The importance of recreation grew considerably with the increase of leisure time and cultural level of the workers. In our country, on the other hand, there are no complete hygienic studies on the problems of organization and necessary equipment for recreation. In that connection, hygienic-urbanization, hygienic-sociological, microclimatic and physiological studies, as well as investigations on ultraviolet radiation, level of noise and extent of air pollution in some typical recreation places (Sofia and Varna) were carried out. Better conditions were established in nature - town ourskirts environment (as regard ultraviolet radiation, noise level, degree of air pollution, temperature-humidity and radiation microclimate) and their favourable effect on the physiological functions of organism (particularly as regards thermoregulation and cardiovascular system). A dependence was also established between the population recreation needs and social-demographic and nature - climatic factors. Tangible recommendations for the improvement of the state of short-term town outskirts recreation in Sofia and Varna are proposed on the base of the results.

In June 1992 prior to a community-based screening campaign for malnourished children, a cross-sectional survey was conducted in a semi-urban area located ten kilometers from Niamey, the capital city of Niger. Nutritional data gathered from a representative sample of children under the age of 5 years were compared to corresponding data from the National Center for Health Statistics. Acute malnutrition among children under six months was 8%. In the sample as a whole, the rates acute and chronic malnutrition were 17.7% (13.4-22.0) and 28.7% (23.4-33.9) respectively with 4% accuracy. These rates were similar to those of a previous regional study made in 1990. Nutritional status was related to the identity of the caretaker. Mothers had a mean of 4.07 living children and 0.99 dead children. The study indicated a child mortality rate of 181 (136-225) per thousand. The primary anamnestic causes of death were diarrhea and measles. Malnutrition was the fifth cause of death. Twenty-two percent of children screened had been sick in the month preceding the study. Low cure rates of the malnourished at the nutritional center of Niamey (7.42%) and high level of chronic malnutrition underline the need for solutions better suited to semi-urban areas.

Sexual behaviors and condom use were surveyed in a sample of 1,312 male adolescents (ages 12-19 years) in Laval, the second largest city in the province of Quebec. The data were obtained via a self-administered questionnaire to which 98.8% responded. The study showed that male adolescents became sexually active, on average, at 13.9 years, an age younger than previously reported. Further, 60% of the adolescent males used a condom at their first intercourse, a figure which doubled that previously reported; its use, however, decreased to one-third when utilization at each intercourse was examined. Overall, condom use was greatest in 14-year-olds and decreased in older adolescents, being replaced by the pill. The study further revealed that 12-13-year-olds were less likely to use condoms and more likely to use ineffective methods than were 14-year-olds. Although condoms seem to be gaining a following among male adolescents, their use is transitory and is being replaced by the pill and other methods with low protective value against sexually transmitted disease (STD) and AIDS. These results underline the necessity to reorient our educational and promotional activities in STD prevention to the realities of this group.

Munson Army Community Hospital has successfully realized substantial cost savings by instituting psychiatric managed care. The development of an external partnership with a civilian psychiatric facility is a unique aspect of the psychiatric managed care initiative and has resulted in most of the savings. We staff this partnership hospital with one-half full-time-equivalent psychiatrist. Other psychiatric managed care program elements include: (1) using CHAMPUS "recapture" funds to hire additional personnel; (2) maximizing personnel utilization by combining mental health staff from the social work and psychiatry services; (3) working closely with the community to identify local mental health needs; (4) offering additional therapeutic modalities; and (5) reducing rehospitalization rates through improved discharge planning. We reduced our financial-year first quarter costs by 76% from 1993 to 1995. We attributed much of these savings to reduced residential treatment admissions. Although the total number of outpatient visits increased, Munson's psychiatric services reduced outpatient costs by increasing clinic access. We have used the Gateway to Care program to prepare for the initiation of Tri-Care in our region.

Dog's ectoparasites were collected from 3 different parts of the body (back of the ears, environs of the loins, and beginning of the tail) during seven months (1993/1994). Material originated from two dog's shelters: Paluch in Warsaw and Celestynów near Warsaw. All 50 inspected dogs had the fleas Ctenocephalides canis (CURTIS, 1826). 63% of fleas occupy back of the body, 31% the tail and 6% the ears. Number of the females was higher than males. The highest number of fleas was stated in November 1993 and April 1994.

Two newly developed Bacillus sphaericus larvicidal formulations, VectoLex CG (corncob granules) and VectoLex WDG (water dispersible granules), were tested against Culex quinquefasciatus larvae in 4 highly polluted breeding sites in Thailand. VectoLex CG, applied at rates of 0.5-2 g/m2, gave satisfactory to complete control of late-instar larvae and pupae for up to 4 wk after treatment. The VectoLex WDG, which had higher potency and was applied at rates of 0.1-0.5 g/m2, gave satisfactory control for 1-4 wk after treatment. Among the factors influencing longevity of control were dosage of a given formulation, precipitation, and flooding of the treated sites; the latter had the greatest impact. Presence of larvivorous fish did not seem to influence larval populations because there were heavy populations of mosquito larvae present in the test sites in the presence of moderate numbers of fish before the application of B. sphaericus treatments.
