[Complex drug therapy of patients with juvenile uterine hemorrhage dependent on the status of the hemostasis system].

The authors analyze the results of examinations and treatment of 125 patients, suffering from juvenile uterine bleedings, divided into three age groups and subgroups with different status of the hemostasis. They consider the suggested complex of drug therapy as an additional diagnostic test for the detection of the underlying pathogenetic component of such bleedings and recommend a pathogenetically-based hemostasis and adequate therapy to regulate menstrual function.

[Microcirculatory hemostasis and its changes in cardiovascular diseases].

The indices of microcirculatory hemostasis were studied in 955 persons, 200 of whom were healthy while 755 had diseases of the heart and vessels. The results were compared with blood fibrinolytic and coagulation activity and with the functional condition of the basophil-mast cell apparatus. A reverse trend in the functional shifts of microcirculatory and plasmo-coagulative hemostasis in the reaction of emergency adaptation was established. Delayed formation and increased resistance of platelet aggregates with simultaneous decrease in the number of platelets in circulation were revealed in cardiovascular diseases during a remission, which reflects intensified expenditure of the platelets or their aggregation energy in the microcirculatory channel. The administration of exogenous heparin, just like the period of basophil degranulation in stresses of various origin, is attended with distinct platelet hyperfunction which allows small doses of heparin to be used for the prevention of diapedetic hemorrhages in hypertensive disease.

ARIC hemostasis study--II. Organizational plan and feasibility study.

In our previous paper, we reported the development of a blood collection and processing system (BCPS) suitable for the ARIC multicenter hemostasis study. As an additional step of preparation for the ARIC study, we incorporated this BCPS into an organizational plan to increase efficiency and minimize errors. We initially designed organizational trays for blood collection tubes and aliquot tubes and developed a coordinated step-by-step plan for the orderly processing of blood samples. Once the plan was considered workable, we carried out a pilot study to test the feasibility of this integrated organizational plan. Included in the pilot study were 95 healthy subjects randomly selected from 4 ARIC field centers, whose age and gender were comparable to those projected for the ARIC population. We determined the time lapse of filling the first tube as an index of blood flow. The overall mean time-lapse was 23 s (S.D. = 5). There was no significant difference among the field centers. We also determined the entire time lapse required for completing the sample processing. The total processing time was always less than 60 min. By performing the processing of samples in pairs, all the samples from two subjects could be completely processed in 70 min. This greatly increased the efficiency of field center operation. We evaluated the potential in vitro hemostasis activation by measuring plasma beta-thromboglobulin and platelet factor 4 levels. The geometric means of both proteins were comparable to our previously reported results. Fibrinogen, factor VII, factor VIII, von Willebrand factor, antithrombin III, protein C and activated partial thromboplastin time were analyzed.(ABSTRACT TRUNCATED AT 250 WORDS)

[Characteristics of disorders of hemostasis and lipid metabolism in patients with different pathogenetic forms of angina pectoris].

The likely mechanisms responsible for the occurrence of angina pectoris, which were revealed by 24-hour ECG monitoring, were correlated with abnormalities in hemostasis and blood lipid spectrum. Changes in the blood lipid spectrum were demonstrated to play the leading role in the development of angina due to higher myocardial oxygen demand. Those in platelet hemostasis are mostly pronounced in impaired myocardial oxygen supply due to vessel spasm and/or formation of platelet aggregates.

[Hemostasis in patients with systemic skin diseases].

Examinations of the hemostasis system parameters in patients with scleroderma, psoriatic arthritis, malignant lymphoma of the skin have revealed a significant elevation of fibrinogen concentration, acceleration of blood clotting time, increased prothrombin consumption in all the patients. Elevation of the prothrombin index was detected in the patients with scleroderma and psoriatic arthritis. A direct relationship between the disease severity, size of the involvement of the skin integument, inflammation activity in the joints, and severity of hemostasis impairment has been revealed.

[The effect of plasmapheresis on hemostasis in lung cancer patients].

To correct hemostasis, 32 patients with lung carcinoma underwent plasmapheresis with exfusion of 50% the circulating plasma volume one day before surgery. Plasmapheresis was coupled with the conventional drug therapy. As compared with the control group patients (n-32) who did not receive plasmapheresis, the main group patients demonstrated that the use of the multimodality correcting therapy made all the components of hemostasis return to normal and minimized the rate of postoperative thromboembolic complications by 40.6% and the postoperative lethality by 18.7%.

Effects of red blood cell concentration on hemostasis and thrombus formation in a primate model.

Because the effects of red blood cell (RBC) concentration on hemostasis and thrombus formation have not been studied experimentally under conditions of whole blood flow without anti-coagulation, normal baboons were bled or transfused to obtain three different groups: a low hematocrit (Ht) group (20% less than Ht less than 25%), a normal Ht group (35% less than Ht less than 40%), and a high Ht group (50% less than Ht less than 55%). Measurements of platelet count, bleeding time, platelet aggregation, fibrinogen level, and coagulation time (APTT) were equivalent to normal values in each group. Thrombus formation was induced using a device composed of collagen-coated tubing followed by two sequentially placed expansion chambers designed to exhibit flow recirculation and stasis. The device was exposed for up to 40 minutes in an arterio-venous shunt system. Wall shear rates in the tubular collagen segment were 100 seconds-1 and 500 to 750 seconds-1. The accumulation of 111In-platelets and 125I-fibrinogen/fibrin was measured radioisotopically; RBC incorporation was determined from measurements of total thrombus hemoglobin. Thrombus that formed on the collagen substrate was rich in platelets and poor in fibrin and RBCs. Under high flow conditions, thrombus composition showed no dependence on Ht. Surprisingly, under low flow conditions, platelet thrombus volume was negatively correlated with Ht (r = -.73, P = .005), and was increased by greater than twofold in the low Ht group as compared with the high Ht group. Thrombus that formed in the disturbed flow regions contained relatively few platelets but was rich in fibrin and RBCs. The predominant finding was a positive correlation between RBC incorporation and Ht at both high and low shear rates (r = .90, P = .00003; and r = .77, P = .002, respectively), with thrombus volume increasing three- to sixfold between the low and high Ht groups. Thus, in vivo variations in Ht ranging between 20% and 55% did not affect hemostasis, but were found either to promote or inhibit the net accumulation of thrombus, depending on local flow conditions.

[Availability of DDAVP in hemostasis control for the breast tumor resection in a patient with thrombasthenia (type II)].

We reported a 33-year-old woman with thrombasthenia (type II) in whom 1-deamino-8-D-arginine vasopressin (DDAVP) was available for the hemostasis control during breast tumor resection. She has had recurrent nasal bleeding and purpura since 2 years old. On the first admission to our hospital because of hematuria at 18 years old, she was diagnosed as thrombasthenia (type II) from hemostatic studies. At 20 years old, she had a healthy baby by the cesarean section with transfusion of fresh blood and platelet concentrates. On the 5th admission for the breast tumor resection, defect of glycoprotein II b-III a of her platelets was confirmed by using SDS gel electrophoresis. Recently, there are some reports on availability of DDAVP for hemostatic control in platelet dysfunction of various etiologies as well as mild hemophilia and von Willebrand disease. So, DDAVP (0.4 microgram/kg) was used for hemostasis control. After 1 hour, the bleeding time was shortened from over 10 to 4 min, platelet adhesiveness to glass beads increased from 1.8 to 37%. Furthermore, the levels of Ristocetin cofactor and von Willebrand factor antigen (especially large multimer) also increased. But platelet aggregation with various inducers remained unchanged before and after infusion. Breast tumor (fibroadenoma) resection.

Hematin: effects on hemostasis.

Extensive studies performed over the past 6 years have shown that a degradation product of hematin produces a unique coagulopathy, characterized by thrombocytopenia with platelet degranulation, alteration in the function of numerous clotting and fibrinolytic proteins, and reversible changes in endothelial cells. With the use of degraded hematin, it can be demonstrated that platelet aggregation is stimulated, that platelet adhesion to endothelial cells is enhanced, that the dissociation of factor VIII: C from von Willebrand factor is inhibited, and that the binding of the factor VII/von Willebrand factor complex to platelets is impaired. Even freshly reconstituted solutions of sorbitol-stabilized hematin affect hemostasis and induce thrombophlebitis, presumably because of in vivo degradation of the hematin. Recently, a new formulation of hematin, heme arginate, has been shown to be extraordinarily stable and to have virtually no effects on coagulation. This review compares and summarizes the effects of these various hematin compounds on hemostasis.

Use of microcrystalline collagen powder and fibrinogen tissue adhesive for hemostasis and prevention of rebleeding in patients with hepatocellular carcinoma associated with cirrhosis of the liver.

Partial resection of the liver is the only curative treatment for patients with hepatocellular carcinoma associated with severe cirrhosis of the liver. Surgical hemostasis on the cut surface of the cirrhotic liver appears very difficult because of the resultant deep cavity and the marked hemorrhagic diathesis. However, by using the microcrystalline collagen powder and the fibrinogen tissue adhesive, complete hemostasis and prevention of postoperative bleeding can be obtained, with minimal blood loss and hepatic ischemia.

[State of the blood platelet link of hemostasis and microcirculation in patients with ischemic heart disease during plasmapheresis treatment].

The paper provides the results of studies into plasmapheresis-induced changes in the thrombocytic link of hemostasis and microcirculation in 40 patients with coronary heart disease (CHD). Repeated sessions of plasmapheresis (P) were found to produce an inhibitory effect on platelet aggregation and to improve microcirculation in CHD patients with signs of increased platelet functional activity. In CHD patients with low platelet functional activity, the first P session was demonstrated to cause an increase in platelet functional activity whereas the subsequent sessions inhibited platelet aggregation. No significant microcirculatory improvement was seen in this group of patients. Heparin and rheopolyglucin given to control patients exerted no substantial effect on the thrombocytic link of hemostasis and microcirculation.

[Use of plasmapheresis in multimodality correction of hemostasis disorders in patients with calculous pyelonephritis].

As many as 142 patients with calculous pyelonephritis were examined for the system of hemostasis. It is shown that pyelonephritis is characterized by hypercoagulation shifts according to the echitox test, by the high content of fibrinogen and its degradation products, thrombinemia, a decrease of the antithrombin III level, and by high heparin resistance of plasma. At the same time there was a reduction of fibrinolysis. Thirty-six patients with the most pronounced disorders of hemostasis underwent multimodality treatment including plasmapheresis which favoured an improvement of the patients' status. The body temperature declined, the signs of intoxication disappeared, and the well-being improved. Hypercoagulation tended to disappearance, whereas the content of fibrinogen and its degradation products decreased. The level of antithrombin III rose and plasma resistance to heparin disappeared. At the same time the characteristics of the paracoagulation tests identifying thrombinemia remained unchanged. No noticeable changes in fibrinolysis were recorded. It is suggested that plasmapheresis should be widely used in multimodality treatment of pyelonephritis.

The effect of human recombinant tissue-type plasminogen activator on clinical and laboratory parameters of hemostasis and systemic plasminogen activation in the dog and the rat.

The effect of human recombinant tissue-type plasminogen activator (rt-PA) on parameters of hemostasis and systemic plasminogen activation was studied in the dog and rat. Effects on screening coagulation times, fibrinogen concentration, fibrin/fibrinogen degradation products, and plasminogen and alpha 2-antiplasmin (alpha 2-AP) activities in plasma were examined following single bolus injections of 0.5-5.0 mg/kg, single and repeated 3 hr infusions of 0.75-7.5 mg/kg and 24 hr infusions of 6.0 and 30.0 mg/kg administered intravenously to dogs. Rats receiving single or 14 daily injections of 5.0-30.0 mg/kg i.v. were similarly monitored. Systemic fibrinogenolysis (greater than 50% decrease in fibrinogen, plasminogen or alpha 2-AP values) was observed in dogs receiving greater than or equal to 1.0 mg/kg as a bolus, greater than or equal to 3.75 mg/kg (20.8 micrograms or 1.19 x 10(4) IU kg-1min-1) as a 3 hr infusion and greater than or equal to 6 mg/kg (4.2 micrograms or 2.42 x 10(3) IU kg-1min-1) as a 24 hr infusion; and in rats treated with bolus injections of 30 mg/kg rt-PA. Clinical and laboratory indications of impaired hemostasis and bleeding (anemia, prolonged coagulation times and post-mortem evidence of hemorrhage) were associated with these effects, which together were dose-dependent and influenced by the rate of infusion. The incidence of major hemorrhage was variable and limited to animals receiving prolonged (24 hr) or repeated infusions.

Heme arginate: effects on hemostasis.

Hematin, the drug used for acute porphyric attacks, has been shown to cause disturbances in hemostasis, mainly because of its degradation products. Lately a new heme compound, heme arginate, has been developed for the treatment of porphyrias. In experimental animal studies as well as in clinical use it has proved to be well tolerated. To find out whether heme arginate has any effects on hemostasis we have studied a number of parameters of coagulation and fibrinolysis after a heme arginate infusion in seven healthy volunteers. All parameters studied remained practically unchanged except the coagulation factor X, which showed a transient, insignificant decrease during the maximal heme concentration. We believe that the lack of side effects is due to a better stability of heme arginate, the degradation rates being 1% for heme arginate and 61% for hematin in four hours. Our data favor the use of heme arginate in acute porphyrias as well as in other deficiency states of heme.

[Parameters of hemostasis in persons with neurocirculatory dystonia under conditions of "dry" immersion].

Twelve volunteers, aged 45-55 years, with hypertension type neurocirculatory dystonia were exposed to 7-day "dry" immersion. Plasma, platelet and vessel hemostasis was investigated. "Dry" immersion was found to stimulate hypercoagulatory changes in the above hemostasis systems. It was also shown that the test subjects developed a slow process of readaptation.

[Clinical importance of changes in thrombocytic hemostasis in lung cancer].

Clinico-laboratory examinations of 86 patients have shown that lung cancer develops against the background of activation of the thrombocytic link of hemostasis which manifested itself in thrombocytosis, greater degree of destruction of the lysosomal membranes, lower energy resources of the cells, their tendency to slow and little reversible aggregation. The authors consider that the indicators of the ADP-induced aggregation of thrombocytes may be used for prognosis of postoperative pyo-inflammatory and thromboembolic complications. HBO is good for correction of alterations in the thrombocytic link of hemostasis in lung cancer and its suppurative complications.

[Effect of anti-arrhythmia preparations on thrombocytic and vascular hemostasis].

Platelet aggregation, prostacyclin and thromboxane were studied in 35 individuals: 20 coronary patients with angina of effort and 15 high-risk patients before and after treatment with lidocaine and pyromecaine. Oral administration of lidocaine and pyromecaine pills considerably depresses platelet aggregation and even causes platelet disaggregation. However, lidocaine disaggregating effect is greater than that of pyromecaine. Both agents affect vascular hemostasis by reducing thrombaxane and raising prostacyclin levels. Hence, the anti-aggregant effect of anti-arrhythmic proocaines may be attributed to changes in vascular hemostasis. The results obtained are expanding the range of indications for the use of these drugs.
