Glycosidase activities in sera from convalescent patients who ingested a toxic oil.

Some glycosidase activities have been determined in blood sera from 21 patients who ingested a toxic oil (rapeseed oil denatured with aniline(s) and treated by a thermal process). The samples were collected from the same patients on 3 or 4 occasions during a period of 11-12 mth. During this period, the clinical state of the patients improved and, generally, they were restored to health. beta-N-Acetylglucosaminidase, alpha-L-fucosidase, beta-D-glucuronidase, beta-D-glucosidase and alpha-D-mannosidase activities, which were higher in patients than in 17 controls during the first mth decreased to normal values in the period studied, 11-12 mth. In contrast, beta-D-galactosidase, alpha-D-galactosidase and alpha-D-glucosidase activities, which were initially lower in patients than in controls, were finally similar or higher than in controls. One explanation for these results could be the possible alteration of the cell membrane(s) by the toxic substance(s).

Convalescent excretion of Salmonella enteritidis in infants.

OBJECTIVES: To review the excretion of Salmonella enteritidis PT4 in the faeces of infants involved in a point-source outbreak in a nursery, and to relate these findings to advice given by the Outbreak Control Team (OCT). METHODS: Retrospective laboratory-based survey. RESULTS: Infection with S. enteritidis PT4 was microbiologically confirmed in 33 primary cases and one secondary case. Of the faeces submitted 4 weeks from exposure, 96% remained positive. None of the infants was symptomatic by this time, and none received antimicrobial treatment. Two infants aged less than 1 year were still excreting 22 weeks after the onset of the outbreak. CONCLUSIONS: As for other serotypes, S. enteritidis PT4 causes prolonged symptomless excretion after infection, particularly in infants aged less than 1 year. Infection control measures, including exclusion criteria, may need to be modified as an outbreak progresses.

Experimental allergic neuritis: cytolysin mRNA expression is upregulated in lymph node cells during convalescence.

Experimental allergic neuritis (EAN) is a T cell mediated animal model of Guillain-Barré syndrome, characterized by inflammation and demyelination of the peripheral nervous system (PNS). To study the involvement of immunoregulatory cytokines, we induced EAN in Lewis rats by immunizing with bovine PNS myelin (BPM) and Freund's complete adjuvant. mRNA expression of the cytokines IL-1beta, IL-6, IL-10, IL-12, TNF-alpha and TNF-beta, and the cytolytic effector molecule cytolysin was examined in lymph node mononuclear cells (MNC) over the course of EAN by in situ hybridization after culture without antigen and in the presence of BPM, the myelin P2 protein, the control antigen acetylcholine receptor, or the mitogen PHA. Three patterns of cytokine mRNA expressing MNC in relation to clinical EAN could be distinguished: (i) IL-1beta mRNA expressing cells peaked already on day 3 post immunization (p.i.), and BPM- and P2-reactive TNF-alpha, and BPM-reactive IL-6 mRNA expressing cells were also detected already on day 7 p.i., i.e., before onset of clinical EAN; (ii) BPM- and P2-reactive TNF-alpha peaked together with P2-reactive TNF-beta, IL-6 and IL-12 mRNA expressing cells at height of clinical EAN, consistent with a disease-promoting role for these four cytokines; (iii) high levels of BPM- and P2-reactive IL-10 and cytolysin mRNA expressing cells were observed only during recovery (day 28 p.i.), consistent with a disease down-regulating role of IL-10 and cytolysin. The results suggest a major proinflammatory role for IL-1beta, TNF-alpha, TNF-beta, IL-6 and IL-12 and a disease down-regulating function of IL-10 as well as cytolysin in EAN.

Differentiation of convalescent animals from those vaccinated against foot-and-mouth disease by a peptide ELISA.

We have identified continuous antigenic determinants within the amino acid sequences of the conserved nonstructural region containing proteins 2C and 3ABC of foot-and-mouth disease virus which can distinguish between the sera from vaccinated and infected animals. An ELISA based on a 3B peptide gave a positive reaction with sera from cattle, pigs, sheep and guinea pigs infected with all seven serotypes of the virus, but not with sera from vaccinated animals. In experiments with cattle and pigs to determine the duration of the antibody response, positive reactions were obtained as late as one year after infection. The advantages of using peptides from the nonstructural viral proteins instead of recombinant proteins for differentiating vaccinees from infected animals include their exquisite specificity, nonreactivity with antibodies against host cell-derived proteins (e.g. E. coli and insect cell proteins), and their ease of preparation.

Intraarticular glucocorticoid, morphine and bupivacaine reduces pain and convalescence after arthroscopic ankle surgery: a randomized study of 36 patients.

In a double-blind randomized study, 36 patients undergoing arthroscopic removal of bony spurs and synovitis causing impingement of the ankle were allocated to intraarticular saline or bupivacaine 15 mg + morphine 5 mg + intraarticular methylprednisolone 40 mg. Combined methylprednisolone, bupivacaine and morphine reduced pain, joint swelling, time of immobilization, duration of sick leave and return to sports after the arthroscopic procedure. In the treatment group, 1 patient had transitory purulent arthritis requiring antibiotics and arthroscopic synovectomy occurred.

Medical aspects of delayed convalescence.

Disease and illness are not synonymous. In most instances, disease is demonstrable by anatomic, physiologic, biochemical, microbiologic, or immunologic abnormalities. Disease is a pathologic process. Not all persons with a disease are sick or ill. Symptoms of illness associated with a disease may be manifest or persist after the disease has disappeared. The absence of demonstrable disease, however, does not necessarily mean that symptoms of illness are unreal. Recovery from disease and recovery from illness are not always equated. Many factors, including personal characteristics and social circumstances, can be responsible for recovery from disease and illness. Chronic fatigue syndrome or symptoms of illness can persist in some patients but not in others after many different diseases.
