[Systemic concepts in residential placement: considerations for pedagogics, counseling and organization].

The paper starts from an analysis of typical problems in residential child care, which can be identified on different system levels (family, family and residential child care unit, family and larger systems, child care as a profession). More recent systems concepts (such as autopoiesis, role of the observer, "fitting", limits of planning in human systems) are used to develop suggestions for everyday practice of residential child care--from intake to dismissal, from crisis intervention to family education, from job satisfaction to management issues.

[Clinical continuing education of younger physicians. A pedagogic trial with postgraduate clinical training using the clinical decision process].

The aim of this study was to improve the clinical training of the staff of a department of internal medicine. A total of 16 clinical pathological conferences with patient demonstrations were given. The doctor in charge of the conference motivated his choice of patient and made a critical review of the clinical decision process according to a 16 item check list. The median time used for planning of the presentation was five hours, range 2.5-8 hours. Sixteen of the participants (94%) found the training programme of very high or high quality while 1 (6%) found it less good. Fifteen (88%) of the doctors indicated higher clinical skill after than before the conferences (p less than 0.01). The registrars revealed a significantly better improvement of their clinical skills than did the senior registrars and consultants (p less than 0.05). The median educational value of the items of the check list was stated to be 1.3 (0-2). Statement of the probability of the final diagnosis of the patient, value judgement in clinical decisions and costs of diagnostic examinations were considered of highest educational value, 1.5-1.6. Postgraduate clinical pathological conferences with patient demonstrations including systematic reviews of the clinical decision process are valuable in the clinical training of doctors.

[Differential results of treatment in pedagogic-therapeutic long-term intervention in residential treatment centers for children].

In a longitudinal evaluation study the developmental progress of 223 children, who have been treated between 1968 and 1987 in a residential facility for psychiatric care, was explored from referral until time of discharge. Initial state and state at discharge were defined multidimensionally according to MAS. Comparisons between children with different initial clinical diagnosis met expectations, that introversive disturbances recover more frequently than autistic as well as conduct disorders or hyperactivity. Associations between recovery and initial cognitive-intellectual state did not reach statistical significance, when type of disorder was controlled for. But rate of recovery appeared to be throughout higher for children with learning disabilities than for children without such deficits. Age at onset and age at referral were apparently rather irrelevant for the state at discharge. But rate of recovery was in general clearly reduced, when length of time between onset and referral had been above average. The relation between duration of treatment and state at discharge depended on the type of disorder. For children with conduct or autistic disorders the rate of recovery increased with longer duration of treatment, whereas for children with introversive disturbances the contrast was true. This could suggest confounding effects of severity of disorder. When the treatment in the facility had been precociously terminated, rate of recovery in general was remarkably lower than in case of a planned termination. Analysis on effects of alternative methods of treatment was only with restrictions possible and did not suggest a general or differential superiority of a special type of treatment.

[Street work and AIDS--experiences and considerations from a socio-pedagogic viewpoint].

Streetwork has proved its worth as a method that is useful in AIDS work. The emphasis is on rendering direct help to the affected persons. This help has indirectly a preventive effects as well because it initiates and supports action that is directly related to social care. Self-initiatives of patients with a positive HIV finding should be given support and should be promoted by communal bodies. AIDS help offers must be re-examined and changes appropriately in accordance with the requirements of the patients and the acceptability of the measures. The AIDS or HIV patients themselves must be basically involved in this permanent orientation process.
