Evaluation of calcium alginate nasal packing (Algostéril) versus Polyvinyl acetal (Merocel) for nasal packing after inferior turbinate resection.

Nasal packing is commonly used to control postoperative bleeding in patients undergoing endonasal surgical procedures. Bleeding and pain on packing removal are frequently reported. The principal objective of this study was to investigate the efficacy and safety of Algosteril to control post-operative nasal bleeding. The secondary objective was to assess nasal bleeding and pain on packing removal, and to evaluate the endoscopic appearance of nasal mucosa 9 days after the procedure. This open, multicenter, randomized, controlled study was conducted on 50 patients undergoing partial bilateral inferior turbinectomy, packed with Algosteril on one side versus Polyvinyl acetal tampon (Merocel) on the other side following a left/right randomization. Both nasal packs effectively prevented post-operative bleeding. However, bleeding on packing removal was statistically less frequent and less severe with Algosteril than with Polyvinyl acetal (p = 0.016). In addition, pain was statistically lower with Algosteril than with Polyvinyl acetal (p = 0.0001). A trend to a better healing of the wound on day 9 was observed with Algosteril, leading us to further investigations. Algosteril nasal packing is as effective as Polyvinyl acetal in preventing postoperative bleeding following partial inferior turbinate resection. Its removal, however, is less traumatic for the nasal mucosa and less painful for the patient, therefore improving patient's comfort.

In vitro evaluation of bovine turbinate cells and Sarcocystis falcatula proliferation in response to serum-free medium and other media supplements.

A serum-free medium (SFM) was evaluated for the growth of bovine turbinate (BT) cells used for the production of Sarcocyvstis falcatula merozoites. Serum free cultures used to propagate S. falcatula were compared to cultures maintained in media supplemented with fetal calf serum (FCS) or horse serum (HS). Serum free cultures were more.effective and very promisin, than the others in supporting the proliferation of S. falcatula merozoites. However, the serum free cultures were unable to adequately support BT cell proliferation compared to the serum-supplemented cultures. No significant differences were seen between cultures supplemented with HS or FCS used for the production of S. falcatula merozoites or BT cells. The rate of BT cell proliferation in response to SFM and different media supplements was assessed in a 96-well plate format using methylene blue staining assay. This technique was superior to manual counting method and allowed quick and accurate quantitative comparison bet-ween the response of proliferating BT cells to different growth conditions

Obstruction of naso-endotracheal tube by inferior turbinate.

An unusual complication of naso-endotracheal tube obstruction by the inferior turbinate is reported. The delay of onset of signs and symptoms of endotracheal tube obstruction, and the lack of hemorrhage, made a difficult diagnosis.

Metabolism of alachlor by rat and mouse liver and nasal turbinate tissues.

The metabolism of alachlor was studied using in vitro incubations with microsomal fractions prepared from liver and nasal turbinates of rats and mice. Specifically, the transformation of alachlor to 3,5-diethylbenzoquinone-4-imine was examined. A key intermediate in this pathway was identified as 2,6-diethylaniline, the formation of which required catalysis by microsomal arylamidases. 2,6-Diethylaniline was oxidized to 4-amino-3,5-diethylphenol and the electrophilic 3,5-diethylbenzoquinone 4-imine. Rat nasal tissue possessed high enzymatic activity which can promote the formation of the reactive quinone imine. Whole body autoradiographic analysis demonstrated localization of radioactivity in the rat nasal tissue following oral administration of alachlor. A methylsulfide metabolite of alachlor was shown to be a precursor to 2,6-diethylaniline. The deposition of radioactivity in the rat nasal tissue was more pronounced following oral administration of the methylsulfide metabolite of alachlor.

Inflammatory disease involving a concha bullosa (enlarged pneumatized middle nasal turbinate): MR and CT appearance.

An enlarged pneumatized middle turbinate, called a concha bullosa, may be the site of inflammatory disease ranging from simple mucosal thickening to a mucocele. Four patients with inflammatory disease involving a concha bullosa were studied with MR and/or CT. Both imaging techniques detected the presence of mucosal inflammation and a mucocele within the concha bullosa, but CT better demonstrated the thin bony wall of the mucocele, allowing differentiation from other soft-tissue nasal masses.

Screening pig herds for toxigenic Pasteurella multocida and turbinate damage in a health scheme for atrophic rhinitis.

The Pig Health Control Association launched a health scheme for atrophic rhinitis in 1978. For several years pig herds were monitored by scoring the degree of turbinate damage and by clinical inspections. When laboratory facilities became available for detecting toxigenic Pasteurella multocida, nasal swabs were taken from pigs in Association herds during 1988 and 1989 to determine whether the organism was present. Sows were screened routinely and in the worst affected herds, sucklers and weaners were also swabbed. In 12 of 19 herds with consistently low snout scores toxigenic P multocida were not isolated, and in 15 herds which developed higher snout scores with time toxigenic P multocida were also not found. Eleven herds had never been listed by the Association, either because their snout scores were consistently high or because they had received importations of stock from herds with high snout scores; of six of these herds with the most persistently high snout scores five showed varying degrees of the clinical signs of atrophic rhinitis, but none of the six showed evidence of infection with toxigenic P multocida, and the organism was not found in the other five herds in the group. There seems to be an overlap between the clinical and gross pathological signs of atrophic rhinitis seen in some herds not infected with toxigenic P multocida and the mild and spasmodic signs of atrophic rhinitis seen in some herds which are substantially infected with the organism.

Mast cell distribution and morphology in human nasal turbinates following decalcification.

Whole inferior nasal turbinates were used to evaluate six different fixatives followed by decalcification. H and E staining was used to assess general fixation and toluidine blue and thionin to stain mast cells metachromatically. We show that neutral buffered formalin or paraformaldehyde give the best overall fixation. Very long times (six days) are needed in TB to show maximum mast cell numbers; these numbers are never as high, nor the cells as densely stained as those in undecalcified controls. The difference in number between the two supports the hypothesis that there is more than one subset of mast cells in nasal mucosa. The more superficial mast cells have a smaller size and may be dendritic, with relatively few granules which stain faintly. Those in deeper situations are often larger, ovoid, very granular and intensely stained.

[Functional microsurgery of the ethmoid sinus using an endonasal approach and preserving the middle turbinate].

The paper deals with the endonasal approach and the functional surgery of the ethmoidal region with respect to the middle concha. First of all, an anatomical survey of the ethmoid, pointing out the many useful topographic and vicinity particulars for endonasal management are displayed. His technique is detailed step by step, adding some modifications and variations introduced as a result of his own experience gained through 36 years of practice. The A stresses to the specialists the importance of being acquainted with the endonasal approach to the ethmoid sparing the middle turbinate, because it is very useful and gratifying.

[Concentration of trace elements in the inferior turbinates and in the nasal septum of the human].

The trace elements in 47 inferior turbinates (concha nasalis inferior) and in the nasal septum were examined by means of synchrotron radiation, spectroscopy of characteristic x-rays, and "sub-micron elemental mapping with the Oxford scanning proton microprobe", and the results compared. Both first methods displayed an age-dependent reduction of Zn and an increase of Pb concentration. In some specimens the As concentration was very high. The disposition of all elements except Zn, As and Fe was uniform. Zn and As were concentrated on the surface of the septum and Fe in the region of vascular channels.

Morphologic features, structure, and adherence to bovine turbinate cells of three haemophilus somnus variants.

Three colony variants (translucent, small opaque, and large opaque) of Haemophilus somnus recovered during infection studies of chicken embryos were examined using electron microscopy. Ruthenium red-strained H somnus preparations revealed no capsule in any variants. Pili were not seen on phosphotungstate-negative stained preparations. The translucent variant was thin-walled and had an irregular, pleomorphic rod shape. The small opaque variant had a thicker cell wall and an even rod shape. The large opaque variant had the thickest, most rigid wall of the 3, with uniform rod morphologic features. When each of the 3 variants was allowed to contact bovine epithelial turbinate cells, the translucent and small opaque were significantly (P less than 0.01) more adherent than was the large opaque variant.

Pyocele of the middle turbinate. A case report.

Pneumatization of the middle turbinate bone is a result of migration of ethmoid air cells. The accumulation of mucus when the ostium is blocked results in the formation of a mucocele or, if superadded infection is present, a pyocele (empyema).

Endoscopic sinus surgery with middle turbinate resection: results and complications.

Numerous articles have been published on the techniques of endoscopic sinus surgery. Fewer articles are available regarding the effectiveness of these procedures in curing the patient's symptoms. A retrospective review of eighty patients was performed to outline their symptoms and determine the success of the surgery. Partial middle turbinate resection was performed in all patients. The high overall success rate of 93.5% and the high antrostomy patency rate of 94.8% demonstrates the benefits of partial resection.

[Comparative study of most recent surgical techniques for the treatment of the hypertrophy of inferior turbinates].

The choice among the several techniques carried out to treat the hypertrophy of inferior turbinates is still difficult and matter of great interest. In the present study the Authors evaluated the efficacy and sequelae of the most common operations suitable for this pathology. The surgical procedures taken into consideration were (1) electrocautery; (2) cryotheraphy; (3) lasertherapy; (4) submucosal decongestion without lateral outfracture; (5) sub-mucosal decongestion with lateral outfracture; (6) turbinectomy. The six groups of patients (total 382) have been followed up for 4 years after surgery. At this aim the rhinomanometry, acoustic rhinometry, MCTt and SIgA dosage have been checked up together with the symptomatologic score. The conclusion was that the submucosal decongestion with lateral outfracture was the most effective technique for the chronic nasal obstruction, able to respect all the nasal functions.

[Endoscopic surgery of turbinates and nasal obstruction].

Nasal blockage is one of the most habitual symptoms in otolaryngologist office and the concha nasalis inferior hypertrophy the commonest cause of the trouble. Cases in which medical treatment is refractory, surgery could be an optional resort in accordance of a great deal of specialists. The possibility of give rise to atrophic rhinitis or even ozenatous lesions has been refraining this surgery lately. In our series are contemplated the outcomes of several surgical procedures as mode of management of the hypertrophy of the lower turbinal. In the article are assessed the results of several surgical techniques dealing with hypertrophy of concha nasalis inferior. 75 patients were operated under endoscopic control. Some underwent a submucous decompression (SD) other partial resection (PR). Results at half and long term of both techniques-the conservative one (SD) and that a little more aggressive (PR)-are compared, and so are the factors conditioning the selection of the surgical procedure done. We consider the partial resection of the inferior concha, under endoscopic vision, as the best way for improving the nose obstruction due to inferior concha hypertrophy, provided conservative measures are not wise at all. Furthermore because with our own hands we reach very good outcomes, both in breathing and nasal comfort. Only the patient's age may influence the last decision.
