Cultural beliefs about rape and victims' response in three ethnic groups.

Cultural definitions of rape were assessed among 101 African-American, Hispanic, and non-Hispanic white female rape victims and 89 nonvictims matched for ethnicity, age, marital status, and socioeconomic status. Hispanics scored highest and whites lowest both in perceived community victim-blaming and in victims' psychological distress. Social and treatment implications are discussed.

Issues in balancing teenage clients' confidentiality and reporting statutory rape among Kansas Title X clinic staff.

Through Federal welfare reform, Congress directed states to aggressively enforce statutory rape laws. Family planning professionals deal with many adolescent clients, and their support for such enforcement or willingness to report is unclear. The authors of this study examined current attitudes and practices of family planning program managers (FPPMs) about statutory rape law enforcement, including current reporting practices. In 1997, all 77 local Kansas Title X FPPMs were surveyed. Structured telephone interviews were conducted with 10 FPPMs to add detail to quantitative responses. Sixty-eight FPPMs responded to the written survey (88%). Of these, 79% supported aggressive enforcement, and 43% thought enforcement would reduce adolescent pregnancy rates. With increased enforcement, 38% believed teenagers would be discouraged from seeking reproductive health care, compared to 41% who believed they would not. Among key informants, all of whom were FPPMs, willingness to report cases was mixed, with those who would report wanting the flexibility to judge on a case-by-case basis. For those not reporting cases, confidentiality concerns overrode beliefs in any positive outcome of enforcement. Kansas Title X FPPMs strongly supported aggressive enforcement, but had mixed beliefs about negative consequences. Among those interviewed, there were also mixed beliefs and practices about reporting. Reporting from FPPMs will be sporadic and arbitrary unless protocols are developed and laws are clarified.

[Extragenital injuries in rape].

The documentation of extragenital injuries to verify violent cohabitation has considerable forensic relevance. 1,875 police files in Hamburg were investigated. Injuries were analysed under the following aspects: kinds, localisations and patterns. According to the most frequent conduct of the doer (manual fixation of the victim and beats in 60% of all cases), consequences of contused violence (i.e. effusions of blood, contusions, dehiscences) were found in 50% of all cases. Injuries due to semi-sharp violence, wounds of genitals or signs of choke were relatively rare (15-3-3%). An isolated state of genital organs is of minor importance. The most frequent localisations of injuries were face and arms, less frequently legs and back. Injuries as result of manual fixation, defence or abutment formed the main patterns. "Banal" hurts were also found. In numerous cases, the outer appearance of the lesions enabled conclusions regarding the instrument used (i.e. double wales after beats with a stick). Self-inflicted injuries can mostly be discerned by their kind and localisation. Rape without any injuries is conceivable as the result of missing defence in the presence of severe threat. Psychic alterations were found in 20% of the cases, half of them continuing for a long time. The medical examination of the suspected man must include the search of signs of a fight (scratch marks, bites), localised at the hands, face and neck (besides ascertainment of traces, for example, smears of semen and smegma, i.e. from the glans penis). Extent and thoroughness of medical examinations as presently practised are discussed critically. The manner in which these examinations are conducted, is considered to be the main cause of a "secondary victimisation" of the raped woman.

Straw men and fairy tales: Evaluating reactions to A Natural History of Rape.

In this paper we respond to two frequent criticisms of our book, A Natural History of Rape (Thornhill & Palmer, 2000). The first criticism portrays the book as little more than a "just-so story" that human rape is an adaptation. We demonstrate that this portrayal is not accurate. The second criticism reflects a common response to the book s challenge of the popular assertion that rapists are not motivated by sexual desire but instead commit these crimes motivated by the urge to power, domination, and violence, and the urge to degrade and humiliate women. We demonstrate that such criticisms of our book are inherently contradictory and illogical. We believe it is important for sex researchers to understand that these sorts of criticisms are seriously flawed so that future research efforts toward understanding the causes of sexual coercion are not stalled.

Acquaintance rape: the effect of race of defendant and race of victim on white juror decisions.

Racial bias appears to lead jurors in trials of stranger rape to convict Black defendants more readily and to sentence them more harshly than White defendants. It was hypothesized that jurors in an acquaintance rape case would demonstrate a different pattern of bias, based not only on the race of the defendant but also on the racial nature of the defendant-victim relationship. White American undergraduates read a trial transcript that established defendant-victim familiarity and sexual contact but was ambiguous about the victim's consent. Race of defendant and of victim (Black or White) were varied on a 2 x 2 design. The participants were asked to rate the guilt of the defendant and to recommend a sentence. Both Black and White defendants were rated as more guilty when the victim's race differed from their own.

Group therapy: an alternative treatment approach for rape victims.

The current study describes a pilot project in which rape victims were treated in group therapy. Objective measures of psychological disturbance before and after 10 weeks of treatment revealed the victims experienced substantially reduced anxiety, fear, and hostility levels. This finding was independent of the amount of time between the rape and the initiation of treatment. The data also suggest that some victims do not benefit and may even worsen with treatment.

Incidence and types of sexual dysfunctions in rape and incest victims.

Many authors have commented on the impact of sexual assault on the sexual lives of the victims. Eighty-three victims were interviewed with regard to their sexual histories, and completed the Sexual Arousal Inventory, to determine the incidence and types of sexual dysfunctions in rape and incest victims: 56% of these victims experienced sexual dysfunctions postassault; 71% of these subjects reported that the sexual assault precipitated the dysfunction. Fear of sex, arousal or desire dysfunctions were the most common sexual problems presented within this victim sample. The Sexual Arousal Inventory was not successful in discriminating dysfunctional and nondysfunctional rape and incest victims.

Athletes as rape-awareness educators: athletes for sexual responsibility.

Sexual assault, including both date rape and gang rape, is an area of great concern for college students today. The University of Maine is addressing this serious national problem through a unique peer education program that enlists athletes as role models for appropriate social and sexual behavior, as well as of physical strength, agility, and stamina.

Prostatic acid phosphatase. Current assessment in vaginal fluid of alleged rape victims.

Although elevated prostatic acid phosphatase activity (ACP) in vaginal fluid is compatible with recent coitus, the finding of spermatozoa in vaginal fluid is usually considered the diagnostic indicator for semen. When 80 alleged rape cases during an 18-month period were reviewed and the results of cytologic examination for the presence of spermatozoa compared with quantitative ACP determinations, the latter appeared to be a more reliable and sensitive indicator of semen. The normal range of ACP in semen, as well as persistence of ACP in vaginal fluid, was also defined. It is concluded that vaginal fluid ACP is a reliable and sensitive method for identification of semen. Furthermore, the results confirm that quantitative ACP determination of vaginal fluid specimens may substantiate the allegation of rape with respect to time.

Epidemiology of sexually transmitted diseases in rape victims.

Rape is a violent crime that is increasing rapidly in incidence. Victims can include male and female adults and children. These victims may be at risk for the acquisition of many sexually transmitted conditions. The risk will vary with the sex and socioeconomic status of the assailant as well as with the nature of the sexual contact. Infections with Neisseria gonorrhoeae, Chlamydia trachomatis, Trichomonas vaginalis, herpes simplex virus, human papillomavirus, and Treponema pallidum have all been found to occur following sexual assault of female adults or children. Although data are lacking, male victims of homosexual rape are presumably at risk for the acquisition of the above infections as well as hepatitis B, the acquired immunodeficiency syndrome, and other infections prevalent among homosexual men. All victims of sexual assault should undergo a comprehensive physical examination with a complete history. Baseline and follow-up laboratory tests should be performed. In some instances, prophylactic administration of antimicrobial agents may be warranted.

A study of the effectiveness of commercially available drink test coasters for the detection of "date rape" drugs in beverages.

The use of illicit substances for the purpose of drug-facilitated sexual assault (DFSA) poses a significant problem. There has been an increase in public awareness of this problem, and a recent invention in the form of a drink coaster claims to detect whether or not a beverage has been spiked with a so-called date rape drug. A person is instructed to place a drop of the suspect beverage onto two spots of the test, smear gently, and wait until dry. If either spot turns to a darker blue color, then a possible date rape drug has been detected by the coaster test. In an effort to determine the effectiveness of the coasters, various drugs that have been associated with drug-facilitated sexual assault were tested at different concentrations in a variety of common alcoholic and non-alcoholic beverages. It was found that although the coasters do detect the presence of GHB and ketamine, two drugs that have been associated with DFSA, there are limiting factors such as the high concentration of the drugs required, hindrance of the reaction due to beverage matrix, and extensive time requirements for ketamine analysis.

False rape: a case report.

A 16-year-old girl was admitted to the emergency department for sexual assault. The forensic examination revealed genital lesions of an age that were incompatible with her statements. She also presented extragenital lesions that resembled self-inflicted lesions. The reports of false rape allegations in the literature have all dealt with the motivations of the false victims. This case report is a reminder that an allegation of rape can be considered only on the basis of proof and not on speculation.

Multiple group rape: psychosocial considerations.

Psychiatric examination of the participants in repeated episodes of rape demonstrated that sexual assault combined with stealing represents symbolically an attack upon the bad mother, such hostile acting out enabling the individual to defend against strong, but ambivalent, wishes to be nurtured. Separation from home in each case was considered a crucial precipitant of such behavior. It is suggested that exhibitionistic-voyeuristic conflict may be an important determinant of whether or not rape is committed in group fashion.

Trauma-related sleep disturbance and self-reported physical health symptoms in treatment-seeking female rape victims.

The purpose of the study was to assess the relationship between trauma-related sleep disturbance and physical health symptoms in treatment-seeking female rape victims. A total of 167 participants were assessed for PTSD symptoms, depression, sleep disturbance, and frequency of self-reported health symptoms. Results demonstrated that trauma-related sleep disturbance predicted unique variance in physical health symptoms after other PTSD and depression symptoms were controlled. The findings suggest that trauma-related sleep disturbance is one potential factor contributing to physical health symptoms in rape victims with PTSD.

Relative preference and adaptiveness of behavioral blame for observers of rape victims.

Observers viewed one of nine dramatized videotaped interviews of a rape victim describing her rape. Information in the interview varied the prudence of the victim's behavior (careful, careless, no information provided) and the respectability of her character (good, bad, no information provided). Behavioral blame was significantly greater than characterological blame when the victim was careless or when no information was provided about behavior, regardless of the victim's character. When the behavior was careful, behavioral blame was equal in magnitude to characterological blame. In no case was characterological blame preferred. The adaptive value of behavioral blame for preserving a belief in a controllable and meaningful world was examined using a hierarchical multiple regression. After removing the effects of the prebeliefs of the subjects and the independent variable manipulations, only behavioral blame was significantly related to the maintenance of adaptive beliefs. Implications of the adaptive value of behavioral blame are discussed along with the importance of distinguishing observers' behavioral and characterological blaming strategies in the victimization literature.

Should medical students be taught about rape? A survey of UK medical schools.

Rape and other serious sexual assaults are common and result in long-term morbidity. Increasing numbers are reported but conviction rates are low. Victims of sexual assault present to a wide range of healthcare settings. Good immediate medical care and evidence collection are important in engaging victims with the criminal justice system, avoiding the loss of crucial evidence and minimising long-term morbidity. Of 21 UK medical schools surveyed, only eight provided teaching about sexual assault and ten provided other forensic teaching. Sixteen schools provided guidance about personal safety. As rape is so common and traumatic, medical schools should seriously consider providing teaching about this area.

Rape flashbacks: constructing a new narrative.

This article offers an approach to the dissolution of rape flashbacks when the usual treatment of talk and group support doesn't work. The client, with her family, constructs a new rape scenario to replace her abusive story. In the new story, the client introduces a chosen protective figure into the flashback, which has the effect of interrupting the old scenario so that it cannot take place in the same way again. This approach relies on the client finding a new or freed voice in the treatment conversations in order to tell her story differently. The new, repopulated story eventually has the effect of altering her memory and renewing her life direction and belief in herself.
