Comparison of near-infrared spectroscopy with air plethysmography in detection of deep vein thrombosis.

BACKGROUND: As a noninvasive screening test, air plethysmography (APG) is a reliable and frequently used modality in the detection of deep vein thrombosis (DVT). Although APG is highly sensitive for the diagnosis of proximal DVT, its sensitivity for identifying calf DVT is poor. Near-infrared spectroscopy (NIRS) is a new modality which can be used to evaluate venous retention during walking. The purpose of this study was to investigate the diagnostic value of NIRS for the detection of DVT, particularly isolated calf DVT, in comparison with APG. METHODS: Fifty limbs of 39 consecutive patients with clinically suspected DVT were studied. All patients were examined by venography and APG. Patients also underwent a treadmill-walking test with simultaneous NIRS. Deoxygenated hemoglobin was continuously measured by NIRS during exercise. The ambulatory venous retention index obtained from serial DeoHb changes was calculated in each patient. RESULTS: Venography demonstrated DVT in 35 limbs, confined to the calf in 9 limbs, and with the involvement of a proximal deep vein with thrombi in 26. The overall sensitivity of NIRS and APG was 97% (34/35) and 80% (28/35), respectively. NIRS was more sensitive than APG for detecting isolated calf DVT (89% [8/9] and 22% [2/9], respectively), while both tests identified proximal DVT in all limbs. CONCLUSIONS: NIRS is a highly sensitive method for the diagnosis of haemodynamically significant DVT. NIRS may become a useful screening test because of its reliability in detecting calf vein thrombi that cannot be identified by APG.

Reliability of plethysmography, the single breath oxygen test, and spirometry in population studies.

Three thousand five hundred and twenty-eight residents of a census tract in Los Angeles, California underwent pulmonary function testing in a mobile laboratory (the Breathmobile). Tests included respiratory questionnaire, electronic spirometry, the single breath O2 test and body plethysmography. The reliability and sensitivity of the tests performed in the Breathmobile was measured by repeating the same tests and others on a 3% probability sample at the University of California at Los Angeles Pulmonary Function Laboratory. The reliability of the FVC and FEV1 was excellent; of the FEF 25-75% and the FEF 50-75% -- good; and of the VTG, RA and delta N2 -- reasonably good. As yet the reliability of the closing volume fraction is inadequate in our study. Grouping by individual reported symptom complexes identified different groups of people. The questionnaire and the forced expiratory flow rates identified the most people as having decreased respiratory function. Selection of the best breath or average of the two best breaths affected results of spirometry very little.

Differntiation of primary from secondary varicose veins by Doppler ultrasound and strain gauge plethysmography.

The Doppler ultrasonic velocity detector and the strain gauge plethysmograph were used to differentiate qualitatively and quantitatively primary from secondary varicose veins. Studies were performed in 21 limbs of 14 normal persons, nine limbs of eight patients with varices secondary to deep venous disease. The latter group of patients was identified by ultrasonic evidence of venous reflux through imcompetent deep venous valves. There was no significant difference in the maximum venous outflow of the limbs of the three groups of persons as determined by plethysmography. The primary hemodynamic abnormality in patients with varicose veins was significant venous reflux through imcompetent venous valves. This reflux was obliterated by tourniquet compression of the varices in patients with primary varicose veins. However, secondary varicose veins are distinguished by persistent abnormal deep venous reflux after compression of the varices. These diagnostic techniques not only provide accurate noninvasive diagnostic information but also may facilitate determination of prognosis and therapy of patients with varicose veins.

Hyperaemia of the calf after arterial reconstruction for lower limb atherosclerosis. A study with mercury strain gauge plethysmography.

Postoperative hyperaemia in the calf was studied in 25 patients successfully operated for obstructing atherosclerosis of the lower limbs. The calf flow was measured by strain gauge plethysmography. No difference in the preoperative calf flow between the patients and the control group was found. The flow at rest in the operated limb increased to a maximum on the 5th postoperative day, at which time it was 2.4 times the flow in the control leg. A significant correlation was found between the postoperative hyperaemia and the preoperative ankle pressure. These findings support the theory of Husni & Manion (1966), that there is a greater postoperative hyperaemia in patients with severe atherosclerosis. No significant correlation was found between the postoperative hyperaemia on the one hand and (1) the preoperative arterial flow increase after reconstruction, (2) the maximum flow increase in the artery after intra-arterial injection of papaverin or (3) clinical symptoms and angiographic findings on the other.

[Methodical aspects of pneumatic segment plethysmography. I. The problem of posture and distal venous occlusion].

Pneumatic segment-plethysmography was used to investigate the influence of the position and of distal venous occlusion on the arterial inflow to the calf and on the pressure-dependent venous capacity. It was demonstrated that (1) two of three investiaged positions have no significant influence on the arterial inflow to the calf; (2) the pressure-dependent venous capacities, however, show statistically significant differences related to position of the leg. The following reasons for these results were discussed: the varying tension of the surrounding tissue of the vessels, the basic volumes and the relation of intravenous pressure and the flexibility of the vessels. (3) It could be shown that the arterial inflow at rest measured without distal venous occlusion is significantly higher than the flow measured with distal venous occlusion. (4) On the contrary distal venous occlusion is of no importance in reactive hyperemia after 3 minutes of arterial obstruction. The arterial inflow to the following distal tissue was assumed to be the reason of these findings.

[Methodical aspects of pneumatic segment plethysmography. II. The problem of cuff width and cuff-pressure].

Pneumatic segment-plethysmography was used to investiage the influence of different cuff-widths and cuff-pressures on arterial inflow to the calf. It was demonstrated that 1. using consistently 80 Torr for the small respectively 50 Torr for the wide cuff no significant difference for small suprapatellar thigh-circumferences was found. On the contrary for large suprapatellar thigh-circumferences there was a statistically significant difference of the arterial inflow; 2. using both cuffs for all thigh-circumferences, a confirmity of arterial inflow was found if a coupling-factor was individually calculated in each case from upper arm- and thigh-pressure; 3. the arterial inflow to the calf did not differ at pressures of 42 Torr and 50 Torr, whereas at 70 Torr arterial inflow was significantly diminished.

[A new apparatus for finger occlusion plethysmography. Disucssion of the method and diagnostic possibilities].

Based on the principles of an apparatus for the venous-occlusion plethysmography of the finger, which was developed by Goetz in 1934 and which was adapted for clinical usage by the authors of this article in 1971, a largely improved apparatus, which is also suitable for the usage in hospitals, is now presented. The various clinical methods of examination for the diagnosis of acrale bloodflow disturbances are discussed. Angioneuropathies (as for instance the Raynaud-syndrome) of diseases, which are connected with injuries of vessel-walls and stenosis resp. occlusion (M. Raynaud), can be differentiated with great certainty by means of the temperatures (15 degrees C and 32 degrees C), the quotient and the partial analysis of the volume-pulse form.

Capillary filtration in venous hypertension: evaluation with the vacuum suction chamber device and strain-gauge plethysmography.

The aim of this study was to evaluate local capillary filtration with the vacuum suction chamber (VSC) and the rate of ankle swelling (RAS) in patients with ankle edema due to venous hypertension before and after treatment with oral TTFCA (60 mg tablets twice daily) for 4 weeks. Strain-gauge plethysmography (SGP) was used to assess RAS and local capillary filtration was studied with the VSC (applied on the perimalleolar region); the disappearance of the weal was measured (minutes). Fifty patients with chronic venous insufficiency and edema were included (M:F= 25:25) after informed consent and randomized into a treatment (mean age 43; SD 7) and a control (mean age 44; SD 8) group. Compliance was very good (100% completed the 4-week trial); no side effects were observed. The two groups were comparable for age/sex distribution. Values of RAS and VSC time were comparable in the two groups, at inclusion. After 4 weeks there were no changes in the control group. A significant reduction was observed in the treatment (RAS decreased to 34% of the initial value; the VSC time decreased 48%; p<0.02). Treatment with TTFCA in chronic venous insufficiency is useful as soon as edema is detected to control the progressive alterations leading to ulcerations. This action is produced by complex actions on the microcirculation reducing and controlling edema and modulating collagen synthesis. This results in an improvement of the microcirculation, skin and subcutaneous tissue perfusion and functions.

[Optoelectronic plethysmography: a new tool in respiratory medicine].

Opto-electronic plethysmography (OEP) is a new noninvasive technique, recently introduced, which is highly accurate in the measurement of total chest wall volume variations, allowing partitioning of the complex shape of the chest wall into different functional compartments. It can measure breathing patterns and, if combined with pressure measurements, can be used to study statics, dynamics and energetics of the respiratory system. OEP does not require connection to the patient and can be used without a subject-specific calibration. All these features and the results obtained in several studies make the OEP not only a reliable system for basic physiological and pathophysiological research studies, but also an attractive tool to be used in clinics.

[Determination of the microcirculatory blood flow by pulp plethysmography].

A method for clinical determination of blood inflow at the tip of digit, using mercury strain gauge plethysmography with venous occlusion, is presented, including a satisfactory way of calibration of the "closed type" gauges. Theoretical considerations and experimental data allow to consider the accuracy of the method (of +/- 7 p.cent) to be of clinical value.

[A method of venous occlusion plethysmography for quantitative study of peripheral hemodynamics in cranio-cerebral trauma].

A method of quantitative investigation of the peripheral blood flow (in ml/100 cm3/min) involving the use of venous occlusion plethysmography is described. Mathematical procedures applied in processing the arterial in flow curves are adduced. Conditions attending an investigation of the blood flow in the forearm vessels are depicted. The expediency of a bilateral examination of the peripheral circulation and vascular resistance in a cerebro-cranial injury is shown, there being, as a rule, a considerable asymmetry between the sides.

[Plethysmography on patients with tracheal and laryngeal stenosis (author's transl)].

Body plethysmography developed from pulmonary physiology in recent years has application to measurement of nasal airway resistance. It is a practical method of measurement of resistance due to upper respiratory tract obstruction. A report on cases with stenosis of the trachea and larynx is here made and it shows that this is a suitable method to assess the indication for or results of surgery to the larynx or trachea.

Body composition in Mexican adults by air displacement plethysmography (ADP) with the BOD-POD and deuterium oxide dilution using infrared spectroscopy (IRS-DOD).

Thirty four subjects (13 men and 21 women), 24 to 70 years old from northern Mexico, were measured for body density by air displacement plethysmography (ADP) with the BOD-POD, and for total body water by deuterium oxide dilution and infrared spectroscopy (IRS-DOD). Subjects were given a 30 g dose of deuterium oxide. Saliva samples were filtered, sublimated, and deuterium was measured using a Miran 1 FF, IRS. Linear regression of the fat mass (FM) derived from both methods showed that the intercept (0.071) was not different from zero (p = .96) and the slope was 0.96 (p < .0001) demonstrating the techniques to be equivalent. Further, mean FM was 26.7 +/- 12.4 and 25.6 +/- 12.4 kg, for IRS-DOD and ADP techniques, respectively (p = .08). Precision analysis by the model R2 showed that 92.3% of the variability was explained (SEM = 3.4 kg). Bland-Altman analysis showed no significant bias (r = 0.017; p = .93). Mean difference between methods was -1.08 (CI: -2.3 to + 0.13) kg FM.

[beta 2-adrenoceptor polymorphism and effect of inhaled beta 2-stimulant (procaterol) on airway resistance measured by body plethysmography in healthy volunteers].

This study was designed to evaluate, in healthy volunteers, differences of bronchodilator responsiveness to a beta 2-agonist between those with the Arg 16/Gly 16 allele and those with the Gly 16/Gly 16 allele at codon 16 of beta 2 adrenoceptor polymorphism. Peak flow and pulmonary functions were measured by body plethysmography after the inhalation of procaterol. In the Arg 16/Gly 16 group, the resulting acute increase of V 25 and acute decrease of airway resistance were greater, and the bronchodilator effect of a single inhaled beta 2-agonist was longer, than those observed in the Gly 16/Gly 16 group. The present study showed that a single inhalation of procaterol has a varying bronchodilator effect on healthy adults, depending on the genotype of beta 2 adrenoceptor polymorphism at codon 16.

Early diagnosis of deep vein thrombosis following total hip replacement using impedance plethysmography: advantages and limitations of this approach.

Early diagnosis of deep vein thrombosis (DVT) following total hip replacement is of fundamental importance given the particularly high incidence of thromboembolic complications and the limited effectiveness of available preventive measures. The authors report the results of postoperative monitoring of the lower limbs using impedance plethysmography (IPG). The validity of the computerized plethysmograph used in this study has been confirmed by previous clinical studies, in which this instrument demonstrated high sensitivity and specificity of diagnosis of deep vein thrombosis in the symptomatic patient. In the present study, however, the instrument is applied to a group of mostly asymptomatic patients, many of whom have non-occlusive DVT. One hundred thirty-two patients underwent IPG both one the day before surgery and on the seventh day after surgery. The postoperative IPG was positive in seven asymptomatic patients (5.3%). Proximal DVT was confirmed by phlebography in five patients (3.7%) and found to be absent in two patients (false positives: 1.5%), for a positive predictive value of 71%. IPG also found proximal DVT to be absent in three patients with postoperative signs and/or symptoms compatible with a thrombotic complication in the lower limbs. The character of the study did not permit collection of data as to IPG sensitivity in the asymptomatic patient, which is presumably quite lower than in the symptomatic patient. Therefore, phlebography remains the most reliable test for diagnosis of asymptomatic DVT. However, systematic monitoring by phlebography is not feasible for logistic, economic, and ethical reasons. Postoperative use of IPG may at least partially reduce the risks connected with thromboembolic complications.

[Venous occlusive plethysmography in assessment of hemodynamics in arterial thrombosis of the lower extremities].

With the use of a method of venous occlusive plethysmography changes, in regional arterial and venous hemodynamics in 75 patients with arterial thrombosis as well as in 47 after reconstructive intervention at the immediate period, and in 25--at the late period were studied. It was established that in ischemia progression, the volume blood flow rate at rest and maximum rate decreased, indicating severe impairement in reserve possibilities of blood flow, maximum venous outflow reduced, venous filling of the vessels increased. In thrombosis, rough disorders in reserve possibilities of blood flow in the aorto-iliac segment were noted. After operation, dynamics of the indices didn't depend on type of reconstructive intervention and was determined only by adequacy of blood flow restoration in the extremity.

[Acute bronchodilator effect of chinoxaline, prednisolone, ipratropium bromide and fenoterol tested by whole-body plethysmography (author's transl)].

The acute bronchodilator effect of chinoxaline, prednisolone and ipratropium bromide was tested by whole-body plethysmography in 18 patients with chronic obstructive airway disease over a period of 60 min. The subsequent administration of fenoterol by a metered dose inhaler demonstrated the possibility of further bronchodilation. Chinoxaline (80 mg i.v.) was followed by a statistically-significant decrease in airway resistance to 82.66% of the baseline value after 5 min. (maximum reduction) and to 87.11% after 60 min.; fenoterol (0.4 mg = 2 puffs) caused a further marked decrease to 49.61% after 20 min. Prednisolone (250 mg i.v.) did not produce statistically-significant acute bronchodilation. Airway resistance decreased to a minimum level of 89.16% after 20 min. and to 90.77% after 60 min.; however, following fenoterol (0.4 mg = 2 puffs) the airway resistance was reduced to 45.72%. Ipratropium bromide inhalation (0.08 mg = 4 puffs) resulted in the greatest bronchodilatory response, with a statistically-significant effect from the 5th to the 60th min. Airway resistance was 64.66% after 5 min., 63.22% after 10 min., 58.94% after 20 min., 57.72% after 30 min. and 51.22% after 60 min. (maximum reduction); fenoterol (0.4 mg = 2 puffs) caused only a slight further decrease to 48.32%.

The role of air plethysmography in monitoring results of venous surgery.

The development of an objective, noninvasive method to assess the hemodynamic effects of venous surgery has long been awaited. Previous methods used to evaluate the results of surgery for varicose veins and venous stasis ulceration have been limited in their quantitative assessment. Now, by use of air plethysmography (APG), we can accurately quantify the effectiveness of corrective venous surgery. Twenty-five extremities that had evidence of venous insufficiency were examined with use of APG before and after venous surgical procedures. Surgery was directed at specific sites of venous incompetence as defined by physical examination and high-resolution duplex imaging. Twenty-one extremities underwent ligation and stripping of the greater saphenous vein. In these patients, APG showed an improvement in venous reflux as demonstrated by a decrease in the venous filling index from 6.6 +/- 0.7 ml/sec to 1.8 +/- 0.3 ml/sec (p = 0.0001) and venous volume from 177.1 +/- 14.5 ml to 139.2 +/- 8.9 ml (p = 0.0008). In addition, these patients showed a mild improvement in calf muscle pump function as noted by an improvement in ejection fraction from 45.8 +/- 2.0% to 50.8% +/- 2.5% (p = 0.07). The residual volume fraction decreased from 45.0% +/- 3.4% to 42.0% +/- 3.7%, a difference that was not statistically significant (p = 0.4). Four extremities with grade III chronic venous insufficiency underwent popliteal vein valve transplantation with use of an autogenous axillary vein valve.(ABSTRACT TRUNCATED AT 250 WORDS)
