[Nervous disorders in those engaged in the cleanup of the accident at the Chernobyl Atomic Electric Power Station subjected to ionizing radiation exposure at low doses].

Neurological, psychiatric, somatic and immune status were studied in 256 patients subjected to ionizing radiation at the dose of 10-45 cGy during liquidation of aftermath of the Chernobyl accident. In 61% of them neurocirculatory dystonia was found, 39% of patients revealed dyscirculatory encephalopathy. Alongside with dystonic disorders structural changes of vessels were detected. Asthenoneurosis diagnosed in 97% of patients was recognized as a key syndrome in 53%, while in 23%--obsessional-phobic syndrome dominated, in 7%--depressive syndrome and in 14%--psycho-organic syndrome were at the foreground. Somatic status in most patients (67%) was burdened by diseases of digestive tract. 191 patients revealed considerable immune imbalance. In 95 patients (33%) it was less pronounced and consisted in moderate decrease of TPR/TPS ratio. Degrees of immune and neurological disorders correlated closely. The conclusion was made that low-dose radiation induces primary damage of immunity and vessels with secondary nervous system involvement. That is why connection between neurological symptoms and radiation in subjects who took part in liquidation of Chernobyl accident aftermath may be considered probable only in association with immune and circulatory disorders.

[The results of an examination of those who took part in extinguishing the fire at the Chernobyl Atomic Electric Power Station on 11-12 October 1991].

Out of 106 firemen who had been engaged in extinguishing fire at Chernobyl NPP in October 11-12, 1991, 39 were examined immediately on the spot and 67--in out-patient department of the Ukrainian Scientific Research Centre of Radiation Medicine. Examination included anamnestic inquest, peripheral blood study and determination of incorporated irradiation. Besides this, about 50 measurements of exposure dose were carried out in the town of Chernobyl, in the NPP territory and in immediate proximity to the site of the accident. Analysis of the data showed that there were no signs of primary reaction of acute atomic disease in all subjects. Measurement of exposure and incorporated doses proved their insignificance with respect to those examined. Complaints and health disorders in firemen are likely to be related to somatic diseases and prolonged emotional exertion during combating fire.

[The probable causes of thyroid diseases in the victims of the Chernobyl accident].

Ideas about the thyroid gland diseases development in people suffering from the Chernobyl accident explaining the absence of straight dose dependence of pathologies raise from the irradiation were elaborated. It is supposed that the probable reason for a number of these pathologies can be the inadequate and inopportune preventive iodine administration. It is known that significant amount of iodine being incorporated in children or persons with compromised thyroid gland could lead to a number of pathological changes, primarily to autoimmune thyroiditis which is a reason of other consequent pathologies of this organ. The principal mechanism of the process can be presented as following: iodine transformation (oxidation) in the thyroid gland accompanies with the generation of considerable quantities of various radicals imitating the radiation damage. This hypothesis allows to consider the children suffered from the Chernobyl accident with autoimmune thyroiditis as belonging to the group of risk as the literary data testify that the presence of autoimmune thyroiditis rises 75 time the risk of malignant lympho-proliferated diseases appearance. A simple immunological method of autoimmune thyroiditis diagnostics was developed which informativity is comparable with the method of ultrasound diagnostics. Serial production (the antibodies to microsomal fraction of thyroid gland) was organized. This method can be used in screening investigations.

[Free radical lipid peroxidation in clean-up workers from the Chernobyl nuclear reactor accident depending on working conditions in the area of radioactive contamination].

Plasma lipids and antioxidant defense factors were examined 5-6 years after the exposure in 49 subjects who had taken part in decontamination of the territories after the Chernobyl accident against those in non-exposed donors. Elevated levels of heptane-soluble lipoperoxides and of dienketones were registered, though malonic dialdehyde changed little. The above parameters deviated from the control level more in subjects exposed to higher ecological risk. In this group of examinees the antioxidant activity failed to neutralize lipoperoxide excess under low activity of plasma catalase. Antioxidant drugs are recommended to manage disorders of lipoperoxide homeostasis in subjects exposed to radiation as a result of Chernobyl accident.

[The natural body resistance of the inhabitants of the city of Kiev subsequent to the accident at the Chernobyl Atomic Electric Power Station].

A study carried out among residents of the city of Kiev indicates that combined chronic radiation after the Chernobyl accident in small doses resulted in a reduction of the natural killer activity level, decrease of the number of leucocytes, lymphocytes and in their qualitative disorders. With time after the Chernobyl accident there occurred a deterioration of the above values.

[Genetic disorders in the pollen cells of waxy barley in radioactive contamination after the Chernobyl accident].

Genotoxic effects during microsporogenesis and gametogenesis were studied in barley of the waxy strain grown on 3 experimental plots situated in the region of the Chernobyl disaster (plot 1 in the town of Chernobyl, plots 2 and 3 in the town of Yanov, 3 km away from the damaged power plant). It was determined that an increased level of radionuclide pollution and chronic irradiation during ontogenesis results in higher incidence of meiotic disturbances and abnormalities during formation of the male gametophyte. Incidence of waxy reversions in pollen grains depends on the activity of radionuclide pollutants as well. We suggest that selection affecting both diplont and haplont forms will provide for the absence of significant aberrations in subsequent plant generations.

[Disorders of spermatogenesis in people working at the clean-up of the Chernobyl nuclear power plant accident].

It has been studied the state of ejaculate of 125 men, participated in liquidation of Chernobyl disaster consequences in 1986-1986 and irradiated by doses up to 25 rem. For all checked men the decrease of ejaculate volume and the number of spermatozoa with increasing of part of immovable and degenerated forms has been found. Maximal changes have been observed for men being at Chernobyl NPS in 1986 and exposed by dose more than 10 rem. The majority of these persons have asthenooligospermia in 1st degree. The majority of men exposed by total dose more than 1 rem have teratozoospermia. Two men exposed by dose more than 10 rem suffer from azoospermia.

[Changes in the level of the angiotensin-converting enzyme activity in the spermatozoa of patients with chronic prostatitis and of participants in the cleanup of the accident at the Chernobyl Atomic Electric Power Station].

The method of determination of angiotensin-converting enzyme activity in human spermatozoa and sperm plasma was developed. The amount of total and active-mobile spermatozoa was shown to be less in Chernobyl victims than in healthy donors. Moreover, the specific activity of angiotensin-converting enzyme in Chernobyl victims spermatozoa calculated per 10(6) cells was 12-fold greater than in spermatozoa of the donors. Similar phenomenon, although to a les extent, was observed in patients with chronic prostatitis. The enzyme activity in blood serum and sperm plasma was also demonstrated to be similar for all groups investigated and equal to that in healthy donors.

[Immunologic methods in epidemiologic monitoring of population, affected by radioactive iodine as a result of the accident at the Chernobyl nuclear plant].

Ultrasound investigations of the thyroid gland and determinations of microsomal antibodies have been performed in persons who lived in the town of Korosten (Zhitomir Region) during the Chernobyl accident. A high correlation has been found between ultrasound and immunological results. The immunological screening of the population suffered from the Chernobyl disaster might be successfully used for the autoimmune thyroiditis detection. These data complete those obtained by the ultrasound tests.

Chernobyl nuclear catastrophe and the high risk potential for mental retardation.

The nuclear explosion at Chernobyl nuclear reactor on April 26, 1986, continues to have wide political, social, and medical ramifications. Hot debris from the Chernobyl reactor covered an area of more than 5,000 square kilometers with nearly 20 million curies of radionuclides. Eleven regions with a population of nearly 17 million people, of whom 2.5 million were children below the age of 5 years, suffered some degree of radioactive contamination. These children are currently of elementary school age. One of the tragedies of the explosion is that thousands of these children are at high risk for mental retardation and learning disorders.

[The radioecological lessons of Chernobyl].

This paper presents the results of radioecological studies undertaken within the area exposed to ionizing radiation after Chernobyl disaster. Conclusions are made concerning the major regularities in radionuclide migration within various natural media and action of ionizing radiation on natural and artificial ecosystems. The efficiency of basic protective ecological measures in eliminating the accident consequences has been determined. The contribution of radioecological studies to the elimination of Chernobyl disaster sequences assessed.

[Health effects in Sweden of the Chernobyl accident].

An international committee (2) which has studied the health and environmental effects of the Chernobyl accident within the borders of the former Soviet Union does not expect that any increase in cancer frequency among adults can be demonstrated epidemiologically. However, the possibility of an observable increase in the incidence of thyroid cancer and increase in juvenile leukemia could not be excluded. The statistics in Sweden is more accurate, though the radiation doses were significantly lower. It would appear to be unlikely that any changes in cancer incidence (even among children), in mental development in children, in malformations or genetic changes ascribable to the Chernobyl accident will ever be demonstrated in Sweden.

High prevalence of RET rearrangement in thyroid tumors of children from Belarus after the Chernobyl reactor accident.

RET rearrangement was studied in papillary thyroid carcinomas (PTC) of children exposed to radioactive fallout in Belarus after the Chernobyl accident. To detect RET rearrangement in small tissue samples from thyroidectomy specimen (12 PTC of children; 2 PTC and 1 follicular carcinoma of adults; non-tumorous thyroid tissue of 4 children and 4 adults as controls), a RT-multiplex PCR was developed using primers suited to amplify fragments in different quantities depending on the presence or absence of RET rearrangements in the tissues. The type of rearrangement was determined by RT-PCR and direct sequencing using primers for ret/PTC1, 2 and 3. Two-thirds of the papillary thyroid carcinomas of the children revealed a RET rearrangement, with ret/PTC3 being more frequent by a factor of 3 than ret/PTC1. ret/PTC2 was not detected. All RET rearrangement-positive tumors had lymph node metastasis while half of the tumors with wild-type cRET had not. More than half of the cases with ret/PTC3 expressed not only the ELE/RET transcript as expected, but also the RET/ELE transcript. Intrachromosomal rearrangement involving RET and the adjacent H4 or ELE gene on chromosome no. 10 is a very frequent event in thyroid cancer of children of the Chernobyl-contaminated zone of Belarus.

[The systematics of mental disorders related to the sequelae of the accident at the Chernobyl Atomic Electric Power Station].

A dynamic survey was undertaken of mental status of those subjects exposed to ionizing radiation after Chernobyl NPP breakdown (n = 476). The examinees exhibited delayed course (up to 8 years) of psychopathologic disorders with the onset consequent upon working in the alienation zone or evacuation. On instituting some measures to form the pathological mental changes in the subjects described a process or psychopathologic development was noted to occur in 74 (15.5%) and 402 (84.5%) respectively. The psychopathologic disorders detected were arranged in the following classes: psycho-organic, neurotic, chiefly posttraumatic stressor, and psychosomatic ones, the relevant figures being as follows: 14.5% (n = 68), 12.05% (n = 58) and 58.0% (n = 176). A scheme of mental disorders related to the Chernobyl NPP breakdown aftereffects was suggested, which is likely to be amended in the course of further study. A specialized Ukrainian Psychoneurologic Rehabilitation Centre is envisaged, designed to give help to the Ch NPP accident survivors.

A post-Chernobyl rise in thyroid cancer in Connecticut, USA.

Recent analyses of children in Belarus and the Ukraine are the first to document large numbers of excess thyroid cancer cases only 4 years after exposure to radiation. In Connecticut (USA), a thyroid cancer increase of a much smaller magnitude occurred in 1990-93, 4-7 years after the Chernobyl accident, for both children and adults. Similar changes also occurred in the states of Iowa and Utah, which like Connecticut were exposed to low levels of radionuclides from Chernobyl fallout during May and June of 1986. Historical data from Connecticut also reveal substantial increases in thyroid cancer incidence about 5 years after large releases of iodine-131 from distant US nuclear weapons plants, after the largest atmospheric US atomic weapons tests in Nevada, and after substantial releases of iodine-131 from the Millstone nuclear power plant in Connecticut. Further analysis of this apparent 5-year latency period will enhance understanding of ionizing radiation's effects on thyroid function and on human health in general.

[The effect of chronic ionizing radiation at low doses on a number of reproductive indices in CC57W/Mv mice from an experimental Chernobyl population].

The effects of low doses of environmental ionizing radiation on reproductive functions of seven generations of CC57W/Mv mice from Chernobyl experimental population has been investigated. A decrease in the number of litters per female during reproductive period, the number of newborn animals per litter, and an increase in the postnatal mortality during the first month of life were shown. These effects are due to the stable disorders of parental reproductive system and the high level of in-vivo preimplantation deaths in F1--F7 generations of CC57W/Mv mice from the Chernobyl experimental populations.

[Respiratory organ pathology in the participants in the cleanup of the accident at the Chernobyl Atomic Electric Power Station].

The authors performed this investigation to ascertain the role of radioactive dust inhalation in the onset of respiratory diseases and determination of clinicomorphological features of such pathology. The authors have found that work in conditions of massive dust exposure, stay in the dust zone for a month and longer in 1986-1987 were the decisive risk factors of respiratory impairment. The examination covered 28 patients who had acute symptoms of inhalation damage to respiratory system when performing their job in the contaminated zone. Later on they developed chronic progressive respiratory affection having the following clinicomorphological characteristics: 1) persistent cough with hard to expectorate sputum, addition of respiratory insufficiency, sluggish recurrent infection and inflammation, deterioration of working capacity and quality of life, 2) isolated decline of specific diffuse capacity of the lungs in combination with obstructive ventilation, 3) marked disturbances of cellular immunity (reduced fraction of T-helpers and increased fraction of T-suppressors), 4) x-ray picture of diffuse pneumosclerosis and hyperairiness of the lung tissue, 5) endoscopic evidence of inflammation and atrophy of the bronchial tree mucosa, 6) numerous alveolar macrophages with foreign inclusions in the lavage fluid, such markers of Chernobyl dust inhalation as Sr, Zr, I, Cs, Ce, Np, Pu, Am, Cm registered in alveolar macrophages at roentgenospectral and roentgenofluorescent microtests, 7) defective functional activity of alveolar macrophages (free radical activation). Long-term persistence of the Chernobyl dust in the lung tissue results in essential functional and morphological changes in cell composition of the alveolar area, lung tissue undergoes structural alterations contributing to specific respiratory diseases with typical clinicomorphological characteristics.

[Respiratory tract damage in those who cleaned up the aftermath of the accident at the Chernobyl Atomic Electric Power Station].

The authors have established the fact of acute inhalation damage of the "Chernobyl aerosol" to respiratory organs of the exposed subjects. The complaints correlation to the site of work, time and kind of job shows that respiratory symptoms arose more frequently in those members of emergency teams who arrived at the station in May, August-October 1986, March 1987, stayed for 30-40 days, worked at the site of the damaged reactor, were engaged in decontamination, degasification, drilling, construction and other "dusty" jobs. As a result of inhalation of the Chernobyl dust previously healthy patients developed various respiratory diseases.
