The expanding role of pulmonary surgery could be expected to lead to an increase in postoperative morbidity and mortality unless surgical risk factors are identified by means of a careful preoperative assessment. A careful and intelligent preoperative evaluation can reduce the morbidity and mortality after lung resection by ensuring the appropriate selection of patients and a rational choice of operative procedure. To evaluate the cardiologic risk in patients undergoing lung resection, 18 of them underwent a diagnostic protocol based on non invasive analysis including ECG, stress test with basal and exercise blood gas analysis, M- and B-mode and Doppler echocardiography. This protocol was able to identify possible cardiologic risk in the perioperative period without applying invasive techniques and with a good cost-benefits ratio.

The aqueous suspensions for intramuscular administration belong to the group of the dosage forms, that have a sustained release behaviour. These are based on the low aqueous solubility of the administered molecule whose aqueous suspensions form a depot in the muscular tissue which controls the absorption. The assays performed previously with two molecules of these characteristics have shown the inability of the habitual methods used for evaluating the dissolution rate of a drug, to determine the possible behaviour of these drugs in a definite and concrete formulation. For this reason, we have designed and developed a new method which allows us to support future processes, such as agglomeration and, possibly, cyst forming, that could be developed "in vivo". In this way, a more reliable prediction of the efficiency of inefficiency of this kind of formulation will be possible. Tha validity of our system has been studied with three formulations. Two of them were sustained release cefazolin molecules showing agglomeration problems "in vivo" and the other one a commercial penicillin-G benzathine commercial with a great efficacy demonstrated in habitual clinical practice.

It is sound medical education discipline to ask for a review of the literature, patient to patient, before setting up any global appraisal or clinical judgement. Next it appears legitimate to conduct similar searches with a local or institutional data base management system to find similar local patients to be compared with those of the literature. Both actions need communication facilities to be rooted in the patient ward unit. It is shown how well the new workstations are suited to these communication expectancies, with an appropriate capability of providing a user-friendly human interface, international bibliographical research (such as MEDLINE), a remote computer assisted learning programme library, image manipulation and local medical records handling. It is also emphasized that the expectancies related to UMLS (Unified Medical Language System of the National Library of Medicine in Washington DC) or to telemedicine are closely dependent on the availability of the high networking performances now available.
A survey has been carried out to evaluate the recovery of Enterobacteriaceae in freshly consumed horticultural products. 64 samples of these vegetables random chosen in different stores in the General Vegetable Market of Ferrara have been examined among the families of Compositae (lettuce, prickly lettuce, cabbage lettuce, common chicory, artichoke), Umbrelliferae (curly parsley, carrot, celery, fennel), Cruciferae (garden cabbage, red radish), Liliaceae (onion), and Solanaceae (tomato). 654 bacterial lines were isolated, of which 92.5% were Enterobacteriaceae, found in all kinds of horticultural products, the more contaminated being celery, followed by fennel, onion, common chicory, curley parsley, tomato; less contaminated were the other vegetables. Serratia was found in 29.6% of the samples, Escherichia in 28%, Enterobacter in 21.6%, Hafnia in 8.3%, citrobacter in 4.9%, Klebsiella in 2.3% and Yersinia enterocolitica in 1.5%. The importance of horticultural products as source of contamination and colonization by Enterobacteriaceae, especially in hospital, is emphasized. On the basis of reported data, the Authors suggest an accurate observance on hygienic regulations in order to contain the microbic charge under borderline values, even if it is not demonstrated that environmental Enterobacteriacee have the same pathogenicity than clinical ones.

We have examined the possible role of clathrin-coated vesicles (CVs) in the genesis of the sarcoplasmic reticulum (SR) in developing chick skeletal myotubes. Calsequestrin (CSQ) a luminal Ca2+ binding protein of the terminal SR cisternae, is contained within the vesicle lumen of skeletal muscle CVs in substantial amounts, approximately four molecules/CV. Employing 3-day cultures of chick skeletal myotubes we demonstrate that after a 30-min labeling with [35S]methionine and cysteine, radioactivity in CSQ remains high in the CVs 45 min later and then declines, while labeled CSQ in the SR continues to rise. No CSQ appears to be secreted. All of the CSQ in both the CVs and SR is sensitive to the activity of endoglycosidase H, and a significant fraction also binds to wheat germ agglutinin. Based on these results, we discuss the hypothesis that a selective CV-mediated pathway exists in developing skeletal muscle cells for the transport of CSQ from the early/intermediate Golgi apparatus to the SR.
