[The stomatology and maxillo-facial surgery department].

Initially devoted to emergencies, the Department of Oral and Maxillofacial Surgery has grown considerably since 1979. The department now consists of a Maxillofacial Surgery Unit supplemented by several specialised Units in charge of oncology, orthodontics and orthognathic surgery, implantology and all dental specialities. The main themes of research developed in the department include the healing process of oral tissues, the teratogenic effects of various compounds on the cranio-facial region, the study of apoptosis, the role of heat shock proteins, and the management of craniofacial dysmorphosis. These studies have a direct impact on patient management.

Physicochemical properties of stomatological dressings on the basis of biodegradable nonionic polymers.

The properties of polyvinyl alcohol (PVA), such as the ability to form membranes, good relative bioadhesiveness, the ability to mix with other polymers, biodegradability, swelling in water, lead to an assumption that the polymer may be used effectively as a carrier of the active substance onto the mucous membranes, what has led us to undertake present studies [1-4]. They revealed that depending on the degree of polyalcohol hydrolysis, the dressings made on the basis of PVA and methylcellulose (MC) with the addition of hydrophilic substances were characterised by distinct rheological properties and released the active substance in various ways. The studies on the kinetics of the release of a locally applied antiseptic, chlorhexidine digluconate, in biopharmaceutical model provided evidence of close to rectilinear relationship between the amount of released substance and the time of release for selected formulations.

[Platelet aggregation inhibitors and vitamin K antagonists in stomatology and maxillo-facial surgery].

Anti-thrombin, anti-platelet and anti-vitamin K agents are common treatments given for the prevention of thrombotic or embolic events. In oral or maxillofacial surgery, the question is whether oral anticoagulants or anti-platelets should be discontinued or not: temporary interruption could limit the risk of bleeding, but would increase the risk of thrombosis, and inversely. The best risk/benefit ratio must be achieved, but in many cases anti-platelet agents or even anti-vitamin K agents can be continued. In case of "potentially hemorrhagic" oral surgery, anti-platelet agents should be interrupted about ten days before the intervention. Anti-vitamin K agents should be interrupted four days before surgery and if required, replaced by low-molecular weight heparin at preventive or curative doses. In the very large majority of patients however, surgery (avulsion for example) can be performed without interrupting anticoagulation. Data in the literature have demonstrated that despite the theoretical risk of hemorrhage, bleeding is minimal and can be easily controlled by local treatments; the risk of thrombotic or thromboembolic events after discontinuing anticoagulation is much greater. A close collaboration between the dentist, the stomatologist, and the clinician is required to determine whether anti-thrombosis prevention should be continued and whether the anticoagulation level is adapted to the thrombotic risk. Good surgical technique and local anti-hemorrhagic treatment are essential in oral surgery, particularly in patients taking anti-platelets or oral anticoagulants.

[Dental wear as a stomatologic manifestation of bulimia nervosa--a clinical case].

The number of patients with anorexia and bulimia nervosa is increasing nowadays. The typical oral feature of these eating disorders is the dental erosion which causes sensitivity of the teeth and esthetic problems for the involved patients. This phenomenon is a characteristic feature in these cases and it may be the first sign of the mentioned disorders. The purpose of the study was to describe the generally the most characteristic oral findings of bulimia nervosa and anorexia nervosa because the dentists play a significant role in recognizing the basic problem of the patients, and they can send them for medical treatment of the serious general problems.

Towards higher standards in dental and stomatological education in Europe.

In the creation of the European Union, attention was given to portability of licensure for professionals. Considerable differences exist among countries in culture, economic conditions, and educational resources and practices. In dentistry, these differences in professional training have been addressed through a peer consultative process rather than through political and legal means. The process of visits to dental schools throughout Europe and the organizational structure (DentEd) used to conduct the visits and summarize findings are described.
