The postoperative course was good. Preoperative constitutional signs such as multiple arthralgia and abnormal sensation disappeared on the 2nd postoperative day. Open heart surgery performed early after the onset of cerebral embolism is generally considered contraindicated due to problems of hemorrhagic infarction or brain edema. Because relapse of embolism may deteriorate the condition, losing the chance of surgery, extirpation of left atrial myxoma early after the onset of cerebral infarction is advisable in cases of small infarction.

[Role of drivers' epilepsy in traffic accidents].

The study covered materials on 48 drivers who underwent 2 and more traffic accidents during the year. The highest share of traffic accidents due to the morbid conditions of the drivers is associated with epilepsy (20 examinees), acute alcohol and drugs intoxication occupies the second place (15 examinees), medical history of head injury appeared in 13 drivers, arterial hypertension-in 3 examinees, diabetes mellitus-in 2 subjects. The examinees having such conditions demonstrated variable consciousness disorders (short-time coma, dazed conditions) during driving, so traffic accidents occurred as the result.

[Cyclosporin A in the treatment of Behçet's disease].

The results of Cyclosporin A treatment of 15 patients with Behçet's disease were examined in the study--9 females (aged from 32 to 34 years, mean 33 years) and 12 males (aged from 25 to 55 years, approximately 34.33 years). Among them, the majority were with chorioretinitis (10/15 or 66.6%). Besides chorioretinitis two patients were simultaneously with iridocyclltis, while in three the whole nervous system was affected (meningoencephalitis, simultaneously pyramidal syndrome with the signs of extrapyramidal lesion and consciousness disorder in one patient). In one patient myocarditis was revealed, and the one was with deep aphtous ulcer. The approximate daily dose was 5 mg/kg in 13 patients, and 7.5 mg/kg/d in 2 patients. For that, the level of the drug in blood elevated from 79 to 380 ng/ml, approximately 170 ng/ml. The effect was favourable in 12 patients (complete restraint of the disease evolution), partial in 2 patients (slowing of the disease course) while it was incomplete in one patient for the interruption of the therapy 15 days after its beginning.

Attempt to establish a classification of patients suffering from coma and admitted to a hospital structure for short or medium term treatment: medical aspects and costs of hospital care.

We dispose of a database, constituted between 1987 and 1993, containing medical and cost information concerning 515 patients suffering from coma and admitted, after a period of resuscitation, to a French hospital establishment--Etablissement Hélio-Marin of Berck-sur-Mer (EHMB)--for short and medium term treatment, between 1974 and 1986. From this base, which contains demographic and clinical data (age, sex, condition upon admission, duration of consciousness disorders, Glasgow Outcome Scale (GOS) upon discharge) we devised a hierarchical classification analysis following a factorial analysis of multiple correspondences, on 2 sets: a sample of 515 patients (all causes of coma being merged) and a sample of 266 patients suffering from brain injuries. Four groups were determined for each typology. These groups were first described on the basis of the variables used for their construction, and later by considering other available variables: origin of coma, duration of stay at EHMB, future evolution of patients and cost of treatment (cost of specific care, average daily cost, total cost of hospitalization). Thus, typical clinical situations were identified in each classification, depending on age of patient, origin of coma and condition upon admission. These situations led to extremely different treatment costs (ratio from 1 to 5 in the general typology and 1 to 2.85 in the classification of brain injuries.

[A case of primary mucinous adenocarcinoma of jejunum].

A 73-year old woman consulted our hospital for consciousness disorder and was hospitalized under a diagnosis of cerebral infarction. On admission, retention of a large volume of ascites was noted. There were also marked increases in tumor markers. Serum CEA, CA19-9 and CA125 levels were 871.9 ng/ml, 1048.2U/ml, and 444.7U/ml, respectively. Cytological examination of the ascites showed class V. Abdominal CT showed a mass measuring 4 cm in diameter and dilatation of the small intestinal tract. During detailed examination, she developed ileus. Fluoroscopy through the lung tube suggested a small intestinal tumor. Laparotomy was performed to relieve ileus. In the abdominal cavity, the greater omentum formed a mass due to dissemination. Tumor was found in the jejunum approximately 60 cm from Treitz's ligament toward the anal side. Omentectomy and partial jejunectomy were performed. Macroscopic findings showed tumor protruding from the mucosal surface. Histological diagnosis was mucinous adenocarcinoma. Primary small intestinal cancer is very rare. In particular, only one case of mucinous adenocarcinoma of the small intestine was reported in Japan. We reported the second case in Japan with a review of the literature.

[Ventricular arrhythmia in hypertrophic cardiomyopathy. When and how to treat].

Patients with apical hypertrophy have a natural favourable history. Non Specific Ventricular Tachycardia (NSVT) at ambulatory monitoring is more favourable if it is not associated with consciousness disorders. A high rate of NSVT episodes at Holter or the association with syncope can lead to a less favourable prognosis and therefore require pharmacological or electric treatment. The role of ET (electrophysiological test) has not yet been clearly described and is in progress. Recent studies of molecular genetics help to identify high-risk patients. Sustained monomorphic VT is not frequent but when it occurs it should be treated with BT. Patients with a light risk of VT should be treated with pharmacological therapy (white amiodarone and/or sotalol) and preferably with implantable defibrillator (ID) if VT cannot be eliminated. ID should be implanted also in the few patients surviving heart attack to avoid the risk of relapses.

[Psychopathologic symptomatology and its prognostic value in the acute period of right hemisphere strokes].

In order to evaluate the possible psychopathological symptomatology the authors analyzed the results of a clinico-dynamical observation of 176 patients in the acute period of a stroke. Among 128 patients with the right hemisphere localization of the focus (the main group) in 60 cases (47%) there were revealed psychopathological symptoms whereas in the control group (48 cases) with left hemispheric foci only 2 patients (4.2%) such disturbances were noted. The main expressions of psychopathological syndromes in patients with lesions of the right hemisphere were anosognosia, certain emotional reactions in the form of euphoria, a drop in purposeful activity, motor and mental aspontanity and specific confabulatory disorders. The qualitative and quantitative specificity of psychopathological syndromes in focal vascular lesions of the right hemisphere is supplementary material for further deep studies of the functional asymmetry of the brain hemispheres.
