Association of Chlamydia pneumoniae (strain TWAR) infection with wheezing, asthmatic bronchitis, and adult-onset asthma.

UNLABELLED: OBJECTIVE--To study the clinical characteristics of respiratory tract illness caused by Chlamydia pneumoniae. DESIGN: -Prospective clinical, bacteriologic, and serologic study. Secondarily, a matched comparison of patients with and without evidence of C pneumoniae infection (serologic titers greater than or equal to 1:64 and less than 1:16, respectively). SETTING: --Four primary care (family practice) clinics in Madison, Wis, and nearby towns. PATIENTS: --The study included 365 white males and females (mean age, 34.2 years). MAIN OUTCOME MEASURES: --Association of acute C pneumoniae infection with signs and symptoms of respiratory illness and the relationship of C pneumoniae antibody titer with wheezing at the time of enrollment in the study, and with the diagnosis of asthmatic bronchitis. RESULTS: --Nine (47%) of 19 patients with acute C pneumoniae infection had bronchospasm during respiratory illness, and there was a strong quantitative association of C pneumoniae titer with wheezing at the time of enrollment in the study (P = .01). In the matched study, C pneumoniae antibody was significantly associated with asthmatic bronchitis after, but not before, respiratory illness (odds ratio, 7.2; 95% confidence interval, 2.2 to 23.4). Four infected patients had newly diagnosed asthma after illness, and four others had exacerbation of previously diagnosed asthma. There was no serologic evidence of coexisting Mycoplasma pneumoniae, Chlamydia trachomatis, or respiratory viral infection in 96% of patients with asthmatic bronchitis and asthma. CONCLUSIONS: --Some C pneumoniae antibody titers, although not diagnostic of chlamydial infection by present criteria, probably represent acute reinfection or ongoing chronic infection. Repeated or prolonged exposure to C pneumoniae may have a causal association with wheezing, asthmatic bronchitis, and asthma.

[Immunological disorders in chronic bronchitis].

The paper considers changes of cellular and humoral immunity in 181 chronic sufferers with bronchitis. The disease forms and severity were suggested to be evolutionary stages. There was suppression of immunological reactivity most evident in T-cell system and interleukin-1 in progressive chronic bronchitis. Immunological shifts proved more severe in obstructive chronic bronchitis complicated by pulmonary emphysema and chronic cor pulmonale. Alterations in immunological status may contribute to progression of chronic bronchitis.

Alpha-1-antitrypsin and chronic bronchitis in adult Nigerians.

Alpha-1-antitrypsin (A1AT) serum levels and phenotypes were determined in 49 Nigerians with chronic bronchitis and 100 normal controls. The A1AT phenotypes encountered were PiMM, 69% in chronic bronchitics, 98% in controls; PiMZ, 23% in chronic bronchitics, 1% in controls; PiLM, 2% in chronic bronchitics, 1% in controls. There were three patients (6%) with the homozygous-deficient phenotype PiZZ. Spirometry confirmed obstructive ventilatory pattern in the patients with chronic bronchitis, and the difference in the values obtained between the patients and controls was statistically significant (P less than 0.01). Serum A1AT levels were within the normal range of 1.4-2.7 g/l in all except the three patients with PiZZ. There was no significant difference in the A1AT serum levels between patients with chronic bronchitis and control subjects with the PiMM phenotype; tests of significance were not possible for the other phenotypes because of the small number of subjects. The observation of PiZZ in 6% of our patients with chronic bronchitis is in support of screening of this category of patients. Replacement therapy with alpha-1-antitrypsin inhibitor is currently under investigation and may be worthwhile in these patients if detected early.

[Nonspecific bronchial reactivity in chronic bronchitis].

In a segment of the population of Zagreb, Crnomerec, which was included in the epidemiological study on chronic bronchitis in Croatia (1969-1972-1982-1989) the values of ventilatory function and non-specific reactivity of airways were examined. The sample consisted of 34 women and 29 men with chronic bronchitis. The control group consisted of 34 men and 33 women--without cough. The average age was 58.6 (M) and 59.4 (F). Investigation of non-specific reactivity was carried out on the dosimeter apparatus APTA-Jaeger, Wurtzburg, W. Germany (by using histamine dehydrochloride (1-30 mg/ml dose) with expressed reactivity PC 40 Raw. PC 40 in subjects with chronic bronchitis amounted to 7.79 mg/ml in men and 8.89 mg/ml in women, which is significantly lower than the control group (14.04 M, 17.8 F). We did not determine significant difference between smokers and non-smokers with regard to PC 40. By factor analysis in an orthogonal projection a link was noted between symptoms of chronic bronchitis, basic values of ventilatory function and PC 40. This is in agreement with known results.

[Bronchitis after pneumonectomy in patients with lung cancer].

Postoperative bronchitis developed in 99 (8.3%) of 1,189 patients who underwent pneumonectomy for lung carcinoma. The sequelae of postoperative bronchitis were the cause of death in more than one third of patients who died from pneumonectomy. The high incidence of bronchopleural complications in this group of patients is emphasized. Data are presented which provide evidence that the functions of external respiration and blood circulation are disturbed in patients with postoperative bronchitis. A complex of measures for the prevention and treatment of this complication are suggested, which made it possible to reduce the frequency and mortality rate of postoperative bronchitis in the last years.

[Incidence of chronic bronchitis in coal miners].

An epidemiological survey of 2000 Donbass coal miners revealed data on the initial dust bronchitis prevalence and its progress. It was established that the coal miners who work in mines with steep coal strata exhibit higher bronchitis morbidity in comparison with the miners engaged in sloping strata mines. A correlation was established between dust bronchitis prevalence and length of professional service and coal miners' labour specificity. Tobacco smokers displayed a markedly higher percentage of chronic dust bronchitis cases.

[The diagnosis and treatment of pulmonary tuberculosis combined with chronic bronchitis in middle-aged and elderly subjects].

Investigation of patients 60 years of age and older with different clinical forms of pulmonary tuberculosis revealed chronic bronchitis in 76-80%, obstructive bronchitis--in 1/3 of all cases. The diagnosis of chronic bronchitis in most cases is established on the basis of clinico-roentgenological and functional examination and partially by fiber bronchoscopy. Chronic bronchitis essentially changes the semeiotics, exacerbates the course and worsens the outcome in tubercular patients with associated pathology.

[Study of the effect of salbutamol inhalation on the function of small airways in patients with chronic bronchitis without ventilatory disorders using simple spirographic tests].

In 68 males with chronic bronchitis and normal standard spirometric parameters and in 30 healthy males the following were assessed: VC, FEV1, V50 and V75 before and after Salbutamol inhalations. In chronic bronchitis Salbutamol slightly decreased FEV1 values. Significant improvement of V50 and V75 was seen after Salbutamol inhalations in chronic bronchitis. This was observed more promeinently in comparison with the control group. Significant increase of V50 and V75 was seen in 14% of chronic bronchitics. The improvement in pattency of small airways following administration of bronchodilators can be an index for early identification of patients with chronic bronchitis, although this needs to be verified in prospective spirographic studies.

[Regional blood flow and air level in the lung segments in recurrent and chronic bronchitis in children].

Transbronchial regional electroplethysmography was employed to measure the blood flow and air content in 67 children aged 3-15 years. Of these, 44 children suffered from chronic bronchitis and 23 from bronchiectasis. Children with recurrent bronchitis examined during the inactive phase of the disease marked by clinicoendoscopic silence were used as controls. In children with chronic bronchitis who escaped bronchial deformity, the levels of the regional blood flow were similar to those seen in the controls. Meanwhile in diffuse deforming bronchitis the reduction of the blood flow was noted in the posterobasal segments on both sides. In local deformities and bronchiectasis the levels of the air content and blood flow were decreased in the affected area whereas in the adjacent part the blood flow was more intense as compared with the symmetrical part on the opposite side, which was likely to be connected with the irritative effect of the focus of lesion.

[Comparative immunologic characteristics of patients with acute and chronic bronchitis].

Twenty patients with acute, 24 with chronic nonobstructive and 16 with chronic obstructive bronchitis were examined over time. The activity of the inflammatory process was taken into account while analyzing the data obtained in the course of immunologic studies. The patients with chronic bronchitis demonstrated a noticeable decrease in the immune response as compared to those with acute bronchitis. Application of multidimensional statistic analysis made it possible to reveal the most informative characteristics of immune responsiveness in acute and chronic bronchitis depending on the intensity of obstruction and the phase of the inflammatory process.

[Pathogenic and clinical differences between stasis and chronic bronchitis].

There is often a misguide of chronic bronchitis and passive pulmonary hypertension. In 42 patients with chronic bronchitis a revision of the results was made and it was found that in 28 (67%) patients the former diagnosis of chronic bronchitis was confirmed, while in 8 (19%) patients a new diagnosis was established--a passive pulmonary hypertension and in 6 (14%) patients a combination of chronic bronchitis and passive pulmonary hypertension was found. It is considered that the symptoms like cough, sputum and dyspnea as well as the similar physical finding may lead the clinician to change the diagnoses.

[Significance of genetic and environmental factors in the occurrence of chronic bronchitis (a study of twins)].

A study of a sample of 47 pairs of twins with chronic bronchitis made it possible to determine a qualitative and quantitative role of hereditary factors in "exposure" to this disease. A lower inheritability ratio was revealed in chronic obstructive bronchitis as compared to chronic nonobstructive bronchitis indicating leading role of the environmental factors (smoking and industrial hazards) in the development of this type of the disease. The importance of these factors in the etiology of different types of chronic bronchitis is also confirmed.

[Chronic bronchitis. Value of RU 41740].

Chronic bronchitis is responsible for 20,000 deaths per annum in France, i.e. 5 per cent of the overall mortality rate. Infection of the bronchi and lung tissue is a frequent cause of death in these patients. Acute on chronic bronchitis ranks fifth among the causes of disablement and admission to hospital. Pneumococci and Haemophilus influenza are the organisms most frequently isolated. the incidence and potential severity of acute episodes of infection account for the repeated use of antibiotics which carries a risk of promotion bacterial resistance. RU 41740 is a non-specific immunomodulator agent which reinforces the non-specific means of the respiratory tract against infections. Three double-blind, drug versus placebo and therefore reliable therapeutic trials have shown that the drug is effective in preventing airway infection. In patients with moderately advanced chronic bronchitis, RU 41740 reduces the number and duration of acute infectious episodes as well as antibiotic consumption. This positive effect persists in patients with chronic respiratory failure, including those who present with extensive bronchial dystrophy. RU 41740 is particularly effective in patients with numerous previous episodes of infection, but it also acts at all stages of chronic bronchitis.

[New aspects in the pathogenesis of chronic bronchitis].

Excretory and incretory testicular function was studied in 16 patients with chronic bronchitis and in 5 healthy males. Sustanon-250 was added to combined therapy of 15 patients. A decrease in excretory and incretory testicular function was noted in the patients with chronic bronchitis. Disturbed sexual function made an effect on chronic bronchitis development by means of lessening mucociliary clearance. A method of pathogenetic therapy is incorporation of testosterone in multimodality therapy of male patients with chronic bronchitis for improving mucociliary clearance and sexual function in them.

[Gastroesophageal reflux in asthmatic and chronic bronchitis patients].

To determine the relationship between gastroesophageal (GE) reflux and pulmonary disease, we studied 21 asthmatics, 30 chronic bronchitics, 6 patients with GE reflux and no pulmonary symptoms, and 10 control subjects; GE reflux was diagnosed by pH monitoring and GE scintiscanning. Frequency of GE reflux in the asthmatics was 57%; in chronic bronchitis it was 56%. Pulmonary function tests did not show any differences between patients with or without reflux. The GE reflux episodes were more numerous but shorter in asthmatics than in chronic bronchitis. Patients with digestive symptoms alone were no different from chronic bronchitis with respect to reflux. The mechanism whereby reflux triggers pulmonary problems was investigated using the following 2 tests: scintiscan for pulmonary aspiration, and esophageal acid infusion (0.1N HCI). Six pulmonary aspirations were detected. Only asthmatics, with or without reflux, showed any significant variations in maximal expiratory flow at 50% and 25% of VC after HCI infusion. Thus, our results show that asthmatics differ from bronchitis patients by the characteristics of their reflux.

[Treatment of bronchitis with cefaclor (Panoral) (author's transl)].

14 patients from the Allergy Unit of an outpatient clinic suffering from acute exacerbation of chronic bronchitis and 13 outpatients with acute bronchitis were treated with 250 mg cefaclor (Panoral) 3 times daily per os for 5 days. 59% of the organisms isolated from cefaclor-sensitive sputum at the time of prominent clinical symptoms were resistent to tetracycline, 53% of them were resistant to penicillin, and 37% were resistant to ampicillin. 12 out of the 14 patients with acute exacerbation of chronic bronchitis became asymptomatic, and no organisms could be detected in the sputum of 13 out of the same 14 patients two days after cessation of cefaclor treatment. In 12 out of the 13 patients with acute bronchitis, the acute clinical symptoms disappeared and in 11 out of the 13 patients the initial sputum organisms were two days after stopping cefaclor treatment.

Precipitins against microbes in mouldy hay in the sera of farmers with farmer's lung or chronic bronchitis and of healthy farmers.

The aim of this study was to determine which precipitins against four antigens in 2,440 farmers are associated with the occurrence of farmer's lung (FL). The antigen panel consisted of mycelial antigens of Micropolyspora faeni, Thermoactinomyces vulgaris, Aspergillus fumigatus and Aspergillus umbrosus. As reference groups we used healthy farmers and those with chronic bronchitis. For the occurrence of precipitins against the four antigens there was a statistically significant difference between farmers with FL and healthy farmers but not between farmers with chronic bronchitis and healthy farmers. In a stepwise logistic linear regression analysis, farmers with FL and chronic bronchitis were compared to healthy farmers with respect to precipitins to the four antigens. Precipitins against Thermoactinomyces vulgaris differentiated farmers with FL (p less than 0.05) but not farmers with chronic bronchitis from healthy farmers. In Finland the occurrence of FL seems to be associated mainly with precipitins against Thermoactinomyces vulgaris, not with precipitins against Micropolyspora faeni as in Great Britain, and not with precipitins of Aspergillus umbrosus, which occurred most frequently in the sera of Finnish farmers. This association is in accordance with the exposure to spores of airborne moulds in farmers' work environment, where spore concentrations of Thermoactinomyces vulgaris have been measured to be about six times higher than those of Micropolyspora faeni.

[Immunological status and the condition of local defense factors in patients with various forms of chronic bronchitis treated by using performed physical factors].

Physiotherapeutic and other methods were employed for the treatment of patients with chronic nonobstructive bronchitis (CNB), chronic bronchitis with transient obstruction of the small bronchi of inflammatory genesis, chronic asthmatic bronchitis (CAB) and chronic obstructive bronchitis (COB). The level of secretory IgA and relative E-RFC number in the bronchial wash off, lysozyme concentration, the level of IgA and titer of heterophilic normal antibodies in the saliva were assessed. The content of T- and B-lymphocytes in the blood was studied. The results were assessed with a method of discrete-dynamic analysis. It was established that the incorporation of inductothermia with ethonium-electrophoresis and the magnetic field effect on the thyroid in CAB and inductothermia with ethonium-electrophoresis in COB in the complex of therapeutic measures produced a positive effect. In CNB (inactive phase) and COB patients inductothermia resulted in restoration of some interrelationships in the immune system whereas in CAB patients in their dysbalance. Different changes in the levels of interrelationships of certain parameters of the immune system were observed in the incorporation of sinusoidal modulated currents in the therapeutic complexes.
