[Comparative study of histology and flow cytometry results obtained in 2 groups of laryngeal tumors, glottic and supraglottic, in T-1 stage].

We studied histophatological parameters and DNA content in 27 supraglottic and 37 glottic T-1 squamous cell carcinoma of the larynx. The DNA content was determined by the Hedley method in paraffin-embedded sections of formalin-fixed blocks in an EPICS Profile flow cytometer. We studied 14 histopathological parameters and only 3 were statistically different in both groups of tumors: Differentiation, infiltrative degree and mitoses. There weren't statistical differences in DNA content (DNA index and proliferation index).

Concomitant boost radiotherapy of supraglottic cancer--preliminary results, morbidity.

Forty-two patients with supraglottic squamous cell carcinoma were irradiated with Co60 using concomitant boost technique, a variant of accelerated fractionation. This technique is characterized by administering the boost as a second daily fraction during the basic wide-field irradiations. Daily dose in the first 4 weeks was 1.8 Gy and during the last 2 weeks it was 1.6 Gy twice daily with 4-6 hours separation. Total dose ranged from 60 to 76 Gy, median 66 Gy. The treatment time ranged from 36 to 56, median 42 days. Acute mucosal reactions were more severe than those of conventional irradiation but acceptable. The follow-up time ranged from 9 to 31 months, median 16 months. The actuarial 18 months survival for all patients was 75%. The actuarial 18 months control for primary site and lymph node was 75% for T1-2 stage, and 47% for T3-4 stage. Severe late complications were not observed so far.

[Distribution and prognostic significance of microvessel density in supraglottic laryngeal squamous cell carcinomas].

OBJECTIVE: To study the distribution and prognostic significance of microvessel density (MVD) in supraglottic laryngeal squamous cell carcinomas (SGLSCC). METHOD: The patients of 59 primary SGLSCC received surgical treatment and 19 vocal cord polyps (VCP) were studied with CD31 immunohistochemical staining using Two Steps System Kits. RESULT: There were many microvessels in the stroma of SGLSCC compared with the connective-tissue under epithelium of VCP. Kendall's ranked correlation analysis indicated that only MVD and nodal status had significant correlation (P<0.01) in all clinicopathological parameters. Both between N0 and N+ and between SGLSCC and VCP MVD were significantly different (P<0.01) by independent-samples t test. Kaplan-Meier survival analysis showed significant relationship existed in MVD and cumulative survival rate of SGLSCC patients (P<0.01). CONCLUSION: This study further proves that angiogenesis plays important roles in tumorigenesis, invasiveness and metastasis. MVD could predict the lymph node metastasis and are significantly associated with the prognosis of SGLSCC patients.

[Problems in swallowing after partial supraglottic and supracricoid laryngectomy].

The results of above 30 year research of pharyngeal phase of deglutition act in patients after classical and extended supraglottic laryngectomies and supracricoid laryngectomies are presented and compared with physiologic data. Initially roentgenocinematographic method has been used, computerized topokinetic analysis of acquired images was introduced later. With the latter method visualization of the deglutition act was better and the analysis more objective. The main purpose of the study was to find which functions and structures were spared after operations, that were necessary to facilitate pharyngeal phase of deglutition act and make it similar to physiologic act of deglutition.

Laryngeal tuberculosis presenting as supraglottic carcinoma: case report and literature review.

Laryngeal tuberculosis is a rare extra-pulmonary manifestation of tuberculosis, and frequently presents with tumour-like symptoms and clinical findings. Uncommon clinical features are to be expected by the ENT specialist, who should be aware of these recently changing presentations. Despite the dramatic reduction of the incidence of the disease during the last few decades, a perceptible increase in case reports has lately been noted and may prove significant. Treatment remains conservative and long-term follow-up is suggested. A relevant case of laryngeal tuberculosis presenting as a supraglottic carcinoma is presented, its diagnosis and management are detailed, and suggestions are offered based on a literature review.

[The relation of metastasis and prognosis with transforming growth factor-beta1 and E-cadherin expression in supraglottic larynx squamous cell carcinoma].

OBJECTIVE: To study the expression of transforming growth factor-beta1 (TGF-beta1) and E-cadherin (E-cd) in Supraglottic larynx squamous cell carcinoma (SGLSCC) and the correlation with lymph node metastasis of neck. METHOD: The expression of TGF-beta1 and E-cd were studied in 60 cases of samples with SGLSCC and 10 cases of normal laryngeal mucosa by immunohistochemical staining (SABC method). They connected with clinical and followed data were analyzed. RESULT: Among 60 cases of human SGLSCC, 81.7% (49/60), 70.0% (42/60) were respectively defined as TGF-beta1 and E-cd - positive. Among 10 cases of normal laryngeal mucosa, 20.0% (2/10) 90.0% (9/10) were respectively defined as TGF-beta1 and E-cd - positive. There is a positive correlation between the expression of TGF-beta1 and the groups of lymph node metastasis, pathologic grade and clinical stage. However, there is a negative correlation between the expression of E-cd and the groups of lymph node metastasis, pathologic grade and clinical stage. The expression of the two indexes were significantly related to lymph node metastasis and not related to the age and sex of the patients,as well as the size of tumors. Although the two indexes were not concerted, they were inversely correlated with each other. E-cd expression was directly correlated with the survival functions of operated patients. CONCLUSION: The expression of TGF-beta1 and E-cd are correlated with lymph node metastasis in SGLSCC. There was negatively related to the two indexes. The two indexes may become the marker to predict the lymph node metastasis in SGLSCC, and E-cd may become the marker to judge the prognosis of SGLSCC.

Supraglottitis: the need of a standardized protocol.

Supraglottitis is an acute airway emergency requiring prompt medical intervention. Ten cases of epiglottitis treated at San Pablo Hospital are reviewed retrospectively. The controversies regarding this diagnosis are discussed. Due to a wide range of opinions concerning the proper management and treatment of epiglottitis, and the need of a standardized protocol, a simple and flexible protocol to be used by all physicians is proposed.

[Radiologic findings following supraglottic and total laryngectomy].

The treatment of carcinoma of the larynx has recently been greatly improved with the development of supraglottic and total laryngectomy. This has also led to a high five year survival rate. The normal postoperative radiological appearances are presented together with the spectrum of relevant radiological investigations and their indications. The most frequently seen postoperative complications and findings in patients with dysphagia are described with special reference to pharyngograms and cineradiography. The morphology of the vibrating segment after laryngectomy in patients who do not develop effective alaryngeal speech is demonstrated.

[The rehabilitation of swallowing after a supraglottic laryngectomy].

We report the swallowing rehabilitation experience of two patients submitted to supraglottic laryngectomy. Rehabilitation was obtained by training of specific swallowing movements to increase hypertrophy of the true vocal cords, which avoid pulmonary aspiration of foods. The first patient failed owing to unfavorable factors. The second patient was able to swallow pastes after eleven days of training, and the swallowing was normal forty days after the surgery. Success in this case was the result of patient's motivation and persistent efforts of the nursing personnel.

[Cancers of the supraglottic larynx: results and therapeutic indications apropos of 124 cases].

The authors report their experience of the treatment of the supraglottic laryngeal carcinoma in a retrospective study of 124 patients primarily treated in the ENT department of Grenoble University hospital. Fourteen patients received only radiotherapy, the others 110 patients were treated by surgery: partial or reconstructive surgery in 47 cases, total surgery in 63 cases. The carcinological results demonstrate the good prognosis of the intra-laryngeal tumors. Their treatment allows a good loco-regional control (tumoral recurrences: 4%, regional recurrences: 2%). The main prognostic factor is the nodal involvement with capsular rupture which is responsible for the most locoregional and metastatic recurrences.

[Supraglottic jet ventilation during direct laryngoscopy].

During 1983-1987 at the ENT Clinic, Faculty of General Medicine, Charles University, Prague 779 direct laryngoscopic examinations made where the technique of supraglottic ventilation was used. The method was introduced for the sake of greater safety during laser surgery of the larynx in the first 57 patients analysis of the blood gases in capillary blood was performed and the results were satisfactory. The technique which is described in detail is considered equally reliable and safe as ventilation by classical intubation. It provides, however, a better overall orientation of the larynx, better use of the laser technique, it saves operation time and intubation material.

[Cancer of the valleculal and laryngeal vestibule treated by supraglottic horizontal laryngectomy. Apropos of 173 surgically treated cases at the Institut Gustave-Roussy].

The authors make a report on a series of 173 supraglottal horizontal laryngectomies carried out from 1970 to 1984 at the Gustave-Roussy Institute of Villejuif. After recalling of operability conditions and tackling characteristics of this series are quoted variants required from tumours localisations diversity and importance of extension. Then are treated clinical and anatomo-pathological classification as briefly surgeries results. This work is specially concerning in survival by analysing different topographical clinical and histological criteria. Survival of 173 patients, having gone through an supra-glottal laryngectomy is 45.5% at 3 years and 31.3% at 5 years. At least, reasons of failure are analysed. This kind of partial surgery caring of voice, should be proposed to patients affected by vallecula or epiglottis carcinomas as regards of good functional and improved carcinological results.

Supraglottic laryngectomy: series report and analysis of results.

Forty-one consecutive patients who underwent supraglottic laryngectomy at UCLA were reviewed. All of the operations were performed by or under the direct supervision of the same surgeon (T.C.). The majority (68%) had advanced squamous cell carcinoma (Stage III-IV). Approximately one half received radiation therapy as part of planned combined therapy. The overall tumor-free survival, with a two-year minimum follow-up period, was 90%. The most common site of tumor recurrence was neck metastasis. There were four laryngeal recurrences, three of which were salvaged with completion laryngectomy. There was only one completion laryngectomy for severe aspiration. The favorable results in this series are attributed to frozen section control of surgical margins, surgical or radiation therapy treatment of cervical lymph nodes at risk for metastatic disease, and the employment of surgical techniques designed to minimize aspiration.

[Functional complications following supraglottal laryngectomy].

85 patients who had undergone supraglottic laryngectomy was necessary including the removal of one arytenoid cartilage or one vocal cord which was replaced by a thyroid cartilage graft. The latter group had greater difficulty in learning to swallow, a longer stay in hospital, more deaths as a result of lung infection, and an increased incidence of later lung infection.

[Phonatory airflow in the supraglottal space].

The phonatory airflow can be measured by means of a hot wire tube placed in the supraglottic space without tying down the tongue. The velocity of airflow above the glottis reaches values around c = 50 to 150 cm/s. The variations in airflow oscillations were recorded. The voice of the person under examination was picked up by a condenser microphone (Bruel & Kjaer No. 2112). According to D'Alembert's wave equation, the sound intensity is related to the velocity of the phonatory air stream. The validity of this statement has been confirmed by repeated testing. The fundamental frequency of voice and of the airflow were analysed synchronously by means of the Nicolet analyser. The air consumption is not utilized for sound production in phonation by breathing. A "hard" or "pressed" voice is associated with diminished or irregular air consumption. The method can be employed in assessing the conditions of phonetic airflow in normal and dysphonic voices.

[Treatment of carcinomas of the vallecula and epiglottis by supraglottal surgery. Indications and therapeutic results].

A study of 87 cases of carcinoma of the vallecula and the epiglottis (margin and vestibulum) with the common feature of an essentially identical surgical technique: supraglottal horizontal laryngectomy. In the case of vallecular lesions (34 cases from T1 to T4) this operation, performed from below upwards with lymph node dissection and associated with postoperative radiotherapy, gave oncological results identical to those of total laryngectomy and better than those of other protocols based upon radiotherapy. From a functional standpoint, despite two deaths due to bronchopneumonia, with careful respect paid to local and general contraindications there were no prolonged problems, even if the postoperative course was somewhat longer in lesions of the epiglottis. Carcinomas of the epiglottis (53 cases) did not receive radiotherapy where resection was adequate and lymph node histology negative. The results, better than in those cases involving the vallecula, seemed to be identical to those published in the literature. Such partial surgery, with preservation of the voice, should thus be beneficial in patients suffering from such lesions, in particular when the larynx is unaffected.
