Projective identification, countertransference, and hospital treatment.

The principal purpose of this article is to illustrate the pivotal function of projective identification within the therapeutic milieu of the hospital. A detailed description of crises in the treatment of two patients undergoing long-term inpatient psychiatric treatment is given. These patients suffered from very different psychiatric illnesses; hence, their cases illustrate some features of hospital treatment that cut across different types and degrees of severity of psychopathology.

The older analysand: countertransference issues in psychoanalysis.

The apparent reluctance among analysts to begin an analysis with a patient beyond his or her fifth decade, a view first articulated by Freud, is examined as an instance of counter-transference resistance. Although there is no greater consensus among analysts than the continuing necessity of self-analysis irrespective of age, it is an attitude which does not appear to be so readily extended to the older prospective analysand. This report examines counter-transference issues within the context of a seven year classical analysis of an older female who began an analysis at the end of her sixth decade. The case illustrates the timelessness of psychological conflicts at all levels and the particular character of the analyst's counter-transferences that surfaced listening to the material of this older analysand.

Empathy, counteridentification, countertransference: a review, with some personal perspectives on the "analytic instrument".

To understand the affect-laden psychological processes of patients in a fully informed way, analysts must be able to tolerate a regression in which their unconscious or primary-process modes of functioning are given full sway. With the aid of impressions acquired in this way, cognitive functions in the analytic task are provided with a kind of authenticity not otherwise achievable. There are potential problems that can interfere with the analyst's functioning in the desired manner, and an awareness of these problems is essential. Basic issues of theory and technique that are involved are considered herein.
