[Insurance medicine and accident surgery].

A definition of insurance medicine is presented with due consideration for national peculiarities (social insurances) in Switzerland. The historical development from original trauma-directed accident medicine to the present-day insurance medicine with its more comprehensive nature is shown with particular consideration being given to the close connection with traumatology (trauma surgery/orthopaedic surgery). The guarantee of smooth-running and constructive cooperation between traumatologists and welfare organizations is one of the major tasks of doctors employed in insurance companies, whose sphere of influence and qualifications are under discussion.

[Expert testimony on rotator cuff rupture].

In giving an expert opinion, it is difficult to differentiate between the two possible causes of damage of the rotator cup, degeneration and trauma. After description of the anatomical problems, different causes for degenerative changes, especially of the supraspinatus tendon, are discussed. Various possibilities of accidents are analysed if they can cause damage of the rotator cup. The special conditions of the private accident insurance are discussed.

[Myocardial infarct and occupational accident].

According to our knowledge to day the cause of a heart infarction is a stenosing atheromatosis of the coronary vessels or an associated coronary thrombosis. Here are described two cases with unusual precipitating factors of a heart infarction, one by an anaphylactic reaction caused by smallpox vaccination, the other case caused by a very intensive stress situation. As the basic condition in the two cases was accepted a coronary heart disease but the described events were considered by the author as the determinating concomitant cause for the social accident insurance in consequence of their intensity and acuteness.

Decreasing incidence of occupational contact urticaria caused by natural rubber latex allergy in German health care workers.

BACKGROUND: The development of allergy to natural rubber latex (NRL) is a risk for health care workers. A regulation banning powdered NRL gloves was implemented in Germany in 1998. OBJECTIVE: This study assesses the effects of this regulation on the development of occupational skin disease caused by NRL in health care personnel working in facilities insured by the German statutory accident insurance company, covering some 1.8 million insured health care workers. METHODS: We reviewed the annual numbers of reported suspected cases of NRL-induced occupational allergies (mainly contact urticaria) from 1996 through 2002, as well as the number and type of gloves purchased in acute-care hospitals from 1986 through 2002. RESULTS: The number of purchased nonsterile examination gloves increased by 1387% between 1986 and 2002. The purchase of powder-free NRL examination gloves exceeded that of powdered gloves in 1998. The incidence of suspected cases increased until 1998 and has since decreased by 79.9%. Most suspected cases (67.9%) were identified as occupationally caused. CONCLUSIONS: Reducing the purchase of powdered NRL gloves is associated with a decrease in suspected and proved cases of occupational contact urticaria caused by NRL. Development of occupationally caused contact urticaria by NRL can be reduced by banning the use of powdered NRL gloves.

Primary prevention of natural rubber latex allergy in the German health care system through education and intervention.

BACKGROUND: The development of occupational asthma and allergic skin reactions caused by natural rubber latex (NRL) allergy are risks for health care workers. There are few published studies to suggest that intervention programs to reduce exposure will lead to primary prevention of sensitization. OBJECTIVE: This study assesses the effects of intervention to reduce the incidence of NRL allergy in personnel working in health care facilities insured by the German statutory accident insurance company for health care workers, Berufsgenossenschaft für Gesundheitsdienst und Wohlfahrtspflege, with approximately 3 million insured employees, by switching to powder-free NRL gloves. METHODS: The timing of introduction of intervention strategies, such as education of both physicians and administrators, together with regulations demanding that health care facilities only purchase low-protein, powder-free NRL gloves are reported. We reviewed the annual numbers of reported suspected cases of NRL-caused occupational allergies and the amount and type of gloves used in German acute-care hospitals since 1986. RESULTS: The purchase of powder-free NRL examination gloves exceeded that of powdered gloves for the first time in 1998. This only became true for powder-free NRL sterile gloves 2 years later in 2000. The incidence of suspected occupational NRL allergy cases rose until 1998 and has declined steadily since. There was a 2-year lag between the beginning of the decline in the purchase of powdered NRL examination gloves and the beginning of a decline in suspected NRL-caused occupational asthma cases. CONCLUSIONS: Despite the effect of increased recognition of NRL allergies, education about NRL allergies in health care facilities combined with the introduction of powder-free gloves with reduced protein levels has been associated with a decline in the number of suspected cases of occupational allergies caused by NRL in Germany on a nationwide scale. These results clearly indicate that primary prevention of occupational NRL allergies can be achieved if these straightforward and practical interventions are properly carried out and maintained.

The need for replantation centers in view of the incidence of traumatic amputations.

During one year a total of 451 amputations in 360 patients were registered by the Danish National Accident Insurance to which all such accidents must be reported, all except 6 of these were finger amputations. An attempt is made to estimate the candidates for replantation surgery in Denmark. Following a somewhat restricted policy the number may be no more than twenty-five to thirty-five per year. This calls for limitation of the numbers of centers which deal with such accidents.

[About insurance medical evaluation of psychoreactive disturbances (author's transl)].

Results arising from injuries within the valid field of the statutory Accident Insurance and of the Federal Law Governing Pensions and Grants can only be recognized as such and give rise to financial compensations if bodily or psychic damages within the legally protected Risk Area are made "probable", and if a connexion between these injuries and the unbiased finding of the actual health disorders prove to be "substantial". The assessment of the damage must bear in mind the individual conditions of the victims. Symptomatology, differential diagnosis and medical insurance estimation of the "neurosis in the true sense", of the personality alteration brought about by selfexperienced hardships (concentration camp neurosis) and the psychogenic breeding of symptoms with a final view to having an ailment recognized, are discussed with a special bearing on the phenomenons which cause the will to express itself.

[The importance of ADP for medical statistics].

Recent german articles concerning medical statistics give priority to data acquisition. This contribution describe the statistical analysis and presentation of the results of the Swiss accident insurance Company (SUVA) with 1.9 million of insured workers. This documentation covers per year approximately 450,000 accidents at work and in free time. The documentation include primarily the insurance-data, for medical purposes we use the adjacent diagnoses (up to 20 per accident). The new medical statistic (SUMEST) begins with the accidents of the year 1984, updated until 1988, this means, that we have a statistic of the outcome of accidents. For the analysis we use properties of the personality, details of what happened, injury, expenses for healing and results. The primary presentation is a collection of tables for every diagnosis (total 417) with the most important criteria for the cases. It is provided to publish results of special problems like time of disability, influence of algodystrophy etc. The possibility to analyse a big documentation (25 MBytes) with a personal computer give to the physician the possibility to perform the statistical analysis himself and to select the significant results immediately.

[Is Swiss national accident data useful for epidemiological studies if the latter is intended for validating preventive measures?].

In the general frame of an inquiry concerning the development of occupational health in Switzerland and its forthcoming place inside the Swiss health system, data on accidents were communicated to us by the Swiss National Accident Insurance Fund. In this paper, the question is raised whether those data may be effectively submitted to the epidemiological analysis in order to quantify the accident risk and to validate preventive measures. Results will be presented in a subsequent article.

[The history of insurance medicine in Germany].

Necessity for insurance was recognized even in ancient and medieval times. In modern times, mathematical and statistical research into mortality ratios, led to the start up of insurance companies. Medical advice was needed. Family doctors, medical advisers and chief physicians of the insurance companies became essential for insurance medicine. The "numerical method" and improved tarif systems were used to investigate the survival rates of people in poor health. In Germany, extensive statistical analysis of medical records was performed in a central data management office (Mitteilungsstelle für Sonderwagnisse). The Dr. Karl-Wilder-Foundation of the German insurance companies subsidized research into causality of diseases, course and prognosis affecting life expectancy. In health insurance publications on historical insurance medical work are rare. In accident insurance medicine, concrete conception of terms started especially after the second world war. Insurance medical knowledge was promoted in the department for insurance medicine of the German Association for Insurance Science (Deutscher Verein für Versicherungswissenschaft). Scientific publication started in 1886. Collaboration with related disciplines such as traumatology, forensic medicine and biostatistic was beneficial and should be extended in the future.
