The long-term of endodontic treatment performed by under graduated students in Dakar were assessed. The overall success rate was 83.84% Vital teeth showed better results than the non vital teeth (necrosis).
Iatrogenic perforations from the pulp cavity towards the periodontium result in complications that often lead to root or tooth removal. Identification of perforations is possible by direct observation of bleeding, indirect bleeding assessment using paperpoints, radiography and an apex locator. Successful treatment depends mainly on immediate sealing and prevention of infection. Several factors affect the achievement of these goals, such as time of occurrence, size and location of the perforation and the lability of the repair material to seal the lesion. An overview is presented of diagnosis, prognosis and non-surgical treatment of perforations.
The retrospective study on the endodontic retreatments done at the Odonto-Stomatology Institute of Dakar from 1986 to 1997 shows a small rate of 1.94% (unreached canalar root filling) for 4325 endodontic treatments. The endodontic retreatments concern almost women (54.76%), ageing between 20 to 30 years (45.24%); maxillary central incisors (21.43%) and first molars (21.43%). The clinical reason having conducted to a retreatment is prosthetic (46.4%) and symptomatic (38.10%); the immediate success rate of these retreatment stays favourable at 59.52%.

Periradicular lesions are diseases either primarily or secondarily caused by microorganisms and therefore they must be prevented or treated accordingly. If the professional is well-versed in both preventing and eliminating the root canal infection, the success rate of endodontic therapy may exceed 90 percent. The present paper discusses theoretical and practical aspects of effective antimicrobial endodontic therapy and delineates strategies to effectively control root canal infections.

A postal questionnaire was used to evaluate endodontic treatment performed in the general dental services in Birmingham, assessing whether practice type--private or National Health Service (UK), or year of graduation, influences compliance with established guidelines. For the aspects of treatment investigated, many dentists were not following guidelines. More private practitioners adhered to the guidelines. Practitioner age was not related to compliance with guidelines, although there were differences in the procedures and materials employed among the different age groups. Within the survey's limits, it was concluded that endodontic performance varies from established guidelines.
A 12-year-old boy presented with a fractured sewing needle within the palatal root canal of a nonvital permanent maxillary first molar. The patient admitted that he had often placed a sewing needle in the tooth to relieve discomfort associated with it. A minimal amount of tooth structure was removed around the foreign object to facilitate access for K-type files. Under copious irrigation, the files were worked around the object to loosen it. The fractured needle was removed with a tweezers that had long, narrow beaks. Use of this simple technique allowed the object to be removed from the root canal with minimal damage to internal tooth structure.
The importance of apical seal has always been stressed in endodontic therapy. Root canal sealers have been used along with gutta percha to enhance the seal. The apical sealing ability of AH26/silverfree, a modified version of AH26, a resin based sealer was compared with tubliseal, a zincoxide eugenol based sealer using methylene blue dye penetration method. AH26/silverfree showed superior sealing ability.
