South Verona's psychiatric service: an integrated system of community care.

Italy's mental health law of 1978 mandated the closing of state mental hospitals to new and former patients and the creation of comprehensive community-based service systems. The authors describe the organization, facilities and services, and treatment principles of one system created in accordance with the law--the South Verona Community Psychiatric Service, which serves a population of 75,000. Case register data for South Verona for the years 1978 to 1984 reflect the changes mandated by the law. They also indicate that despite an increase in admissions to the psychiatric ward in the general hospital following the closing of the state hospital, there was a substantial overall decrease in the number of hospital beds occupied per day by South Verona residents after passage of the law, and that a substantial proportion of patients with functional psychosis were able to be treated as outpatients.

[Sanitary monitoring of an integrated system for the treatment of wastewaters from pig farming].

OBJECTIVE: To assess the potential for contamination of wastewaters from pig farming. METHODS: Wastewaters from pig farming were stored in a tank. After 0, 30, 60, 90, and 120 days of hydraulic retention, they were added to lysimeters filled with argillaceous, sandy, or medium soil. Finally, these lysimeters were submitted to simulations of either a rainy season or a dry season. The number of colony-forming units (CFUs) of total coliforms, fecal coliforms, and fecal streptococci was measured in the effluents of the storage tank (for the various periods of hydraulic retention), in the percolate from the lysimeters, and in the three types of soil. The microbiological analyses were carried out using the membrane filter technique. The pH analyses were done potentiometrically. RESULTS: For the three microorganisms, the largest decrease in bacterial counts in the storage tanks occurred with 90 or 120 days of retention. There was a marked decrease in the bacterial count in the percolates of the three soils. For the three soil types the greatest reduction in bacterial counts was found in medium soil, due to its acidity (pH < 7.0). Hydraulic retention was not sufficient to ensure the sanitary adequacy of the wastewaters and their use for irrigation, given that fecal coliform values were above 1 000 CFU per 100 mL. Therefore, adding the residues to the soil was considered a second stage of treatment. CONCLUSIONS: The retention of wastewaters followed by adding them to soil was effective in minimizing the contaminating effect of pig farming residues. The storage time for wastewaters from pig farming could be decreased from 120 to 90 days.

Videoendoscopy and photodetection: linearity of a new integrated system.

Simultaneous videoendoscopy and photodetection has potential for improving the assessment of changes in velopharyngeal (VP) movements and closure over time during speech. The purpose of this project was to examine the linearity of a new system, including a pediatric bronchoscope with an internal instrument channel through which the photodetector fiber was positioned. The results led to the conclusion that the response of the new system was linear. However, further examination indicated that system positioning variables had important effects on the overall level of photodetector light detection. These observations confirm the importance of monitoring and controlling the position of the endoscope-photodetector apparatus in the nasopharynx when using the system to evaluate velopharyngeal movement and closure for speech.

Dissecting the conditioned pecking response: an integrated system for the analysis of pecking response parameters.

The conventional pecking response key, although an excellent transducer of response rate, can provide minimal information on the topography, coordination, or localization of conditioned pecking. We describe the hardware and software components of a system that, in addition to recording response rates, permits simultaneous "on-line" monitoring of head acceleration, jaw movement, terminal peck location, and duration of pecking response. Head movements are monitored with a miniature accelerometer, jaw movements with a magnetosensitive transducer, and peck location with modified touch screen technology. Initial experiments with the system suggest that it will be useful in studies of response differentiation, acquisition and maintenance of complex discriminations, and interaction of conditioned and unconditioned stimuli in the control of pecking response probability and response topography.

Integrated systems analysis of sow replacement rates in a hierarchical swine breeding structure.

Sow replacement rates in a three-tiered breeding structure were investigated for a 10-yr planning horizon using a stochastic life-cycle swine production model. Market hogs were produced in a three-breed static crossing program and marketed on a liveweight basis. Growth and reproductive traits of individual pigs were simulated using genetic, environmental, and economic parameters. Sows were culled after a maximum of 1, 5, or 10 parities. Systems were defined by maximum sow age at culling and included combinations of 1- and 5-parity nucleus and 1-, 5-, and 10-parity multiplier and commercial tiers. Economic response to index selection was considerable for all culling alternatives with yearly increases in system profits ranging from $1.06 to 1.44 for each commercial hog marketed. When sows were culled after one parity in nucleus, multiplier, and commercial tiers, respectively (1,1,1), annual changes in net returns and all cost measures were 40 to 50% larger than responses in systems with lower sow replacement rates. Based on 10-yr averages for net returns, systems with low multiplier- and commercial-level replacement rates were more profitable than systems with higher replacement rates. The most profitable system (5,10,10) differed from the least profitable system (1,1,1) by more than $10 per pig, but when the (1,1,1) system was excluded, the range was only $3 per pig. The system with lowest replacement rates supported 3,388 more multiplier and 34,151 more commercial sows from a 750-sow nucleus level than the (1,1,1) system. Output from the two extremes differed by > 664,000 commercial market hogs sold.(ABSTRACT TRUNCATED AT 250 WORDS)

Caring for the severely asthmatic child and family. I. The rationale for family systems integrated medical/psychological treatment.

Comprehensive care for the severely asthmatic child includes psychological as well as medical treatment. Family therapy is a recognized modality of therapy. Investigations have examined the role of emotion and asthma as well as psychosomatic asthma. Pharmacological treatment of beta 2-agonist and cromolyn prior to disciplining the child prevents psychosomatic asthma secondary to crying or shouting. Adherence to multiple asthma therapeutic modalities is imperative, and a psychological and medical team can address these important clinical issues in a high-risk population.
