Frustrated fertility: a population paradox.

This Bulletin examines the causes of subfecundity -- the diminished ability to reproduce -- and its effect today and in the past on the fertility, or actual reproductive performance, of individuals and, hence, populations. By definition, all real populations are subfecund since all experience some degree of involuntary biological factors affecting coitus, conception, or the ability to carry a conceptus to live birth which reduces their fecundity below the estimated biological population maximum of 15 children per woman. Affecting both men and women, these factors fall into 5 categories: genetic factors such as blood group incompatibilities and inherited sickle cell anemia or diabetes; psychopathology, including psychic stress and behavioral disorders (e.g., drug and alcohol abuse); infectious diseases such as gonorrhea, malaria, tuberculosis, and postabortion infection; malnutrrition, including the chronic undernutrition of the 3rd World and the overnutrition of developed societies; and hazards posed by increasing amounts of radiation and toxic chemicals in the environment. Reducing subfecundity requires improved living conditions, avoidance of or protection from known hazards, and adoption of medical advances which now can help 40 to 60% of subfecund couples. But even in the U.S. fertility would certainly rise among the 15% of couples now estimated to be involuntarily childless and the 10% who have fewer children than they want, and among disadvantaged groups, and teenagers.

Evaluation of vaginal tubal ligation.

A series is described of 150 cases of sterilization through posterior colpotomy. The operation was mainly elective or postabortal. The average hospital stay was short--1 or 2 days and complications were few-mainly pelvic peritonitis. There was only 1 death in the series covering 5 years--from peritonitis. The method is recommended for widespread use throughout the country both in rural and urban centres.

Anaerobic infections in surgery: clinical review.

Anaerobic bacteria are being recognized with increasing frequency as important micro-organisms in surgical infections. Clostridium, Bacteriodes, Fusobacterium, and Peptostreptococcus are the clinically prominent pathological anaerobes. All are commensals and, consequently, most anaerobic infections are endogenous in origin. In the colon, anaerobes are 1,000 times more prevalent than aerobes. This has important implications regarding the management of gastrointestinal tract operations and the treatment of infections originating from the bowel. Typical anaerobic infections include gas gangrene, brain abscess, oral infections, putrid lung abscesses, intra-abdominal abscesses, and wound infections following gynecologic and bowel surgery, perirectal abscesses, postabortal infections, and septic thrombophlebitis. Infections with anaerobic organisms must be suspected when there is feculent odor and/or gas production following gynecologic or bowel surgery, when there are organisms on gram staining but no growth on aerobic cultures, or when septicemia is associated with repeatedly negative blood cultures. Debridement and drainage constitute the main stay of treatment. All anaerobes are sensitive to chloramphenicol and clindamycin and all but Bacteroides fragils are sensitive to penicillin. Identification of anaerobes requires proper specimen sampling, immediate culturing on prereduced media, and careful gram staining of clinical material. The frequency of anaerobic organisms in surgical infections generally is not recognized by many surgeons; their importance needs to be stressed in the future.

Electroencephalographic changes following intraamniotic prostaglandin F2 alpha administration for therapeutic abortion.

Electroencephalograms were obtained on 43 patients undergoing therapeutic second trimester abortion using intraamniotic prostaglandin F2alpha. Recordings were made before and 4 hours after prostaglandin administration, as well as 4 hours after the abortion. At 4 hours after prostaglandin administration, 30% of the women had abnormal tracings, while 50% of the women had abnormal recordings 4 hours postabortion. Although the changes consisted primarily of mild generalized or lateralized slowing, 4 patients developed paroxysmal activity not initially present. This finding would seem to suggest a potential relationship between PGF2alpha administration and clinical seizures.

Post-abortion contraceptive acceptability in Blantyre, Malawi.

BACKGROUND: Unsafe abortion remains one of the major reproductive and public health challenges globally, but more so in developing countries. Lack of or poor contraception has been shown to be one of predisposing factors. OBJECTIVE: To determine contraceptive acceptance amongst post-abortion patients in Blantyre, Malawi. DESIGN: A cross-sectional pilot study. SETTING: The study was conducted in the gynaecological ward of the Queen Elizabeth Central Hospital, Blantyre, Malawi, the University Teaching Hospital. RESULTS: A total of 464 patients were studied. Their mean age and parity were 24 (SD 6.1) and 1.5 (SD 1.9) respectively. The single comprised 20.3% and students 16.4%. The index pregnancy was reportedly unwanted by 45.3% and 17.2% said they had unsafe abortion. Of these 373 (80.4%) accepted contraception. There was no relationship between acceptance and one's age (p = 0.28), marital status (p = 0.59); or parity (p = 0.5). The most commonly chosen methods were oral pills (45.3%), DMPA (21.8%) and male condoms (20.7%). The young (<25 years old), single, more educated, and women of low parity, preferred relatively less reliable contraceptives. CONCLUSION: The results of this study indicate that with proper planning postabortion contraception is acceptable in Malawi. It will provide an additional contraceptive service opportunity, thus increasing the currently low National CPR, and reduce repeat unwanted pregnancy, unsafe abortion, with all its potential sequel. We therefore recommend its integration into emergency post-abortion care.

Pelvic inflammatory disease.

Pelvic inflammatory disease is a common serious complication of the sexually transmitted pathogens Neisseria gonorrhoeae and Chlamydia trachomatis. There are more than 800,000 cases of pelvic inflammatory disease annually accounting for approximately 200,000 hospital admissions for acute and chronic infections. Early accurate diagnosis and treatment are essential to prevent the serious sequelae including ectopic pregnancy, tubal disease infertility, chronic pain, and disability requiring multiple hospitalizations and surgery. Although clinical models to aid in the diagnosis and management of pelvic inflammatory disease have been developed by numerous investigators, all have lacked the sensitivity and specificity to be helpful to the clinician. Laparoscopy, considered by many to be the "gold standard" for diagnosis, is underutilized, and the definition of pelvic infection differs between investigators. Improved patient compliance and safety may be seen if single-agent therapy for acute pelvic inflammatory disease becomes a reality. In a small prospective randomized study, oral ofloxacin was as effective as cefoxitin plus doxycycline for outpatient treatment of chlamydial and gonococcal pelvic inflammatory disease. Treatment of tuboovarian abscess appears to be successful with single agent and combination therapy. Risk factors for developing postabortion endometritis continue to be identified, and the most efficacious prophylactic antibiotic regimen has not been determined to date.

The snare method of laparoscopic sterilization: an analysis of 1,000 cases with 4 failures.

The snare method has proven to be an effective method of tubal resection for sterilization. In the most experienced hands, significant complications are indeed rare. However, the failure rate appears to be higher than that of the coagulation-alone method, at least in the postabortal patient. The necessity for pathologic confirmation, a tradition of surgery, is critically questioned. Additional coagulation of the tubal ends after resection is now recommended, especially in the patient seeking abortion as well as sterilization. The authors await the comparative long-term results of bipolar forceps and the newer mechanical methods of laparoscopic tubal sterilization. It is conceivable that the snare and other unipolar electrosurgical instruments designed for female sterilization will become obsolete.

TRAUMATIC INTRAUTERINE ADHESIONS. (THE FRITSCH-ASHERMAN SYNDROME).

In contrast to the foreign literature, there are no large North American studies on the sequelae to trauma and infection in the recently pregnant uterus. For this reason, the present status of these sequelae was reviewed and re-evaluated. They constitute a long-recognized, well-established clinical syndrome comprising: (1) past history of puerperal or postabortal infection and/or curettage, followed by (2) amenorrhea or hypomenorrhea, (3) dysmenorrhea, (4) habitual abortion, and (5) sterility. Knowledge of the existence of the entity is of great importance for its prevention and treatment. Strict maintenance of aseptic technique during curettage, use of a dull or serrated curette, and proper use of antibiotics are essential preventive measures. Treatment measures for this condition are solely surgical and consist of (1) dilatation and curettage, (2) hysterotomy, (3) transplantation of living tissues, and finally (4) hysterectomy.

Maternal mortality in a subdivisional hospital of eastern Himalayan region.

This study was conducted in a subdivisional hospital of eastern Himalayan region among 5,273 pregnant women over a period of 8 years. There were 29 deaths, the maternal mortality rate was 55 per 10,000. Septic abortion was encountered in 4 among them. Direct obstetric cause was responsible in 72.41% of cases and indirect cause in 27.59% cases. Sepsis, both puerperal and postabortal resulted in 24.14% followed by postpartum haemorrhage in 20.69%. Two of these cases were associated with inversion of the uterus. Preeclampsia caused 10.34% and eclampsia 6.9% of the deaths. Among the indirect causes severe anaemia and pulmonary tuberculosis accounted for 10.34% and 6.9% respectively. Infective hepatitis was the cause in 6.9% cases. Only 17% of the cases were booked and the rest were unbooked. Majority of the cases (62.07%) belonged to the age group of 20-30 years. Primigravida constituted 41.38% of the cases.

Induced abortion and traumatic stress: a preliminary comparison of American and Russian women.

BACKGROUND: Individual and situational risk factors associated with negative postabortion psychological sequelae have been identified, but the degree of posttraumatic stress reactions and the effects of culture are largely unknown. MATERIAL/METHODS: Retrospective data were collected using the Institute for Pregnancy Loss Questionnaire (IPLQ) and the Traumatic Stress Institute's (TSI) Belief Scale administered at health care facilities to 548 women (331 Russian and 217 American) who had experienced one or more abortions, but no other pregnancy losses. RESULTS: Overall, the findings here indicated that American women were more negatively influenced by their abortion experiences than Russian women. While 65% of American women and 13.1% of Russian women experienced multiple symptoms of increased arousal, re-experiencing and avoidance associated with posttraumatic stress disorder (PTSD), 14.3% of American and 0.9% of Russian women met the full diagnostic criteria for PTSD. Russian women had significantly higher scores on the TSI Belief Scale than American women, indicating more disruption of cognitive schemas. In this sample, American women were considerably more likely to have experienced childhood and adult traumatic experiences than Russian women. Predictors of positive and negative outcomes associated with abortion differed across the two cultures. CONCLUSIONS: Posttraumatic stress reactions were found to be associated with abortion. Consistent with previous research, the data here suggest abortion can increase stress and decrease coping abilities, particularly for those women who have a history of adverse childhood events and prior traumata. Study limitations preclude drawing definitive conclusions, but the findings do suggest additional cross-cultural research is warranted.

Outcome and management of crisis pregnancy counselling.

Between October 1989 and April 1991, 215 women were seen in the pregnancy centre. Of these women, 139 were in crisis pregnancy situations. Of the remaining 76 women, 42 attended for pregnancy education, 10 for counselling regarding adoption options, eight for postabortion counselling, and 10 for other reasons. All 139 women in crisis pregnancy situations received counselling and assistance with the resolution of the specific life crises that the pregnancy precipitated and were informed about the physical and emotional effects of pregnancy, normal fetal development, and the options that were available to them. These 139 women were divided into three groups at the start of their initial interview, according to whether they planned to continue with the pregnancy (18), seek termination of pregnancy (72), or were ambivalent (49). The outcome for each of these groups has been evaluated separately. Overall 65 (47%) of the women changed an initial decision or reached their final decision regarding the future of their pregnancy during the time of counselling, and 64 (46%) ultimately continued with their pregnancies. These outcomes support the need for a specific approach to crisis pregnancy situations such as that provided at the pregnancy centre and provide support for considering the crisis pregnancy an obstetric emergency.

[Postpartum contraceptive practice in hospitals of the Federal District].

For many women living in developing areas, childbirth is the only time when they receive medical care in a clinical setting. These women may not return until they are ready to deliver their next baby. Without access to family planning counseling and effective contraceptive methods, they are likely to become pregnant again within a year or two. Where contraceptive information and services are available to women receiving maternity care, many choose to begin using contraception in the postpartum period. Some elect to have an IUD inserted immediately after delivery or to undergo sterilization while still in the hospital. Under a postpartum contraceptive program administered by the IMSS in one hospital and the same program established in another hospital of the SSA, more than half of postpartum and postabortion women began using contraception or underwent sterilization before they left the hospital. In contrast, the other hospitals under the analysis showed a wide range of contraception coverage from 7 to 55 percent. A formal postpartum contraception program is necessary to establish in all the hospitals in order to improve coverage. Counseling is an important component for all women who must have adequate information before they choose a contraceptive method.

Abortion in Europe, 1920-91: a public health perspective.

This article grew out of a keynote address prepared for the conference, "From Abortion to Contraception: Public Health Approaches to Reducing Unwanted Pregnancy and Abortion Through Improved Family Planning Services," held in Tbilisi, Georgia, USSR in October 1990. The article reviews the legal, religious, and medical situation of induced abortion in Europe in historical perspective, and considers access to abortion services, attitudes of health professionals, abortion incidence, morbidity and mortality, the new antiprogestins, the characteristics of abortion seekers, late abortions, postabortion psychological reactions, effects of denied abortion, and repeat abortion. Special attention is focused on the changes occurring in Romania, Albania, and the former Soviet Union, plus the effects of the new conservatism elsewhere in the formerly socialist countries of central and eastern Europe, particularly Poland. Abortion is a social reality that can no more be legislated out of existence than the controversy surrounding it can be stilled. No matter how effective family planning services and practices become, there will always be a need for access to safe abortion services.

Gestational trophoblastic neoplasia: clinical trends in 8 years at Hospital Universitario de Caracas.

OBJECTIVE: To analyze the clinical trends of gestational trophoblastic neoplasia (GTN) at the Department of Obstetrics and Gynecology, Hospital Universitario de Caracas (HUC). STUDY DESIGN: A medical record review was performed of epidemiologic, clinical and diagnostic features of 25 cases of GTN at HUC from 1997 to 2004. RESULTS: During the study period, 35,300 deliveries occurred, and 25 patients were diagnosed with GTN; the prevalence was 0.70:1,000 deliveries. The mean age was 29.2 years. Fifty-six percent were posthydatidiform mole (HM), 36% postchoriocarcinoma (CC), 4% postinvasive mole and 4% postabortion with abundant intermediate trophoblast. Vaginal bleeding was the main symptom in patients with CC. Two cases resembled ectopic pregnancy, and another resembled a vaginal endometrioma. Fifty-two percent of cases were at stage Ib; 76% received single-agent chemotherapy. Hysterectomy was performed in 6 cases. Twenty-one patients achieved remission, 2 showed regression and 2 died. CONCLUSION: GTN had a high prevalence because HUC is a reference center. The most common presentation was post-HM GTN. Vaginal bleeding is frequent in CC and can mimic other gynecologic diseases. Chemotherapy is helpful, and hysterectomy can be performed in selected cases at early stages or with severe vaginal bleed-with a good ing. GTN has a good prognosis, and early diagnosis is possible.

Prevalence of Chlamydia trachomatis in women seeking termination of pregnancy.

Fifty consecutive unmarried women seeking termination of pregnancy in Toa Payoh Hospital were analysed to determine the prevalence of Chlamydia trachomatis infection as compared to gonococcal infection. Cases harbouring chlamydial infection were followed up to exclude postabortal pelvic infection. Incidence of positive cultures were also compared among the various ethnic groups in the population. Results from the data analysed showed that chlamydial infection was significantly more prevalent than gonorrhoea in our local women seeking therapeutic abortion. However, further research to define the exact size of the problem is indicated.

[Sacral epidural anesthesia as a method in technically complicated abortions].

An experience is reported with the use of epidural-sacral anesthesia for artificial abortion in 102 primiparae+- and nulligravidas and in 86 patients with scarring deformity or partial atresia of the endocervix. Two percent trimecaine was used in a single dose 6 mg/kg. A high efficiency of anesthesia was seen in 92% of cases. There was a significant relaxation of the cervix which facilitated cervical dilation and reduced the incidence of tissue injury. The population under study showed a lower incidence of intraoperative blood loss and early postabortal complications as compared with a population which received other anesthesia. These results suggest that epidural-sacral anesthesia is a promising technique for artificial abortion in primiparae+- and nulliparous women and in women with scarring cervical lesions.

[Use of group-specific contaminants A and B in albumin preparations produced from the donor, placental and abortion blood].

Assay of group-specific blood substances (GSBS) A and B was conducted in 21 series of an albumin preparation produced from donors' blood serum, in 11 series of placental and in 5 series of "Postab" albumin. GSBS content was estimated by a comparative use of poly- and monoclonal antibodies prepared in this country. More than 80% of the preparations from donors' blood serum did not contain GSBS A and B. In the preparations obtained from placental and postabortion blood the mean levels of GSBS significantly fluctuated, and with the use of poly- and monoclonal antibodies in comprised 8.3-9.1 micrograms/ml GSBS A and 16.7-42.8 micrograms/ml GSBS B, respectively. Administration to recipients of albumin preparations contaminated with GSBS may produce an immunomodulating side effect.

Prenatal diagnosis of and midtrimester pathology with karyotype 46,XY,del(4)(q22q26). A case report.

A prenatal diagnosis of an interstitial deletion with chromosome 4,46,XY,del(4)(q22q26) was obtained on amniotic fluid cells drawn at 19 weeks' gestation from a 35-year-old gravida. Counseling on the basis of unusual or tenuous data is always difficult, but comparisons with similar deletions in 4q suggested a substantial risk of anomalies. A comparison of the postabortal autopsy findings with those from other reported cases of interstitial deletions of chromosome 4q suggested different pathology with this area of deletion than previously reported for other areas of 4q.
