Computer evaluation of cytostatic properties of compounds. I. In vitro studies on inhibitory and stimulative activities by CY-VTP/1 program for ODRA-1204 computer.

For the first step of in vitro investigation on the basis of NCI, NIH examination procedure of cytostatics, an evaluation program (CY-VTP/1 in ALGOL-1204) of stimulative and inhibitory activity was prepared for the Polish ODRA-1204 computer. The program allows: a) variety of investigations in connection with possibilities of alternative usage of examination variants; b) comparison with NCI, NIH results, with an optimum for verification and unification with every program; c) to show the inhibitory and stimulative activity of compounds; d) to obtain precise data from a great series of compounds; e) to determine the range of concentration for detailed examinations using CY-VTP/2 program (second step).

Computer evaluation of cytostatic properties of compounds. II. In vitro studies on the activities by CY-VTP/2 program for ODRA-1204 computer.

For the second step of in vivo investigation on the basis of the NCI, NIH examination procedure of cytostatics, an evaluation program CY-VTP/2 in ALGOL-1204) of cytostatic activities was prepared for the Polish ODRA-1204 computer. The program allows: a) variety of investigation in connection with possibilities of alternative usage of examination variants; b) comparison with NIC, NIH results with an optimum for verification and unification with every program; c) to obtain precise data from a great series of compounds with additional information (inactive concentration ED0, range of ED50, activities, a degree of linear correlation; d) graphic presentation of data. Program may be useful for evaluation of other effects dependent on doses and the I-range type reactions.

[The assessment of the severity of pulmonary edema by computed tomography. A comparison between high-resolution computed tomography, objective evaluation with density masks and functional tests].

The results of high-resolution computed tomography (HRCT) were correlated with those of pulmonary function tests, chest films and CT expiratory density mask values in the evaluation of pulmonary emphysema in 33 symptomatic subjects. Emphysema was quantitated with both subjective and objective measurements. Conventional chest films were useful to diagnose severe emphysema but its actual extent was more reliably evaluated with CT scoring systems. HRCT and density mask correlated well with function tests, but the former method exhibited stronger correlation with carbon monoxide diffusion capacity. The opposite was true for hyperinflation and expiratory obstruction variables. Subjective CT estimates, which are quick and easy to perform, were seen to correspond more specifically to the pathophysiologic derangement and should therefore be used to evaluate the anatomic extent of disease. The functional severity of emphysema correlated only with the overall extent of disease and not with its regional distribution in the upper or lower lungs. Finally, in 4 cases (12.1%) with low CT scores, FEV1 was reduced but diffusion capacity values were normal. In one of these patients HRCT showed signs of bronchiolitis. In fact, small airway disease might be a more critical factor in determining functional impairment than the actual anatomical emphysema.

[Invasive aspergillosis in the hematologic patient: the usefulness of computed tomographic and high-resolution computed tomographic studies].

Invasive aspergillosis is an emerging cause of death in hematologic patients. Several patterns of lung involvement are described: acute tracheobronchitis, bronchopneumonia, pleural aspergillosis and angioinvasive aspergillosis. The latter pattern is the most common one; it is characterized by different signs, some of which, supported by clinical data, are quite suggestive for fungal etiology. Particularly, nodules and/or wedge-shaped lesions with a ground-glass halo are a useful early feature, best detected by HRCT. Early therapy with amphotericin B may improve survival chances. Therefore, in neutropenia patients we decided, when possible, to perform high-resolution computed tomography (HRCT) as soon as fever appears. This was feasible in 8 of 32 patients with invasive aspergillosis examined with HRCT. Immediate treatment with amphotericin B in one such patient showing a nodule with the halo sign allowed the lesion to completely disappear. The authors describe the frequency of different radiologic signs in 32 patients, as observed in 54 HRCT exams; the results are compared with those obtained with conventional CT and chest X-ray. Compared to chest X-ray, CT detects more lesions and is more sensitive to small pneumothorax and minimal pleural effusion or thickening. HRCT is more suitable to detect initial cavitation and thin ground-glass haloes.

Computed tomography of pancreatic adenocarcinoma: comparison of tumor size measured by dynamic computed tomography and histopathologic examination.

Our objective was to assess the detection rate and the accuracy of tumor size determination in pancreatic ductal adenocarcinoma using dynamic computed tomography (CT). Preoperative dynamic CT was evaluated in 35 surgically resected pancreatic ductal adenocarcinomas and the findings were compared with the results of histopathological examination. Pancreatic adenocarcinoma was visualized by dynamic CT as a low-density mass in 97% of cases. All three lesions < or = 2 cm in size were demonstrated in good contrast to normal pancreatic parenchyma. The tumor size measured on dynamic CT showed a good correlation with that measured histopathologically, especially in small tumors. Dynamic CT was useful for evaluating the extent of pancreatic adenocarcinoma, especially for small tumors. The detectability of lesions by CT depended on the manner of tumor extension and the degree of associated pancreatitis as well as the tumor size.

Pelvic computed tomography of breast carcinoma patients. Should it routinely be added to abdominal computed tomography?

BACKGROUND: This study was conducted to determine if pelvic computed tomography (CT) should routinely be appended to abdominal CT in the workup of patients with breast carcinoma. METHODS: The abdominal-pelvic CTs of 139 breast carcinoma patients (195 exams) were reviewed. Scans were grouped by indication and whether pelvic pathology was known before CT. Pelvic CT results were correlated with their effect on patient management. RESULTS: Among the 119 patients without pre-CT evidence of pelvic disease, a nonosseous pelvic metastasis was identified in only 1; this patient also had liver metastases and management was not changed. No unsuspected pelvic CT finding altered therapy for breast carcinoma. However, three patients underwent surgery for asymptomatic masses discovered on pelvic CT; all were benign. CONCLUSIONS: Pelvic CT is unlikely to affect the management of patients with breast carcinoma by detecting occult metastatic disease.

Accuracy of nuclear classification in cervical smear images. Quantitative impact of computational deconvolution and 3-D feature computation.

OBJECTIVE: To investigate the accuracy with which the nuclei of cells in overlapped and thick clusters in cervical/ vaginal smears can be classified independent of the segmentation algorithm used and to determine the influence of three-dimensional (3-D) processing as compared to two-dimensional (2-D) methods on classification of the nuclei. STUDY DESIGN: Cell clusters were imaged from 31 ThinPrep smears composed of 808 nuclei, of which 420 were determined to be abnormal by a cytotechnologist. Sets of 2-D and 3-D volumetric features of the detected nuclei were formulated, and classifiers were constructed. The effect of computational deconvolution on classification was assessed using nearest-neighbor and Wiener filter in 2-D and 3-D before calculating features. A "best focus plane" was calculated for each nucleus from the 3-D data set, and the 2-D features in this plane were also analyzed.

Relationship between structural parameters, bone mineral density and fracture load in lumbar vertebrae, based on high-resolution computed tomography, quantitative computed tomography and compression tests.

Different noninvasive techniques for the assessment of the individual fracture risk in osteoporosis are introduced, and the relation between structural properties of high-resolution computed tomography (HR-CT) images of vertebral bodies, their bone mineral density (BMD) and the fracture load is analyzed. In 24 unfractured lumbar vertebrae with different degrees of demineralization from six specimens, the trabecular and cortical BMD was determined using quantitative CT. A lateral X-ray image revealed the number of fractures in the entire spine. A structural analysis of spongy and cortical bone was performed based on the HR-CT images. In the spongiosa, the fractal dimension was calculated as a function of the threshold value. In the cortical shell, the maximum number of clusters of low BMD was determined at varying threshold values. After the CT measurements the vertebrae were excised and compressed until fractured. On the basis of the spongiosa BMD and the number of fractures, 3 cases were found to be severely osteoporotic; the other 3 cases showed osteopenia. The average fracture loads were determined as 3533 N for the non-osteoporotic cases (range 2602-5802 N) and 1725 N for the osteoporotic cases (range 1311-2490 N). The parameters were determined as follows: average spongiosa BMD 115.2 mg/ml (101.8-135.3 mg/ml) for the non-osteoporotic cases, 46.2 mg/ml (34.8-57.6 mg/ml) for the osteoporotic cases; average cortical BMD 285.1 mg/ml (216.4-361.9 mg/ml) for the non-osteoporotic cases, 136. 1 mg/ml (142.5-215.2 mg/ml) for the osteoporotic cases; spongiosa structure: average 0.5 (range 0.32-0.75) for the non-osteoporotic cases, average 1.05 (range 0.87-1.24) for the osteoporotic cases; cortical structure: average 81 (range 55-104) for the non-osteoporotic cases), average 136 (range 102-159) for the osteoporotic cases. Single parameters (BMD and structure) and weighted sums of these parameters were correlated with the fracture load, resulting in correlation coefficients of r(sBMD) = 0.82 (spongiosa BMD), r(cBMD) = 0.82 (cortical BMD), r(sStr) = -0.75 (spongiosa structure) and r(cStr) = -0.86 (cortical structure). The weighted sum of cortical and spongiosa BMD resulted in r(BMD) = 0.86, of cortical and spongiosa structure in r(Str) = -0.86. A weighted combination of all four parameters correlates with the fracture load at r(4) = 0.89, all correlations being statistically significant (p<0.0001). The four individual parameters show only a slight overlap between non-osteoporotic and osteoporotic subjects. The high correlation of the cortical BMD and the structural parameter in cortical bone indicates the important contribution of the cortical shell to vertebral stability. A weighted sum of multiple parameters results in a higher correlation with the fracture load and does not show an overlap between the two groups. It is best suited to estimate the individual fracture risk. The presented methods are generally applicable in vivo; and allow an improvement of the diagnosis of osteoporosis compared with the measurement of the BMD alone.

Computational chemistry on commodity-type computers.

A number of inexpensive computers were benchmarked with the ab initio program Gaussian 94, using both small standard test jobs and larger density functional (DFT) calculations. Several varieties of Pentium (x86) and Alpha CPU based systems were tested. Most of them were running under the open source code operating system Linux. They were compared with several workstations and supercomputers. The most powerful of today's commodity-type processors surpassed current supercomputers in speed. The choice of compilers and compilation options was often found to have a larger influence on job CPU times than details of the hardware. Especially on the x86 type machines, the jobs always ran faster the less memory (RAM) they were given. The fastest machine on a per-CPU basis was an Alpha/Linux system. For the DFT calculation, it was close to twice as fast as a Cray J90 supercomputer.
