Malignant acanthosis nigricans: a review.

Malignant acanthosis nigricans is a dermatosis that appears grossly as a hyperpigmented, velvety, or verrucous hyperplasia of the epidermis, most marked in flexural areas. It is always associated with a malignancy. In 277 cases reviewed, the condition was associated with gastric carcinomas in 55.5%, with other intra-abdominal carcinomas in 17.7%, and with malignancies in other sites in 26.8%. Current theories for the association and pathogenesis of the cutaneous process are discussed.

Granulosa theca cell tumor with luteoma in the ovary of a bonnet monkey (Macaca radiata).

A mass was identified on the left caudal region of the abdomen in a 13-year-old bonnet monkey (Macaca radiata). The mass was excised and diagnosed as granulosa theca cell tumor accompanied with luteoma based on the microscopic findings. Morphologically it appeared pink, round, firm multilobulated measured approximately 5 x 3 x 2.5 cm in dimension. Histologically the luteoma composed of polyhedral cells with pale strained vacuolated cytoplasm, centrally located nuclei with distinct cytoplasmic borders. Granulosa theca cell tumor appeared as densely packed spindle shaped fusiform cells arranged in interlacing bundles and whorled pattern with neoplastic cells appearing irregular shaped solid sheets. The concomitant development of granulosa theca cell tumor with luteoma in a single ovary is very rare and is the first reported case in a bonnet macaque to our knowledge.

Ovarian neoplasia in ornamental hybrid carp (Nishikigoi) in Japan.

Massive abdominal enlargements were observed in 21 adult carp 4--6 years old living in the breeding ponds of carp fisheries in northern Japan. The abdominal enlargement rapidly increased, and the affected fish died within a few months. At necropsy, single or multiple tumors, 1.5--25 cm (average, 10.6 cm) in diameter, were found in the abdominal cavity. Ovarian tissue was identified close to the tumors in 15 cases, and there was no evidence of anatomic continuity between the tumors and other viscera. It seems likely that all of these tumors arose in the ovaries. Histologically, the 21 tumors observed exhibited great variation, but they were composed mainly of various types of cells similar to those of human dysgerminoma, granulosatheca cell tumors, or embryonal carcinomas. Squamous cell nests were found with mesenchymal elements in one tumor. Thus, ovarian tumors in carp may have diverse histogenetic origins and probably arise both from germ cells and from ovarian mesenchyme. Areas of cellular pleomorphism, which suggested a malignant character, were seen in some of the tumors, but no evidence on metastasis was found. Tumor-bearing carp had generally been raised in ponds supplied by wells or underground springs in mountainous areas. Evidence of a possible environmental factors(s) is yet to be demonstrated.

Use of stapling instruments to aid in the removal of ovarian tumours in mares.

Ovarian granulosa theca cell tumours were removed surgically in 10 mares. Stapling instruments were used to facilitate haemostasis of the ovarian pedicle in five out of 10 cases and as a sole method of haemostasis in the remaining five cases. Postoperative haemorrhage from the ovarian pedicle was suspected in only one case. All cases survived long term follow-up from six months to four years. Results from this series of cases indicate that stapling instruments provide a rapid, easy and efficient means of ligating ovarian pedicles when performing ovariectomy in the mare.

Malignant fibrothecomatous tumour of the ovary: diagnostic value of anti-inhibin immunostaining.

Malignant ovarian tumours of the fibrothecoma group are rare. The clinicopathological features of a case of ovarian malignant fibrothecoma in which there was metastatic disease in the small intestine and peritoneum at presentation are described. A number of differential diagnoses were considered but positive immunohistochemical staining of the resected ovarian and small intestinal neoplasms with anti-inhibin was of value in confirming a sex cord-stromal tumour and in excluding other lesions. The two tumours were also ultrastructurally identical. Classical malignant fibrothecomas are said to show four or more mitotic figures per 10 high power fields (HPF). Although the intestinal secondary was mitotically active, the primary ovarian tumour contained only one to two mitoses per 10 HPF, showing that formal mitotic counts are not an absolute indicator of malignant behaviour in this group of tumours.

The use of ultrasound in the evaluation of trophoblastic disease and its response to therapy.

Thirty-nine patients with trophoblastic disease were studied to determine the usefulness of ultrasound in identifying risk patterns and response to therapy. Serial measurements of serum human chorionic gonadotropin-beta subunit (HCG-BSU) were compared with ultrasonographic uterine and theca lutein cyst volumes. In 16 patients ultrasound demonstrated theca lutein cysts, many of which were not palpable on physical examination. Although there was a significant decrease in uterine volume and a change in the sonographic pattern following evacuation, volume slowly returned to normal over a period of several months. Persistent trophoblastic disease was more accurately detected by HCG-BSU measurements than by ultrasound. Persistent disease developed in 44% of those patients who had theca lutein cysts and in 22% of those without cysts. Patients with theca lutein cysts did not consistently have higher HCG-BSU levels than patients without cysts, and it is concluded that ultrasound is the best method for detecting these cysts.

Fibroma and fibrothecoma of the ovary: MR imaging findings.

PURPOSE: To describe the morphologic and signal intensity characteristics on magnetic resonance (MR) images of fibromas and fibrothecomas. MATERIALS AND METHODS: MR images of 11 female patients with histologically proved fibromas or fibrothecomas were reviewed, and morphologic and signal intensity characteristics of the lesions were analyzed. MR imaging findings were correlated with histologic findings. RESULTS: All fibromas and fibrothecomas showed homogeneous low signal intensity on T1-weighted images. On T2-weighted images, the two smallest lesions showed homogeneous low signal intensity, and eight of the other nine lesions showed predominantly low signal intensity. Edema was noted only in larger lesions, and cystic degeneration was noted only in three of the largest lesions. On T2-weighted images, the percentage of low signal intensity in the lesion was not found to be related to lesion size, and the percentage of low signal intensity in fibromas was not significantly different from that in fibrothecomas (P = .55). Many lesions showed heterogeneous signal intensity; the solid component was distributed peripherally, and the cystic component was located centrally or eccentrically. Free intraperitoneal fluid was noted in 10 of 11 lesions and was not significantly correlated with lesion size (r = .52 and P = .10). CONCLUSION: Because of their predominantly low signal intensity on T2-weighted images, fibromas and fibrothecomas display a relatively specific appearance on MR images.

Management of non-epithelial ovarian tumours.

Ovarian tumours of non-epithelial origin are less common than those of epithelial origin but must be distinguished from these as their natural history and management differ. As these tumours are rare, histological review by an expert in the field is essential. There have been no randomised trials to outline the management of these tumours and therefore this paper represents a review of descriptive data. Non-epithelial ovarian tumours, which represent approximately 50% of all ovarian tumours and approximately 25% of malignant ovarian tumours, are outlined. Lymphomas and metastatic tumours should be treated the same as when found in other sites and will not be discussed further.

Lectin histochemistry of sex cord-stromal tumors and small cell carcinoma of the ovaries.

Binding sites of peanut agglutinin (PNA), Ulex europaeus (UEA-1), concanavalin A (Con A), and wheat germ agglutinin (WGA) were localized in 10 granulosa cell tumors, 10 Sertoli-Leydig cell tumors, 4 theca cell tumors, and 5 small cell carcinomas. Con A and WGA reacted positively with the majority of the studied neoplasms. PNA and UEA-1 were persistently negative in the sex cord-stromal tumors (SCST) but showed focal positivity in small cell carcinomas. Negative reactions of SCST with PNA and UEA-1 may serve for differentiation between them and common epithelial tumors that are usually PNA and UEA-1 positive.

Aspiration cytology of 147 adnexal cysts with histologic correlation.

OBJECTIVE: To assess the usefulness of cytology in the diagnosis of 147 histologically established adnexal cysts. STUDY DESIGN: Retrospective, macro-microscopic study based on fluid aspirated from 132 ovarian and 15 extraovarian cysts and projected as a cytohistologic correlation. RESULTS: Typical macroscopic features were identified in 76% of endometriotic cysts, in 53% of mucinous neoplasms and in 67% of dermoid cysts. Cytology helped to identify 67% of nonneoplastic and 56% of neoplastic cysts. The lowest diagnostic sensitivities were observed in functional cysts and benign serous neoplasms (50%), while the highest were shown by endometriotic cysts (76%) and malignant epithelial neoplasms (71%). Inadequate samples were obtained from all types of cysts, even malignant ones (two mucinous cystadenocarcinomas). Diagnostic cytology was useless in extraovarian cysts (33% sensitivity). An adult granulosa cell tumor was erroneously diagnosed as a follicular cyst by cytologic examination. CONCLUSION: Examination of the cyst fluids obtained by aspiration demonstrated low sensitivity, with 43% of inadequate samples obtained from all types of cysts. Malignant cystic neoplasms may be overlooked in inadequate samples. Our study also revealed that specificity in this type of analysis is high in inadequate samples, provided that the technique is carried out correctly.

Survival after ovarian granulosa and theca cell tumours.

Ninety-two cases of ovarian granulosa and theca cell tumours were recorded in the Tayside Gynaecological Tumour Registry during the 30 year period 1948-78. A high follow-up rate was achieved through this centralised registry and exhaustive annual recall. At 20 years after initial treatment patient actuarial survival was 92% for the theca cell tumours and 34% for granulosa cell tumours.
