Is pregnancy smoking causal to testis cancer in sons? A hypothesis.

Inquiry studies into smoking during pregnancy as a possible cause of testis cancer in sons are found misinterpreted as purely negative. Danish quinquennial incidence rates for patients aged 25-39 years are compared with rates for tobacco-induced neoplasms among the corresponding maternal generation, primarily of bladder tumours including papillomas, and of lung both for ages 50-64 years, allowing for period of latency. Corresponding to a delay of increase, earlier demonstrated for incidence rates of testis cancer among men born 1939-1945, delays of increase are found for female bladder and lung with allowance for lag-times of 30 and 20 years respectively. Comparison of data from Scandinavian nationwide cancer registries show a noteworthy parallelism of trends from first to last plotting of testis/female bladder ratios. The rise for lung is found steeper in consequence of age distribution.

A variant chromosome 17 in a mother with repeated abortions and a 46, XY/47, XXY Klinefelter son.

A female with a satellited chromosome 17 is presented. She had suffered repeated abortions and later gave birth to a 46,XY/47,XXY Klinefelter boy. The significance of the variant chromosome 17 in the etiology of the mother's reproductive failure is discussed. The mental and physical development of her now 8-year-old 46,XY/47,XXY son has been checked regularly since birth. The boy showed a significant deviation in behaviour pattern and development of body habitus already from early infancy.

HLA-B27 negative ankylosing spondylitis in a father and a son.

Only 5% of AS patients are B27 negative. We describe two cases of HLA-B27 negative AS in a father and a son who both developed clinical and radiological features characteristic of AS. Tissue typing for HLA-A,-B,-C was performed in all 1.degree family members except for the father who died in 1990. All family members possessed B40. HLA-B40 may contribute to an increased susceptibility for AS not only in B27 positive individuals but also in B27 negative cases.

Pars planitis in father and son.

The authors examined a family in which father and son presented with pars planitis. Both of them and the paternal grandparents were HLA typed; no association between HLA antigens and pars planitis was found. Present and previous data suggest a relation between pars planitis and allergic predisposition.

Congenital alacrima without associated manifestations (AD). An affected father and son.

The authors report a Kuwaiti Arab family in which the father and one of his two sons have severe hypolacrimation with blotchy staining of the cornea and punctate staining of the interpalpebral bulbar conjunctiva by fluorescein and Rose Bengal. Pharmacologic testing together with biochemical analysis and systemic examinations and investigations suggest an isolated dysfunction of lacrimation. The authors' small Arab family differs from the only other recorded pedigree (Irish) in which all five affected members in four generations also had atopy.

{Social support and the help offered by mothers to their psychologically troubled sons or daughters}.

We describe in this paper the everyday support mothers give to their adult sons or daughters suffering from psychotic disorders. We also describe the support these women receive from their social network regarding specific needs related to their children's disorders. Ninety-nine women, representing all socioeconomic groups, were interviewed. They were recruited through hospitals and self-help groups. The results of this study highlight the fact that participants give a substantial amount of support to their children, that they have considerable concern for their children's well-being, and that members of their family are their main source of emotional and practical support, and of advice. Mental health professionals give primarily informational support to respondents, in part through community-based agencies. The respondents who belong to self-help groups have significantly more needs related to their children's disorders and have more concern for their children's well-being than do the other respondents.

Adolescent sexual matricide following repetitive mother-son incest.

A case of a 16-year-old male who committed a sexual matricide following years of mother-son incest is reported. After murdering his mother by strangulation, which itself was sexually arousing, the youngster engaged in both vaginal and anal necrophilia. The homicide occurred while the perpetrator was in a dissociative state and experiencing what has been referred to as a catathymic crisis: the sudden release of emotionally charged psychic conflict and tension, resulting in extreme violence within an interpersonal bond. Discussion of maternal image and maternal sexual conduct in relationship to the psychosexual development of adolescent males offers insight into the motivation in this extremely rare case.

Sons as sole caregivers for their elderly parents. How do they cope?

OBJECTIVE: To examine the experiences of men who are sole caregivers for their elderly parents. DESIGN: Semistructured in-depth interviews. SETTING: Family practice clinic attached to a large tertiary care centre in north central Toronto. PARTICIPANTS: A convenience sample of 10 men who identified themselves as sole caregivers in that they had no particular women assisting them with caregiving. METHOD: Interviews were analyzed by standard qualitative methods. MAIN FINDINGS: Emerging themes were the spectrum of caregiving, the experience of caregiving, and the use of formal support systems. Scope of care varied from very little to total care, including personal care. Participants described positive and negative aspects of and the nature of their relationships with those for whom they cared. Avoiding institutionalization was seen as positive; effects on work and social life were negative. Use of more than homemaking services was associated with previous hospitalization; participants complained about difficulties accessing services. CONCLUSIONS: The nature of sons' relationships with their parents and the amount of time they have available can predict how much caregiving they can undertake. Information about community support services is not readily accessible to these men.

Acrogeria of the Gottron type in a mother and son.

We report a familial case of acrogeria in a mother and son, with characteristic cutaneous involvement and no clinical signs of vascular Ehlers-Danlos syndrome (former EDS type IV) in spite of some tendency to bruising. The biochemical and molecular studies did not disclose any abnormality of collagen type III, which favours the diagnosis of acrogeria. It appears that recognition of acrogeria as an entity is of clinical significance since these cases are not associated with systemic involvement, and specifically with rupture of vessels and internal organs, occasionnally occurring in EDS.

X-linkage does not account for the absence of father-son similarity in plasma uric acid concentrations.

Plasma uric acid concentration aggregates in families, and this similarity has been suggested to be due, in part, to multiple shared genes. Men have higher plasma uric acid concentrations than women and are affected with gout nine times more frequently. Rare forms of hyperuricemia and gout are due to mutations of X-linked genes (HPRT1 and PRPS1). Given these observations, we tested the hypothesis that normal variation in plasma uric acid levels would display a pattern of familial similarity consistent with X-linkage in 892 individuals from 196 obese but otherwise healthy families. As predicted by X-linked inheritance, fathers and sons showed no resemblance in plasma uric acid concentration (r = 0.013, NS), while all other pairings showed moderate-to-strong familial resemblance (ranging from 0.167, P < 0.01, parent-offspring to 0.415, sister-sister, P < 0.01). We then tested the hypothesis that loci along the X chromosome would influence plasma uric acid concentration. We conducted both single-point and multipoint linkage analyses using 17 X-linked markers spaced at approximately 9 cm intervals to determine whether allele sharing among sibs was related to sib similarity in plasma uric acid concentrations (n = 1,100 sib pairs). We found no regions of the X chromosome that cosegregated with plasma uric acid concentrations (P > 0.05). We conclude that variation in genes on the X chromosome contribute little to normal variation in plasma uric acid concentrations.

A father and son with a nonsevere form of Crouzon's syndrome.

Crouzon's syndrome is a hereditary autosomal-dominant disorder. In its classic form, patients experience a premature closure of the cranial sutures, which leads to brachycephaly, proptosis, a small maxilla, and anomalies of the external and middle ear. In this report, we describe the case of a father and son who both had a nonsevere form of this disorder. The two men did not have brachycephaly or proptosis, but they did have ptosis and a mixed-type hearing loss.

Intellectual, cognitive, and academic performance among sons of alcoholics, during the early school years: differences related to subtypes of familial alcoholism.

BACKGROUND: Research on intellectual and cognitive functioning of children of alcoholics has been marked by inconsistency, with some studies unable to document deficits. This discrepancy may reflect the substantial heterogeneity found in the alcoholic population and among families of alcoholics. The current study sought to examine the effects of familial alcoholism subtypes on intellectual, cognitive, and academic performance in early school-aged sons of alcoholics. METHODS: Subjects for the present study were 198 elementary-age boys who were participants in the larger MSU-UM Longitudinal Study. Familial alcoholism subtypes were determined based on fathers' alcoholism and antisocial personality disorder diagnoses. Intellectual functioning was measured with the Wechsler Intelligence Scale for Children-Revised (WISC-R); academic achievement was measured with the Wide Range Achievement Test-Revised. In addition, Mazes and Freedom from Distractability factor scores of the WISC-R were used to assess abstract planning and attention abilities. RESULTS: Children of antisocial alcoholics (AALs) displayed the worst IQ and academic achievement compared with children of nonantisocial alcoholics (NAALs) and controls. In addition, children of AALs displayed relatively poorer abstract planning and attention abilities compared with children from control families. Regression analyses revealed that familial alcoholism subtype continued to account for variance in child intellectual ability even when other factors were excluded. CONCLUSIONS: Findings indicate that children from AAL families are most susceptible to relative intellectual, cognitive, and academic deficits. The study further supports the proposition that familial risk characteristics (i.e., paternal alcoholism and antisociality) may serve as effective indicators of family risk for poor intellectual outcome among offspring as early as the elementary school years.

[Fluorescence diagnosis of a lengthened Y-chromosome in father and son].

Two cases with long Y chromosome in a father and his son have been described. The chromosomes were analysed routinely using the peripheral blood leukocyte culture according to Moorhead et al. (1960) and of the slightly modified fluorescence method of Caspersson (1970, 1971) after QM-staing. In both patients the size of the Y resembled that of the pair 17--18 of autosomes. A brightly fluorescent heterochromatic region of the Y showed normal patterns and size. In one of the patients the analysis of solar and palmar dermatoglyphics, the testicular biopsy, EEG, glucose loading test, as well as laboratory tests evaluating the hormonal function of adrenal cortex and of thyroid gland were performed. The second patient did not agree to any tests.

The elective treatment of graft-versus-host disease following a bone marrow graft from a father to a son with severe combined immunodeficiency.

The details of the first successful bone marrow graft from father to son are described. The recipient developed an acute graft-versus-host reaction on the seventh day which was successfully treated with anit-lymphocyte globulin and methotrexate. This regime did not destroy the graft and the child is alive and well with normal immunological function some 2 1/2 years later.

[The Dodarts- father and son- physicians to the king].

Denis Dodart, barrister grandson, was born in Paris in 1634 and practiced especially botanical studies. He taught pharmacy and was, at first, the Duchesse de Longueville's own Doctor and, then, of the Princesse de Conti before acceeding to the rank of Conseiller-médecin to Louis XIV. In 1673, he was accepted as a member of the Academie Royale des Sciences (as botanist). In 1698, he became medical doctor to the Court retinue and Madame de Maintenon chose him for giving cares to Saint-Cyr schoolgirls. He died november the 5th 1707 and buried in Saint Germain l'Auxerrois Church, in Paris. His son : Claude Jean-Baptiste, born in 1664, was admitted Doctor to the Faculté de médecine of Paris in 1688 ; soon patronized by the Princesse de Conti he obtained the same appointment of Saint-Cyr School's Doctor than his father. Then, in 1693, he got also the same appointment to the Court retinue. In 1701, the Duc d'Orleans took him as "Principal Docteur". He proceeded with that title to the young Duc de Bretagne's suite and, further, to the Duc de Bourgogne. He became Louis XV's "Principal Docteur" on April the 6th, 1718 ; he died, November the 25th 1730, in Paris.

Difference in goiter rates between two areas in Mae Hong Son Province despite an equally sufficient iodine supply.

The study was designed to investigate the problem of endemic goiter and iodine supplementation in Mae Hong Son, Thailand. Routine school data still showed high goiter rates in many areas. Six such schools and five schools with declining prevalence of goiter were selected. A single examiner examined all the children. Their weight, height, body mass index, ethnicity, history of iodine intake and migration were recorded. Urine samples were collected for measuring iodine and thiocyanate levels. From 653 children, 105 and 13 were found to have grade 1 and 2 goiter status, respectively. Median values of urine iodine level in children from all schools suggested sufficient iodine supplementation. Multivariate analysis showed that hilltribe minorities had a 2.09 times higher risk of endemic goiter than Thai children. Urine thiocyanate levels among children from high prevalence schools were significantly higher than those from low prevalence schools. No other significant correlation was found. Possible roles of other known and unknown goitrogens should also be investigated.
