The association of gender identity disorder with parental separation.

56 boys diagnosed with gender disturbance, ages 3 to 18 yr. (mean age 8.4 yr.), were classified according to family structure. The proportion of gender-disturbed subjects separated from one or both parents (66%) was significantly higher than the 35% to 48% separated from one or both parents in comparable US general population statistics.

Decreased incidences of intestinal and diffuse types of gastric carcinoma in Finland during a 20-year period.

The incidence of gastric carcinoma (GCA) has decreased throughout the world. This decrease is attributed to a decline in incidence of the intestinal type of GCA (IGCA), whereas the diffuse (DGCA) type of GCA is considered to be endemic in nature and more stable in incidence. In the present study we have estimated how much the incidences of IGCA and DGCA have decreased in percentage in Finland from 1952-61 to 1972-81. Our calculations are based on the Finnish Cancer Registry data of new GCA cases, on population statistics in Finland, and on the percentage distribution of GCA subtypes in three consecutive samples of GCA patients collected in the time periods 1952-61 (reference series) and 1972-81 (two separate samples: series A and B). The samples totaled 1837 GCA cases. We calculated that the incidence of IGCA had decreased from 1952-61 to 1972-81 by 62-71% (reference series versus series A - reference series versus series B) among men and by 69-70% among women. Correspondingly, the incidence of DGCA was calculated to have decreased by 30-39% among men and 37-42% among women. We conclude that not only has the incidence of IGCA decreased in percentage approximately twice as much as the incidence of DGCA but DGCA has also distinctly decreased in incidence in Finland from 1952-61 to 1972-81.

Non-Hodgkin's lymphomas in elderly patients.

Based on population statistics and institutional reviews, the median age of patients developing non-Hodgkin's lymphomas (NHL's) is around 65 years. A review of retrospective studies suggesting that increasing age imparts an adverse prognosis in patients with NHL's is presented. Interpretation of this data is often confounded by referral bias of patients to specialized centres, multiple other NHL-related risk factors and inadequate chemotherapy administration due to age and toxicity related dose reductions. These factors, as well as alterations in tumour-host biology and comorbid diseases which result in changes in pharmacokinetics and pharmacodynamics, are discussed as possible reasons for poorer outcome in the elderly. In an effort to develop better tolerated and thus more effective combination chemotherapy for older patients, a number of prospective single arm and randomized clinical trials of novel regimens have been undertaken. Improved rates of disease remission and overall survival appear often to have been achieved at the expense of greater morbidity and mortality. Ongoing attempts to improve the therapeutic index include the application of chronic oral chemotherapy, brief duration intensive therapy and fractionation of standard drug doses as well as incorporation of myelo-preserving haematopoietic growth factors. The possibility of developing flexible, "customized" therapy for elderly patients is discussed.

Multistate worklife expectancies.

This communication reviews the demographic concept of worklife expectancy and draws the epidemiologists' attention to its usefulness in occupational health research and pension policy making. The distinctions between different analytic approaches to the quantification of expected workforce status and mobility are pointed out. A recently developed multivariate large-sample regression method for the analysis of worklife tables is placed into the general context of life tables. Given aggregated data from multiple cross-sectional or longitudinal population surveys, a multistate regression model can be used to estimate consistently marginal probabilities that a person is in a given work-health state or transition probabilities between the states and, thereby, worklife expectancies. The methodology is illustrated through the application to data from Finnish population statistics on employment, disability, retirement, and mortality. The paper closes with a discussion of the methodological issues and empirical findings on pension policy in Finland.

Lung cancer in New South Wales: current trends and the influence of age and sex.

OBJECTIVE: To examine the effects of time, sex and age at diagnosis on lung cancer incidence rates and the distribution of the histological types of lung cancer in New South Wales. DESIGN AND SETTING: Retrospective analysis of data from the NSW Cancer Registry and Australian Bureau of Statistics population data for NSW for 1985-1995. MAIN OUTCOME MEASURES: Trends in lung cancer incidence rates between 1985 and 1995 for men and women aged over 30 years; changes in incidence rates within age groups; and incidence rates of histological subtypes relative to sex and age. RESULTS: The incidence of lung cancer in men aged 40-80 years fell, while that in women aged over 65 rose. Rates were stable in younger women and older men. Incidence rates in men aged 40-60 years fell by 40%-60%. Were it not for the reduction in incidence rates in men between 1985 and 1995, the number of male lung cancer cases in 1995 would have been greater by 389 (95% CI, 362-415). In women, increasing incidence rates were responsible for an extra 242 cases (95% CI, 232-253) in 1995. Adenocarcinoma comprised a greater percentage of lung cancer cases in younger people, while squamous-cell carcinoma increases steadily with age in both men and women. Women with lung cancer are less likely to have squamous-cell carcinoma (25% for women v. 40% for men) and therefore more likely than men to have adenocarcinoma (35% of new female cases v. 26% for men) or small-cell lung cancer (24% v. 19%). CONCLUSIONS: Increased smoking cessation has seen a halving of lung cancer rates in middle-aged men. Whether this represents delayed or prevented cases is uncertain. The distribution of histological subtypes of lung cancer in women is different from that in men, and it is not clear whether this difference is hormone-dependent or related to historical patterns of smoking.

Bupropion SR vs placebo for smoking cessation in health care professionals.

OBJECTIVE: To evaluate bupropion SR for smoking cessation in physicians and nurses. METHODS: This double-blind prospective 26-center, 12-country trial randomized 687 subjects to smoking cessation counselling with bupropion SR or placebo for 7 weeks. The participants were followed for 52 weeks. RESULTS: Bupropion SR was superior to placebo (50% vs 40%, P=0.013) on the 4-week primary outcome variable. Due to a high placebo response in this health care population, statistical differences were not maintained after treatment was discontinued. CONCLUSIONS: Bupropion SR is effective and well tolerated in health care professionals. Relapse prevention measures are needed to attain long-term abstinence.

Reducing Molecular Shuttling to a Single Dimension.

An analogy with a cart on a roller coaster partially explains the shuttling motion of macrocycles in peptide rotaxanes. Just above the barrier to shuttling, the macrocycle statistically populates the "track" rather than the low-energy "stations" (see the potential energy curve). The dynamics of the movement is described in terms of a simplified one-dimensional model based on the solution of the relevant quantum-mechanical equation. x=position.

Validation of nuclear texture, density, morphometry and tissue syntactic structure analysis as prognosticators of cervical carcinoma.

OBJECTIVE: To evaluate the performance of karyometry and histometry in the prediction of survival, recurrence and response of early-stage invasive cervical carcinoma. STUDY DESIGN: Nuclear morphometry, chromatin texture and tissue architecture (characterized by syntactic structure analysis) were measured using a semiautomated image analysis system on 46 cases of Feulgen-stained tissue sections. The performance of the features was compared to that of clinical features, reported to be the best prognosticators until now, such as age, lympho-vascular permeation, histologic type, stage and grade. A K nearest neighbor classifier was used for classification. RESULTS: In the prediction of three-year survival, recurrence and response, syntactic structure analysis proved to be the best performer. Classification rates were, respectively, 100%, 94.4% and 94.5%. In all classifications, karyometric and histometric features outperformed clinical features. In general, the best performing features described differences in second-order population statistics (standard deviations). CONCLUSION: The results show that a quantitative analysis based on nuclear morphology, chromatin texture and histology can be considered an excellent aid in the prognosis of invasive cervical carcinoma. The measurements are not hampered by the need to undertake complete resections and are suited to daily practice when implemented in a semiautomated image analysis system.

[Primary hyperparathyroidism treated surgically].

Primary hyperparathyroidism is a systemic disease, more and more frequently recognized-concerning 1 to 3% of the population. Statistically appears in 1 of 1000 adults, with significant advantage of women. In Poland every year about 30 new cases are noticed and incidence increases with an age. In spite of significant advance of the knowledge, it still makes a lot of diagnostic troubles. It appears to be non-specific illness, characterised by just one symptom, mainly urolithiasis, sometimes chronic ulcer disease, chronic pancreatitis, arterial hypertension, disorders of the movement or psychic disorders. Parathyroid adenoma which is the main reason of the disease is usually single and small, multiple and bigger ones are found exceptionally. In about 2% of cases they are localized in mediastinum. In the article the basic symptoms, diagnostic and therapeutic problems were shown, especially concerning surgical treatment which is safe, radical and efficacious method when performed by experienced surgical team and the conduct from choice on primary and secondary hyperparathyroidism.

Prevalence of elder abuse in the United States: a comparative report between the national and Wisconsin data.

This study is designed to calculate the prevalence of elder abuse in Wisconsin and the United States and to perform a geographic comparison of elder abuse prevalence by county in Wisconsin. The national data was obtained from the National Elder Abuse Incidence Study, and the Wisconsin data from the Department of Health and Family Services. The population statistics were obtained from the United States Census Bureau. The prevalence of elder abuse based on substantiated cases was 2.7 per thousand (236,479 cases) in the United States and 1.6 per thousand (2629 cases) in Wisconsin in 1996 (p < 0.05). There was an increase in elder abuse prevalence in Wisconsin between 1990 and 1998. Larger metropolitan counties had the largest number of reported cases but more rural counties had the highest prevalence (14 per thousand in Vilas County) and the highest rate of increase in elder abuse prevalence (increase by 9.4 per thousand in Langlade County). This study shows that there are national and local geographic variations in the prevalence of elder abuse cases reported to Adult Protective Services. Further studies are needed to identify factors that led to these variations.

[Gastric diseases in foreign workers in the German Federal Republic (author's transl)].

Gastric biopsies from foreign workers were compared with those from German patients of the same age group. With reference to population statistics investigations showed that a far higher percentage of foreign workers are gastric patients than are Germans of the same age. Chronic atrophic antral gastritis occurs in foreigners twice as often as in a comparative group of Germans. 77% of the duodenal ulcers and 88% of the gastric ulcers are found in the group of foreign workers examined aged from 21 to 40 years. The German patients, on the other hand, show normal gastric mucosa with more than chance frequency compared with foreign workers of corresponding age and sex.

[Surgical treatment of primary and secondary hyperparathyroidism].

Primary hyperparathyroidism is a systemic disease, more and more frequently recognised--concerning 1 to 3% of the population. Statistically appears in 1 of 1000 adults, with significant advantage of women. In Poland every year about 30 new cases are noticed and incidence of it increases with an age. In about 2% of cases they are localised in mediastinum. In the comparison to primary hyperparathyroidism the secondary one appears as an effect of the other general disorders. Our material contains 12 patients, 9 with primary and 3 with secondary hyperparathyroidism. All of them were diagnosed and prepared to the operation in the departments of internal diseases. We have not observed any serious complications during the operations and in the postoperative period. In the article the basic symptoms, diagnostic and therapeutic problems of primary and secondary hyperparathyroidism were shown, especially concerning surgical treatment which is safe, radical and efficacious method when performed by experienced surgical team.

[Basic theoretical and methodological principles of developing a population and housing census program in the People's Republic of Bulgaria in 1985].

"Proceeding from the Marxist approach to the relation between population and social development, a significantly broader theoretical model of studying the population by means of the forthcoming [Bulgarian] census is justified." The model includes demographic, educational, and socioeconomic aspects, as well as labor force activity. The collection of data by means of sample surveys is assessed, with a focus on areas concerning fertility and reproductive behavior, migration, labor needs and resources, health, and the social system.
