[A clinical case of demyelinating disease with basilar impression].

The authors report a clinical case of a 48-year-old female patient admitted to the Neurological Division following acute symptoms characterised by generalised asthenia, motory disorders (incoordination, equilibrium or gait deficit) accompanied by diplopia. Instrumental (medullary and encephalic NMR) and laboratory tests revealed a malformation of the atlo-occpital hinge with basilar impression and areas of corticosubcortical demyelinisation signifying multiple sclerosis. The liquor test was also positive for the presence of oligoclonal bands of IgG with a Link index of 0.97 (lower v.n. at 0.7). The association between these two pathologies is rare, whereas the need for a differential diagnosis between them often arises. Therefore, two pathologies which are mutually exclusive in many cases were present in an associated form in this case.

Management of hydrocephalus in Paget's disease. Case report.

The authors report on a case of Paget's disease with hydrocephalus in an elderly female with a favourable result after ventriculoperitoneal shunt. The neurological examinations at the time of admission revealed mild dementia, ataxic gait and urinary incontinence. Skull X-ray, CT-scan and MRI showed hydrocephalus and basilar impression. The pathophysiological hypothesis is discussed and the technical difficulties during the shunt surgery are described.

Cervicobasilar relationships.

It is prudent to apply several methods of evaluation of cervicobasilar relationships in basilar impression and, in suggestive cases, to use tomographic evaluation for complete study. This discussion reviews normal cervicobasilar relationships and illustrates selected pathologic processes that produce basilar impression.

[Clinical findings and roentgenological diagnosis of the anomalies in the craniocervial region (author's transl)].

Osseous anomalies of the craniocervical junction may induce grave neurological disturbances and even lead to sudden death in exceptional cases. Therefore a correct diagnosis with painstaking demonstration of the extent of all anomalies must be established. This can be achieved exclusively by roentgenological studies. Clinical symptoms can only provide a lead to the underlying disease. In the present paper, the special problems of the radiology of craniocervical malformations are discussed and illustrated with own observations, which include specimen radiography.

[A case of basilar impression treated with mandible splitting transoral approach].

We report a case of basilar impression treated with mandible splitting transoral approach surgery. A 39-year-old man presented a ten-year history of gait disturbance. He had experienced acceleration of his spastic paraparesis for the past few years. Neurological examination on admission demonstrated tetraparesis (MMT4/5), severe hyperreflexia in his arms and legs, severe sensory loss below C2, urinary retention and inability to open his mouth widely. Radiological findings showed the dens and the body of C2 are deeply invaginated to the atlas and the foramen magnum and medulla and upper cervical cord were markedly compressed by this basilar invagination. After carrying out posterior decompression and occipito-cervical fixation, we attempted to remove the invaginated dens and the C2 vertebral body using a conventional transoral approach. But limitation of mouth opening made operative fields narrow. Consequently because of insufficient anterior decompression his neurological deficits could not be alleviated. Employment of mandible splitting procedure brought about a wide operative view so that complete removal of the residual dens and 70% drilling out of the C2 body was made possible. Postoperative complications were negligible. Finally, great alleviation of his deficits was achieved. Mandible splitting procedure is effective for obtaining a wide operative field on the anterior approach to the craniocervical junction.

[An alternative dorsal fusion technique after transoral dens resection in basilar impression with atlas assimilation].

Current therapy of basilar impression includes transoral dens resection. The essential disadvantage of this procedure is the instability of C1/C2 due to loss of the transverse ligament. We describe two patients in whom this instability was treated by modified screwing. Using MAGERL-screws between C0-C2 an interposition of an iliac graft between the occiput and the arc of C2 was made. Neurological symptoms improved in both patients postoperatively. In both cases stable fusion C0/C2 was achieved with an additional treatment with a HALO-fixateur for eight weeks. In our opinion the combined transoral/dorsal procedure is most suitable and efficient in treatment of basilar impressions. By this approach both, decompression of the myelon as well as stable fusion C0/C2, is achieved.
