[Morphohistological aspects of tumors and tumor-like changes in the thyroid gland].

The results of the thyroid gland (TG) state investigation in 1027 patients, ageing from 14 to 75 years, operated on in 1986-1995 yrs., are adduced. In 60.5% of patients the diffuse-nodular goitre was diagnosed, in 9.5%--Hashimoto's thyroiditis, in 9.5%--TG cancer, in 2.8%--other pathology. To establish the interrelationship of pre-cancer changes and a TG cancer it is necessary to point out the concomitant morphologic change beside the main diagnosis, which determines the clinical tactics.

Pathology results in [18F]fluorodeoxyglucose positron emission tomography of the thyroid gland.

The aim of this retrospective study was to evaluate pathologically increased uptake of [18F]fluorodeoxyglucose (18F-FDG) in positron emission tomography (PET) results of the thyroid gland. Results of 18F-FDG PET and [99mTc]pertechnetate scintigraphy of the thyroid gland are shown, compared to each other and discussed. In a retrospective study 16 patients underwent whole-body 18F-FDG PET and [99mTc]pertechnetate scintigraphy of the thyroid gland within 3 weeks. In addition, an examination of the thyroid gland by using ultrasound and laboratory tests was carried out. The 18F-FDG PET studies were carried out on a dedicated whole-ring PET scanner. Eight patients had a pathological FDG uptake in the thyroid and a cold nodule in [99mTc]pertechnetate scintigraphy of the thyroid gland (in 7/8 cases histology showed malignancy). Five patients had an inhomogeneous FDG uptake in the thyroid gland and were suspected of thyroiditis in 18F-FDG PET (in 3/5 cases thyroiditis was confirmed). Three patients had an especially low FDG uptake compared to normal physiological FDG uptake (no malignancy). Results from studies using 18F-FDG represent a growing body of evidence showing the differentiation between malignant and benign disease: we saw many pathological results in the thyroid gland. High uptake of 18F-FDG in the thyroid gland suggests possible malignancy. Thyroiditis can only be suspected based upon the results of 18F-FDG PET. We conclude that 18F-FDG PET has a potential clinical impact for detecting possible malignant lesions of the thyroid gland, but further studies, in which a higher number of patients are evaluated, are necessary.

[Autoimmune reactions in patients with surgical diseases of thyroid gland].

Possibility of detection of autoantibodies toward thyroidal tissue, using reaction of the compliment bounding in the cold, was studied for raising of the diagnosis and differential diagnosis efficacy in surgical diseases of thyroid gland. The results obtained witness the possibility of application of the method in diagnosis of the thyroid gland surgical diseases. In nodular goiter the antibodies titer had constituted 1:10-1:20, in cancer of thyroid gland-- 1:40-1:80. Accessibility of the method permits to recommend it for application in screening procedures.

Methodology of the thyroid gland disease decision-making using profiling in steroid hormone pathway.

To find out the genetic factors of outbreak of thyroid gland disease, we developed the thyroid gland decision-making system, which processes the metabolic profile in steroid hormone map using a statistical method. Metabolic profile is a measured data of lots of mixed materials that includes not only known metabolites, but also unknown ones, which is estimated to have an influence on the thyroid gland disease. Therefore, to develop thyroid gland disease decision-making system, analyzing metabolic profile containing multi-materials would be useful for diagnosing thyroid gland disease. Because experimental values used for system construction are area values for the retention time, the observations are preprocessed through variable transition and t-test to use the area values concurrently and the highly correlated materials are estimated by principal component analysis. The thyroid gland decision-making system developed through the logistic regression is an excellent system demonstrating 98.7% accuracy in the classification table.

Metastasis to the thyroid gland. A report of 43 cases.

BACKGROUND: The incidence of metastasis to the thyroid gland in autopsy series varies from 1.25% to 24%. Metastasis to the thyroid gland is usually considered a terminal event, and the effectiveness of conventional treatment has been questioned. The authors assessed the effects of current methods of diagnosis and treatment on the course of the disease. METHODS: Forty-three patients with metastasis to the thyroid gland were studied retrospectively. Primary tumor origin was identified in all but two cases. Metastasis to the thyroid gland was confirmed by fine-needle aspiration cytology or histology. Data were analyzed for the frequency and types of malignant lesions, the clinical course of disease, and the prognosis after thyroid involvement. RESULTS: The kidney was the most common primary tumor site (33%), followed by lung (16%), breast (16%), esophagus (9%), and uterus (7%). The time from diagnosis of the primary tumor to metastasis to the thyroid gland was considerable for renal cell adenocarcinoma (mean, 106 months) and for adenocarcinomas of the breast (mean, 131 months) and uterus (mean, 132 months). In 12 patients, this interval was more than 120 months. Fine-needle aspiration cytology detected metastatic malignancy in 29 of 30 patients. Treatment involved surgery alone, surgery with adjuvant therapy, or nonsurgical methods. Two patients with uterine adenocarcinoma and one with breast adenocarcinoma had disease regression with no evidence of tumor recurrence. CONCLUSIONS: In any patient with a previous history of malignancy, no matter how remote that history is, a new thyroid mass should be considered recurrent malignancy until proved otherwise. Although detection of metastasis to the thyroid gland often indicates poor prognosis, aggressive surgical and medical therapy may be effective in a small percentage of patients.

[Nonmedullary apudoma of the thyroid gland].

Eight apudomas of the thyroid gland with nonmedullary structure are described. Their structures were of the follicular-solid, papillary cancer, B-cellular, and microfollicular adenoma types, whereas their cells were impregnated with silver salts which was the inherent property of the APUD-system elements.

Sonographic imaging of the thyroid gland in congenital hypothyroidism.

We have attempted to facilitate differential diagnosis of etiological types of congenital hypothyroidism using real-time ultrasonography. Sonography of the thyroid gland was performed on 418 normal children, and 23 patients with congenital hypothyroidism and hyperthyrotropinemia. The thyroid gland was imaged by transverse scanning at the neck; the maximum thickness and the maximum width of the right and left lobe were measured. On the basis of the normal thyroid gland size obtained from normal children, the thyroid gland image of the patients were classified into 4 types: large image, normal image, small image, and no image of the thyroid gland at the neck: no image of the thyroid gland indicated agenesis or ectopia; large thyroid gland image indicated goitorous hypothyroidism. On the other hand, normal or small thyroid gland image probably included mild or transient forms of hypothyroidism, and transient hyperthyrotropinemia; these 2 types required further examination to complete the diagnosis. We concluded that real-time ultrasonography of the thyroid gland was a useful diagnostic imaging technic for patients who revealed elevated serum thyrotropin on neonatal mass-screening.

[Effect of phenformin on fetal mass in rats previously immunized with an extract of homologous thyroid gland].

Rats immunized with homologous thyroid antigen 9-27 weeks before delivery manifest an increase in the mean weight of fetus as compared with normal pregnant rats. Administration of phenformin (7.5 mg/day per os) during immunization and over 30 days after its discontinuance led to a significant weight reduction as compared with immunized animals not given phenformin. The latter's effect became more potent with an increase in the interval between immunization discontinuance and delivery. Prophylaxis and cure of the latent and evident autoimmune lesions of the thyroid gland during childhood and juvenescence are regarded as approaches to the prevention of endocrine and metabolic disorders favouring large fetus delivery.
