Intraosseous lipoma of the calcaneus. Report of a case and a short review of the literature.

One case of an intraosseous lipoma of the calcaneus bone is presented together with a short review of the literature. Only 20 cases of tumors in the extremities have so far been published. The typical X-ray picture shows a cystic, sharply demarcated cavity, which macroscopically at operation is filled with yellowish, adipose tissue. In the present case and one previously published a central sclerotic mass was observed. The histological picture is typical, with mature adipose tissue mixed with a few degenerated bone trabeculae.

Surgery for fracture of the calcaneus. 5 (2-8) year follow-up of 20 cases.

20 displaced intraarticular fractures of the calcaneus were treated by open reduction and internal fixation. Results were evaluated according to a standardized protocol. With an average follow-up of 5 (2-8) years, results were excellent or good in 15 cases and fair or poor in 5 cases. An accurate radiographic study and CT scanning are essential for understanding these fractures. We feel that restoring calcaneal shape as anatomic as possible, getting an appropriate reduction of the posterior joint surface by a surgical procedure with early mobilization, and delayed weight bearing would be a good option in treatment of these fractures.

Decreased broadband ultrasound attenuation of the calcaneus in women with fragility fracture. 85 Colles' and hip fracture cases versus 77 normal women.

The broadband ultrasound attenuation (BUA) of the right calcaneus was measured in 49 women with Colles' fracture, in 36 women with hip fracture, and in 77 normal women. All were postmenopausal, and the fractures were all of fragility type. BUA correlated with age only in the normals. BUA distinguished between the fracture cases with sensitivities of 97 percent and 70 percent and specificities of 72 percent and 92 percent at 61 dB/MHz and 51 dB/MHz, respectively. The women with hip fracture had lower BUA than those with Colles' fracture. For screening purposes, BUA below 51 dB/MHz may arouse suspicion of a fracture risk, notably if other factors such as a tendency to fall, dietary and other habits, inactive lifestyles and early menopause are present.

A small cell osteosarcoma on the calcaneus--a case report.

Small cell osteosarcoma is rare, representing 1-4% of all osteosarcomas. We experienced a case of small cell osteosarcoma in an 8-year-old girl on her calcaneus. Histologically, the tumor consists of small round cells that resemble those of Ewing's sarcoma, and variable foci of lacy osteoid formation between tumor cells. The rare location, histologic characteristics and differential diagnostic points are discussed.

Anterior bone block: treatment of calcaneus deformity after failed soft-tissue operations.

Calcaneus deformity is almost always the result of a neural disorder. It is progressive, disabling, and resistant to bracing and usually requires surgery. In immature feet, soft-tissue operations are the procedures of choice but have the disadvantage of recurrence due to stretching of soft tissues. However, bony operations will lead to growth center damage. Anterior bone block corrects the deformity without damaging the growth centers.

[Bone mineral density (BMD) of the calcaneus bone in 72 dialysis patients measured by ultrasonic bone absorptiometry].

BACKGROUND: To determine what factors contribute to and change BMD in dialysis patients. METHODS: Bone parameters, namely, osteosono-assessment index (calculated by speed of sound and transmission index) were measured at calcaneus bone by ultrasonic bone absorptiometry with an Aloka Acoustic Osteoscreener (Model AOS-100). Seventy two patients were 62. 8 +/- 10.5 years of age (mean +/- SD) (range 36-81) and 70.8% were males; the patients had received dialytic therapy for 52.7 +/- 41.7 months (range 2-205). The effects of sex, age, height, weight, postmenoposal years, dialysis duration, various blood parameters, oral phosphorus binding agent (CaCO3 dosage) and oral vitamin D3 treatment (D3 dosage) on BMD were assessed statistically. RESULT: BMD in male patients were significantly higher than those in female patients (Mann-Whitney test, p < 0.01). BMD showed significant correlations with age (Spearman's correlation coefficient r = -0.384, p < 0.01), height (r = 0.479, p < 0.00001), postmenoposal years (r = -0.692, p < 0.05), dialysis duration (r = 0.250, p < 0.01), blood Ca level (r = 0.292, p < 0.05) and CaCO3 dosage, postmenoposal years and dialysis duration were the variables that significantly correlated with the BMD (multiple correlation coefficient = 0.646).

Complications of intra-articular fractures of the calcaneus.

Most complications of calcaneal fractures are due to the combination of the dynamic nature and the mechanism of the injury itself. Complications are usually secondary to three integral parts of this complex fracture pattern: 1) fracture and depression of the subtalar joint, 2) loss of the height of the calcaneal body with varus rotation, and 3) expansion of the lateral wall of the calcaneus. Other complications occur with less frequency. This article describes the complications and sequelae of old, malunited intra-articular calcaneal fractures in relation to the nature and the components of these unique fractures.

Focal tuberculous osteomyelitis of the calcaneus secondary to direct extension from an infected retrocalcaneal bursa.

We report a case of focal tuberculous involvement of the posterior margin of the calcaneus with preservation of the articular margin. The route of infection was direct extension through tuberculous retrocalcaneal bursitis, a rare and atypical pathogenesis. Magnetic resonance imaging was helpful in ruling out neoplasm and in limiting the diagnosis to an inflammatory infectious process.

Post-traumatic osteochondroma of the calcaneus.

Osteochondroma is the most common bone and cartilage tumor. It is usually congenital but may be related to trauma. A case of osteochondroma affecting the calcaneus, an unusual location for this lesion, is presented. Surgical excision appears to be as effective a cure in this location as in others.

Intraosseous lipoma of the calcaneus. A case report.

The intention of this report is to heighten the practitioner's clinical suspicion of intraosseous lipoma of the calcaneus. The apparent infrequency of this diagnosis may be attributed to the dependence on radiographic evaluation and the use of closed needle biopsy. Thus, to make a proper diagnosis, a computed tomography scan with the rate of attenuation and, ultimately, a large tissue biopsy with attentive gross and microscopic examination are necessary.

Simple bone cyst of the calcaneus. A case report and literature review.

The authors presented a case report of a patient presenting with heel pain symptoms and physical findings similar to those associated with a heel spur syndrome. With a standard x-ray, the presence of a simple bone cyst was evident. Although mainly reported in tubular bones, steroid injection therapy has been successful and appears to be replacing surgical curettage and packing as the preferred choice of treatment in many cases. As more reports of the successful eradication of simple bone cysts of the calcaneus with steroid injections are reported, greater confidence for using it as a treatment choice for this particular lesion may be gained. The surgical approach in the patient presented here was chosen because of factors that the authors believe increased the likelihood of a pathologic fracture, such as size and extent of the lesion, history of progressive pain, and activity level of the patient. The larger body weight and the resulting stress of the patient more likely to present with a calcaneal cyst may also be factors to consider.

Segmentation of quantitative ultrasonographic images of the calcaneus using elastic deformation of the flexible Fourier contour.

OBJECTIVE: This study investigated a new technique for automatic model-based segmentation of broadband ultrasound attenuation (BUA) images of the calcaneus. We determined whether this technique was able to improve osteoporotic fracture discrimination. METHODS: The segmentation process included 2 major steps: a model-building stage and the automatic segmentation of new image data sets via an elastic deformation of contour models. Broadband ultrasound attenuation was then averaged within the final contour (BUAwhole). The results of the segmentation were validated on a database of 256 patients by comparison of the clinical results obtained with the automatic circular region of interest (BUAcirc) currently implemented on a commercially available ultrasonography unit. All patients were selected by the same physician, who assessed that the fractures were caused by bone fragility on the basis of the circumstances under which fractures occurred and radiologic data. RESULTS: Short-term reproducibility assessed in 49 women was 3.5% and 3.98% for BUAcirc and BUAwhole, respectively. Both BUAcirc (age-adjusted T score, -3.78; P < .0005; age-adjusted odds ratio, 1.92; 95% confidence interval, 1.34-2.75; area under the receiver operating characteristic curve, 0.70) and BUAwhole (age-adjusted T score,-2.73; P < .01; age-adjusted odds ratio, 1.57; 95% confidence interval, 1.12-2.21; area under the curve, 0.67) performed equally well in discriminating healthy postmenopausal patients (n = 150) from those with fractures (n = 60). CONCLUSIONS: Fully automatic segmentation by parametrically deformable elastic models for contour using Fourier descriptors can be achieved with reasonable reproducibility and fracture risk prediction. The method is similar to existing methods (automatic circular region of interest); however, the new contour-based region of interest allows more flexible region of interest geometries and placement and potential adaptation to individual anatomy. The method could also possibly be extended to quantitative ultrasonographic imaging at different skeletal sites.

[Anterior subtalar dislocation associated with a fracture of the calcaneus: a case report and review of the literature].

A new case of uncommon anterior subtalar dislocation associated with a fracture of the calcaneus is reported. Mechanism of production is described with a review of literature. The criteria of radiologic diagnosis are mentioned. Closed reduction with stabilisation by Kirschner wire were undergone. Prolonged physiotherapy following 6 weeks of immobilisation allowed good result. A decreased subtalar motion without pain was found at the last check up.

[Double measurement of the Böhler angle: prognostic value of radiological angles in posterior facet fractures of the calcaneus].

In 1931, Böhler proposed that measuring the radiological angle of the tuberosity could be useful in posterior facet fractures of the calcaneus to evaluate initial damage as well as reduction quality. In opposition to the 1998 SOFCOT symposium, certain authors considered that the Böhler angle has no prognostic value. Progress in pathological anatomy has helped to better understand posterior facet fractures, justifying the use of a "double measurement". The fundamental fracture line separates the posterior facet into a lowered medial fragment and a pivoted lateral fragment. The double contour of the posterior facet visualized radiographically allows measurement of a medial Böhler angle and a lateral Böhler angle. It is demonstrated that is the smaller the medial Böhler angle, the greater the subtalar degeneration. Surgical restoration of a satisfactory Böhler angle is a necessary prerequisite for a good outcome. "Double measurement" of the Böhler angle on the lateral view contributes to the prognostic value of this historical angle.

Fractures of the calcaneus with displacement of the thalamic portion.

The thalamus is the part of the calcaneus that supports the posterior articular facet and continues forward, becoming thinner towards the groove of the sinus tarsi. The main displacements after fracture depend on 1) a primary fracture line dividing the bone into anterior and posterior fragments, and 2) a semilunar fragment in the thalamic region. In the operation advised the sinus tarsi is exposed and the semilunar fragment is reduced by rotation in the opposite direction and is fixed to the medial fragment (the sustenaculum tali not being displaced) by a transverse Kirschner wire. The twp ,aom frag, emts are foxed bu am amtero-posterior wire. Plaster is applied and is retained for twelve weeks. Weight-bearing is not permitted for the first four weeks. There were no major complications in fifty-eight operations. The anatomical results were good: restoration of the tuber-joint angle by reduction of the semilunar fragment was maintained. The functional results were very satisfactory: permanent disability was slight or mild.
