Nutritional factors affecting growth before and after liver transplantation.

Malnutrition is present in approximately 60% of children at the time of liver transplantation. The pathogenesis of malnutrition is complex and includes reduced calorie intake, fat malabsorption, abnormal nitrogen metabolism, and increased energy expenditure leading to increased calorie requirements. As nutritional status at transplantation is a significant factor in both morbidity and mortality post liver transplantation, intensive nutritional support pre-transplant is vital and may be achieved with a modular feed providing 120-150% estimated average requirement (EAR). Approximately 80% of children who survive liver transplantation will achieve normal growth and nutritional status within 12 months post-transplant. Significant factors responsible for growth failure post-transplant include pre-operative nutritional status (height SDS score <-2), continued glucocorticoid administration, recurrent hepatic dysfunction or chronic rejection and reduced calorie intake due to behavioural feeding problems. Effective future strategies include intensive pre- and post-operative nutritional support and early reduction of glucocorticoid administration.

Flap repair: an effective strategy for minimising sexual morbidity associated with the surgical management of vulval intra epithelial neoplasia.

OBJECTIVE: To assess the sexual function after local excision and flap repair for symptomatic vulval intraepithelial neoplasia (VIN). STUDY DESIGN: A retrospective analysis of five sexually active women who had persistent, symptomatic VIN diagnosed in a dedicated tertiary referral vulval clinic and treated with local excision and flap repair. Sexual function was assessed using a modified version of the Sabbatsberg Sexual Self-Rating Scoring (SRS) system. RESULTS: The mean age of the cohort was 32 years (range 30 to 51). Four patients had previously been unsuccessfully treated with local excision. Follow up ranged from 5 to 33 months. The SRS scores were 90, 90, 81, 72 and 25. Endogenous depression may explain this last score. There has been no recurrence of VIN. CONCLUSION: In sexually active women with symptomatic VIN III, a flap reconstruction should be considered in addition to local excision as the treatment of choice.

The efficacy of terbutaline and magnesium sulfate in the management of preterm labor.

Ninety-six patients with preterm labor at 28 weeks to 35 weeks gestation were randomized to terbutaline or magnesium sulfate untill 36 weeks gestation, 25 patients were excluded from the study. Of the remaining 71 patients, 35 patients received terbutaline and 36 patients received magnesium sulfate. The result of the study showed that, there were no significant differences (P > 0.05) regarding time to stop, mean gestational age at delivery, time gained, failure rate, time to recurrent labor and readmission for recurrent labor, birth weight, apgar score and fetal survival. Serious maternal side effects were not observed with terbutaline or magnesium sulfate, although the majority of women also received dexamethasone. Neither drug caused serious adverse neonatal effects.

Quality of life assessment after Nissen fundoplication.

The aim of this study was to evaluate the quality of life of 31 patients presenting with gastroesophageal reflux (GORD) and operated on by Nissen fundoplication. The series consisted of 23 men and 8 women; the median age was 39 years (range 22-65) and the median follow-up 36 months (range 18-74). We used a new questionnaire: the Gastrointestinal Quality of Life Index (GIQLI) that includes 36 items and uses a five-graded Likert scale (from 0 to 4) giving a maximum score of 144. This score includes five dimensions: symptoms, emotions, vitality, social relations and medical treatment. The pre- and postoperative GIQLI scores observed in the Nissen group and the score of a control group of 110 healthy patients were compared with each other. The preoperative score (71 +/- 21) was greatly impaired compared to the score (123 +/- 13) of the control group (p < 0.0001). The postoperative score (109 +/- 21) increased significantly (p < 0.0001) but remained statistically inferior to the score of the control group (p < 0.005). The analysis of the dimensions showed that the postoperative score of the symptoms was lower in the Nissen group: 56 +/- 9 versus 66 +/- 6 in the control group (p < 0.0005) whereas no statistical difference was found for the four other dimensions. This lower symptoms score was not due to recurrence of GORD symptoms but to the occurrence of flatulence and to the persistence of gurgling noises and gas bloating. In conclusion, the quality of life of the patients requiring surgery for gastroesophageal reflux was greatly impaired, it largely improved after Nissen fundoplication but did not reach the level of healthy patients because of unrelated GORD gastrointestinal symptoms.

[Prognostic evaluation of severe viral hepatitis].

The Cox's regression model is used for analysis of registered survival data of severe viral hepatitis. The final regression model showed six covariates, i.e. grade of coma, prothrombin time, the interval between jaundice to bleeding, the interval between jaundice to coma, LDH, and pathological scores as well as the recurrence frequency were associated with survival significantly. The final model can be presented as a "pocket chart" by which a prognostic index (PI) for a new subject can easily be obtained. By a simple graph, the PI can be translated to estimates of the probability of surviving within a given time and the median survival time. A small sample of prospective study showed the predicted survival length did not differ from the observed survival length significantly.

[Immunotherapy by Japanese cedarpollen in atpic dermatitis].

Atopic dermatitis positive to Japanese cedar pollen showed the recurrence or worsening of the symptoms during the pollen season. In 22 cases, 6 children (mean, 11.4 y) and 16 adults (29.5 y) who showed positive to Japanese cedar pollen by RAST, CAST analysis were done by Cry j 1 0.01 approximately 10 micro g/ml and they showed the significant higher simulation indices compared to controls (P<0.01). Randomized analysis of the hyposensitized patients (10 cases) and non-hyposensitized (12 cases) showed significant lower stimulation indices in hyposensitized patients (P<0.01). In six cases stimulation indices were compared after one year of hyposensitization therapy. Four cases to whom hyposensitization were newly introduced showed the significant decreases of stimulation indices: 19.83+/-4.97 (mean+/-SEM) to 6.84+/-6.36 (65.0%) by Cry j 1 0.01 microg/ml, 19.73+/-5.65 to 6.85+/-1.78 (65.3%) by 0.1, 17.88+/-5.11 to 6.36+/-1.53 (64.4%) by 1, and 20.03+/-5.29 to 6.11+/-1.39 (69.5%) by 10, and they showed the significant decreases (P<0.05). By anti-IgE it decreased significantly from 35.08+/-3.42 to 7.00+/-1.77 (79.7%) (P<0.01). In two cases who got hyposensitization therapy for 2 years and 1 1/2 years each, there were no significant decreases of stimulation indices. The symptoms improved significantly and there were little or no recurrence of the symptoms. Symptom scores (Rajka & Langeland) showed significant decreases. Thus, hyposensitization by cedar pollen in atopic dermatitis is a promising treatment.

Colonic transit times and behaviour profiles in children with defecation disorders.

AIMS: To evaluate children referred for defecation disorders using the child behavioural checklist (CBCL). METHODS: A total of 215 patients were divided into three groups: 135 (5-14 years of age) with paediatric constipation (PC), 56 (5-17 years) with functional non-retentive faecal soiling (FNRFS), and 24 (5-16 years) with recurrent abdominal pain (RAP). Behavioural scores were correlated with colonic transit time (CTT) and anorectal function parameters (manometry and EMG). RESULTS: No significant differences in the mean CBCL scores were found among the three patient groups. However, children with PC and FNRFS had significantly more behavioural problems than the Dutch normative sample, while children with RAP had scores within the normal range. No significant differences were found between CTT in the patient groups, with respect to the CBCL. Similarly, no significant difference existed between children able or unable to relax their pelvic floor muscles during defecation attempts and their behaviour profiles. CONCLUSION: There seems to be no relation between colonic/anorectal function and specific behavioural profiles. On the other hand, children with defecation disorders show more behavioural problems than do controls.

[Prognostic value of stromal immunolabelling by MMP-2, MT1-MMP and TIMP-2 in clinically localized prostate cancer].

INTRODUCTION: Metalloproteinases (MMPs) promote cell migration and tumour invasion by degradation of the extracellular matrix. Activators and inhibitors (such as membrane type 1-matrix metalloproteinase or MT1-MMP and tissue inhibitor metalloproteinase 2 or TIMP-2) regulate metalloproteinase activity. The objective of this study was to determine the prognostic value of stromal immunolabelling with MMP-2, MT1-MMP and TIMP-2 in clinically localized prostate cancer. MATERIAL AND METHODS: The immunohistochemical study was performed on 30 radical prostatectomy specimens. The results were compared to preoperative PSA, Gleason score, pT stage and biochemical recurrence with a minimum follow-up of 5 years. RESULTS: Stromal immunolabelling with MMP-2 is correlated with stage pT3 (p=0.0022, OR=17.5). No correlation was observed with the other histoprognostic parameters. No statistical link was established with MT1-MMP and TIMP-2. CONCLUSION: Stromal immunolabelling with MMP-2 is a histoprognostic marker of capsular effraction in prostatic adenocarcinoma. When performed on prostatic biopsies, it can predict the pT3 stage, allowing adaptation of the initial treatment in some patients.

Choroid plexus papilloma in an infant.

A case of choroid plexus papilloma (CPP) associated with overproduction of cerebrospinal fluid (CSF) in an infant is reported. The tumor was totally resected. Histologically, it was diagnosed as CPP with several mitosis, however, there were no other features indicating malignancy. Transthyretin immunoreactivity was found in the tumor cells on CSF cytological examination as well as in surgical specimens. Though the percentage of PCNA was high (10.8%), it was not higher than other benign CPPs (mean score: 18.2%). There was no recurrence or dissemination on follow-up CT or MRI 1 year after surgery. The patient has grown normally after surgery without a shunting procedure.

[Cytogenetics of myelodysplastic syndromes].

The authors review the main cytogenetic abnormalities encountered in MDS particularly del 5q, -7, +8, and complex abnormalities. Their presence may be useful to the diagnosis of MDS in difficult cases. Their prognostic value is important, and cytogenetics have recently been included in prognostic scores in MDS. Knowledge of recurrent chromosomal rearrangements is finally a first step in the discovery of genes implicated in the pathogenesis of MDS.

Preoperative parameters for predicting early prostate cancer recurrence after radical prostatectomy.

OBJECTIVES: To determine whether easily measurable prostate biopsy features could complement Gleason score, prostate-specific antigen (PSA), or clinical stage in predicting recurrence-free survival after prostatectomy. Information relating preoperative parameters to recurrence-free survival is needed to counsel patients with newly diagnosed prostate cancer regarding expectations for postprostatectomy cancer control. METHODS: The data of a cohort of 588 consecutive prostatectomy patients (median age 61 years, range 39 to 83) with ascertained preoperative data and up to 4 years of postprostatectomy follow-up were analyzed. Bivariate and multivariate Cox proportional hazards analysis evaluated preoperative factors (clinical stage, PSA, biopsy Gleason score, greatest percentage of a biopsy core involved by cancer [GPC], number of biopsy cores containing cancer, perineural invasion) to identify those relating significantly to recurrence-free survival. Functional forms of these factors were evaluated to optimize accuracy in predicting cancer control. RESULTS: The baseline parameters significantly affecting PSA-free survival included PSA level (P <0.01), biopsy Gleason score (P = 0.04), and GPC (P <0.01). Although clinical stage and perineural invasion had a marginal association with PSA-free survival as univariate factors, this association was not independently significant in multivariable analysis. The multivariate Cox model using PSA, Gleason score, and GPC was highly predictive of PSA free-survival (chi-square = 48.2, P = 0.0001). A set of plots representing these data can be used to identify the risk of early postoperative PSA recurrence on the basis of specific preoperative PSA, Gleason score, and GPC values. CONCLUSIONS: These findings provide a highly significant model and a simple tool for assisting preoperative patient counseling regarding predicted cancer control after radical prostatectomy.

Postoperative prediction of and strategy for metastatic recurrent hepatocellular carcinoma according to histologic activity of hepatitis.

BACKGROUND: The hepatitis activity index (HAI) score describes the histologic status of accompanying chronic hepatitis and was established by pathologists. The aim of this study was twofold: 1) to investigate the correlation between intrahepatic metastatic recurrence (IM) and the HAI score of the noncancerous region of the liver and 2) to estimate the usefulness of postoperative preventive chemotherapy in patients with hepatocellular carcinoma (HCC). METHODS: The study included 158 consecutive patients who underwent curative resection for HCC and had been observed for > 1 year. Based on the HAI scores of the noncancerous region the patients were classified into 3 groups: those with mild hepatitis (n = 33) (i.e., with HAI scores of 0-5), those with moderate hepatitis (n = 77) (with HAI scores of 6-9), and those with severe hepatitis (n = 48) (those with HAI scores of > or = 10). In addition, a prospective randomized trial of postoperative adjuvant chemotherapy was performed for 21 patients with moderate hepatitis. RESULTS: The patients in the moderate hepatitis group were found to be at higher risk for IM recurrence within 2 years after HCC resection compared with those patients in the mild (P = 0.05) and severe (P < 0.01) hepatitis groups. The incidences of more than two tumors and portal vein involvement in patients with moderate hepatitis were much higher than in those patients with mild or severe hepatitis. Multivariate analysis showed that intraoperative bleeding volume, the number of nodules, portal vein involvement, and moderate hepatitis were independent predictive factors for IM recurrence free survival. Ten patients with moderate hepatitis had received postoperative intrahepatic arterial chemotherapy (2-3 courses with a maximum dose of 80 mg of cisplatin and 10 mg of mitomycin C at 1-month intervals) for the last 3 years. Although the number of patients was small, the therapy improved the disease free survival rate significantly compared with 11 patients who received no therapy. CONCLUSIONS: The patients with moderate hepatitis (HAI score of 6-9) had the highest rate of IM recurrence among the three HAI groups. Postoperative hepatic arterial chemotherapy may be useful in improving the rate of disease free survival after surgery among these patients.

Analysis of intra-uterine cytokine concentration and matrix-metalloproteinase activity in women with recurrent failed embryo transfer.

BACKGROUND: In all IVF programmes, some patients fail to achieve an ongoing pregnancy, even after numerous embryo transfer procedures. An unfavourable environment within the uterus might be a contributory factor to such recurrent implantation failure. This question was addressed by measuring cytokine concentrations and matrix metalloproteinase activities in fluid derived from uterine irrigation of such patients. METHODS AND RESULTS: The uterine cavities of 22 patients who had previously undergone embryo transfer of at least 10 embryos without ongoing pregnancy were irrigated during the luteal phase. The resultant fluid was assayed for the concentration of interleukin (IL)-1beta, tumour necrosis factor (TNF)-alpha, interferon (IFN)-gamma, leukaemia inhibitory factor (LIF), IL-10 and matrix metalloproteinase (MMP) -2 and -9 activity. The results were compared with those of a control population of women known to be previously fertile (n = 16) and also with women with recurrent spontaneous abortion (n = 13). In the recurrent implantation failure group, the MMP score and IL-1beta concentration were significantly higher than those in the control group, whereas concentrations of IFN-gamma and IL-10 were significantly lower. CONCLUSIONS: In IVF patients with recurrent implantation failure, an altered pattern of intra-uterine cytokine concentration and MMP activity was observed.

Survival and functional status after resection of recurrent glioblastoma multiforme.

OBJECTIVE: To determine the selection factors for and results of second resections performed to treat recurrent glioblastoma multiforme (GM), we studied 301 patients with GM who were treated from the time of diagnosis using two prospective clinical protocols. METHODS: The patients were prospectively followed from the time of diagnosis, using clinical and radiographic criteria after maximal surgical resection and external beam radiotherapy with or without adjuvant chemotherapy. Resection of recurrent GM was performed at the recommendation of the treating clinicians. The results of the second resections were retrospectively reviewed and analyzed using multivariate logistic regression, Kaplan-Meier-Turnbull survival analysis, Cox regression, and propensity score stratification. RESULTS: Forty-six patients underwent second resections during the study period. The actuarial rate of the second resections was 15% of the patients 1 year after diagnosis and 31% 2 years after diagnosis. Younger age (P = 0.01) and more extensive initial resection (P = 0.02), but not Karnofsky Performance Scale (KPS) score at the time of diagnosis or recurrence, predicted a higher chance of selection for reoperation after initial tumor recurrence. Twenty-eight percent of the patients had improved KPS scores after undergoing reoperation, 49% were stable, and 23% had declines in KPS scores of 10 to 30 points. There was no operative mortality. After reoperation, 85% of the patients received chemotherapy, 11% received brachytherapy or underwent stereotactic radiosurgery, and 17% underwent third resections. The median survival period after reoperation was 36 weeks. Higher preoperative KPS scores predicted longer survival periods after reoperation (P = 0.03). Age and interval since diagnosis were not significant prognostic factors. The median high-quality survival period (KPS score, > or =70) was 18 weeks. The median survival period after first tumor progression was 23 weeks for 130 patients treated using the same protocols who did not undergo reoperations. Patients who did undergo reoperations experienced clinically and statistically significantly longer survival periods. However, this was determined to be partially because of selection bias. CONCLUSION: Survival after resection of recurrent GM remains poor despite advances in imaging, operative technique, and adjuvant therapies. High-quality survival after resection of recurrence to treat GM seems to have increased significantly since an earlier report from our institution.

Female urethral strictures: successful management with long-term clean intermittent catheterization after urethral dilatation.

OBJECTIVE: To report our experience in the diagnosis and treatment of urethral stricture in women. PATIENTS AND METHODS: A retrospective review of records and video-urodynamics identified women treated for urethral stricture between 1999 and 2004 at one institution by one surgeon. Urethral stricture was defined as a fixed anatomical narrowing between the bladder neck and distal urethra of <14 F preventing catheterization, and the diagnosis was confirmed by cysto-urethroscopy, and/or video-urodynamics. Women with a history of external beam radiotherapy to the pelvis, or of gynaecological, urethral or bladder malignancy, were excluded, and the women had a urethral biopsy to exclude a malignant cause of the stricture. Initial treatment consisted of urethral dilatation to > or = 30 F. After a period of indwelling catheterization, the women were placed on clean intermittent self-catheterization (CISC) at least once daily, and monitored every 3-6 months. At each follow-up, the urethra was catheterized to exclude recurrence. American Urological Association (AUA) symptom scores were obtained at presentation and at the initial 3 month follow-up. RESULTS: Seven women met the criteria for urethral stricture, and were followed for a mean (range) of 21 (6-34) months. All were initially maintained on daily CISC, and some were gradually reduced to weekly CISC for the duration of follow-up. No patient had a recurrent stricture while on CISC, and none has had a urethral reconstruction to manage their condition. AUA symptom scores improved in all of the women by a mean of 10.7 points. No complications related to catheterization were noted. CONCLUSION: Urethral stricture is rare in women. Long-term CISC in these women is safe and effective, and can avoid the need for major reconstructive surgery.

Interphase argyrophilic nucleolar organiser regions and nucleolar counts in transitional cell bladder tumours.

AIMS: To see whether a correlation exists between clinicopathological parameters, argyrophilic nucleolar organiser regions (AgNORs), and nucleolar counts in the nuclei of tumour cells in patients with transitional cell bladder carcinoma. METHODS: Paraffin wax embedded sections from a total of 62 cases of primary transitional cell bladder carcinoma were stained with the silver colloid method. The numbers of individual silver grains (AgNORs) in nucleoli and the numbers of nucleoli were counted in 100 nuclei. The correlation between AgNORs and nucleolar counts and patients' sex, tumour grade, disease stage, recurrence pattern, and tumour related survival was analysed. RESULTS: The numbers of nucleoli in tumour cells were higher in male patients (p < 0.032). AgNOR numbers correlated with tumour grade (p = 0.017) and recurrence (p = 0.046). In multivariate analysis, the variation coefficient of AgNOR scores was found to be the only independent predictor of the duration of tumour free period in patients with recurrent disease (p < 0.002). AgNOR scores and nucleolar counts were of no value in distinguishing superficial and invasive tumours or in predicting tumour related survival. CONCLUSIONS: AgNOR scores in transitional cell bladder carcinoma reflect variations in tumour biological behaviour; however, the clinical value of this technique in patients with urinary bladder carcinoma is limited.

Quanticyt: karyometric analysis of bladder washing for patients with superficial bladder cancer.

OBJECTIVES: Quantitative cytology by image-analysis techniques enables objective interpretation of nuclear features in light microscopic images. QUANTICYT, a quantitative karyometric cytology system, was used in the follow-up of patients with superficial bladder cancer. METHODS: From 1992 to 1995, 4137 samples from 1412 patients were obtained. At 1-year follow-up after the initial bladder washing, a tumor recurrence rate of 21% was found. In this period, tumor progression to invasive disease occurred in 1.6% of patients. Scoring of tumor by the QUANTICYT system was based on two nuclear features: the 2c deviation index and the mean of a nuclear shape feature: MPASS. RESULTS: The method was found to be reproducible and superior to visual cytologic interpretation. QUANTICYT analysis of the bladder washings resulted in a score of low, intermediate, and high risk. In a multivariate analysis, highest grade of earlier tumor and QUANTICYT risk score were the best predictors of tumor recurrence and progression. For the easy application of QUANTICYT analysis in daily routine, a report form that included patient history and DNA histogram was developed. CONCLUSIONS: QUANTICYT karyometric analysis of bladder-wash material proved a useful, clinically applicable grading tool in the follow-up of patients with superficial bladder cancer, with sufficient power to be used in decision-making in the individual patient.

Simple aspiration versus chest-tube insertion in the management of primary spontaneous pneumothorax: a systematic review.

BACKGROUND: The initial treatment of a primary spontaneous pneumothorax (PSP) is controversial. Guidelines of the British Thoracic Society recommend simple aspiration for all PSP requiring intervention. The placement of chest tubes is only advocated for patients who fail simple aspiration. However, the American College of Chest Physicians Delphi Consensus Statement found simple aspiration to be rarely appropriate in the management of PSP. AIMS: To compare simple aspiration with chest-tube drainage in the initial management of PSP. METHODS: Meta-analysis of randomized controlled trials (RCTs). OUTCOME MEASURES: Reductions in duration of hospital stay, recurrence rate and pain or dyspnoea score were classified as benefits, whereas reductions in successful events were classified as risks. DATA COLLECTION AND ANALYSIS: For dichotomous data, the relative risk (RR) and 95% confidence intervals were calculated. For continuous data, weighted mean differences (WMD) were used. RESULTS: Three RCTs were identified with a combined total of 194 patients. Simple aspiration was associated with shorter hospitalization (WMD -1.30 days [-2.20 to -0.39]). The results for success rate could not be combined because of differences in outcome definitions. However, a pooled result for "success at 1 week or more" showed no significant difference between either intervention (RR 0.86 [0.67, 1.11]). Results of recurrence at 1 year were also not significantly different (RR 0.73 [0.39-1.38]). Different reporting systems for pain scores meant that data could not be pooled. Only one trial reported dyspnoea scores. CONCLUSION: RCT evidence in this field is limited, and the total sample size is too small to make any firm conclusion. On the basis of current available evidence, simple aspiration is advantageous in the initial management of PSP because of shorter hospitalization. There is no significant difference in recurrence at 1 year using either modality, and the efficacy data are inconclusive.
