Maternal deaths at Pelonomi Hospital, Bloemfontein, 1980-1985. A survey of 81 consecutive cases.

From 1 January 1980 to 31 December 1985, 81 maternal deaths occurred at Pelonomi Hospital, Bloemfontein; these were classified as direct obstetric-related (74 cases), indirect obstetric-related (6) and non-obstetric (1). The overall maternal mortality rate was 2.87 per 1000 deliveries, including deaths related to abortion and ectopic pregnancy, but excluding the non-obstetric death. Among booked patients the maternal mortality rate was 0.32 and among unbooked patients 11.13 per 1000 deliveries. The maternal mortality rate for patients from the Bloemfontein area was 0.72 per 1000. Puerperal sepsis and postabortal sepsis accounted for 45.7% of the deaths. Avoidable factors were considered to be present in 65 cases (80.2%).

Abortion in adolescence.

Sexual attitudes and behavior of adolescent females have been the topic of much interest over the past decade. Feelings about contraception, conception, and abortion have been described in relation to the adolescents' beliefs about the possibility of becoming pregnant, who will or will not "protect" them, and the influence of significant others on their decision making. This study explores differences in 35 women who had abortions during their teenage years with 36 women whose abortions occurred after the age of twenty. A demographic questionnaire, the Millon Clinical Multiaxial Inventory, and the Beck Depression Inventory were completed by women who were members of a patient-led support group. Premorbid psychiatric histories, the decision-making process itself, and distressing symptoms postabortion are reported. Specific differences in perceptions of coercion, preabortion suicidal ideation, and nightmares post-abortion were found in the adolescent group. Antisocial and paranoid personality disorders as well as drug abuse and psychotic delusions were found to be significantly higher in the group who aborted as teenagers. Hypotheses regarding the influences of adolescent development on mother/child relationships, power struggles, and the use of fantasy as a coping device are explored.

Maternal rubella and the congenital rubella syndrome.

The major goal of rubella immunization is the prevention of the congenital rubella syndrome. As many as 20 per cent of women in the reproductive age group in the United States continue to be susceptible to rubella despite the immunization programs currently in place. Intensified efforts are therefore needed to identify persons at risk for infection and to vaccinate them. Women who develop a rubella-like illness during pregnancy should have the diagnosis confirmed serologically because a diagnosis based on clinical criteria alone is unreliable and because of the serious implications of gestational rubella infection. The rubella virus can infect the fetus at any stage of pregnancy, but defects are rarely noted when this occurs after the 16th week of gestation. The most common abnormalities in the congenital rubella syndrome are hearing loss, mental retardation, cardiac malformations, and eye defects. Diabetes mellitus, thyroid disease, glaucoma, and other delayed manifestations of congenital rubella syndrome are common, thereby necessitating long-term followup of these patients. The detection of rubella-specific IgM antibodies in fetal blood is helpful in establishing the diagnosis prenatally and can aid in the management of pregnancies complicated by this infection. Susceptible women identified through screening during pregnancy should be immunized in the immediate postpartum or postabortion period. Although the live, attenuated rubella vaccine is contraindicated during pregnancy, pregnant women who are inadvertently immunized are not candidates for pregnancy termination because no defects consistent with congenital rubella have been reported to date in the offspring of other similarly vaccinated women.

Clinicopathologic implications and diagnosis of nongranulomatous nonspecific endometritis.

The analysis of 600 subsequent endometrial biopsies from urban gynecologic outpatient clinics was carried out with special regard to nongranulomatous and nonspecific endometritis. The total incidence of inflammatory changes reached 10.8%; after excluding all postabortion and puerperal endometria 9.4% of the curettings showed endometritis. The relative occurrence of acute, subacute, and chronic disease was respectively 6.2, 30.8, and 63.0%. The etiologic factor remained obscure in 64.7% of all the cases. The influence of inflammatory changes on the hormonally induced modifications of the endometrium are discussed.

Randomized, comparative trial of imipenem/cilastatin and moxalactam in the treatment of serious obstetric and gynecologic infections.

Thirty-four patients with pelvic inflammatory disease, postoperative, postabortal and postpartum infections were randomized to intravenous therapy with either 500 milligrams of imipenem and cilastatin sodium every six hours or 2 grams of moxalactam every eight hours for a minimum of four days. One patient in the moxalactam group was nonevaluable because of protocol noncompliance; three more patients had no bacteriologic pathogen isolated (two in the moxalactam group and one patient in the imipenem/cilastatin group). The two groups were similar with respect to age, diagnosis, etiologic agents and duration of therapy. Of the 17 evaluable patients in the imipenem/cilastatin group, all were complete clinical cures. Three patients in the imipenem/cilastatin group had persistence of at least one bacteriologic pathogen despite clinical cure and apparent laboratory evidence of susceptibility. Of the 13 evaluable patients in the moxalactam group, eight were complete clinical cures. Two more patients in that group were clinically improved enough to be discharged on oral antibiotics. There were three clinical failures in the moxalactam group, all of whom had group D streptococcus resistant to moxalactam. An additional three patients in the moxalactam group had other resistant organisms isolated despite clinical cure. Both drugs were well tolerated and no serious complications or side effects occurred in either group. Despite small numbers, our data suggest that imipenem and cilastatin is a more appropriate agent for initial treatment of obstetric and gynecologic infections than moxalactam.

[Dynamics of Shigella interaction with the epithelium in the infection process].

The study of the adhesion of Shigella flexneri to intestinal mucosal explants from human postabortion fetuses, used as an experimental model, has revealed that the process of interaction between the infective agent and the epithelium develops in accordance with Langmuir's equation of the adsorption isotherm. The specific biological feature of the adhesive interaction between bacteria and the mucous membrane is the fact that the effective adhesion of microbes is possible only in case of their high concentration on the surface of the mucous membrane. In case of their low concentration in the parietal layer no microbial adhesion is observed, whereas epithelial villi infected with shigellae and fixed to the mucous membrane of the explant produce a high parietal concentration of the infective agents, which leads to the increase of adhesion by more than two orders.

Combined pregnancy: the Mount Sinai experience.

Nine recent cases of coexisting intra- and extrauterine pregnancy at The Mount Sinai Medical Center are presented. The myriad of clinical presentations is exemplified in the postabortal, endometriosis, posttreatment; ovulation induction, and routine "rule-out ectopic" situations. A brief review of the literature follows detailing historical reports of diagnostic inaccuracy and delayed diagnosis. Several studies are cited supporting the difficulty of correctly diagnosing heterotopic pregnancy as well as factors which may predispose to it. The advantages and pitfalls of ultrasonography are mentioned as well as recommendations for early laparoscopic intervention when clinical history and ultrasound are suggestive. Caution is advised in those infertility patients who may undergo "routine" curettage at laparoscopy for ectopic pregnancy and thus, be at risk for inadvertent termination of an otherwise salvageable intrauterine pregnancy. This report further supports several other reviews in noting that the incidence of combined pregnancy may be much greater than formerly appreciated.

[Abortion in the 2d trimester using carboprost tromethamine].

The authors present their own experience (1980-1985) in the application (im. so called "monotone rhythm and doses") of Prostin 15-M (Upjohn) in legal abortions in the second trimester (14th-20th gestational weeks). This preparation was applied in 470 patients divided into 3 groups for the purpose of differential treatment and analysis: A--gravidae juvenae 13-18 years of age, B--gravidae adultae (19-40 years of age) C--gravidae vetustae 41-46 years of age. The mean age of patients by age groups was: A-16 years + 8 months; B-26 years + 3 months; C-42 years + 7 months. Indications for abortion were: medical in 53.83%, eugenic in 30.21%, medico-social in 14.25%, and ethico-legal in 1.70%. The percentage of primigravidae was 40.42 and of multigravidae 59.76%. The average length of the abortion interval and the average dose of Prostin 15-M (in ml) were as follows: group A 28 h 30' and 8.92 ml; group B 23 h 23' and 8.23 ml. and group C 16 h and 4.97 ml. All recorded side-effects and complications were analysed. Significant complications occurred in 5 cases (1.06%): cervical rupture in 3 cases and massive postabortal haemorrhage in 2 cases. No deaths occurred. The authors elaborated a system of the differential evaluation of the efficacy of this method and its advantages over other methods previously applied. The method has proved to be very simple, efficacious, suitable from the medico-clinical point of view, with a relatively low percentage of complications, and economical (costs of treatment and hospitalisation).

Legalized abortion: the Singapore experience.

Abortion has been legal since 1970 in Singapore. This report traces the events leading to the liberalization of abortion laws and examines women's changing attitudes toward abortion, in Singapore. The method of abortion practiced by obstetricians, postabortal sterilization, and the effect of legalized abortion on the incidence of illegal abortion in the country are also examined.

Correlation between the incrustation of intrauterine devices and the duration of their use.

The usefulness and side-effects of intrauterine contraceptive devices (IUDs) are evaluated on the basis of various aspects (e. g. effect of postabortal and post-partal insertion, expulsion of device, frequency of pregnancies, bleeding and inflammation, etc.) [1, 2, 3, 4]. However, the incrustation of the device has been ignored although the coating on the major part of the surface is easy to study. The rough surface coating is very likely to be associated with some side-effects [5]. In our report we have tried to find some links between the degree of incrustation and the duration of using the device.

[Carcinoma of the uterine cervix associated with pregnancy--analysis of 458 cases].

From 1958-1978, 458 patients with carcinoma of the cervix complicated with pregnancy were treated in our hospital. It comprised 4.3% of the total number of cervical carcinoma admitted for radiotherapy during the same period. In these patients, 112 cases were antenatal and 346 cases were postnatal. The five year survival rates were 56.2% and 44.7%, respectively, both being lower than that (69.4%) of nonpregnant patients. Statistics showed: 1. the earlier the period of pregnancy, the better the prognosis. 2. within six months postpartum, the earlier the treatment, the higher the survival rate. These results suggest that pregnancy has an unfavorable effect on cervical carcinoma. Irradiation treatment was essentially the same for patients who were pregnant or not. Termination of pregnancy, however, was a prerequisite. In the first trimester of pregnancy, spontaneous abortion with radiation was advocated. In the second and third trimesters of pregnancy, hysterotomy and removal of foetus for proper irradiation was done. Intracavitary irradiation was recommended before termination of pregnancy and postabortal or postpartum external radiation was started as early as possible in order to achieve more favorable results.

Infection in acute renal failure.

The results are reported of the incidence, severity, and description of infectious complications in 646 cases of acute renal failure (ARF). Infection was felt to represent an important cause of ARF in 34% of the cases. However, the frequency and extent of the infection varied according to the biologic classification. It was present constantly in postabortion ARF, was frequent in medical and in postoperative ARF, and was rare in traumatic and postpartum ARF. The renal lesions vary according to the cause of the infections. Staphylococcus septicemia, leptospiral infections, and rickettsial infections are the causes of interstitial nephritis; whereas gram-negative septicemia, probably via infectious shock, leads more readily to tubular lesions. Hemolytic septicemias most often cause tubular necrosis, although this is usually reversible. When the initial clinical picture is complicated by disseminated intravascular coagulopathy, bilateral cortical necrosis is a distinct possibility. During the established phase of ARF, infections are equally frequent, whether primary or secondary. The most frequent complications are septicemia and bacteremia--or local complications, most often pulmonary or urinary tract infections. The organisms are mainly Staphylococcus (often mephicillin-resistant) and gram-negative, usually E. coli. The most effective treatment is a combination of cephalothin and gentamicin. Pseudomonas infections are the most difficult to treat. The frequency of these serious infections and the difficulty with antibiotic therapy, often dangerous in renal insufficiency, stress the importance of preventive treatment of prophylactic measures. Infection was primarily responsible for 19% of the deaths in our series and may be also largely responsible for some of the persistent residual renal functional impairments.

Early complications and sequence of pregnancy interruption with hypertonic saline.

Midtrimester abortions and a number of cases with missed labor were induced by intrauterine instillation of 30% saline solution in 103 women. The instillation-abortion time was relatively short (mean 25.4 h) while oxytocin was not needed for the augmentation of contractions either in elective abortions or missed labor. Eighty-seven women had a curettage after the abortion; this was directly related to fetal weight. The first puncture was unsuccessful in 11 women (10.7%) and a second puncture was made a week later. Ten women had early complications, mainly fever for a few days; there were no instances of uterine perforations or disseminated intravascular coagulopathy (DIC). Postabortal curettage decreased the incidence of early complications. All study patients had a follow-up examination after 2 to 22 yr from the abortion. In 10 women some sequelae were found, but only in three women was the problem possibly related to the saline abortion (hydrosalpinx, incompetent cervix, and intrauterine adhesions). Of 71 women who subsequently wished to become pregnant, 69 succeeded: seven had more than one pregnancy, in three there were recurrent abortions. In 94 patients menstruation was normal, in eight bleeding became heavier, and there was one case of oligomenorrhea. In the authors' experience and contrary to other publications, midtrimester abortion with concentrated saline has been safe and rapid, and late sequelae have been rare and less important than after early induced abortions.

Total prevention of Rh hemolytic disease: possible or impossible?

Rh immune globulin prophylaxis has reduced the incidence of Rh immunization in Manitoba from 10.6 per 1000 births in 1964 to 1.8 per 1000 births in 1979. With total physician, patient compliance, and universal antenatal, postnatal postabortion, postamniocentesis Rh prophylaxis programs and Kleihauer fetal red cell screening, the incidence of Rh immunization will be reduced even further. However, since there is a 0.106% incidence of Rh immunization appearing before antenatal Rh prophylaxis is given and a 0.085% incidence of Rh immunization occurring in which the cause of the immunization is unknown, total prevention of Rh immunization is impossible. Assuming a family size of 2.2 to 2.5 children, reduction of the incidence of Rh immunization to 0.4 per 1000 births is possible and should be our goal.

Uterine T-mycoplasma colonization in reproductive failure.

In 46 patients with a history of habitual abortion and in 18 women with primary unexplained infertility, samples from cervical mucus and endometrial tissue have been examined for presence of T-mycoplasma, and the results compared to those obtained among 45 control subjects. Colonization of T-mycoplasma in the cervix was found to be common in all patient groups (49 to 83 per cent), whereas colonization in the endometrium was found to be significantly more frequent among the habitual abortion patients (28 per cent) and the infertility patients (50 per cent) than among the control subjects (7 per cent). In all patients having T-mycoplasma-positive endometria the organism was also isolated from the cervix, thus indicating an ascending route for the colonization of the uterine cavity. Bacteria belonging to the normal vaginal flora were frequently co-isolated from the endometrial samples along with T-mycoplasma. The women with T-mycoplasma-positive endometria showed a high incidence of postabortion fever. Treatment with doxycycline eradicated T-mycoplasma from the cervix and the endometrium and apparently led to an improvement with respect to the outcome of the posttreatment pregnancies.

Use of contact hysteroscopy in evaluating postpartum bleeding and incomplete abortion.

Contact hysteroscopy is a new endoscopic technique that allows direct visual examination of the uterine cavity. Such direct visualization has several potential advantages, including localization of pathology, determination of the adequacy of dilatation and curettage and of biopsy, and precise diagnosis of uterine anomalies. Contact hysteroscopy was used to examine 95 patients with postabortal or postpartum bleeding. The technique was judged successful from the standpoint of technical simplicity, diagnostic accuracy and absence of complications.

Voluntary termination of pregnancy.

Multiple factors are involved in the decision to terminate an unplanned pregnancy. Gestational age is the most important factor affecting the risk of abortion; after six weeks gestation, the earlier the abortion is performed, the safer. Currently, 90% to 95% of abortions occur within 12 weeks of gestation. However, abortion availability and accessibility remain a problem. A discussion of abortion incidence, procedures, complications, and pre- and postabortal evaluation is presented. Implications for the nurse practitioner are reviewed.

Fertility after induced abortion: a prospective follow-up study.

Abortion has been alleged to cause sterility. To address this issue, 1235 postabortal women, 912 women recently delivered, and 939 women from the authors' gynecology clinics were studied for as long as 3.5 years. Cumulative pregnancy rates were similar for abortion group patients, delivery group patients, and the previously pregnant clinic group. When pregnancy rates were examined by different assumptions as to entrance into follow-up, the abortion group differed only in a somewhat lower pregnancy rate after nine months, when the use of contraceptives was greater than that of the other two groups. Pregnancy rates for all three groups were influenced by age of subject, parity, race, marital and Medicaid status, and a history of urinary tract infections. Induced abortion status did not influence pregnancy rates except that women reporting three or more induced abortions had a higher pregnancy rate.
